In a recent article,[1] we have demonstrated a significant difference in the theoretical knowledge of both junior and senior emergency department (ED)medical offi cers (mOs) from Salem, Tamil Nadu, India,and the Royal Adelaide Hospital (RAH) Adelaide, South Australia, in the management of severely injured trauma patients, when comparing mOs who had completed an ATLS-type program with those who had not undertaken such a program.

Application of Predictive model for Efficient Cassava (manihot esculenta Crantz) Yield in the Face of Climate Variability in Enugu State, Nigeria

Climate variability as occasioned by conditions such as extreme rainfall and temperature, rainfall cessation, and irregular temperatures has considerable impact on crop yield and food security. This study develops a predictive model for cassava yield (manihot esculenta Crantz) amidst climate variability in rainfed zone of Enugu State, Nigeria. This study utilized data of climate variables and tonnage of cassava yield spanning from 1971 to 2012;as well as information from a questionnaire and focus group discussion from farmers across two seasons in 2023 respectively. Regression analysis was employed to develop the predictive model equation for seasonal climate variability and cassava yield. The rainfall and temperature anomalies, decadal change in trend of cassava yield and opinion of farmers on changes in rainfall season were also computed in the study. The result shows the following relationship between cassava and all the climatic variables: R2 = 0.939;P = 0.00514;Cassava and key climatic variables: R2 = 0.560;P = 0.007. The result implies that seasonal rainfall, temperature, relative humidity, sunshine hours and radiation parameters are key climatic variables in cassava production. This is supported by computed rainfall and temperature anomalies which range from −478.5 to 517.8 mm as well as −1.2˚C to 2.3˚C over the years. The questionnaire and focus group identified that farmers experienced at one time or another, late onset of rain, early onset of rain or rainfall cessation over the years. The farmers are not particularly sure of rainfall and temperature characteristics at any point in time. The implication of the result of this study is that rainfall and temperature parameters determine the farming season and quantity of productivity. Hence, there is urgent need to address the situation through effective and quality weather forecasting network which will help stem food insecurity in the study area and Nigeria at large. Folate and vitamin B12 in idiopathic male infertility

Although methylenetetrahydrofolate reductase, a folate enzyme gene, has been associated with idiopathic male infertility, few studies have examined other folate-related metabolites and genes. We investigated whether idiopathic male infertility is associated with variants in folate, vitamin B12 （B12） and total homocysteine （tHcy）-related genes and measured these metabolites in blood. We conducted a case-control study that included 153 men with idiopathic infertility and 184 fertile male controls recruited at the Fertility Center and Antenatal Care Center, University Hospital, malmo and Lund, Sweden. Serum folate, red cell folate （RCF）, serum B12, plasma tHcy and semen quality were measured. Subjects were genotyped for 20 common variants in 12 genes related to folate/B12/ homocysteine metabolism. metabolite concentrations and genotype distributions were compared between cases and controls using linear and logistic regression with adjustment for covariates. The phosphatidylethanolamine N-methyltransferase （PEmT） m 175V and TCblR rs173665 polymorphisms were significantly associated with infertility （P=0.01 and P=0.009, respectively）, but not with semen quality. Among non-users of supplements, infertile men had lower serum folate concentrations than fertile men （12.89 vs. 14.73 nmoll^- 1 P=0.02）, but there were no significant differences in RCF, B 12 or tHcy. Folate, B 12 and tHcy concentrations were not correlated with any semen parameters. This study provides little support for low folate or B12 status in the pathogenesis of idiopathic male infertility. Although additional data are needed to confirm these initial findings, our results suggest that PEmTand TCbIR, genes involved in choline and B12 metabolism, merit further investigation in idiopathic male infertility.

A patient with myoclonus- dystonia syndrome was treated by implanting electr odes in the internal segment of the globus pallidus (GPi) and applying deep brai n stimulation. Surgery was done in two sessions. The most affected limb was trea ted first and the other limb one year later. Neuronal recordings showed that mos t pallidal neurones discharged in bursts at a relatively low firing rate (mean ( SD), 46 (18) Hz) compared with cells in the GPi in patients with Parkinson’ s d isease. Neurones modified the rate and mode of discharge with dystonic postures and rapid involuntary contractions of limb muscles. Neurological examination at 24 months after surgery showed a decline of 47.8% and 78.5% in the Burke- F ahn- marsden and disability rating scales, respectively.
