Cochlear Implantation in Patients with Eosinophilic Otitis Media

It is known that cochlear implantation for deaf patients with eosinophilic otitis media (EOM) is safe and can provide good speech perception. However, the best timing of implant surgery in patients with EOM is not yet known. The aim of this case report is to suggest the appropriate timing of the surgery in EOM patients with deaf. Cochlear implantation was indicated in two patients with EOM. One underwent cochlear implantation in the absence of any ear discharge. In the other case, implant surgery was delayed for three years due to persistent ear discharge. No complications related to implant device or skin flap were observed in either case. The speech recognition score after implantation was good in the first case and poor in the second case. Perioperative complications were manageable even in the patient with persistent ear discharge. However, the delay in implant surgery due to the persistent ear discharge resulted in a poor speech recognition score. Early implantation should be considered even in EOM patients with ear discharge, although the presence of active middle ear inflammation is regarded as one of the contraindications for implantation according to the current Japanese guidelines.

Pneumoscrotum is uncommon and also rarely reported as a complication associated with colonic perforation. A case of colonic perforation in delayed fashion associated with EMR, revealed by pneumoscrotum, is reported and the associated literatures are reviewed. A 52-year-old male received piecemeal EMR for a laterally spreading tumor 35 mm in size in our hospital. He complained of enlargement of the scrotum and revisited our hospital the day after the procedure. A diagnosis of pneumoscrotum was made, and as most such cases have been reported to be associated with pneumoperitoneum, colonic perforation was suspected. Free air but no fluid collection was found by abdominal computed tomography, and delayed colonic perforation was diagnosed. However, as there were no clinical signs of peritoneal irritation,conservative treatment was administered and the patient recovered uneventfully. Pneumoscrotum could be a sign of colonic perforation after EMR, and treatment should be carefully chosen.

The propagation of pressure waves in water and metal generated by focusing of pulsed Nd:YAG laser beam on to the metal surface is studied. When a metal material is ablated in water, a pressure is enhanced near laser-irradiated area and some stress waves generate. The understanding of the process is the basis of laser peening. The effects of thickness of specimen (SUS304) ranging from 0.1mm to 10mm on wave generation in water were investigated using visualization with shadowgraph technique and measurement of over pressure in the water. The shape of a hemispherical shock wave and the peak pressure behind it were independent on thickness of the specimen. On the other hand, the shapes and the behaviors of visualized other waves depend on plate thickness.

The linearity, stability, accuracy and inter-day precisions of the assay method were evaluated in methanolic aerial-part extracts of Paepalanthus giganteus and Syngontnhus nitens from the Eriocaulaceae family. Their small capitulae hinder morphological analysis, and thus complicate taxonomic studies of these species, which present anti-ulcer, antimutagenic and antioxidant activities. Taxonomic studies of these plants revealed that the Paepalanthus genus presents flavonols and naphthopyranones while the Syngontnhus genus has flavone and xanthone as majority compounds. The prepared samples were analyzed quantitatively by High Performance Liquid Chromatography with PDA detection for the presence of quercetin, luteolin, 3,6-dimethoxy-1,5,7-tri-hydroxyxanthone and paepalantine. The substances were recovered from these samples at rates from 98.01 to 99.99%. The coefficient of variation in the quantitative analysis of the sample com- pounds was under 5%. The linearity of the method was determined by linear regression. The analysis of the samples spiked with known amounts of analyte demonstrated that the response was proportional to the concentrations of the sam- ples with re-spective determination coefficients of r2 = 0.9999 (luteolin and 3,6-dimethoxy-1,5,7-tri-hydroxyxanthone) and r2 = 0.9998 (quercetin and paepalantine) for the linear range of the analytical calibration curves of the samples. The detection limits were 0.07 μg?mL–1 for quercetin and luteolin, 0.06 μg?mL–1 for 3,6-dimethoxy-1,5,7-tri-hydroxyxanthone and 0.10 μg?mL–1 for paepalantine. The quantification limits were 0.23 μg?mL–1 for quercetin and luteolin, 0.20 μg?mL–1 for 3,6-dimethoxy-1,5,7-tri-hydroxyxanthone and 0.33 μg?mL–1 for paepalantine by LC. The method was considered sen- sitive for quantification of the quercetin, luteolin, 3,6-dimethoxy-1,5,7-tri-hydroxyxanthone and paepalantine in plant samples.

Anisakiasis can involve any part of the digestive tract, but most cases reported have involved the stomach;few reports have described colonic anisakiasis. Furthermore, asymptomatic colonic anisakiasis has been reported to be very rare. A 58-year-old Japanese male asymptomatically received colonoscopy due to a fecal occult blood testing positive, and an Anisakis larva was removed in the ascending colon. After colonoscopy, an detailed questionings concerning eating raw fish revealed that the patient ate the liver of raw filefish 21 days before the colonoscopy. Thus, questionings concerning eating the raw fish were very important and helpful for correct diagnosis. This case report demonstrated that colonic anisakiasis can be diagnosed by colonoscopy before severe complications (intestinal obstruction, perforation, and cancer development) occur. Also, biopsy forceps could be used to remove the Anisakis worms, demonstrating that diagnosis and treatment can be simultaneously performed.

Constructing Genetic Linkage Maps Using the Whole Genome Sequence of Pacific Bluefin Tuna (Thunnus orientalis) and a Comparison of Chromosome Structure among Teleost Species

Pacific bluefin tuna (Thunnus orientalis) is one of the most economically important species in the Percomorpha group of teleost fishes. Their migrations are extensive and depend upon continuous swimming at a high rate of speed throughout their life. The draft genome sequence of this species has been reported but remains highly fragmented. We constructed a Pacific bluefin tuna genetic linkage map using microsatellite markers developed on each of the scaffolds from the draft genome sequence to link these genome fragments and understand the genomic structure of species in Percomorpha. Of the 606 polymerase chain reaction microsatellite primer pairs tested, 473 were polymorphic in the mapping populations for the linkage analysis. We constructed sex-specific maps for 24 linkage groups consisting of 470 markers, which allowed us to place scaffolds that cumulatively represented 20.8% (153.8 Mb) of the sequenced genome onto the linkage groups. The distribution of orthologous genes on the chromosomes of tuna and four other teleost fish species suggested that the constitution of tuna chromosomes is closest to that of medaka. Both species have the 24 chromosomes of the ancestral teleost, including several chromosomal inversions. The integrated map developed in this study will be useful to construct a complete physical map to conduct comparative teleost genomics and genetic studies on economically useful traits in Pacific bluefin tuna.

Dental?hemorrhagic?complications,?including?postoperative?bleeding?and?traumatic?hemorrhage?as?emergency?cases,?often?occur?in?patients?undergoing?oral?anticoagulant?therapy?such?as?warfarin?therapy.?Recent?research?recommends?that?warfarin?dosage?should?be?assessed?every?12?weeks.?Therefore,?most?physicians?generally?accept?international?normalized?ratio?(INR)?monitoring?at?longer?intervals.?However,?cases?are?encountered?in?which?the?INR?prolongation?is?observed?despite?of?invariable?dosage?of?warfarin.?In?this?report,?we?present?2?cases?of?patients?with?dental?hemorrhagic?complications?undergoing?oral?anticoagulant?therapy?who?exhibited?excessive?INR?prolongation.?These?patients?exhibited?decreased?appetite?and?hypoalbuminemia. We?speculate?that?long-term?appetite?loss?resulted?in?the?increase?in?the?serum?concentration?of?free?warfarin and?vitamin?K deficiency. Our?study?indicates that we?should?notice malnourishment?when?we?treat patients?who?have?undergone?warfarin?therapy with dental?surgical procedures.?It?is?recommended?that measurement?of?INR just before?a?dental?surgical?treatment.
