Computerized Scheme for Histological Classification of Masses with Architectural Distortions in Ultrasonographic Images

Architectural distortion is an important ultrasonographic indicator of breast cancer. However, it is difficult for clinicians to determine whether a given lesion is malignant because such distortions can be subtle in ultrasonographic images. In this paper, we report on a study to develop a computerized scheme for the histological classification of masses with architectural distortions as a differential diagnosis aid. Our database consisted of 72 ultrasonographic images obtained from 47 patients whose masses had architectural distortions. This included 51 malignant (35 invasive and 16 non-invasive carcinomas) and 21 benign masses. In the proposed method, the location of the masses and the area occupied by them were first determined by an experienced clinician. Fourteen objective features concerning masses with architectural distortions were then extracted automatically by taking into account subjective features commonly used by experienced clinicians to describe such masses. The k-nearest neighbors (k-NN) rule was finally used to distinguish three histological classifications. The proposed method yielded classification accuracy values of 91.4% (32/35) for invasive carcinoma, 75.0% (12/16) for noninvasive carcinoma, and 85.7% (18/21) for benign mass, respectively. The sensitivity and specificity values were 92.2% (47/51) and 85.7% (18/21), respectively. The positive predictive values (PPV) were 88.9% (32/36) for invasive carcinoma and 85.7% (12/14) for noninvasive carcinoma whereas the negative predictive values (NPV) were 81.8% (18/22) for benign mass. Thus, the proposed method can help the differential diagnosis of masses with architectural distortions in ultrasonographic images.

Successful treatment of giant rectal varices by modified percutaneous transhepatic obliteration with sclerosant:Report of a case

We present a female patient with continuous melena, diagnosed with rectal variceal bleeding. She had a history of esophageal varices, which were treated with endoscopic therapy. Five years after the treatment of esophageal varices, continuous melena occurred. Since colonoscopy showed that the melena was caused by giant rectal varices, we thought that they were not suitable to receive endoscopic treatment. We chose the modified percutaneous transhepatic obliteration with sclerosant, which is one of the interventional radiology techniques but a new clinical procedure for rectal varices. After the patient received this therapy, her condition of rectal varices was markedly improved.

Dynamic viscoelasticity in a temperature ramp sweep test showed the starch granules of fermented sample were more rigid and resistant to breakdown than nonfermented *** scanning calorimetry revealed that the gelatinization temperature of fermented rice flour shifted to lower *** partial impairment of granule structure may release the internal stress during swelling,endued the granule with more resistance to *** fermented or nonfermented rice flour,sample formed an elastic gel just after the end of gelatinization during *** fermented rice flour formed gel earlier with a better gel structure,however it showed slower retrogradation during *** result also indicated the retrogradation of amylopectin impaired the favorite texture of rice noodles.

Aged Garlic Extract Reduces ROS Production and Cell Death Induced by 6-Hydroxydopamine through Activation of the Nrf2-ARE Pathway in SH-SY5Y Cells

Many degenerative or pathological processes, such as aging, cancer and coronary heart disease, are related to reactive oxygen species (ROS) and radical-mediated reactions. We examined the effectiveness of aged garlic extract (AGE), a garlic preparation rich in water-soluble cysteinyl moieties, for protection of cells from ROS produced by 6-hydroxy-dopamine (6-OHDA) using human neuroblastoma SH-SY5Y cells. Concomitant treatment of cells with AGE (2 and 4 mg/ml) showed the dose-dependent protective effect on the cell death induced by 6-OHDA. In addition, the AGE treatment significantly suppressed the increase of ROS generation by 6-OHDA. Furthermore, the protective effect of AGE was accompanied by activation of the nuclear factor erythroid 2-related factor 2 (Nrf2)-antioxidant response element (ARE) pathway and the increase of mRNAs of heme oxygenase-1 and NAD(P)H quinone oxidoreductase 1. These two enzymes are important in the cellular antioxidant system. These results indicated that AGE protected cells from ROS damage by not only capturing ROS directly but also activating the cellular antioxidant system by stimulating antioxidant gene expression via the Nrf2-ARE pathway. The present study suggested that AGE may be useful for prevention and treatment of cell damage caused by ROS.

Variations in the mental health and sense of coherence (SOC) of new graduate nurses and the effects of SOC on variations in mental health

The current research conducted a longitudinal study of new graduate nurses at 3 times-prior to their employment as nurses and 3 months and 1 year afterward. This study yielded 3 findings. 1) The mental health of new graduate nurses significantly worsens 3 months after employment as a nurse and mental health 1 year after employment as a nurse was significantly worse than that before employment. 2) A sense of coherence (SOC) is the ability to cope with stressors, and the 3 components of SOC decreased significantly in the year after employment as a nurse. 3) The mental health of new graduate nurses was significantly affected by a low SOC and by previous diminishing of SOC. Findings suggested the importance of support to sustain and improve the mental health of new graduate nurses and the importance of approaches in the workplace to encourage the development of SOC.

Screening of the Growth-Inhibitory Effects of 168 Plant Species against Lettuce Seedlings

The methanol extracts of 168 plant species from 68 families were evaluated for their inhibitory activity against lettuce seedling elongation. Among the plant species tested, 12 species had EC50 values for radicle growth inhibition ranging from 0.01 to 5.00 mg fresh weight equivalent mL-1. Enterolobium contortisiliquum, a traditionally used herbal medicine, exhibited the strongest inhibitory activity (estimated EC50: 0.28 fresh weight equivalent mL-1). Among the 12 species, Pachysandra terminalis, Tamarindus indica, and Albizia guachapele required investigation, because only little has been reported about their chemical constituents to date. The data in the present study would be useful in finding new lead compounds for natural herbicides.

The Wheat Plastochron Mutant, fushi-darake, Shows Transformation of Reproductive Spikelet Meristem into Vegetative Shoot Meristem

In wheat plants at the vegetative growth stage, the shoot apical meristem (SAM) produces leaf primordia. When reproductive growth is initiated, the SAM forms an inflorescence meristem (IM) that differentiates a series of spikelet meristem (SM) as the branch. The SM then produces a series of floret meristem (FM) as the branch. To identify the mechanisms that regulate formation of the reproductive meristems in wheat, we have investigated a leaf initiation mutant, fushi-darake (fdk) which was developed by ion beam mutagenesis. The morphological traits were compared in wild type (WT) and fdk mutant plants grown in the experimental field. WT plants initiated leaves from SAM at regular intervals in spiral phyllotaxy, while fdk plants had 1/2 alternate phyllotaxy with rapid leaf emergence. The fdk plants have increased numbers of nodes and leaves compared with WT plants. The time interval between successive leaf initiation events (plastochron) was measured in plants grown in a growth chamber. The fdk plants clearly show the rapid leaf emergence, indicating a shortened plastochron. Each tiller in fdk plants branches at the upper part of the culm. The fine structure of organ formation in meristems of fdk plants was examined by scanning electron microscopy (SEM). The SEM analysis indicated that fdk plants show transformation of spikelet meristems into vegetative shoot meristems. In conclusion, the fdk mutant has a heterochronic nature, i.e., both reproductive and vegetative programs were simultaneously in operation during the reproductive phase, resulting in a shortened plastochron and transformation of reproductive spikelets into vegetative shoots.

Risk factors for bleeding after endoscopic mucosal resection

AIM： To clarify the risk factors for bleeding after endoscopic mucosal resection （EMR）. METHODS： A total of 297 consecutive patients who underwent EMR were enrolled. Some of the patients had multiple lesions. Bleeding requiring endoscopic treatment was defined as bleeding after EMR. Odds ratios （OR） with 95% confidence intervals （CI）, calculated by logistic regression with multivariate adjustments for covariates, were the measures of association. RESULTS： Of the 297 patients, 57 （19.2%） patients with bleeding after EMR were confirmed. With multivariate adjustment, the cutting method of EMR, diameter, and endoscopic pattern of the tumor were associated with the risk of bleeding after EMR. The multivariate-adjusted OR for bleeding after EMR using endoscopic aspiration mucosectomy was 3.07 （95%CI, 1.59-5.92） compared with strip biopsy. The multiple-adjusted OR for bleeding after EMR for the highest quartile （16-50 mm） of tumor diameter was 5.63 （95%CI, 1.84-17.23） compared with that for the lowest （4-7 mm）. The multiple-adjusted OR for bleeding after EMR for depressed type of tumor was 4.21 （95%CI, 1.75-10.10） compared with elevated type. CONCLUSION： It is important to take tumor charactedstics （tumor size and endoscopic pattern） and cutting method of EMR into consideration in predicting bleeding after EMR.

On the other hand, visible light is reported to be most important as a zeitgeber to entrain circadian rhythms in animals. However, effects of UV on the circadian rhythms such as locomotor activity and body temperature rhythms have not been investigated well. Therefore, in this study, we examined the effects of UV irradiation on the circadian rhythms of locomotor activity and body temperature in hairless mice.

A Greater Association of Hyperuricemia than of Metabolic Syndrome with the New Incidence of Chronic Kidney Disease

Background: To what extent uric acid (UA) levels and/or metabolic syndrome (Mets) contribute to the onset of chronic kidney disease (CKD) is largely unknown. The present study explores how these two factors have an association with the new incidence of CKD. Methods: Study design is a cohort study. A total of 14,485 participants were eligible for the cross-sectional analysis on UA levels and the prevalence of Mets. Among those individuals, 8,223 participants without CKD and 4,839 without Mets were eligible for the longitudinal analysis of the new incidence of CKD. Parameters monitored were body mass index, systolic and diastolic blood pressure, serum creatinine concentration, estimated glolerular filtration rate, lipid profiles, plasma glucose, HbA1c. The primary predictor was the level of UA and Mets to explain the newly-developed CKD. The observation period was 4 years. Results: In a cross-sectional analysis, higher UA levels were associated with the greater prevalence of Mets. In addition, UA levels were associated with the numbers of the Mets constituents in both genders. In a longitudinal analysis, higher UA levels were associated with the greater rate of CKD and the greater incidence of Mets. In addition, the incidence of CKD at year 4 was influenced by the presence of hyperuricemia, but not by that of the Mets. The odd ratio (OR) to predict the CKD incidence was 1.42 (95% confidence intervals (CI), 0.52 to 3.78) in the presence of Mets alone, 2.10 (95% CI, 1.36 to 3.23) in the presence of hyperuricemia alone, and 3.56 (95% CI, 1.55 to 8.21) in the presence of both. Conclusion: Hyperuricemia has a greater association with the incidence of CKD than Mets does. Hyperuricemia complicated by Mets is additionally detrimental.
