A pancreatic tumor was suspected on the abdominal ultrasound of a 72-year-old man. Abdominal computed tomography showed pancreatic enlargement as well as a diffuse, poorly enhanced area in the pancreas; endoscopic ultrasound-guided fine needle aspiration biopsy and endoscopic retrograde cholangiopancreatography failed to provide a definitive diagnosis. Based on the trend of improvement of the pancreatic enlargement, the treatment plan involved follow-up examinations. Later, he was hospitalized with an alveolar hemorrhage and rapidly progressive glomerulonephritis; he tested positive for myeloperoxidase-anti-neutrophil cytoplasmic antibody(ANCA) and was diagnosed with ANCArelated vasculitis, specifically microscopic polyangiitis. It appears that factors such as thrombus formation caused by the vasculitis in the early stages of ANCArelated vasculitis cause abnormal distribution of the pancreatic blood flow, resulting in non-uniform pancreatitis. Pancreatic lesions in ANCA-related vasculitis are very rare. Only a few cases have been reported previously. Therefore, we report our case and a review of the literature.

Direct numerical simulation(DNS)of turbulent planar jet with a second-order chemical reaction(A+B→R)is performed to investigate the processes of mixing and chemical reactions in spatially developing turbulent free shear *** A is premixed into the jet flow,and reactant B is premixed into the ambient *** is performed at three different Damk¨ohler numbers(Da=0.1,1,and 10).Damk¨ohler number is a ratio of a time scale of a flow to that of chemical reactions,and in this study,the large Da means a fast chemical reaction,and the small Da means a slow chemical *** visualization of velocity field shows that the jet flow is developed by entraining the ambient *** visualization of concentration of reactant A shows that concentration of reactant A for Da=1 and 10 becomes very small in the downstream region because the chemical reaction consumes the reactants and reactant A is diffused with the jet *** comparison of the profiles of chemical reaction rate and concentration of product R,it is found that product R for Da=10 is produced by the chemical reaction at the interface between the jet and the ambient fluids and is diffused into the jet flow,whereas product R for Da=0.1 is produced in the jet flow after reactants A and B are well mixed.

AIM To determine the usefulness of assigning narrow-band imaging(NBI) scores for predicting tumor grade and invasion depth in colorectal *** A total of 161 colorectal lesions were analyzed from138 patients who underwent endoscopic or surgical resection after conventional colonoscopy and magnifying endoscopy with NBI. The relationships between the surface and vascular patterns of the lesions, as visualized with NBI, and the tumor grade and depth of submucosa(SM) invasion were determined *** were assigned to distinct features of the surface microstructures of tubular and papillary-type *** a multivariate analysis, a model was developed for predicting the tumor grade and depth of invasion based on NBI-finding *** NBI findings that correlated with a high tumor grade were associated with the "regular/irregular"(P < 0.0001)surface patterns and the "avascular area" pattern(P =0.0600). The vascular patterns of "disrupted vessels"(P = 0.0714) and "thick vessels"(P = 0.0133) but none of the surface patterns were associated with a depth of invasion of ≥ 1000 μm. In our model, a total NBIfinding score ≥ 1 was indicative of a high tumor grade(sensitivity: 0.97; specificity: 0.24), and a total NBIfinding score ≥ 9(sensitivity: 0.56; specificity: 1.0) was predictive of a SM invasion depth ≥ 1000 μm. Scores less than these cutoff values signified adenomas and a SM invasion depth < 1000 μm, respectively. We have inquired into a means to decrease the number of charged particles escaping from a loss cone of a magnetic mirror reactor as many as possible. We previously reported the way of installing a supplemental magnetic mirror (which has a cyclotron heating space within) at the exit of a main magnetic bottle. The cyclotron heating space was set to increase a reflection-efficiency of the supplemental mirror. We could not suppress the loss of the escaping deuteron ions sufficiently even with a very long mirror and a very powerful electric field. Then, in this work we propose a new plan of installing another supplemental mirror besides the previous supplemental mirror. A new mirror is set perpendicularly to the center axis of the main bottle. By the addition of the perpendicular mirror, an efficiency of sending back of escaping deuteron ions is considerably theoretically improved. Also in the previous work, since we did not touch how to supply a high-frequency electric field to the cyclotron heating space, here we consider supplying it by an extraordinary-wave with a cyclotron frequency. It is mentioned that propagation of an extraordinary-wave with an electron cyclotron frequency depends on a magnetic field strength and density of escaping electrons.

Barros discusses that [Jose Acacio de Barros, Int. J. Theor. Phys. 50, 1828 (2011)] nagata derives inconsistencies from quantum mechanics [K. nagata, Int. J. Theor. Phys. 48, 3532 (2009)]. Barros considers that the inconsistencies do not come from quantum mechanics, but from extra assumptions about the reality of observables. Here we discuss the fact that there is a contradiction within the quantum theory. We discuss the fact that only one expected value in a spin-1/2 pure state 〈σx〉rules out the reality of the observable. We do not accept extra assumptions about the reality of observables. We use the actually measured results of quantum measurements (raw data). We use a single Pauli observable. We stress that we can use the quantum theory even if we give up the axiomatic system for the quantum theory.

Different traction devices that can provide a visual field and attain appropriate tension at the dissection plane during endoscopic submucosal dissection(ESD)have been ***-with-line(CWL)is a classic traction device that can offer per-oral traction toward the direction where the line is drawn.A multicenter randomized controlled trial(CONNECT-E trial)comparing the conventional ESD and CWL-assisted ESD(CWL-ESD)for large esophageal tumors was conducted in *** study showed that CWL-ESD was associated with a shorter procedure time(defined as the time from initiating submucosal injection to completing tumor removal)without increasing the risk of adverse *** analysis revealed that whole-circumferential lesion and abdominal esophageal lesion were independent risk factors for technical difficulties,which were defined as a procedure time of>120 min,perforation,piecemeal resection,inadvertent incision(any accidental incision caused by the electrosurgical knife within the marked area),or handover to another ***,techniques other than CWL should be considered for these *** studies have shown the usefulness of endoscopic submucosal tunnel dissection(ESTD)for such lesions.A randomized controlled trial conducted at five Chinese institutions showed that compared with the conventional ESD,ESTD had a significantly reduced median procedure time for lesions covering≥1/2 of the esophageal *** addition,a propensity score matching analysis conducted at a single Chinese institution showed that compared with the conventional ESD,ESTD had a shorter mean resection time for lesions at the esophagogastric *** the appropriate use of CWL-ESD and ESTD,esophageal ESD can be performed more efficiently and ***,the combination of these two methods may be effective.

Studies on mitochondrial macromolecular syntheses in various organs of aging animals labeled with 3H-precursors as revealed by electron microscopic radioautography

In order to study the aging changes of intramitochondrial macromolecular synthesis in various organs of aging animals during the development and aging, 10 groups of developing and aging mice, each consisting of 3 individuals, total 30, from fetal day 19 to postnatal newborn at day 1, 3, 9, 14 and adult at month 1, 2, 6, 12 to 24 were injected with 3H-thymidine a DNA precurson, another 10 groups consisting of 3 individuals, total 30, were injected with 3H-uridine a RNA precursor, and another 10 groups of 30 individuals were injected with 3H-leucine a protein precursor, total 90 individuals. Then, all the animals were sacrificed 1 hr after the injections and the liver tissues, the lung tissues, the kidney tissues, the testis and ovary tissues, the adrenal tissues were taken out, fixed and processed for electron microscopic radioautography. On electron microscopic radioautograms obtained from each animal, ten photographs in respective groups, numbers of mitochondria per cell profile area, numbers of labeled mitochondria per cell and the mitochondrial labeling index (LI) labeled with 3H-thymidine showing DNA synthesis, LI labeled with 3H-uridine showing RNA synthesis, and LI labeled with 3H-leucine showing protein synthesis, in various organs, were counted and the results in various organs in respective aging groups were compared, respectively. From the results, it was demonstrated that the numbers of mitochondria in hepatocytes as well as in 3 zones of the adrenal cortex, the zona glomerulosa, fasciculata and reticularis of mice at various ages increased from fetal day 19 to postnatal month 1 due to development and aging of animals. On the other hand, the numbers of labeled mitochondria and the labeling indices of intramitochondrial DNA syntheses incorporating 3H-thymidine in hepatocytes and 3 zones of cortical cells increased from fetal day 19 to postnatal month 1 and decreased to month 24. Electron microscopic radioautographic study on mitochondrial RNA synthesis in adrenal cortical and medullary cells of aging mice

In order to study the aging changes of intramito-chondrial RNA synthesis of mouse adrenal cells, 10 groups of developing and aging mice, each consisting of 3 individuals, total 30, from fetal day 19 to postna-tal newborn at day 1, 3, 9, 14, adult at month 1, 2, 6 and senescent animals at month 12 (year 1) and 24 (year 2) were injected with 3H-uridine, an RNA pre-cursor, sacrificed 1 hr later and the adrenal tissues were fixed and processed for electron microscopic radioautography. On electron microscopic radio-autograms obtained from each animal, the number of mitochondria per cell, the number of labeled mito-chondria with 3H-uridine showing RNA synthesis per cell and the mitochondrial labeling index in each adreno-cortical cells, in 3 zones, as well as in each adreno-medullary cells, 2 types of cells in the medulla, the adrenalin cells and the noradrenalin cells, were calculated and the results in respective aging groups were compared with each others. The results demon-strated that the number of mitochondria in adreno-cortical cells in 3 zones, the zona glomerulosa, fasciculata and reticularis of respective mice at vari-ous ages increased from fetal day 19 to postnatal month 1 reaching the plateau from month 1 to 24 due to development and aging of animals, respectively, while the number of labeled mitochondria per cell and the labeling index of intramitochondrial RNA synthesis incorporating 3H-uridine increased from fetal day 19 to postnatal month 2, reaching the maxima and decreased slightly from month 6 to month 24. On the other hand, the number of mito-chondria per cell in the medulla increased from fetal day 19 to postnatal month 1 reaching the plateau from month 1 to 24, while the number of labeled mi-tochondria per cell and the labeling index of in-tramitochondrial RNA synthesis increased from fetal day 19 to postnatal day 14, reaching the maxima and decreased from month 1 to 24. In this paper, we consider the finite sample property of the ordinary least squares (OLS) estimator for an AR(1) model with measurement error. We present the Edgeworth approximation for a finite distribution of OLS up to O(T1/2). We introduce an instrumental variable estimator that is consistent in the presence of measurement error. Finally, a simulation study is conducted to assess the theoretical results and to compare the finite sample performances of these estimators.
