Mild drinking habit is a risk factor for hepatocarcinogenesis in non-alcoholic fatty liver disease with advanced fibrosis

AIM The impact of mild drinking habit(less than 20 g/d of ethanol) on the clinical course of non-alcoholic fatty liver disease(NAFLD) has not been determined. We examined the influence of a mild drinking habit on liver carcinogenesis from NAFLD. METHODS A total of 301 patients who had been diagnosed as having NAFLD by liver biopsy between 2003 and 2016 [median age: 56 years, 45% male, 56% with nonalcoholic steatohepatitis, 26% with advanced fibrosis(F3-4)] were divided into the mild drinking group withe thanol consumption of less than 20 g/d(mild drinking group, n = 93) and the non-drinking group(n = 208). Clinicopathological features at the time of liver biopsy and factors related to hepatocellular carcinoma(HCC) occurrence were compared between the *** We observed significant differences in male prevalence(P = 0.01), platelet count(P = 0.04), and gammaglutamyl transpeptidase(P = 0.02) between the test groups. Over 6 years of observation, the HCC appearance rate was significantly higher in the mild drinking group(6.5% vs 1.4%, P = 0.02). Multivariate survival analysis using Cox's regression model revealed that hepatic advanced fibrosis(F3-4)(P < 0.01, risk ratio: 11.60), diabetes mellitus(P < 0.01, risk ratio: 89.50), and serum triglyceride(P = 0.04, risk ratio: 0.98) were factors significantly related to HCC in all NAFLD patients, while the effect of a drinking habit was marginal(P = 0.07, risk ratio: 4.43). In patients with advanced fibrosis(F3-4), however, a drinking habit(P = 0.04, risk ratio: 4.83), alpha-fetoprotein(P = 0.01, risk ratio: 1.23), and diabetes mellitus(P = 0.03, risk ratio: 12.00) were identified as significant contributors to HCC occurrence. CONCLUSION A mild drinking habit appears to be a risk factor for hepatocarcinogenesis in NAFLD patients, especially those with advanced fibrosis.

The Treatment Strategy of Patients with Sleep Apnea Syndrome in Our Department and Subsequent Course

It is known that acute or chronic heart failure can develop in patients with grave sleep apnea syndrome (SAS), and known to have the social problem including an accident and the decrease of the labor will. Several medical treatments are now available for SAS. We considered problems of the treatment based on the practice situation of our department about the present conditions and subsequent treatment. The subjects consisted of 306 patients. We measured Epworth Sleepiness Scale (ESS) and apnea hypopnea index (AHI) after polysomnography in all patients at the first time and 6 months later. So, it was found that AHI and ESS decreased when we could be treated in the SAS patients appropriately. It was confirmed that the choice of the appropriate regimen was important.

Roles and Hopes of Family Members Living with SMID Children in Japan

The number of children with Severe Motor and Intellectual Disabilities (SMID) receiving medical treatment/recovering at home is rising yearly. Although benefits of this care are emphasized, the stress and duties of family members in the household are extremely great, especially because Japan is becoming a society of nuclear families. In this study, we described the lifestyles of nuclear families providing in-home medical care for children with SMID, focusing on family members’ roles. Roles of mothers, fathers, and siblings of children with SMID were summarized from semi-structured inter-views. As a result, for “the roles of each family member living with a child with SMID”, mothers had five roles, fathers seven, and siblings five. For “the hopes of each family member living with a child with SMID”, parents desired the whole family collaboration in care for children with SMID, and as caregivers, parents’ common thoughts included wanting siblings in order to help care for the child with SMID and wanting siblings to treasure their own lives. Siblings wanted their mother to have some time for rest and expected their fathers to have two main roles. They also expected their grandparents and other siblings to fulfill roles.

This research proposes a new offshore wind energy generation system that uses a tension leg platform (TLP) and describes experiments performed on a TLP type wind turbine in both waves and wind. The following conclusions can be made from the results of this research. 1) In the case of coexisting wave-wind fields, the wind effect stabilizes the pitch motion. 2) The wind effect decreases vibration of the mooring lines when waves and wind coexist. In particular, the springing (2nd or 3rd order force) also decreases in this field. 3) It can be estimated that the reduction in the rate of generation of electrical power can be up to about 6% as a result of the heel angle. In addition, the annual amount of electricity generated was estimated along with the utilization factor based on the experimental results.

Genetic Background Influences Brassinosteroid-Related Mutant Phenotypes in Rice

In two cases, mutations in the same brassinosteroid-related genes caused different phenotypes in japonica varieties Nipponbare and Taichung 65. The mutant phenotypes were less severe in the Taichung 65 background than in the Nipponbare background. Three newly isolated brassinosteroid-insensitive mutants (d61-1N, d61-11, and d61-12) derived from a Nipponbare mutant library were found to be alleles of d61, which represent defects in the OsBRI1 gene. Although the Nipponbare-derived mutant d61-1N had the same nucleotide substitution as the previously characterized Taichung 65-derived mutant d61-1T, these two mutants showed different phenotypes for plant stature, internode elongation pattern, and seed shape;in each case, d61-1N (in the Nipponbare genetic background) had the more severe mutant phenotype. Similar trends were seen for phenotypes caused by mutants of d2, a brassinosteroid biosynthesis gene. Consistent with these phenotypes, the expression of brassinosteroid-responsive genes was lower in the Nipponbare-derived mutants. These results can be explained by our findings that feed-forward up-regulation of OsBRI1 did not occur in the Nipponbare-derived mutants and that an mPing transposon is inserted into the promoter region of Nipponbare OsBRI1. Based on these results, we conclude that the expression of OsBRI1, especially its feed-forward up-regulation, is misregulated in wild-type Nipponbare and in brassinosteroid-related mutants in a Nipponbare genetic background. Although Nipponbare is a model rice genotype, it can be categorized as an OsBRI1 mutant that has reduced sensitivity to brassinosteroid.

Membrane trafficking to the protein storage vacuole （PSV） is a specialized process in seed plants. However, this trafficking mechanism to PSV is poorly understood. Here, we show that three types of Beige and Chediak-Higashi （BEACH）-domain proteins contribute to both vacuolar protein transport and effector- triggered immunity （ETI）. We screened a green fluorescent seed （GFS） library of Arabidopsis mutants with defects in vesicle trafficking and isolated two allelic mutants gfs3 and gfs12 with a defect in seed pro- tein transport to PSV. The gene responsible for the mutant phenotype was found to encode a putative pro- tein belonging to group D of BEACH-domain proteins, which possess kinase domains. Disruption of other BEACH-encoding loci in the gfs12 mutant showed that BEACH homologs acted in a cascading manner for PSV trafficking. The epistatic genetic interactions observed among BEACH homologs were also found in the ETI responses of the gfs12 and gfs12 bchb-1 mutants, which showed elevated avirulent bacterial growth. The GFS12 kinase domain interacted specifically with the pleckstrin homology domain of BchCl. These results suggest that a cascade of multiple BEACH-domain proteins contributes to vacuolar protein transport and plant defense.

Fractional Optical Solitons and Fractional Noether’s Theorem with Ortigueira’s Centered Derivatives

This paper shows that the centered fractional derivatives introduced by Manuel Duarte Ortigueira in 2006 are useful in the description of optical solitons. It is shown that we can construct a fractional extension of the nonlinear Schrödinger (NLS) equation which incorporates Ortigueira’s derivatives and has soliton solutions. It is also shown that this fractional NLS equation has a Lagrangian density and can be derived from a variational principle. Finally, a fractional extension of Noether’s theorem is formulated to determine the conserved quantities associated to the invariances of the action integral under infinitesimal transformations.
