We report the case of a patient who developed pyogenic spondylitis after surgery for oral floor cancer. This 85-year-old man was evaluated at our hospital for a mass in the left floor of the mouth. Oral floor cancer (T2N0M0) was diagnosed. Surgical resection of the tumor was performed twice. On postoperative day 14, he developed sudden neck pain, followed by fever the next day and a gradual decrease in blood pressure. Septic shock was diagnosed, and echocardiography was performed, but no evidence of infective endocarditis was found. Findings on neck magnetic resonance imaging suggested pyogenic spondylitis, and ultimately pyogenic spondylitis causing septic shock was diagnosed. In patients with a compromised immune system after surgery who develop neck pain and fever, the possibility of pyogenic spondylitis as a complication should be kept in mind.

＜正＞ The present paper deals with the pandaloid shrimp material collected from the northern part of the South China Sea during the Chinese government’s "National Comprehensive Oceanography Survey" program in 1958-1960. The collection includes two species of Thalassocarididae and 18 species of Pandalidae. A new species, Plesionika longidactylus, is described and compared with P. exigua, P. mexicana, P. pumila and P. izumiae. Four species are recorded from Chinese waters for the first time: Plesionika kensleyi Chace, 1985, P. philippinensis Chace, 1985, P. pumila Chace, 1985 and P. spinensis Chace, 1985.

Extracellular adenosine induces apoptosis in a variety of cancer cells via intrinsic and extrinsic pathways. In the former pathway, adenosine uptake into cells triggers apoptosis, and in the latter pathway, adenosine receptors mediate apoptosis. Extracellular adenosine also inducesapoptosis of gastric cancer cells. Extracellular adenosine is transported into cells through an adenosine transporter and converted to AMP by adenosine kinase. In turn, AMP activates AMP-activated protein kinase(AMPK). AMPK is the factor responsible for caspase-independent apoptosis of GT3-TKB gastric cancer cells. Extracellular adenosine, on the other hand, induces caspase-dependent apoptosis of MKN28 and MKN45 gastric cancer cells by two mechanisms. Firstly, AMP, converted from intracellularly transported adenosine, initiates apoptosis, regardless of AMPK. Secondly, the A3 adenosine receptor, linked to Gi/Gq proteins, mediates apoptosis by activating the Gq protein effector, phospholipase Cγ, to produce inositol 1,4,5-trisphosphate and diacylglycerol, which activate protein kinase C. Consequently, the mechanisms underlying adenosine-induced apoptosis vary, depending upon gastric cancer cell types. Understand the contribution of each downstream target molecule of adenosine to apoptosis induction may aid the establishment of tailor-made chemotherapy for gastric cancer.

We present a case of Cronkhite-Canada syndrome(CCS) in which the entire intestine was observed using a prototype of magnifying single-balloon enteroscope(SIF Y-0007, Olympus). CCS is a rare, non-familial gastrointestinal polyposis with ectodermal abnormalities. To our knowledge, this is the first report showing magnified intestinal lesions of CCS. A 73-year-old female visited our hospital with complaints of diarrhea and dysgeusia. The blood test showed mild anemia and hypoalbuminemia. The esophagogastroduodenoscopy and colonoscopy revealed diffuse and reddened sessile to semi-pedunculated polyps, resulting in the diagnosis of CCS. In addition to the findings of conventional balloon-assisted enteroscopy or capsule endoscopy, magnifying observation revealed tiny granular structures, non-uniformity of the villus, irregular caliber of the loop-like capillaries, scattered white spots in the villous tip, and patchy redness of the villus. Histologically, the scattered white spots and patchy redness of the villus reflect lymphangiectasia and bleeding to interstitium, respectively.

AIM To investigate the post-colonoscopy colorectal cancer(PCCRC) rate for high-definition(HD) colonoscopy compared with that for standard-definition colonoscopy reported *** Using medical records at Sano Hospital(SH) and Dokkyo Medical University Koshigaya Hospital(DMUKH), we retrospectively obtained data on consecutive patients diagnosed as having CRC between January 2010 andDecember 2015. The definition of PCCRC was diagnosis of CRC between 7 and 36 mo after initial high-definition colonoscopy that had detected no cancer, and patients were divided into a PCCRC group and a non-PCCRC group. The primary outcome was the rate of PCCRC for HD colonoscopy. The secondary outcomes were factors associated with PCCRC and possible reason for occurrence of early and advanced *** Among 892 CRC patients, 11 were diagnosed as having PCCRC and 881 had non-PCCRC. The PCCRC rate was 1.7%(8/471) at SH and 0.7%(3/421) at DMUKH. In comparison with the non-PCCRC group, the PCCRC group had a significantly higher preponderance of smaller tumors(39 mm vs 19 mm, P = 0.002), a shallower invasion depth(T1 rate, 25.4% vs 63.6%, P = 0.01), a non-polypoid macroscopic appearance(39.0% vs 85.7%, P = 0.02) and an earlier stage(59.7% vs 90.9%, P = 0.03). Possible reasons for PCCRC were "missed or new" in 9 patients(82%), "incomplete resection" in 1(9%), and "inadequate examination'" in 1(9%). Among 9 "missed or new" PCCRC, the leading cause was non-polypoid shape for early PCCRC and blinded location for advanced *** The PCCRC rate for HD colonoscopy was 0.7%-1.7%, being lower than that for standard-definition colonoscopy(1.8%-9.0%) reported previously employing the same methodology.

Safety and efficacy of self-expandable metallic stents in malignant small bowel obstructions

In this report, we present 3 cases of malignant small bowel obstruction, treated with palliative care using endoscopic self-expandable metallic stent(SEMS) placement, with the aim to identify the safety and efficacy of this procedure. Baseline patient characteristics, procedure methods, procedure time, technical and clinical success rates, complications, and patient outcomes were obtained. All 3 patients had pancreatic cancer with small bowel strictures. One patient received the SEMS using colonoscopy, while the other 2 patients received SEMS placement via double balloon endoscopy using the through-the-overtube technique. The median procedure time was 104 min. The technical and clinical success rates were 100%. Post-treatment, obstructive symptoms in all patients improved, and a low-residue diet could be tolerated. All stents remained within the patients until their deaths. The median overall survival time(stent patency time) was 76 d. SEMS placement is safe and effective as a palliative treatment for malignant small bowel obstruction.

目的:观察完整保留C2棘突半棘肌附着的颈椎管扩大椎板成形术的手术疗效。方法:A组27例行改良颈椎板成形术,即显露时保留半棘肌在C2棘突上的附着,切除C3椎板,C4～C7双开门术式。B组33例行常规颈椎板成形术,即分离切断C2棘突半棘肌附着,关闭时缝合重建,C3～C7双开门成形术式。比较两组手术时间、出血量、术后神经功能、并发症、颈椎曲度和屈伸活动度(ROM)、病人自我评价。JOA计分法评价神经功能、Cobb’s法测量颈椎曲度和ROM。结果:所有病例随访至少1年,A、B组术后JOA评分平均改善率分别为63.01%、60.83%(P>0.05)。手术时间A、B组分别123±17.4min、175±12.7min,出血量分别380±18.6ml、564±21.5ml;A组较B组手术时间短、出血量少(P<0.05)。术后1年A、B组ROM分别保留术前的81.6%和65.3%(P<0.01);A组无后凸畸形、手术并发症少、局部症状轻微。结论:改良颈椎管扩大椎板成形术脊髓减压满意,手术时间短、出血量少,无严重并发症;而且有助于早期颈椎功能锻炼、维持良好颈椎曲度和ROM。

目的:比较限制性(Constrained)和动力性(Dynamic)颈椎前路钢板融合率、对融合节段及全颈椎矢状面曲度影响。方法:回顾分析97例(C组59例;D组38例)颈椎前路钢板内固定术后的影像学资料,Cobb's法测量颈椎侧位X线片上融合节段和C3～C7矢状位曲度、融合节段前柱高度,比较其术后即刻及随访末期参数的变化。连续骨小梁通过融合区、X线动态侧位片显示融合节段棘突顶点间距离无变化判定为融合。结果:平均随访16.9个月,C、D组融合率分别为91.6%、97.3%。C组融合节段前凸角丢失单节段为1.17°(0～2.9°)、双节段为1.83°(0～3.5°),D组单节段2.57°(0.5°～4.9°)、双节段3.59°(1°～6.1°),两组有显著性差别(P0.05)。结论:动力性颈椎前路钢板可获得较高的融合率,与限制性钢板相比,融合节段前凸角丢失和前柱高度丢失虽然较大,但不影响整个颈椎的前凸角度。

The near wake of a circular cylinder in linearly stratified flows of finite depth was experimentally investigated by means of flow visualization and measurements of vortex shedding frequencies, at Reynolds numbers 3.5 × 103-1.2 × 104 and stratification parameters kd 0-2.0. The non-dimensional parameter kd is defined as kd = Nd/U, where N is the Brunt-Vaisala frequency, d, the diameter of the cylinder, and U, the approaching flow velocity. The study demonstrates that as kd increases from zero, the vortex shedding from a circular cylinder progressively strengthens, while the Strouhal number gradually becomes lower than that for homogeneous flow. This phenomenon can be explained by the effect of the increasingly stable stratification which enhances the two-dimensionality of the near-wake flow of the circular cylinder;the enhanced two-dimensionality of the flow strengthens the roll-up of the separated shear layer. Above a certain value of kd, however, vortex formation and shedding are strongly suppressed and the Strouhal number rises sharply. This observation is attributable to the development of stationary lee waves downstream of the circular cylinder because the lee waves strongly suppress vertical fluid motions.
