A Case of Transient Advanced Atrioventricular Block after Aortic Valve Replacement, Report of a Case

Approximately 3%-11.8% of cases require permanent pacemaker implantation due to atrioventricular block (AVB) after aortic valve replacement (AVR), and determination of conduction disturbances such as left or right bundle branch block by preoperative electrocardiography is correlated with high risk postoperative permanent pacemaker implantation. Intraoperative risk factors include severe calcification of the aortic valve, prolonged cardiopulmonary bypass time, aortic clamp time. Recently, there have been reports of high rates of pacemaker implantation (14.2%) after transcatheter aortic valve implantation. The time of permanent pacemaker implantation after AVB is often 4-10 days, and the European Society of Cardiology guidelines recommend a period of seven days of persistent atrioventricular block postsurgery prior to permanent pacemaker implantation. We report a 79-year-old woman in which the patient developed high-degree AVB after AVR was performed for severe aortic stenosis with complete right bundle branch block. However, her pulse returned to sinus rhythm 7 days postsurgery.

Evaluation of Deformable Image Registration and Dose Accumulation Using Histogram Matching Algorithm between kVCT and MVCT with Helical Tomotherapy

Purpose: To evaluate the accuracy of deformable image registration (DIR) between the planning kVCT (pCT) and the daily MVCT combined with the histogram matching (HM) algorithm, and evaluate the deformable dose accumulation using a suggested method for adaptive radiotherapy with Helical Tomotharapy (HT). Methods: For five prostate cancer patients (76 Gy/38 Fr) treated with HT in our institution, seven MVCT series (a total of 35 series) acquired weekly were investigated. First, to minimize the effect of different HU values between pCT and MVCT, this image-processing method adjusts HU values between pCT and MVCT images by using image cumulative histograms of HU values, generating an HM-MVCT. Then, the DIR of the pCT to the HM-MVCT was performed, generating a deformed pCT. Finally, deformable dose accumulation was performed toward the pCT image. Results: The accuracy of DIR was significantly improved by using the HM algorithm, compared with non-HM method for several structures (p ±0.05, 0.83 ±0.06, and 0.90 ± 0.04 for the CTV, rectum, and bladder, respectively, while that of the HM method was 0.81 ±0.06, 0.81 ±0.04, and 0.92 ±0.06, respectively. For the deformable dose accumulation, some difference was observed between the two methods, particularly for the small calculated regions, such as rectum V60 and V70. Conclusion: Adapting the HM method can improve the accuracy of DIR. Furthermore, dose calculation using the deformed pCT using HM methods can be an effective tool for adaptive radiotherapy.

Trefoil Factor 3 (TFF3) mRNA Expression Level in Follicular Thyroid Tumors Using Formalin-Fixed, Paraffin-Embedded (FFPE) Blocks

Background: Differential diagnosis of follicular thyroid carcinoma (FTC) from follicular thyroid adenoma (FTA) is often difficult since presence or absence of capsular/vascular invasion can not be determined by preoperative fine needle aspiration cytology, and may not be judged unanimously on permanent sections even among experienced pathologists. Determination of molecular-genetic factors such as trefoil factor 3 (TFF3) mRNA in the follicular thyroid tumors may be useful aid to improve the accuracy of diagnosis, though it is considered to be unstable and relatively low concentrated genetic substance. Purpose of our study is to investigate expression level of TFF3 mRNA of thyroid follicular tumors using formalin-fixed, paraffin-embedded (FFPE) tissue. Methods: Study population included FFPE sections from 19 FTC cases, 20 FTA cases, 11 adenomatous goiter (G) cases and 12 samples of normal thyroid tissue (N) adjacent to thyroid tumors. RNeasy FFPE kit was used for extraction of total RNA. Purification and concentration values were determined by spectrophotometer. Extracted RNA was used for cDNA synthesis in reverse transcription. Synthesized cDNA subsequently proceeded for relative quantification of TFF3 mRNA by RT-qPCR using TFF3 primers. Glyceroldehyde-3-phosphate dehydrogenase (GAPDH) and hypoxanthin phosphorobosyltransferase1 (HPRT1) were used as control genes. The mean and standard deviation of TFF3 mRNA expression level were analyzed by software Multiplate RQ. Results: Extraction by the FFPE kit yielded high concentration of RNA in all cases. Purification values were 1.8 in average. Concentration values were significantly higher in FTC and FTA relative to G and N tissues, possibly due to high density of thyrocytes in the samples. Relative quantification of TFF3 mRNA expression level showed broad ranges both in FTC and FTA, while the analyses in G and N tissues indicated narrow ranges. 陈国达矿(Ag9FeTe2S4),产于中国胶东金矿区埠南金矿的含金-银的黄铜矿-石英脉中,呈5～70μm的半自形-他形粒状与方铅矿、黄铜矿、碲银矿、银金矿、自然银、未定名的Ag6TeS2,Ag16FeBiTe3S8共生,并被自然银和螺状硫银矿包围交代.其平均化学成分(16个EPMA分析值)为Ag8.97Fe1.00Te1.99S4.04,理想化学式Ag9FeTe2S4.利用甘多菲照相及同步辐射迴摆照相技术,获得陈国达矿的67个衍射线,主要衍射强线的d值和相对强度(括号中)为:6.742(69),6.416(39),5.951(33),3.265(100),2.981(24),2.649(22),2.25(24),2.188(71),2.142(22),2.123(31),2.044(23),1.949(33).该衍射数据指标化为斜方晶系,a=12.769(2),b=14.814(2),c=16.233(1),V=3070.63,Z=9,dcal=6.85g/cm3.反光显微镜下,陈国达矿的光学性质与硫银锡矿接近,浅灰色,无明显非均质性,摩氏硬度2～3,颜色指数:x=0.3027,y=0.3076,Y=25.78%,λd=474nm,Pe=3.68%.该矿物以中国地质学家、地洼学说的创立人陈国达教授的姓名命名.该矿物及命名经IMA-CNMMN于2004年投票通过.2005年作者补充数据后,由CNMMN主席正式公布,批准号2004-042a。

The transformation of nonmetallic inclusions and the change of MnS precipitation ratio on oxide particles during heat treatment were studied by using Fe-10%Cr alloy containing different Si and S content in the present *** phenomenon of oxide transformation in alloy in this research was characterized according to Si *** the Si content was lower,the typical inclusions changed from MnO·SiO2 type to MnO·Cr2O3 type by heat *** the case of high S concentration,the obvious MnS area in inclusions was observed after heat *** was more favorable for MnS precipitation as the S content was improved from approximately 20×10-6 to 60×*** MnS precipitation ratio on oxide particles was higher on MnO·SiO2 type than MnO-Cr2O3 type oxide ***,the ratio on the two kinds of oxides increased with increasing the time for heat treatment at 1473K as a *** theoretical Mn content at which MnS can precipitate agrees well with the experimental results.

AIM: To investigate the role of pre-core and basal core promoter(BCP) mutations before and after hepatitis Be antigen(HBe Ag) ***: The proportion of pre-core(G1896A) and basal core promoter(A1762T and G1764A) mutant viruses and serum levels of hepatitis B virus(HBV) DNA, hepatitis B surface antigen(HBs Ag), and HB core-related antigen were analyzed in chronic hepatitis B patients before and after HBe Ag seroconversion(n = 25), in those who were persistently HBe Ag positive(n = 18), and in those who were persistently anti-HBe positive(n = 43). All patients were infected with HBV genotype C and were followed for a median of 9 ***: Although the pre-core mutant became predominant(24% to 65%, P = 0.022) in the HBe Ag seroconversion group during follow-up, the proportion of the basal core promoter mutation did not change. Median HBV viral markers were significantly higher in patients without the mutations in an HBe Ag positive status(HBV DNA: P = 0.003; HBs Ag: P < 0.001; HB core-related antigen: P = 0.001). In contrast, HBV DNA(P = 0.012) and HBs Ag(P = 0.041) levels were significantly higher in patients with the pre-core mutation in an anti-HBe positive ***: There is an opposite association of the pre-core mutation with viral load before and after HBe Ag seroconversion in patients with HBV infection.
