Background:This study aimed to determine the effect of different carbohydrate(CHO)doses on exercise capacity in patients with McArdle disease—the paradigm of“exercise intolerance”,characterized by complete muscle glycogen unavailability—and to determine whether higher exogenous glucose levels affect metabolic responses at the McArdle muscle cell(in vitro)***:Patients with McArdle disease(n=8)and healthy controls(n=9)underwent a 12-min submaximal cycling constant-load bout followed by a maximal ramp test 15 min after ingesting a non-caloric *** a randomized,double-blinded,cross-over design,patients repeated the tests after consuming either 75 g or 150 g of CHO(glucose:fructose=2:1).Cardiorespiratory,biochemical,perceptual,and electromyographic(EMG)variables were ***,glucose uptake and lactate appearance were studied in vitro in wild-type and McArdle mouse myotubes cultured with increasing glucose concentrations(0.35,1.00,4.50,and 10.00 g/L).Results:Compared with controls,patients showed the“classical”second-wind phenomenon(after prior disproportionate tachycardia,myalgia,and excess electromyographic activity during submaximal exercise,all p<0.05)and an impaired endurance exercise capacity(-51%ventilatory threshold and55%peak power output,both p<0.001).Regardless of the CHO dose(p<0.05 for both doses compared with the placebo),CHO intake increased blood glucose and lactate levels,decreased fat oxidation rates,and attenuated the second wind in the ***,only the higher dose increased ventilatory threshold(+27%,p=0.010)and peak power output(+18%,p=0.007).In vitro analyses revealed no differences in lactate levels across glucose concentrations in wild-type myotubes,whereas a doseresponse effect was observed in McArdle ***:CHO intake exerts beneficial effects on exercise capacity in McArdle disease,a condition associated with total muscle glycogen *** of these benefits are dose dependent.

Performance assessment of two-dimensional hydraulic models for generation of flood inundation maps in mountain river basins

Hydraulic models for the generation of flood inundation maps are not commonly applied in mountain river basins because of the difficulty in modeling the hydraulic behavior and the complex topography. This paper presents a comparative analysis of the performance of four twodimensional hydraulic models (HEC-RAS 2D, Iber 2D, Flood Modeller 2D, and PCSWMM 2D) with respect to the generation of flood inundation maps. The study area covers a 5-km reach of the Santa B-arbara River located in the Ecuadorian Andes, at 2330 masl, in Gualaceo. The model's performance was evaluated based on the water surface elevation and flood extent, in terms of the mean absolute difference and measure of fit. The analysis revealed that, for a given case, Iber 2D has the best performance in simulating the water level and inundation for flood events with 20- and 50-year return periods, respectively, followed by Flood Modeller 2D, HEC-RAS 2D, and PCSWMM 2D in terms of their performance. Grid resolution, the way in which hydraulic structures are mimicked, the model code, and the default value of the parameters are considered the main sources of prediction uncertainty.

Effects of organic chromium supplementation to finishing lambs diet on growth performance, carcass characteristics and meat quality

The objective of this study was to evaluate supplemental organic chromium（Cr） to finishing lambs on growth performance, carcass characteristics, and meat quality. Eighteen Suffolk lambs（age（4.5±0.2） mon;（25.8±3.6） kg body weight（BW）） were randomly assigned to three levels of supplemental organic Cr（0.0, 0.2 and 0.4 mg kg^–1 dry matter（DM）） in a complete random design. Growth performance was evaluated for 70 d, and then lambs were slaughtered to study carcass characteristics and chemical composition of meat. Orthogonal contrasts were performed（contrast one-average level 0.2 ppm Cr vs. average level 0.4 ppm Cr; contrast two-level 0 vs. average levels（0.2＋0.4） ppm Cr）. Orthogonal polynomials were used to estimate the linear and quadratic effects of Cr concentrations. Growth and carcass performance were not affected by supplemental organic Cr. Muscle conformation and leg perimeter linearly increased（P〈0.05） as organic Cr level increased in the diet. Kidney fat decreased linearly（P〈0.05） as supplemental Cr increased. In Longissimus dorsi（LD）, the ash content decreased linearly, and shear force（kg cm^–2） increased（P〈0.05） as organic Cr level increased in the diet. It is concluded that organic Cr did not affect growth performance, but it improved positively the muscle conformation, reduced kidney fat, whereas in LD there was an increment in shear force in finishing carcass lambs.

Is Pulmonary Auscultation Alone Sufficient for Clinical Practice?

Objective: The clinical examination is the basis for the diagnosis and rational choice of complementary tests. The aim of the study was to evaluate the performance of auscultation of the chest for screening of disease and for predicting the presence of abnormalities in the other domains of the chest examination. Methods: Patients with COPD, atelectasis, pleural effusion, pneumonia and controls were evaluated by two examiners in the absence of any clinical information, initially only with pulmonary auscultation, and then in the other domains of chest examination. Results: 192 physical examinations were performed in 104 patients. An abnormal pulmonary auscultation had a sensitivity of 85.2%, positive predictive value (PPV) of 84.1%, positive likelihood reason (LR) of 1.53 (95% CI;1.16 to 2.01) and negative LR of 0.33 (95% CI;0.2 to 0.56) to identify the presence of any disease, and also a positive LR of 2.23 (95% CI;1.02 to 4.9) and a negative LR of 0.3 (95% CI;0.17 to 0.51) to predict additional abnormalities. A normal auscultation showed low accuracy to identify healthy individuals, with sensitivity, specificity, NPV and PPV, respectively, of 44%, 43%, 41% and 46%. The agreement between the examiners considering normal versus abnormal findings showed kappa = 0.76 for any changes in the physical examination present (p Conclusions: Auscultation of the chest alone, may not be a sufficient strategy to track diseases or establish whether continuity of the examination is necessary or not.
