Keeping Pregnant Patients Safe During COVID-19 Pandemic

During a crisis such as the coronavirus disease 2019(COVID-19)pandemic,a hospital can suspend all elective surgery to expand capacity for intensive care;1 however,the clinical care on the Labor and Delivery Unit(L&D)cannot be *** patients must come to the hospital for their obstetric care and delivery,and patients may express concerns and fear regarding the risk of nosocomial *** CO VID-19 pandemic poses multiple challenges specifically for L&D Units around the world,such as:(1)caring for CO VID-19 and non COVID-19 pregnant patients simultaneously on the same unit;and(2)the mode of delivery cannot be pre-determined and emergent cesarean delivery is sometimes ***,strategies and guidelines are desperately needed to protect pregnant patients and health care workers(HCWs)from viral transmission while at the same time ensuring safe and swift responses to obstetric *** the CO VID-19 pandemic,we have learned an enormous amount about how to provide safe obstetric *** we report and share our strategies and experience as the CO VID-19 pandemic is accelerating and global peak is still to come.

Chiral Camphor-Based 1,3- and 1,4-Amino Alcohols and Aminodiols as Ligands for Diethylzinc Addition to Aldehydes

Syntheses of (1R,2S,3R,4S)-1,7,7-trimethyl-2-pyridin-2-ylmethylbicyclo[2.2.1]-heptane-2,3-diol (7), (1R,2S,3R,4S)-1,7,7-trimethyl-2-[(6-methyl)-pyridin-2-ylmethyl-bicyclo-[2.2.1]heptane-2,3-diol (13), and (1R,2S,2’R,4R)-1,7,7-trimethyl-2-piperidin-2-ylmethyl-bicyclo[2.2.1]heptan-2-ol (19b) from commercially available (d)-camphor (1) are described. Key steps of the syntheses involved substrate-controlled diastereoselective alkylation and platinum oxide-catalyzed hydrogenation reactions. These compounds, and other intermediate amino alcohols in their syntheses, were successfully utilized as ligands in enantioselective diethyl zinc (Et2Zn) addition to benzaldehyde with moderate enantioselectivity.

Prognostic Value of Lymphocyte Vascular Density and E-Cadherin in Inflammatory Breast Cancer

Background: We recently evaluated four laboratory assays, vascular endothelial growth factor D (VEGF-D), E-cadherin, lymphatic vessel density (LVD) measured by podoplanin, and intra-lymphatic tumor emboli (ILTE), which showed notable differences between inflammatory breast cancer (IBC) and non-inflammatory locally advanced breast cancer (LABC). In this study we investigated the potential of the three most quantitatively measured markers, E-cadherin, LVD and VEGF-D, to predict survival in the IBC patients. Materials and Methods: This study involved the 100 cases identified in the Inflammatory Breast Cancer Registry (IBCR) whose tumors were previously evaluated for the four assays noted above. Living patients were recontacted and survival data were available for up to 17 years. Overall survival (OS) was analyzed through the Kaplan-Meier method stratified by E-cadherin, LVD, VEGF-D, and response to chemotherapy. The differences in OS curves were compared using the log-rank test. Results: The median OS for patients with high LVD was 6.63 years (95% CI: 4.06 to 10.14), compared to median at 10 years not reached in those with low LVD (p = 0.03). There was a trend towards a longer median OS in patients with high E-cadherin (10.14, 95% CI: 6.63 to 11.67), compared with those with low E-cadherin (6.26, 95% CI: 3.42 to undeterminable). VEGF-D levels showed no correlation with survival. Conclusion: Low LVD significantly predicts better survival. High E-cadherin expression, as with non-IBC breast cancer and several other malignancies, tends to be associated with a better prognosis.
