Extent of lymphadenectomy has no impact on postoperative complications after gastric cancer surgery in Sweden

Objective： Curative gastric cancer surgery entails removal of the primary tumor with adequate margins including regional lymph nodes. European randomized controlled trials with recruitment in the 1990＇s reported increased morbidity and mortality for D2 compared to D 1. Here, we examined the extent of lymphadenectomy during gastric cancer surgery and the associated risk for postoperative complications and mortality using the strengths of a population-based study. Methods： A prospective nationwide study conducted within the National Register of Esophageal and Gastric Cancer. All patients in Sweden from 2006 to 2013 who underwent gastric cancer resections with curative intent were included. Patients were categorized into DO, D I, or D I ＋/D2, and analyzed regarding postoperative morbidity and mortality using multivariable logistic regression. Results： In total, 349 （31.7%） patients had a DO, 494 （44.9%） DI, and 258 （23.4%） DI＋/D2 lymphadenectomy. The 30-d postoperative complication rates were 25.5%, 25.1% and 32.2 % （DO, D I and D1 ＋/D2, respectively）, and 90-d mortality rates were 8.3%, 4.3 % and 5.8%. After adjustment for confounders, in mukivariable analysis, there were no significant differences in risk for postoperative complications between the lymphadenectomy groups. For 90-d mortality, there was a lower risk for D1 vs. DO. Conclusions： The majority of gastric cancer resections in Sweden have included only a limited lymphadenectomy （DO and DI）. More extensive lymphadenectomy （DI＋/D2） seemed to have no impact on postoperative morbidity or mortality.

On the Convergence of Observed Partial Likelihood under Incomplete Data with Two Class Possibilities

In this paper, we discuss the theoretical validity of the observed partial likelihood (OPL) constructed in a Coxtype model under incomplete data with two class possibilities, such as missing binary covariates, a cure-mixture model or doubly censored data. A main result is establishing the asymptotic convergence of the OPL. To reach this result, as it is difficult to apply some standard tools in the survival analysis, we develop tools for weak convergence based on partial-sum processes. The result of the asymptotic convergence shown here indicates that a suitable order of the number of Monte Carlo trials is less than the square of the sample size. In addition, using numerical examples, we investigate how the asymptotic properties discussed here behave in a finite sample.

Endoscopic diagnosis of Barrett's esophagus

The Prague C and M Criteria have been developed for the objective endoscopic diagnosis of Barrett's esophagus(BE).BE arises between the squamocolumnar junction and the gastroesophageal junction at the proximal margin of the gastric folds.In this study,we reported that 43.0% of the subjects examined were diagnosed with BE based on the Prague C and M Criteria.Previous criticism by John Dent proposed that our data should be considered invalid because the prevalence of BE reported in our study was extraordinarily high and discordant with previous studies.Dent predicted that the position of the gastroesophageal junction in our study was judged to be lower than the actual position due to the effacement of the proximal ends of the gastric folds because of the routine use of a high degree of air distension during typical Japanese endoscopic examinations.The endoscopic evaluation of the superior gastric folds is certainly influenced by the degree of air distension of the esophagus.However,in our study,the proximal limit of the gastric mucosal folds was prospectively imaged while the oesophagus was minimally insufflated.Then,under a high level of air distension,the distal ends of the palisade-shaped longitudinal vessels were imaged because they are more easily observed when distended.In the majority of patients,the distal ends of the palisade-shaped longitudinal vessels correspond to the proximal limit of the gastric mucosal folds.Our endoscopic evaluation was appropriately performed according to the Prague C and M Criteria.We suspect that the high prevalence of BE in our study may be due to the inclusion of ultrashort-segment BE,which defines BE with an affected mucosal length under 5 mm,in our positive results.

Post-colonoscopy colorectal cancer rate in the era of highdefinition colonoscopy

AIM To investigate the post-colonoscopy colorectal cancer(PCCRC) rate for high-definition(HD) colonoscopy compared with that for standard-definition colonoscopy reported previously.METHODS Using medical records at Sano Hospital(SH) and Dokkyo Medical University Koshigaya Hospital(DMUKH), we retrospectively obtained data on consecutive patients diagnosed as having CRC between January 2010 andDecember 2015. The definition of PCCRC was diagnosis of CRC between 7 and 36 mo after initial high-definition colonoscopy that had detected no cancer, and patients were divided into a PCCRC group and a non-PCCRC group. The primary outcome was the rate of PCCRC for HD colonoscopy. The secondary outcomes were factors associated with PCCRC and possible reason for occurrence of early and advanced PCCRC.RESULTS Among 892 CRC patients, 11 were diagnosed as having PCCRC and 881 had non-PCCRC. The PCCRC rate was 1.7%(8/471) at SH and 0.7%(3/421) at DMUKH. In comparison with the non-PCCRC group, the PCCRC group had a significantly higher preponderance of smaller tumors(39 mm vs 19 mm, P = 0.002), a shallower invasion depth(T1 rate, 25.4% vs 63.6%, P = 0.01), a non-polypoid macroscopic appearance(39.0% vs 85.7%, P = 0.02) and an earlier stage(59.7% vs 90.9%, P = 0.03). Possible reasons for PCCRC were "missed or new" in 9 patients(82%), "incomplete resection" in 1(9%), and "inadequate examination'" in 1(9%). Among 9 "missed or new" PCCRC, the leading cause was non-polypoid shape for early PCCRC and blinded location for advanced PCCRC.CONCLUSION The PCCRC rate for HD colonoscopy was 0.7%-1.7%, being lower than that for standard-definition colonoscopy(1.8%-9.0%) reported previously employing the same methodology.

Safety and efficacy of self-expandable metallic stents in malignant small bowel obstructions

In this report, we present 3 cases of malignant small bowel obstruction, treated with palliative care using endoscopic self-expandable metallic stent(SEMS) placement, with the aim to identify the safety and efficacy of this procedure. Baseline patient characteristics, procedure methods, procedure time, technical and clinical success rates, complications, and patient outcomes were obtained. All 3 patients had pancreatic cancer with small bowel strictures. One patient received the SEMS using colonoscopy, while the other 2 patients received SEMS placement via double balloon endoscopy using the through-the-overtube technique. The median procedure time was 104 min. The technical and clinical success rates were 100%. Post-treatment, obstructive symptoms in all patients improved, and a low-residue diet could be tolerated. All stents remained within the patients until their deaths. The median overall survival time(stent patency time) was 76 d. SEMS placement is safe and effective as a palliative treatment for malignant small bowel obstruction.

Incidence of reflux esophagitis and Helicobacter pylori infection in diabetic patients

AIM: To investigate the incidence of reflux esophagitis (RE) and H pylori infection in the diabetic patient. METHODS: The incidence of RE and H pylori infection were investigated in 85 patients with diabetes mellitus and the results were compared with controls. RESULTS: The incidence of RE in diabetic patients was 17.6%. Although this tended to be higher in diabetic patients, there were no statistically significant differences between diabetic patients and controls. The incidence of H pylori infection in diabetic patients was 53.7% but no statistically significant difference was seen between diabetic patients and controls in the incidence of H pylori infection. CONCLUSION: No significant differences could be seen between diabetic patients and controls in the incidence of RE and H pylori infection.

This article reports a first case of calcinosis circumscripta in a captive African cheetah(Acinonyx jubatus).Histopathology demonstrated well defined multiple cystic structures containing granular,dark basophilic materials with peripheral granulomatous reaction,characterized by presence of multinucleated giant cells surrounded by a varying amounts of fibrous connective tissues.Special staining with von Kossa revealed black stained deposits confirming the presence of calcium salts.

Discontinuous chains of ultramafic rock bodies form part of the 3800–3700 Ma Isua Supracrustal Belt(ISB),hosted in the Itsaq Gneiss Complex of southwestern Greenland.These bodies are among the world’s oldest outcrops of ultramafic rocks and hence an invaluable geologic record.Ultramafic rocks from Lens B in the northwestern limb of ISB show characteristics of several stages of serpentinization and deserpentinization forming prograde and retrograde mineral assemblages.Ti-rich humite-group minerals such as titanian chondrodite(Ti-Chn)and titanian clinohumite(Ti-Chu)often occur as accessory phases in the metamorphosed ultramafic rocks.The Ti-rich humite minerals are associated with metamorphic olivine.The host olivine is highly forsteritic(Fo96-98)with variable Mn O and Ni O contents.The concentrations of the rare-earth elements(REE)and high-field strength elements(HFSE)of the metamorphic olivine are higher than typical mantle olivine.The textural and chemical characteristics of the olivine indicate metamorphic origin as a result of deserpentinization of a serpentinized ultramafic protolith rather than primary assemblage reflecting mantle residues from high-degrees of partial melting.The close association of olivine,antigorite and intergrown Ti-Chn and Ti-Chu suggests pressure condition between$1.3–2.6 GPa within the antigorite stability field(<700°C).The overall petrological and geochemical features of Lens B ultramafic body within the Eoarchean ISB indicate that these are allochthonous ultramafic rocks that recorded serpentine dehydration at relatively lower temperature and reached eclogite facies condition during their complex metamorphic history similar to exhumed UHP ultramafic rocks in modern subduction zone channels.

[Ni3O(TBPLA)(H2O)](ClO4)4(H2O)6 [TBPLA=(S)-1,1',1''-2,4,6-trimethylbenzene-1,3,5-triyl)-tris(methylene)-tris-(pyrrolidine-2-carboxylate)] was found to possibly display multiferroic property (the coexistence of ferroelectricity and ferromagnetism). The magnetic property of this compound was studied and no magnetic hysteresis loop was found probably due to an intrinsic fluctuation for the magnetic exchange energy.
