Morbidity and mortality of coagulase-negative staphylococcal sepsis in very-low-birth-weight infants

background Coagulase-negative staphylococci (CoNS) are the most common cause of late-onset sepsis in the neonatal intensive care unit (NICU) and usually require vancomycin treatment. Our objective was to determine whether CoNS are associated with neonatal morbidity and mortality. Methods This was a retrospective cohort study of very-low-birth-weight (VLbW, ≤ 1500 g) infants from 1989 to 2015. Exclusion criteria were major congenital anomaly or death within 72 h. CoNS was considered a pathogen if recovered from ≥ 2 cultures, or 1 culture if treated for ≥ 5 days and signs of sepsis were present. Logistic regression was used to exam-ine factors associated with morbidity and mortality. Results Of 2242 VLbW infants, 285 (12.7%) had late-onset sepsis. CoNS (125, 44%),Staphylococcus aureus (52, 18%), and Escherichia coli (36, 13%) were the most commonly recovered organisms. In multivariate analysis, CoNS sepsis was not associated with mortality (OR 0.6 (95% CI 0.2-2.6)), but sepsis with other organisms was (OR 4.5 (95% CI 2.6-8.0))CoNS sepsis was associated with longer hospitalization but not risk for bronchopulmonary dysplasia, intraventricular hem-orrhage, or retinopathy of prematurity. Conclusion CoNS sepsis was not associated with mortality or morbidities other than length of stay. These findings support vancomycin-reduction strategies in the NICU.

Lunasin protease inhibitor concentrate(LPIC)is a novel combination of soy bioactive peptide lunasin,Kunitz and bowman-birk protease *** reported anti-inflammatory and anticancer properties of each one of them suggest LPIC as a promising candidate for the treatment of infl ammatory-related *** objective was to assess the in vivo anti-infl ammatory properties of ***,an in vitro test was performed in lipopolysaccharide(LPS)-activated RAW264.7 murine macrophages by measuring the production of nitric oxide(NO),interleukin-6(IL-6),and tumor necrosis factorα(TNF-α)as infl ammatory *** the in vivo model,ulcerative colitis(UC)was induced in mice via oral administration of dextran sodium sulfate(DSS).LPIC treatment was performed via daily intraperitoneal injection of 50 mg/kg body *** weight,visible blood in stool and stool consistency were scored daily as macroscopic indicators of disease *** blood was evaluated by the presence of hemoglobin in stool every third *** length,caecum weight,colonic myeloperoxidase activity(MPO),presence of pro-inflammatory cytokines in blood and colon,changes in the architecture,and expression of inducible nitric oxide synthase(i NOS)in colonic tissue were *** vitro,LPIC induced production of NO and maintained cytokine levels in comparison to activated untreated *** vivo,LPIC increased colonic bleeding and did not improve macroscopic markers of the disease,but reduced colonic IL-1βand IL-6,decreased systemic circulation of TNF-α,attenuated neutrophils infi ltration and i NOS expression in colonic tissue,and diminished the damage in colonic *** results suggest that combinations of peptides in LPIC may counteract the antiinfl ammatory properties in vitro;while in vivo,LPIC can signifi cantly reduce the histopathological damage,hence is a possible therapeutic strategy to attenuate UC.

Nowadays,thermal condition and solute field are considered as the potential dominant factors controlling competitive grain growth during directional solidification ***,the controlling modes and critical conditions of competitive grain growth have been drastically debated over the past two *** this work,thermal condition and solute field are combined to study the competitive grain growth in the converging case by experimental observation and numerical simulation of bicrystal *** find the competitive grain growth is controlled by the cooperative effect of thermal condition and solute field,and the controlling modes are related to the bicrystal misorientation between favorably and unfavorably oriented *** the unfavorably oriented grain is low misoriented,unfavorably oriented grain dominates grain selection,and the competitive grain growth performs as solute field ***,with the increase of unfavorably oriented grain’s misorientation,the grain selection converts into favorably oriented grain domination,and the competitive grain growth changes to thermal condition *** explain these abnormal transformation phenomena,we propose a misorientation dependent thermal condition-solute field cooperative domination model and identify the critical conditions by a critical misorientation(θ_(cm)).According to dynamic equation of dendrite growth,we calculate the critical misorientationθ;to prove this *** theoretical calculation results agree well with the experimental results.

The mechanisms by which titanium carbide （TiC） improves the properties of tungsten carbide （WC） coatings deposited on duplex stainless steels using laser particle injection technique were investigated. The relationships between laser process parameters and the synthesized composite were studied. The morphologies and microstructures of the feedstock powders and composite coatings were characterized using scanning electron microscopy （SEM） and energy dispersive X-ray spectroscopy （EDX） techniques. Surface hardness of the composite layers was determined using the Vickers microhardness tester while its corrosion behaviour in 3.5%NaCl solution was investigated by potentiodynamic polarization curve measurement method. As a result of the laser treatment, microstructures characterized by hard ceramic particles with strong bonding to substrate were formed on the surface layer of the steels. The addition of TiC to WC resulted in microstructures free from cracks, pores and intermetallics which could be detrimental to the properties of the composites. High microhardness was observed and most of the coatings shifted the corrosion potential to more noble values with the pseudo-passive curve.

background:Ascites is a costly,morbid complication of *** a low-sodium diet is central to the clinical management of ascites,its efficacy is limited by poor *** aimed to determine the feasibility and impact of lowsodium medically tailored meals(MTM)***:We enrolled 40 persons with cirrhosis and ascites at the time of a paracentesis in a 12-week,1:1 randomized trial of standard of care(SOC)(low-sodiumdiet educational handout)or MTM with2,100 kcal,and>80 g of protein including a nocturnal protein *** determined the proportion of eligible candidates recruited and adherence to *** primary outcome was the number of paracenteses performed during weeks *** also collected ascites-specific quality-of-life(ASI-7)***:The median age of the enrolled subjects was 54(IQR,47-63)years,46%were female,with median MELD-Na 18(IQR,11-23)and albumin 2.7(IQR,2.5-3.3)g/*** baseline,subjects reported a median of two(IQR,1-3)paracenteses in the prior 4 *** to the meal schedule was excellent save for when hospitalizations *** 12 weeks,patients in the MTM arm required fewer paracenteses per week than those in the SOC group[median(IQR):0.34(0.14-0.54)vs 0.45(0.25-0.64)].During the trial,four(20%)SOC patients died,whereas two(10%)died and one(5%)was transplanted in the MTM ***-specific quality of life improved to a greater degree in the MTM arm compared to the SOC arm,by 25%(IQR,-11%to 61%)vs 13%(IQR,-28%to 54%).Conclusion:A trial of MTM for persons with ascites is feasible and potentially *** arms experienced benefits,highlighting the role for improved education and closer monitoring in this challenging condition.

In this paper,the effect of various electrochemical environments in the galena flotation is *** electrochemical environments consisted of a ball mill for grinding of galena ore and a Denver flotation cell for flotation of galena in the laboratory *** order to achieve the maximum recovery with sodium hyposulfite,the concentrations of 0.06 and 0.12 M of air and nitrogen gases have been used to control the pulp potential in the Denver flotation *** galena sample was from the ''Era mine'' which is located in the Kiyasar area,north of *** mine contains:Galena(PbS) at least 22%,Fluorite(CaF 2) 73.37%,Quartz(SiO 2) 2.54% and other minerals such as Cerussite(PbCO 3) and Kaolinite(Al 2 Si 2 O 5(OH) 4).Flotation of Galena was conducted in a 0.12 M of sodium hyposulfite *** was found that the amount of recovery by this process was 85% and 63% in the presence and absence of potassium amyl xanthate(KAX) and at an estimated potential of 280 to 350 mV respectively while the recovery was 70% and 52% at an approximate potential of 175 to 210 mV in water in the presence and absence of collector(KAX).

Digital forensics is the science of identifying, extracting, analyzing and presenting the digital evidence that has been stored in the digital devices. Various digital tools and techniques are being used to achieve this. Our paper explains forensic analysis steps in the storage media, hidden data analysis in the file system, network forensic methods and cyber crime data mining. This paper proposes a new tool which is the combination of digital forensic investigation and crime data mining. The proposed system is designed for finding motive, pattern of cyber attacks and counts of attacks types happened during a period. Hence the proposed tool enables the system administrators to minimize the system vulnerability.

A 9.25-m-long sediment core from bosten Lake, Xinjiang, provides detailed information about changes in the water budget and biological acticity over the last 8400 calendar years. The chronology is constructed from six AMS radiocarbon dates on the terrestrial plant remains. based on analyses of TOC, CO3, detrital compounds and biogenic SiO2, lake level fluctuations and periods of remarkably-negative water budget appeared at 8.4—8.2 cal ka, 7.38—7.25 cal ka, 5.7—5.5 cal ka, 3.7—3.4 cal ka and 3.3—2.9 cal ka, respectively. As they are in-phase with low lake levels at Sumxi Co and bangong Co in western Tibet Plateau and with paleolakes in Inner Mongolia, a climate-induced change to somewhat drier and warmer conditions is inferred. A further drop in lake level after 1320 AD of about 200 yr duration may be attributed to a negative water balance prior to the main phase of the Little Ice Age. Deep and stable lake phases of 1500 yr and 1800 yr duration at 7.2—5.7 cal ka and 5.5—3.7 cal ka coincide with maximum moisture during the Holocene Megathermal in China. The long term trend towards aridity since about 4.3 cal ka can clearly be recognised. The reduced water budget of bosten Lake from 640—1200 AD may be attributed to local effects.
