Endoscopic treatment of biliopancreatic pathology in patients with Whipple's pancreaticoduodenectomy surgical variants: Lessons learned from single-balloon enteroscopy-assisted ERCP

background: Endoscopic treatment of biliopancreatic pathology is challenging due to surgically altered anatomy after Whipple's pancreaticoduodenectomy. This study aimed to evaluate the feasibility and safety of single-balloon enteroscopy-assisted endoscopic retrograde cholangiopancreatography(SbE-ERCP) to treat biliopancreatic pathology in patients with Whipple's pancreaticoduodenectomy surgical variants. Methods: We retrospectively analyzed 106 SbE-ERCP procedures in 46 patients with Whipple's variants. Technical and clinical success rates and adverse events were evaluated. Results: biliary SbE-ERCP was performed in 34 patients and pancreatic SbE-ERCP in 17, including 5 with both indications. From a total of 106 SbE-ERCP procedures, 76 were biliary indication with technical success rate of 68/76(90%) procedures and clinical success rate of 30/34(88%) patients. Mild adverse event rate was 8/76(11%), without serious adverse events. From a total of 106 SbE-ERCP procedures, 30 were pancreatic indication with technical success rate of 24/30(80%) procedures( P = 0.194 vs. biliary SbEERCP) and clinical success rate of 11/17(65%) patients( P = 0.016 vs. biliary SbE-ERCP). Mild adverse event rate was 6/30(20%)( P = 0.194 vs. biliary SbE-ERCP), without serious adverse events. After SbE-ERCP failure, endoscopic ultrasound-guided drainage, percutaneous drainage and redo surgery were alternative therapeutic options. Conclusions: biliopancreatic pathology after Whipple's pancreaticoduodenectomy variants can be treated using SbE-ERCP without serious adverse events. Technical and clinical success rates are high for biliary indications, whereas clinical success rate of pancreatic indications is significantly lower. SbE-ERCP can be considered as first-line treatment option in this patient group with surgically altered anatomy.

Herein we study the economic performance of hydrochar and synthetic natural gas coproduction from olive tree *** process entails a combination of hydrothermal carbonization and *** a previous work,we evidenced that standalone hydrochar production via HTC results ***,we propose a step forward on the process design by implementing a methanation,adding value to the gas effluent in an attempt to boost the overall process techno-economic *** different plant capacities were analyzed(312.5,625 and 1250 kg/hr).The baseline scenarios showed that,under the current circumstances,our circular economy strategy in *** analysis of the revenues shows that hydrochar selling price have a high impact on NPV and subsidies for renewable coal production could help to boost the profitability of the *** the contrary,the analysis for natural gas prices reveals that prices 8 times higher than the current ones in Spain must be achieved to reach *** seems unlikely even under the presence of a strong subsidy *** costs analysis suggests that a remarkable electricity cost reduction or electricity consumption of the HTC stage could be a potential strategy to reach profitability ***,significant reduction of green hydrogen production costs is deemed instrumental to improve the economic performance of the *** results show the formidable techno-economic challenge that our society faces in the path towards circular economy societies.

Functional recovery from central nervous system(CNS)trauma depends not only on axon regeneration or compensatory sprouting of uninjured fibers but also on the ability of newly grown axons to establish functional synapses with appropriate *** several studies have successfully promoted long-distance axonal regeneration in distinct CNS injury models,none of them have resulted in a viable therapeutic approach for patient recovery.A possible reason may be the lack of new synaptogenesis for reestablishing the circuitry lost after ***,we discuss how our understanding of the mechanisms that instruct synapse formation in the injured nervous system may contribute to the design of new strategies to promote functional restoration in traumatic CNS disorders.

The parasitic hydrogen evolution reaction(HER)in the negative half-cell of vanadium redox flow batteries(VRFbs)causes severe efficiency ***,a deeper understanding of this process and the accompanying bubble formation is *** benchmarking study locally analyzes the bubble distribution in thick,porous electrodes for the first time using deep learning-based image segmentation of synchrotron X-ray *** large three-dimensional data set was processed precisely in less than one minute while minimizing human errors and pointing out areas of increased HER activity in *** study systematically varies the electrode potential and material,concluding that more negative electrode potentials of-200 m V *** hydrogen electrode(RHE)and lower cause more substantial bubble formation,resulting in bubble fractions of around 15%–20%in carbon felt ***,the bubble fractions stay only around 2%in an electrode combining carbon felt and carbon *** detected areas with high HER activity,such as the border subregion with more than 30%bubble fraction in carbon felt electrodes,the cutting edges,and preferential spots in the electrode bulk,are potential-independent and suggest that larger electrodes with a higher bulk-to-border ratio might reduce HER-related performance *** described combination of electrochemical measurements,local X-ray microtomography,AI-based segmentation,and 3D morphometric analysis is a powerful and novel approach for local bubble analysis in three-dimensional porous electrodes,providing an essential toolkit for a broad community working on bubble-generating electrochemical systems.

The study of the propagation of infectious diseases in urban centers finds a close connection with their population's social characteristics and *** work performs a spatial analysis of dengue cases in urban centers based on the basic reproduction numbers,R0,and incidence by planning areas(PAs),as well as their correlations with the Human Development Index(HDI)and the number of *** analyzed dengue epidemics in 2002 at two brazilian urban centers,belo Horizonte(bH)and Rio de Janeiro(RJ),using PAs as spatial *** results reveal heterogeneous spatial scenarios for both cities,with very weak correlations between R0 and both the number of trips and the HDI;in bH,the values of R0 show a less spatial heterogeneous pattern than in *** bH,there are moderate correlations between incidence and both the number of trips and the HDI;meanwhile,they weakly correlate for ***,the absence of strong correlations between the considered measures indicates that the transmission process should be treated considering the city as a whole.
