BACKGROUND Holoprosencephaly(HPE)is a congenital malformation with various degrees of incomplete separation of the cerebral hemispheres due to differentiation disorders of the *** HPE with diabetes insipidus due to associated pituitary dysfunction has been reported,HPE with the syndrome of inappropriate antidiuretic hormone secretion(SIADH)is very ***,a vasopressin V2 receptor antagonist,is effective in adults with ***,there is no report of its efficacy in infants with *** purpose of this report is to demonstrate that tolvaptan is effective for SIADH in infants and that administration of tolvaptan eliminates the need for restriction of water intake and sodium *** SUMMARY A 2414-g female infant was born at 38 wk by normal vaginal *** anomalies and head magnetic resonance imaging indicated semilobar *** birth,she had hyponatremia due to SIADH and was treated using water and sodium ***,she developed an exaggerated response to the fluid restrictions,resulting in large fluctuations in serum sodium *** administration of tolvaptan improved the fluctuations in serum sodium levels without the need for adjustment of water or sodium *** sodium was maintained within the normal range after discontinuation of tolvaptan at 80 d of *** were no side effects,such as hypernatremia or liver dysfunction,during the administration of *** This is the first report on the safety and efficacy of tolvaptan in an infant with SIADH associated with HPE.

AIM:To make clear whether CD147 (EMMPRIN) expression in pathological tumor samples with a fine-needle aspiration biopsy is useful for pathological diagnosis of early hepatocellular carcinoma (HCC). METHODS:Twenty-two patients (15 men and 7 women; median age 68 years,range 56-81 years) underwent a liver tissue biopsy in order to make a diagnosis of HCC. Paraffin-embedded liver biopsy tissue samples from 22 patients were stained with anti-CD147 antibody,murine monoclonal antibody 12C3 (MAb12C3) for immunohistochemical analysis. An immunohistochemical analysis of CD147 was performed and the degree of staining compared between tumor and non-tumor tissue. In addition,the degree of staining within tumor tissue was compared according to a number of clinicopathological variables. RESULTS:The degree of staining of CD147 was significantly higher in tumor tissues than non-tumor tissues,even in tumors less than 15 mm in *** expression of this protein was significantly elevated in HCC tissue specimens from patients with a low value of serum AST and γ-GTP.

错流碟片旋转填充床反应器可有效脱除燃煤烟道气中的二氧化碳和其它有害成分,但液体分布状态对反应速率有很大影响,研究液膜的体积分率可为该类型反应器的设计提供有效的依据参数;实验研究了碟片转速、液体流量、气体流量、碟片间距和碟片开孔率与液膜体积分率的关系,用每张碟片底部对应液体收集器储存并测量碟片表面液膜的体积流量,用高速摄像机记录不同条件下液膜的存在状态和变化规律。研究表明随碟片转速增加液膜分率降低,增强的离心力促使液膜破裂,但液滴的甩出速率增加,促使液滴二次聚并,有不连续液丝产生。增大入口液体流量,液膜分率减小,液膜厚度变化缓慢,空间液滴增大。增大气体流量,液膜分率减小,厚度变薄,碟片表面附着液膜被吹散,促使液体雾化。液膜分率随碟片间距增加减小,提高碟片间距可有效减少液滴二次聚并;碟片开孔率增大液膜分率减小,提高碟片开孔率可有效降低碟片表面液膜的连续性,促使液膜分裂。

Direction without speed information process in the human brain: a magnetoencephalographic study using random dots apparent motion stimulus

Apparent motion stimulus induces visual perception of smooth motion even though there is no speed information. We examined whether human brain response as measured by magnetoencephalography carries direction information in the visually presented apparent motion of a randomdot pattern in a similar manner as continuous motions that have speed and direction information. Although there was no significant effect of motion direction on the peak response latency and amplitude, mutual information entropy (IE) significantly increased after the motion onset at approximately 36 ms after the response latency in 41% of the evaluations. Detailed analysis of the data from five subjects who participated in both the present apparent motion and our previous coherent motion studies revealed that the maximum IE latency (delay) for apparent motion was significantly longer than that for coherent motion, although the mean maximum IE was the same. The results indicate that direction is represented in the response waveform evoked by apparent motion but the manner is different from that for coherent motion probably due to the distinct neural processes engaged only for the apparent motion perception. We consider that direction and speed can be processed separately in the human brain because direction information was generated without speed information for the perception of apparent motion.

The structural interaction is prerequisite for compatibility in car-to-car crashes. Full- width tests have been proposed to assess the structural ground-height alignment and homogeneity, which are key factors for structural interaction. In the present research, full-width rigid and deformable barrier tests were compared with respect to barrier force distributions, vehicle deformation and dummy responses. The average height of force was similar between full rigid and deformable barrier tests. Further information on force distribution such as structure homogeneity can be obtained in full-width deformable barrier tests because forces from structures can be clearly shown in barrier force distributions compared with rigid barrier. However, force distribution measurements and homogeneity criteria are dependent on load cell height from the ground.

Distribution of the P2X2 receptor and chemical coding in ileal enteric neurons of obese male mice(ob/ob)

AIM:To investigate the colocalization,density and profile of neuronal areas of enteric neurons in the ileum of male obese ***:The small intestinal samples of male mice in an obese group(OG)(C57BL/6J ob/ob)and a control group(CG)(+/+)were *** tissues were analyzed using a double immunostaining technique for immunoreactivity(ir)of the P2X2 receptor,nitric oxide synthase(NOS),choline acetyl transferase(ChAT)and calretinin(Calr).Also,we investigated the density and profile of neuronal areas of the NOS-,ChAT-and Calrir neurons in the myenteric *** neurons were labeled using an NADH-diaphorase histochemical staining ***:The analysis demonstrated that the P2X2receptor was expressed in the cytoplasm and in the nuclear and cytoplasmic membranes only in the *** density values(neuron/cm2)decreased 31%(CG:6579±837;OG:4556±407)and 16.5%(CG:7796±528;OG:6513±610)in the NOS-ir and calretininir neurons in the OG,respectively(P<0.05).Density of ChAT-ir(CG:6200±310;OG:8125±749)neurons significantly increased 31%in the OG(P<0.05).Neuron size studies demonstrated that NOS,ChAT,and Calr-ir neurons did not differ significantly between the CG and OG *** examination of NADH-diaphorase-positive myenteric neurons revealed an overall similarity between the OG and ***:Obesity may exert its effects by promoting a decrease in P2X2 receptor expression and modifications in the density of the NOS-ir,ChAT-ir and CalR-ir myenteric neurons.

Ethical Dilemma Factor in Regarding Physical Restraints to Elderly of Female Nurses with the Living Together Experience

Purpose: The aim of this study was to illuminate the connotation of “dilemma” regarding the use of physical restraint on elderly patients as represented by female nurses working in general wards at community hospitals who also live with elderly adults at home. Method: The study used the questionnaire method with an independently prepared questionnaire. Participants: The objective of the study was explained to the persons in charge of nursing in the selected 17 hospitals, and 1929. Finally, this study of participants were 524 female nurses working in general 54 wards (excluding the emergency wards of psychiatry, pediatrics, obstetrics, outpatients, operating rooms, and intensive care units) at community hospitals who also live with elderly adults at home. Results and conclusions: Cronbach’s overall coefficient for the 20 items of dilemma was high (0.78) and the factor analysis extracted four dilemma factors as having a characteristic value of 1 (Kaiser-Meyer-Olkin measure value = 0.81) with a cumulative contribution ratio of 64.5%. The high Cronbach’s for these items (0.86, 0.88, 0.87 and 0.81) confirmed the internal consistencies. With respect to the dilemma where nurses working in general wards at community hospitals who also live with elderly adults at home are faced with the physical restraint of elderly persons, four factors in the clarification of the dilemma were extracted: accomplishment of medical-treatment and accident prevention, characteristic features in nursing for elderly patients with dementia, healthcare professional relationship in nursing for elderly patients with dementia, and priorities on the accident prevention for elderly patients with dementia. Therefore, the construction of four systems to cope with these dilemmas is suggested. These systems would enable practising nurses to: (i) institution of policies to encourage discussion between nurses and other medical staff to reach consensus on treatment;(ii) allocating elderly care specialists to w

Inhibition of gastric perception of mild distention by omeprazole in volunteers

AIM： To evaluate the effects of omeprazole on gastric mechanosensitivity in humans. METHODS： A double lumen polyvinyl tube with a plas- tic bag was introduced into the stomach of healthy volunteers under fluorography and connected to a barostat device. Subjects were then positioned so they were sitting comfortably, and the minimal distending pressure （MDP） was determined after a 30-rain adap- tation period. Isobaric distensions were performed in stepwise increments of 2 mmHg （2 min each） starting from the MDR Subjects were instructed to score feel- ings at the end of every step using a graphic rating scale： 0, no perception; 1, weak/vague; 2, weak but significant; 3, moderate/vague; 4, moderate but signifi- cant; 5, severe discomfort; and 6, unbearable pain. Af- ter this first test, subjects received omeprazole （20 mg, after dinner） once daily for 1 wk. A second test was performed on the last day of treatment. RESULTS： No adverse effects were observed. Mean MDP before and after treatment was 6.3 - 0.3 mmHg and 6.2：1：0.5 mmHg, respectively. One subject before and 2 after treatment did not reach a score of 6 at the maximum bag volume of 750 mL. After omeprazole, there was a significant increase in the distension pres- sure required to reach scores of 1 （P = 0.019） and 2 （P = 0.017） as compared to baseline. There were no changes in pressure required to reach the other scores after treatment. Two subjects before and one after omeprazole rated their abdominal feeling 〈 1 at MDP, and mean （±SE） abdominal discomfort scores at MDP were 0.13±0.09 and 0.04±0.04, respectively. Mean scores induced by each MDP ＋ 2, 4, 6, 8, 10, 12, 14, 16, 18 and 20 （mmHg） were 1.1±0.3, 2.0±0.4, 2.9±0.5, 3.3±0.4, 4.6±0.3, 5.2±0.3, 5.5±0.2, 5.5±0.3, 5.7±0.3, and 5.4, respectively. After omepra- zole, abdominal feeling scores for the same incremen- tal pressures over MDP were 0.3±0.1, 0.8±0.1, 2.0±0.4, 2.8±0.4, 3.8±0.4, 4.6±0.4, 4.9±0.3, 5.4±0.4, 5.2±0.6, and 5.0±1.0, respectively.
