Histopathological confirmation of similar intramucosal distribution of fluorescein in both intravenous administration and local mucosal application for probebased confocal laser endomicroscopy of the normal stomach

Probe-based confocal laser endomicroscopy(p CLE) is capable of acquiring in vivo magnified cross-section images of the gastric mucosa. Intravenous injection of fluorescein sodium is used for confocal imaging. However, it is still under debate if local administration of the dye to the mucosa is also effective for confocal imaging as it is not yet clear if topical application also reveals the intramucosal distribution of fluorescein. The objective of this study was to evaluate the intramucosal distribution of fluorescein sodium after topical application and to compare the distribution to the conventional intravenous injection used for confocal imaging. p CLE of the stomach uninfected with Helicobacter pylori was performed in a healthy male employing intravenous administration and local mucosal application of fluorescein. The mucosa of the lower gastric body was biopsied 1 min and 5 min after intravenous administration or local mucosal application of fluorescein, and the distribution of fluorescein in the biopsy samples was examined histologically. Green fluorescence was already observed in the cytoplasm of fundic glandular cells in the biopsied deep mucosa 1 min after local mucosal application of fluorescein. It was also observed in the foveolar lumen and inter-foveolar lamina propria, although it was noted at only a few sites. In the tissue biopsied 5 min after the local mucosal application of fluorescein, green fluorescence was more frequently noted in the cytoplasm of fundic glandular cells than in that 1 min after the local mucosal application of fluorescein, although obvious green fluorescence was not identified in the foveolar lumen or inter-foveolar lamina propria. The distribution of intravenously administered fluorescein in the cytoplasm of fundic glandular cells was also clearly observed similarly to that after local mucosal application of fluorescein. 疏浚作业时，以底泥释放为代表的内源污染治理问题变得日益突出，减少疏浚带来的水体二次污染是河湖治理中的重点和难点。本文通过使用数学模型FS3M进行全尺寸数值模拟，从流场、悬沙运动及体积变化的角度研究了能减少水体浑浊产生的抓斗最佳下落操作。数值结果表明，抓斗在自重和机械力的控制下，下降过程中会对周围水体产生较大的扰动，卷起较多的床沙，对周边水体环境造成影响。在作业过程中适当的减缓自身下降速度可以有效的抑制床面附近产生的水流速度，随之也会减少悬沙的产生并抑制其随水流运动而向外部扩散。随着计算模型的不断优化升级，今后还需对抓斗铲沙这一动态行为进行模拟分析，深入研究这一行为对泥沙运动规律的影响。该研究的成果可为环保式疏浚工艺提供一定参考。

Relationship between space charge and effective pore size of nanoporous electrode in electric double-layer capacitor

The space-charge distributions of electric double-layer capacitors （EDLCs）, which are energy storage devices, were examined with the pulsed electroacoustic （PEA） method. It was found that the experimental results could be influenced by the reflection and penetration of sound waves when the space-charge dis- tributions of EDLCs were measured with the PEA method. This is because EDLCs have a five-layer structure consisting of three materials （aluminum, cellulose, and activated carbon）. We calculated the reflection wave components that influenced the charge density through the acoustic impedance and the relative permittivity of the materials. In this way, we found that the changes in the space-charge distributions of EDLCs and their charge characteristics corresponded closely. We determined that measuring the space- charge distributions with the PEA method was effective for evaluating the charge accumulation of EDLCs. In this study, a polarized electrode was prepared for use in EDLCs. The ratio of the surface area to the average pore diameter of the polarized electrode was measured with the nitrogen adsorption method at 77 K. The relationship between the ratio of the surface area to the average pore size and the space-charge distributions of EDLCs is also discussed in this paper.

Intraoperative glycemic control procedures and the use of an artificial pancreas

Strict intraoperative glycemic control can significantly decrease the incidence of postoperative infection; however, anesthesiologists must carefully control blood glucose levels as well as properly manage the respiratory and cardiovascular systems. However, standard blood glucose measurement systems and insulin dosing algorithms, which are necessary for achieving strict glycemic control, have not yet been developed. An artificial pancreas (STG-22TM; Nikkiso Co., Tokyo, Japan) is considered a highly accurate blood glucose monitoring system capable of closed-loop control of blood glucose. The device has, however, many problems to be addressed since it is a large and expensive system with little versatility, and it requires a large amount of blood to be collected. Therefore, the development of less invasive and inexpensive systems with future technological progress is greatly anticipated.

Impact of Temperature in Summer on Emergency Transportation for Heat-Related Diseases in Japan

Background： In Japan, the demand for emergency transportation for people with heat-related illness has recently increased. The purpose of this study was to investigate the relationship between incidents of heat-related illness and the daily maximum temperature. Methods： The daily maximum temperatures in Japan＇s 11 districts over the past 10 years were classified into four categories, with cutoff points at the 50th, 75th, 95th, and higher than 95th percentiles. We then conducted a logistic regression analysis of einergency transportation demand in each temperature category by age group, using the 50th percentile as tile reference category for each area. Results： There were 42,931 cases of emelgency transportation due to heat-related diseases during the study period. Classified by age. 12.5%, 43.4%, and 44.1% of cases involved children, adults, and elderly people, respectively. The analysis showed that the number of cases of emergency transportation for people with heat-related diseases （per 100,000 people; corresponding to a 1.0~C increase in the daily maximum temperature） was 0.016-0.106 among children （24.9-169.9 children required emergency transportation for heat-related diseases）, from 0.013 to 0.059 among adults （19.8 98.2 adults required emergency transportation）, and from 0.045 to 0.159 among elderly persons （30.0 145.4 elderly people required emergency transportation）. The risk was highest for elderly persons, followed by children and finally adults. Cases of emergency transportation due to heat-related illness increased by 2.4-8.9 times when the daily maximum temperature was approximately 1.5℃ above the mean daily maximum temperature. In fact, the daily maximum temperature had a larger effect than the daily relative humidity level oil emergency transportation for people with heat-related diseases. Conclusion： Public health organizations and health-care services should support elderly people and children, two high-risk groups for heat-related diseases.

Multi-Class Sentiment Analysis on Twitter: Classification Performance and Challenges

Sentiment analysis refers to the automatic collection,aggregation,and classification of data collected online into different emotion *** most of the work related to sentiment analysis of texts focuses on the binary and ternary classification of these data,the task of multi-class classification has received less ***-class classification has always been a challenging task given the complexity of natural languages and the difficulty of understanding and mathematically"quantifying"how humans express their *** this paper,we study the task of multi-class classification of online posts of Twitter users,and show how far it is possible to go with the classification,and the limitations and difficulties of this *** proposed approach of multi-class classification achieves an accuracy of 60.2%for 7 different sentiment classes which,compared to an accuracy of 81.3%for binary classification,emphasizes the effect of having multiple classes on the classification ***,we propose a novel model to represent the different sentiments and show how this model helps to understand how sentiments are *** model is then used to analyze the challenges that multi-class classification presents and to highlight possible future enhancements to multi-class classification accuracy.
