Proton pump inhibitor step-down therapy for GERD:A multi-center study in Japan

AIM:To investigate the predictors of success in stepdown of proton pump inhibitor and to assess the quality of life(QOL).METHODS:Patients who had heartburn twice a week or more were treated with 20 mg omeprazole(OPZ) once daily for 8 wk as an initial therapy(study 1).Patients whose heartburn decreased to once a week or less at the end of the initial therapy were enrolled in study 2 and treated with 10 mg OPZ as maintenance therapy for an additional 6 mo(study 2).QOL was in-vestigated using the gastrointestinal symptom rating scale(GSRS)before initial therapy,after both 4 and 8 wk of initial therapy,and at 1,2,3,and 6 mo after starting maintenance ***:In study 1,108 patients were *** characteristics were as follows;median age:63(range: 20-88)years,sex:46 women and 62 *** success rate of the initial therapy was 76%.In the patients with successful initial therapy,abdominal pain,indigestion and reflux GSRS scores were *** study 2,83 patients were *** of 83 patients completed the study 2 *** the per-protocol analysis,80%of 70 patients were successful for *** multivariate analysis of baseline demographic data and clinical information,no previous treatment for gastroesophageal reflux disease(GERD)[odds ratio (OR)0.255,95%CI:0.06-0.98]and a lower indigestion score in GSRS at the beginning of step-down therapy(OR 0.214,95%CI:0.06-0.73)were found to be the predictors of successful step-down *** improved GSRS scores by initial therapy were maintained through the step-down ***:OPZ was effective for most GERD ***,those who have had previous treatment for GERD and experience dyspepsia before stepdown require particular monitoring for relapse.

Can the biliary enhancement of Gd-EOB-DTPA predict the degree of liver function?

BACKGROUND: Excretion of gadolinium-ethoxybenzyl-diethyl- enetriamine pentaacetic acid (Gd-EOB-DTPA) in the bile may be related to liver function, because of elimination from the liver after preferential uptake by hepatocytes. The purpose of this study was to investigate the relation between liver and biliary enhancement in patients with or without liver dysfunction, and to compare the tumor-to-liver contrast in these patients. METHODS: Forty patients [group 1: normal liver and Child Pugh class A in 20 patients, group 2: Child-Pugh class B in 18 patients and Child-Pugh C in 2] were evaluated. All patients underwent MR imaging of the liver using a 1.5-Tesla system. T1 weighted 3D images were obtained at 5, 10, 15 and 20 minutes after Gd-EOB-DTPA injection. The relation between group 3 (total bilirubin <1.8 mg/dL) and group 4 (total bilirubin ≥1.8 mg/dL) was investigated at 20 minutes. Liver and biliary signals were measured, and compared between groups 1 and 2 or groups 3 and 4. Tumor-to-liver ratio was also evaluated between groups 1 and 2. Scheffé’s post-hoc test after two-way repeated measures ANOVA and Pearson’s correlation test were used for statistical analysis. RESULTS: Liver enhancement showed significant difference at all time points between groups 1 and 2. Biliary enhancement did not show a significant difference between groups 1 and 2 at 5 minutes, but did at 10, 15 and 20 minutes. At 20 minutes significant differences between groups 3 and 4 were seen for liver and biliary enhancement. At all time points, liver enhancement correlated with biliary enhancement in both groups. At 5 minutes and 20 minutes, statistical differences between groups 1 and 2 were seen for tumor-to-liver ***: The degree of biliary enhancement has a close correlation to that of liver enhancement. It is especially important that insufficient liver enhancement causes lower tumor-to-liver contrast in the hepatobiliary phase of Gd-EOB- DTPA.

Circulating tumor DNA dynamics analysis in a xenograft mouse model with esophageal squamous cell carcinoma

This paper treats the numerical calculation of a steady two-dimensional viscous flow past a flexible membrane. Both edges of the membrane are fixed in the flow and its chord is set normal to the flow. The numerical calculations of the Navier-Stokes equation based on a finite difference method and a relaxation method are carried out for several values of the membrane tension in cases when the Reynolds numbers are 5, 10 and 20. It is found that the membrane tension has a minimum value below which there is no steady shape of the membrane. Two different shapes of the membrane are possible at a given value of tension: the one is of small deformation, while the other a large deformation. There appear twin vortices in the concave region of the membrane shape if its deformation increases to a certain extent.

Acquired drug resistance is the major reason why patients fail to respond to cancer *** is a challenging task to *** the tipping point of endocrine resistance and detect the associated *** from new systems biology theory, the dynamic network biomarker (DNB) method is designed to quantitatively identify the tipping point of a drastic system transition and can theoretically identify DNB genes that play key roles in acquiring drug *** analyzed time-course mRNA sequence data generated from the tamoxifen-treated estrogen receptor (ER)-positive MCF-7 cell line, and identified the tipping point of endocrine resistance with its leading *** results show that there is interplay between gene mutations and DNB genes, in which the accumulated mutations eventually affect the DNB genes that subsequently cause the change of transcriptional landscape, enabling full-blown drug resistance. Survival analyses based on clinical datasets validated that the DNB genes were associated with the poor survival of breast cancer *** results provided the detection for the pre-resistance state or early signs of endocrine *** predictive method may greatly benefit the scheduling of treatments for complex diseases in which patients are exposed to considerably different drugs and may become drug resistant.

Effect of Microwave Irradiation on Oximation of Acetylferrocene

Microwave-assisted reactions are an environmentally friendly approach for synthesizing organic compounds. In this study, oximation of acetylferrocene and acetophenone was conducted under both microwave irradiation and conventional heating conditions. Acetylferrocene and acetophenone were subjected to oximation under the two conditions in various solvent mixtures, and the extent of conversion was determined by 1H nuclear magnetic resonance spectroscopy. Microwave irradiation was found to accelerate the rate of oximation of both acetylferrocene and acetophenone. Acceleration of the reaction under microwave irradiation was attributed to the efficient absorption of microwaves by the ferrocene nucleus.

Design and experimental studies on a hybrid excitation flux switching motor as a traction motor for hybrid electric vehicles drive are presented.A stator body of the motor consists of not only laminated silicon-iron electromagnetic steel and three-phase armature windings,but also both of field excitation coils and permanent magnets working together as a variable field magnetomotive force *** the other hand,a rotor is composed of just laminated silicon-iron electromagnetic steel with salient poles like switched reluctance *** bring out the best in drive performances of the hybrid excitation flux switching motor as a variable flux motor for the application,each material adopted for the stator and rotor body should be designed properly in terms of motor efficiency,maximum torque and power densities and so *** some of them,in this paper,thinner silicon-iron electromagnetic steel sheet and permanent magnets with high remanent and low amount of Dysprosium used are applied for achieving higher motor ***,all coils wound flatwise and edgewise using rectangular wires are introduced to realizing high filling factor for reduced copper *** tests using a 60kW prototype of the motor demonstrates the designed motor has good motor efficiency under frequent operating points expected for the target vehicle drive.

In-vitro proliferation assay with recycled ascitic cancer cells in malignant pleural mesothelioma: A case report

BACKGROUND We report the first case,to the best of our knowledge,of massive ascites due to recurrent malignant pleural mesothelioma that was controlled using KM-cell-free and concentrated ascites reinfusion therapy(KM-CART).The tumor cells derived via KM-CART were utilized secondarily in an in vitro cell growth assay using the collagen gel droplet-embedded culture drug sensitivity test(CD-DST)to investigate anticancer drug *** SUMMARY A 56-year-old man presented with recurrent malignant mesothelioma with massive ascites;more than 4000 mL of ascitic fluid was removed,filtered,and concentrated using KM-CART,and the cell-free ascitic fluid was reinfused into the patient to improve quality of *** cells isolated secondarily in an in vitro proliferation assay using CD-DST exhibited low sensitivity to pemetrexed and high sensitivity to *** with gemcitabine maintained stable disease for 4 *** The combination of KM-CART and CD-DST may be a promising treatment option for malignant ascites associated with malignant mesothelioma.

Pseudolymphoma of the liver associated with primary biliary cirrhosis:A case report and review of literature

We report a case of two pseudolymphomas of the liver in a 63-year-old Japanese woman with primary biliary *** of the lesions was found incidentally during a medical examination,presenting as a 10 mm hypodense nodule that revealed hyperdensity in the early phase and hypodensity in the late phase in computed tomography(CT)after injection of contrast ***,the 10 mm nodule had first been discovered as a 4 mm nodule during CT 4 years *** iron oxide-enhanced MRI revealed another 4 mm hyperintense nodule in segment 6 in addition to the 10 mm hyperintense nodule in segment *** during arterial portography revealed two hypointense *** with other imaging modalities such as ultrasonography,magnetic resonance imaging,and hepatic angiography were consistent with hepatocellular carcinoma.A right posterior segmentectomy was performed,and the lesions were microscopically diagnosed as *** the best of our knowledge,only 31 other cases of this disease have ever been reported,with a highly asymmetrical male:female ratio of 1:*** we could find only one case of transformation of hepaticpseudolymphoma into lymphoma in the liver,the exact nature of development from benign pseudolymphoma to malignant lymphoma is still not fully understood and cases of hepatic lymphoma need to be followed carefully.
