Risk factors for postoperative stoma outlet obstruction in ulcerativecolitis

BACKGROUND Current medical treatments can achieve remission of ulcerative colitis (UC).Surgery is required when potent drug treatment is ineffective or when coloncancer or high-grade dysplasia develops. The standard procedure is restorativeproctocolectomy (RPC) with ileal pouch-anal anastomosis, commonly performedas two- or three-stage RPC with diverting ileostomy. Postoperative stoma outletobstruction (SOO) is frequent, but the causes are not well *** To identify the risk factors for SOO after stoma surgery in patients with *** We retrospectively reviewed the files of 148 consecutive UC patients whounderwent surgery with stoma construction. SOO was defined as small bowelobstruction symptoms and intestinal dilatation just below the penetrating part ofthe stoma on computed tomography. Patients were divided into two groups:Those who developed SOO within 30 d after surgery and those who did *** characteristics, intraoperative parameters, the stoma site, and rectusabdominis muscle thickness were collected. Moreover, we identified the patientswho repeatedly developed SOO. Univariate and multivariate analyses wereperformed to identify risk factors for SOO and recurring *** Eighty-nine patients who underwent two-stage RPC were included betweenJanuary 2008 and March 2020. Postoperatively, SOO occurred in 25 (16.9%)patients after a median time of 9 d (range 2-26). Compared to patients withoutSOO, patients with SOO had a significantly higher rate of malignant tumors ordysplasia (36.0% vs 17.1%, P = 0.032), lower total glucocorticoid dose one monthbefore surgery (0 mg vs 0 mg, P = 0.026), higher preoperative total protein level(6.8 g/dL vs 6.3 g/dL, P = 0.048), higher rate of loop ileostomy (88.0% vs 55.3%, P= 0.002), and higher maximum stoma drainage volume (2300 mL vs 1690 mL, P =0.004). A Quantitative Factorial Component Analysis to Investigate the Recent Changes of Japan’s Weight-Based Food Self-Sufficiency Ratio

We investigate the weight-based food self-sufficiency ratio (WSSR) for Japan over a 50-year period (1961-2011) by applying factorial component analysis technique in order to measure the changes of the WSSR quantitatively. Quantitative data analysis is employed to determine the drivers of those changes. Numerical results show that Japan experienced a drastic decline in its food self-sufficiency ratio (FSSR) during the above period. The factorial component analysis shows that such a decline was caused by the changes in the FSSR of the food groups/items, not in the quantity of the food supply. A number of characteristics of those changes are presented and a list of major food groups that have major impacts on the changes is constructed. The findings in this paper reiterate the alarming food security problem in Japan and provide clear insight into the causes of this problem. The findings in this study pick up where previous studies have left off, aid the food-related policy-making process and identify new ideas for future food research.

Investigating Evacuation Trends and Restoration Processes from Serious Earthquake Damage in Japan

Japan has experienced many large-scale natural disasters, such as earthquakes, typhoons accompanied by heavy rain, and landslides. Based on data for the damage caused by four recent major earthquakes in Japan, we investigate the trends in the number of evacuees and evacuation centres after these disasters as well as the restoration processes for public utilities such as electricity, gas, water, and communication lines. We compare the restoration speeds and trends of the damaged infrastructure systems. We also propose various mathematical models to approximate the recovery trends using both evacuee-related data and damage recovery process data. These results can be used to design natural disaster mitigation policies not only in Japan, but also in other countries. The results of various statistical data analyses and mathematical modelling techniques are applied to provide policy suggestions.

A simplified simulation method based on the FDTD technique that can handle active devices is proposed. This method well suits the electrical crosstalk analysis of multi-channel integrated, opto-electronic mixed modules. We apply this method to an 8-channel integrated super-compact high-sensitivity optical module. The results show good agreement between simulations and measurements.

Greater complexity is involved in the transient pressure analysis of horizontal oil wells in contrast to vertical wells,as the horizontal wells are considered entirely horizontal and parallel with the top and underneath boundaries of the oil ***,there is an essential need to estimate productivity of horizontal wells accurately to examine the effectiveness of a horizontal well in terms of technical and economic *** this work,novel and rigorous methods based on two different types of intelligent approaches including the artificial neural network(ANN)linked to the particle swarm optimization(PSO)tool are developed to precisely forecast the productivity of horizontal wells under pseudo-steady-state *** was found that there is very good match between the modeling output and the real data taken from the literature,so that a very low average absolute error percentage is attained(e.g.,<0.82%).The developed techniques can be also incorporated in the numerical reservoir simulation packages for the purpose of accuracy improvement as well as better parametric sensitivity analysis.

The generation of functional retinal pigment epithelium （RPE） is of great therapeutic interest to the field of regenerative medicine and may provide possible cures for retinal degenerative diseases, including age-related macular degeneration （AMD）. Although RPE cells can be produced from either embryonic stem cells or induced pluripotent stem cells, direct cell reprogramming driven by lineage-determining transcription factors provides an immediate route to their generation. By monitoring a human RPE specific Bestl：：GFP reporter, we report the conversion of human fibroblasts into RPE lineage using defined sets of transcription factors. We found that Bestl：：GFP positive cells formed colonies and exhibited morphological and molecular features of early stage RPE cells. Moreover, they were able to obtain pigmen- tation upon activation of Retinoic acid （RA） and Sonic Hedgehog （SHH） signaling pathways. Our study not only established an ideal platform to investigate the tran- scriptional network regulating the RPE cell fate deter- mination, but also provided an alternative strategy to generate functional RPE cells that complement the useof pluripotent stem cells for disease modeling, drug screening, and cell therapy of retinal degeneration.

Statistical Data Analyses on Aircraft Accidents in Japan: Occurrences, Causes and Countermeasures

We investigate the major characteristics of the occurrences, causes of and counter measures for aircraft accidents in Japan. We apply statistical data analysis and mathematical modeling techniques to determine the relations among economic growth, aviation demand, the frequency of aircraft/helicopter accidents, the major characteristics of the occurrence intervals of accidents, and the number of fatalities due to accidents. The statistical model analysis suggests that the occurrence intervals of accidents and the number of fatalities can be explained by probability distributions such as the exponential distribution and the negative binomial distribution, respectively. We show that countermeasures for preventing accidents have been developed in every aircraft model, and thus they have contributed to a significant decrease in the number of accidents in the last three decades. We find that the major cause of accidents involving large airplanes has been weather, while accidents involving small airplanes and helicopters are mainly due to the pilot error. We also discover that, with respect to accidents mainly due to pilot error, there is a significant decrease in the number of accidents due to the aging of airplanes, whereas the number of accidents due to weather has barely declined. We further determine that accidents involving small and large airplanes mostly occur during takeoff and landing, whereas those involving helicopters are most likely to happen during flight. In order to decrease the number of accidents, i) enhancing safety and security by further developing technologies for aircraft, airports and air control radars, ii) establishing and improving training methods for crew including pilots, mechanics and traffic controllers, iii) tightening public rules, and iv) strengthening efforts made by individual aviation-related companies are absolutely necessary.

Discontinuous chains of ultramafic rock bodies form part of the 3800–3700 Ma Isua Supracrustal Belt(ISB),hosted in the Itsaq Gneiss Complex of southwestern *** bodies are among the world’s oldest outcrops of ultramafic rocks and hence an invaluable geologic *** rocks from Lens B in the northwestern limb of ISB show characteristics of several stages of serpentinization and deserpentinization forming prograde and retrograde mineral ***-rich humite-group minerals such as titanian chondrodite(Ti-Chn)and titanian clinohumite(Ti-Chu)often occur as accessory phases in the metamorphosed ultramafic *** Ti-rich humite minerals are associated with metamorphic *** host olivine is highly forsteritic(Fo96-98)with variable Mn O and Ni O *** concentrations of the rare-earth elements(REE)and high-field strength elements(HFSE)of the metamorphic olivine are higher than typical mantle *** textural and chemical characteristics of the olivine indicate metamorphic origin as a result of deserpentinization of a serpentinized ultramafic protolith rather than primary assemblage reflecting mantle residues from high-degrees of partial *** close association of olivine,antigorite and intergrown Ti-Chn and Ti-Chu suggests pressure condition between$1.3–2.6 GPa within the antigorite stability field(<700°C).The overall petrological and geochemical features of Lens B ultramafic body within the Eoarchean ISB indicate that these are allochthonous ultramafic rocks that recorded serpentine dehydration at relatively lower temperature and reached eclogite facies condition during their complex metamorphic history similar to exhumed UHP ultramafic rocks in modern subduction zone channels.

Investigating Economic Growth, Trade Issues and Future Energy Strategies for Central Asian Countries

We examine the economic growth, energy development policies and strategies for the CAC from various perspectives. We apply statistical data analysis techniques and mathematical modelling methodologies focusing upon regression model analysis in order to deal with the economic and energy related data during the period 1990-2014, and to investigate the relationship among economic growth, energy production, and the trade of energy resources in order to find future desirable policies and strategies for the CAC. Findings show that energy production growth would bring statistically significant positive impact on GDP growth in fossil-fuel rich Kazakhstan, Uzbekistan, and Turkmenistan while we also find negative impact of GDP growth on the trade balance of the CAC except for Turkmenistan. Another finding follows that the foreign direct investment has a significant influence on the trade balance in the cases of Uzbekistan and Turkmenistan, which institute import substitution policies right from their initial years of independence. Based on these quantitative investigations on economy, energy, and trades we propose future energy strategies for the CAC, stressing the importance of diversification of economies.
