Penetration testing(PT)is an active method of evaluating the security of a network by simulating various types of cyber attacks in order to identify and exploit vulnerabilities.Traditional PT involves a time-consuming and labor-intensive process that is prone to errors and cannot be easily formulated.Researchers have been investigating the potential of deep reinforcement learning(DRL)to develop automated PT(APT)tools.However,using DRL in APT is challenged by partial observability of the environment and the intractability problem of the huge action space.This paper introduces RLAPT,a novel DRL approach that directly overcomes these challenges and enables intelligent automation of the PT process with precise control.The proposed method exhibits superior efficiency,stability,and scalability in finding the optimal attacking policy on the simulated experiment scenario.

清代太医外派地方医事探析

外派地方为官员治病是清代太医的重要职责,在性质上属于特派差务,有京城太医派遣、随军太医调派、随扈太医驻留三种形式。从派医缘起角度,此类医事又可分为“请医(药)-赐医”“请求解任-赐医”和“汇报病情-赐医”三种类型。本文基于清宫档案,梳理出48件清太医院医官奉旨外派地方为官员治病的医事,其中“京城太医派遣”方式最为常见,呈现出固定流程。通过分析清宫档案,得出如下结论:御医是外派太医的主力成员,治疗对象主要是一、二品高级官员,患者病情大多得到控制,但痊愈率不高。皇帝以赐医为纽带维护君臣关系,有利于地方日常事务的运转和特殊政治任务的执行。同时也是皇帝对臣下施加控制的一种方式,起到强化“主仆性”君臣关系的效果。

总结1例行左侧乳腺癌改良根治术、右侧乳房重建术患者成功保留患肢PICC的护理经验。护理重点包括术前多学科评估保留PICC的利弊,制定和实施周密的综合护理方案;加强对症管理、康复锻炼,加强营养支持及心理康复;强化延续护理。结果患者术后住院18 d,切口愈合好,携带PICC出院。出院后继续相关治疗至治疗结束拔除PICC,共留置338 d(入院前留置128 d,住院期间留置85 d,出院后留置125 d),未发生PICC相关并发症。

Backgrounds:GALLIUM is a global phase Ⅲ study that demonstrated significant improvements in progression-free survival(PFS)for obinutuzumab plus chemotherapy(G-chemo)vs.rituximab plus chemotherapy(R-chemo)in previously untreated patients with follicular lymphoma(FL).This study aimed to report the results of a subgroup of patients in China.Methods:Patients were randomized to G-chemo or R-chemo.Responders received maintenance therapy for 2 years or until disease progression.The primary endpoint was investigator(INV)-assessed PFS.Secondary endpoints included the overall response rate(ORR)and complete response rate(CRR)at the end of induction chemotherapy,overall survival(OS),and safety.Results:Overall,58 patients with FL were randomized to the G-chemo(n=25)and R-chemo arms(n=33).The INV-assessed PFS rate at 3 years was 81.8%in the G-chemo arm,vs.70.2%in the R-chemo arm(hazard ratio 0.35;95%confidence interval:0.09-1.34;P=0.1120).The INV-assessed CRRs(without positron emission tomography[PET])in these arms were 24.0%and 21.2%,respectively,whereas the ORRs were 80.0%and 90.9%,respectively.INV-assessed CRR-PET was 52.6%in the G-chemo,vs.60.9%in the R-chemo.Median OS was not reached in either arm.Grade 3 to 5 adverse events were more frequent in the R-chemo arm(97.0%vs.88.0%).Conclusions:The results of this subgroup analysis were consistent with those of the global population,and they suggest that G-chemo has a positive benefit-risk profile in patients from China with FL.Trial registration:ClinicalTrials.gov,No.NCT01332968.

目的研究缺氧诱导因子1α(HIF-1α)与微小RNA-210-3p(miR-210-3p)在子宫内膜异位症(EMS)患者异位内膜组织中的表达。方法选取56例EMS手术患者,所有病例均经腹腔镜或开腹手术确诊,经病理证实为EMS,术中留取异位内膜组织为试验组,同期选取正常子宫内膜组织22例为对照组,应用免疫组织化学法(SP)测定2组内膜组织中HIF-1α的表达,并采用实时荧光定量法(qRT-PCR)检测2组标本中miR-210-3p相对表达水平;采用Spearman法分析异位内膜组织中HIF-1α与miR-210-3p表达水平相关性分析。结果异位内膜组织中miR-210-3p相对表达量显著高于正常内膜组织,差异有统计学意义(P<0.05);异位内膜组织中HIF-1α蛋白的阳性表达率高于正常内膜组织,差异有统计学意义(P=0.036);Spearman相关性分析结果显示异位内膜组织中HIF-1α与miR-210-3p表达水平呈正相关(r=0.573,P<0.001)。结论EMS患者异位内膜组织中HIF-1α与miR-210-3p表达上调,两者呈正相关,可能协同参与了EMS的发生发展。

Currently,there is no effective drugs for treating clinically COVID-19 except dexamethasone.We previously revealed that human identical sequences of SARS-CoV-2 promote the COVID-19 progression by upregulating hyaluronic acid(HA).As the inhibitor of HA synthesis,hymecromone is an approved prescription drug used for treating biliary spasm.Here,we aimed to investigate the relation between HA and COVID-19,and evaluate the therapeutic effects of hymecromone on COVID-19.Firstly,HA was closely relevant to clinical parameters,including lymphocytes(n=158;r=−0.50;P<0.0001),C-reactive protein(n=156;r=0.55;P<0.0001),D-dimer(n=154;r=0.38;P<0.0001),and fibrinogen(n=152;r=0.37;P<0.0001),as well as the mass(n=78;r=0.43;P<0.0001)and volume(n=78;r=0.41;P=0.0002)of ground-glass opacity,the mass(n=78;r=0.48;P<0.0001)and volume(n=78;r=0.47;P<0.0001)of consolidation in patient with low level of hyaluronan(HA<48.43 ng/mL).Furthermore,hyaluronan could directly cause mouse pulmonary lesions.Besides,hymecromone remarkably reduced HA via downregulating HAS2/HAS3 expression.Moreover,89%patients with hymecromone treatment had pulmonary lesion absorption while only 42%patients in control group had pulmonary lesion absorption(P<0.0001).In addition,lymphocytes recovered more quickly in hymecromone-treated patients(n=8)than control group(n=5)(P<0.05).These findings suggest that hymecromone is a promising drug for COVID-19 and deserves our further efforts to determine its effect in a larger cohort.

Nitrogen is widely used to prevent the spontaneous combustion of coal in underground coal mines. A spontaneous combustion-prone coal seam was studied to investigate the restraining effect of nitrogen on coal oxidation in different oxidation stages, based on non-isothermal thermogravimetry-differential scanning calorimetry(TG-DSC) and electron paramagnetic resonance(EPR) experiments. We found that the key feature temperatures grow steadily with increasing nitrogen in the oxidation environment,resulting in longer oxidation stages. The most significant finding is that there is a stagnation of the inhibitory effect of nitrogen on coal oxidation in the range of 85.0–95.0% nitrogen in the slow and the rapid oxidation stages, owing to the competitive adsorption of coal by nitrogen and oxygen. However, the restraining effect cannot be reflected by the kinetic parameters of the coal before it reaches the thermal decomposition and combustion stage. Nitrogen can also affect free radical types and free radical concentrations during coal oxidation: the higher the concentration of nitrogen in the oxidation environment, the greater the number of free radical types and the lower the free radical concentration. This experimental study improves the understanding of the restraining effect of nitrogen on coal oxidation in different oxidation stages and provides an important reference for coal fire prevention in spontaneous combustionprone coal seams.
