bACKGROUND Global education in psychiatry is heavily influenced by knowledge from Western,high-income countries,which obscures local voices and expertise.AIM To adapt a human simulation model to psychiatric education in a context that is specific to local languages and cultures.METHODS We conducted an observational study consisting of six human simulation sessions with standardized patients from two host countries,speaking their native languages,and following an adaptation of the co-constructive patient simulation(CCPS)model.As local faculty became increasingly familiar with the CCPS approach,they took on the role of facilitators—in their country’s native language.RESULTS Fifty-three learners participated:19 child and adolescent psychiatry trainees and 3 faculty members in Türkiye(as a group that met online during 3 consecutive months);and 24 trainees and 7 faculty in Israel(divided into 3 groups,in parallel in-person sessions during a single training day).Each of the six cases reflected local realities and clinical challenges,and was associated with specific learning goals identified by each case-writing trainee.CONCLUSION Human simulation has not been fully incorporated into psychiatric education:The creation of immersive clinical experiences and the strengthening of reflective practice are two areas ripe for development.Our adaptations of CCPS can also strengthen local and regional networks and psychiatric communities of practice.Finally,the model can help question and press against hegemonies in psychiatric training that overshadow local expertise.

A Retrospective Analysis of Intoxicated Patients Admitted to the Intensive Care Unit: A Single-Institution Assessment in the Post-Pandemic Period

background: The aim of this study was to evaluate the clinical and epidemiological characteristics of patients with acute intoxication admitted to the Intensive Care Unit (ICU). Material and Methods: An observational retrospective study was conducted on intoxicated patients who admitted to ICU between January 2022 and January 2024. Data were collected from the patients medical records. The demographic characteristics, causes of intoxications, clinical parameters, the mean stay in the ICU, treatment modalities and prognosis were recorded. Results: A total of 2875 critically ill patients were admitted during the study period, and 109 (3.79%) of them were acute intoxications. Their mean of ages was 38.09 ± 12.29. The female-to-male ratio was 1.37/1. Drugs were found to be the primary cause (62.39%) of intoxications, and analgesics, antidepressants, and antibiotics were the most frequent agents. Suicidal attempts were present in 66 patients, most of them were female (62.13%) and between 17 - 24 years (40.91%). The other common causes of intoxications were carbon monoxide (CO) (22.02%), methyl/ethyl alcohol (8.26%) and mushroom (5.50%). The mean stay in the ICU was 2.69 ± 0.89 days. Mechanical ventilation was applied to 10 of our patients. Renal replacement therapy was required in 6 patients. Despite all treatments, 6 of our patients died, and we found the mortality rate to be 5.50%. Conclusion: Intoxications were more frequent in young female patients and drugs were the most common cause with suicidal intent. Unfortunately, CO poisoning continues to be a very important problem in our city. These findings provided significant information about the characteristics of intoxications in Karabuk.

In this study, a series of Gd^(3+)-doped mixed Ni-Cu-Zn ferrites with composition of Zn_(0.5)Ni_(0.3)Cu_(0.2)Fe_(2-x)Gd_(x)O_(4)(x = 0, 0.025, 0.05, 0.075, 0.1) was prepared using self-ignition sol-gel method. The prepared nanoparticles with an average size ranging from 22 to 26 nm show a single-phase cubic structure belonging to the spinel matrix. A rise in the Gd^(3+)concentration leads to an increase in crystallite size and lattice parameter. In Fourier transform infrared spectra, two main absorption bands belonging to the spinel structure are observed. The high-frequency bands(v_(1)) represent the tetrahedral complex, while the lowfrequency bands(v_(2)) signify the octahedral complex. The optical bandgap of the nanoferrites is found within the range of 2.91 to 2.41 eV, depending on their size. The magnetic characteristics of the material,such as saturation magnetization and coercivity are significantly altered with the concentration of Gd^(3+)in the solution. Using Rhodamine b(Rhb) as a model organic pollutant, an in-depth investigation of the photocatalytic activity of the compounds was carried out. The present outcomes show that adding an adequate amount of Gd^(3+)significantly enhances the number of hydroxyl radicals produced by the ferrite,in turn, increasing the photocatalytic activity of the material. Mechanism elucidated by scavenger studies reveals that ·OH and holes are the primary reactive radicals responsible for the degradation process.Prepared photocatalysts show an insignificant performance loss in five consecutive cycles. Thus, it is concluded that these photocatalysts are highly suitable for the remediation of dye-contaminated wastewater.

In recent years,many researchers focused on unsupervised learning for network anomaly detection in edge devices to identify attacks.The deployment of the unsupervised autoencoder model is computationally expensive in resource-constrained edge devices.This study proposes quantized autoencoder(QAE)model for intrusion detection systems to detect anomalies.QAE is an optimization model derived from autoencoders that incorporate pruning,clustering,and integer quantization techniques.Quantized autoencoder uint8(QAE-u8)and quantized autoencoder float16(QAE-f16)are two variants of QAE built to deploy computationally expensive Al models into Edge devices.First,we have generated a Real-Time Internet of Things 2022 dataset for normal and attack traffic.The autoencoder model operates on normal traffic during the training phase.The same model is then used to reconstruct anomaly traffic under the assumption that the reconstruction error(RE)of the anomaly will be high,which helps to identify the attacks.Furthermore,we study the performance of the autoencoders,QAE-u8,and QAE-f16 using accuracy,precision,recall,and F1 score through an extensive experimental study.We showed that QAE-u8 outperforms all other models with a reduction of 70.01%in average memory utilization,92.23%in memory size compression,and 27.94%in peak CPU utilization.Thus,the proposed QAE-u8 model is more suitable for deployment on resource-constrained IoT edge devices.

Gravity models given by higher-order scalar curvature corrections are believed to bear important consequences. Einstein-bel-Robinson (EbR) gravity with quartic curvature modification motivated Sajadi et al. to explore static spherically symmetric black hole solutions using perturbative methods. In this study, inspired by their work, we investigate AdS black hole shadows in EbR gravity and demonstrate how the gravity parameter alters the energy emission rate. Finally, we address the same problem in the presence of plasma, because the black holes are thought to be surrounded by a medium that changes the geodesic of photons.

The electrochemical CO_(2) reduction reaction(CO_(2)RR) to controllable chemicals is considered as a promising pathway to store intermittent renewable energy. Herein, a set of catalysts based on copper-nitrogendoped carbon xerogel(Cu-N-C) are successfully developed varying the copper amount and the nature of the copper precursor, for the efficient CO_(2)RR. The electrocatalytic performance of Cu-N-C materials is assessed by a rotating ring-disc electrode(RRDE), technique still rarely explored for CO_(2)RR. For comparison, products are also characterized by online gas chromatography in a H-cell. The as-synthesized Cu-NC catalysts are found to be active and highly CO selective at low overpotentials(from -0.6 to -0.8 V vs.RHE) in 0.1 M KHCO_(3), while H_(2) from the competitive water reduction appears at larger overpotentials(-0.9 V vs. RHE). The optimum copper acetate-derived catalyst containing Cu-N_(4) moieties exhibits a CO_(2)-to-CO turnover frequency of 997 h^(-1) at -0.9 V vs. RHE with a H_(2)/CO ratio of 1.8. These results demonstrate that RRDE configuration can be used as a feasible approach for identifying electrolysis products from CO_(2)RR.

The marine losses during the Permo-Triassic mass extinction were the worst ever experienced. All groups were badly affected, especially amongst the benthos(e.g. brachiopods, corals, bryozoans, foraminifers,ostracods). Planktonic populations underwent a fundamental change with eukaryotic algae being replaced by nitrogen-fixing bacteria, green-sulphur bacteria, sulphate-reducing bacteria and prasinophytes. Detailed studies of boundary sections, especially those in South China, have resolved the crisis to a ～55 kyr interval straddling the Permo-Triassic boundary. Many of the losses occur at the beginning and end of this interval painting a picture of a two-phase extinction. Improved knowledge of the extinction has been supported by numerous geochemical studies that allow diverse proposed extinction mechanisms to be studied. A transition from oxygenated to anoxic-euxinic conditions is seen in most sections globally, although the intensity and timing shows regional variability. Decreased ocean ventilation coincides with rapidly rising temperatures and many extinction scenarios attribute the losses to both anoxia and high temperatures.Other kill mechanisms include ocean acidification for which there is conflicting support from geochemical proxies and, even less likely, siltation(burial under a massive influx of terrigenous sediment) which lacks substantive sedimentological evidence. The ultimate driver of the catastrophic changes at the end of the Permian was likely Siberian Trap eruptions and their associated carbon dioxide emissions with consequences such as warming, ocean stagnation and acidification. Volcanic winter episodes stemming from Siberian volcanism have also been linked to the crisis, but the short-term nature of these episodes(rapid warming during the crisis makes this an unlikely cause. Anti-ganglioside antibodies are associated with delayed/poor clinical recovery in Guillain-barrèsyndrome,mostly related to halted axon regeneration.Cross-linking of cell surface gangliosides by anti-ganglioside antibodies triggers inhibition of nerve repair in in vitro and in vivo paradigms of axon regeneration.These effects involve the activation of the small GTPase Rho A/ROCK signaling pathways,which negatively modulate growth cone cytoskeleton,similarly to well stablished inhibitors of axon regeneration described so far.The aim of this work was to perform a proof of concept study to demonstrate the effectiveness of Y-27632,a selective pharmacological inhibitor of ROCK,in a mouse model of axon regeneration of peripheral nerves,where the passive immunization with a monoclonal antibody targeting gangliosides GD1a and GT1b was previously reported to exert a potent inhibitory effect on regeneration of both myelinated and unmyelinated fibers.Our results demonstrate a differential sensitivity of myelinated and unmyelinated axons to the pro-regenerative effect of Y-27632.Treatment with a total dosage of 9 mg/kg of Y-27632 resulted in a complete prevention of anti-GD1a/GT1b monoclonal antibody-mediated inhibition of axon regeneration of unmyelinated fibers to skin and the functional recovery of mechanical cutaneous sensitivity.In contrast,the same dose showed toxic effects on the regeneration of myelinated fibers.Interestingly,scale down of the dosage of Y-27632 to 5 mg/kg resulted in a significant although not complete recovery of regenerated myelinated axons exposed to anti-GD1a/GT1b monoclonal antibody in the absence of toxicity in animals exposed to only Y-27632.Overall,these findings confirm the in vivo participation of Rho A/ROCK signaling pathways in the molecular mechanisms associated with the inhibition of axon regeneration induced by anti-GD1a/GT1b monoclonal antibody.Our findings open the possibility of therapeutic pharmacological intervention targeting Rho A/Rock pathway in immune neuropathies associated wi

The recent identification of a neurodevelopmental disorder with cerebellar atrophy and motor dysfunction(NEDCAM)has resulted in an increased interest in GEMIN5,a multifunction RNA-binding protein.As the largest member of the survival motor neuron complex,GEMIN5 plays a key role in the biogenesis of small nuclear ribonucleoproteins while also exhibiting translational regulatory functions as an independent protein.Although many questions remain regarding both the pathogenesis and pathophysiology of this new disorder,considerable progress has been made in the brief time since its discovery.In this review,we examine GEMIN5 within the context of NEDCAM,focusing on the structure,function,and expression of the protein specifically in regard to the disorder itself.Additionally,we explore the current animal models of NEDCAM,as well as potential molecular pathways for treatment and future directions of study.This review provides a comprehensive overview of recent advances in our understanding of this unique member of the survival motor neuron complex.
