BAcKGRoUND: Surgery for intracranial aneurysm often results in postoperative neurologic deficits. We conducted a randomized trial at 30 centers to determine whether intraoperative cooling during open craniotomy would improve the outcome among patients with acute aneurysmal subarachnoid hemorrhage. METHoDS: A total of 1001 patients with a preoperative World Federation of Neurological Surgeons score of I, II, or III (“ good- grade patients"), who had had a subarachnoid hemorrhage no more than 14 days before planned surgical aneurysm clipping, were randomly assigned to intraoperative hypothermia (target temperature, 33° c, with the use of surface cooling techniques) or normothermia (target temperature, 36.5° c). Patients were followed closely postoperatively and examined approximately 90 days after surgery, at which time a Glasgow outcome Score was assigned. RESULTS: There were no significant differences between the group assigned to intraoperative hypothermia and the group assigned to normothermia in the duration of stay in the intensive care unit, the total length of hospitalization, the rates of death at follow- up (6 percent in both groups), or the destination at discharge (home or another hospital, among surviving patients). At the final follow- up, 329 of 499 patients in the hypothermia group had a Glasgow outcome Score of 1 (good outcome), as compared with 314 of 501 patients in the normothermia group (66 percent vs. 63 percent; odds ratio, 1.14; 95 percent confidence interval, 0.88 to 1.48; P=0.32). Postoperative bacteremia was more common in the hypothermia group than in the normothermia group (5 percent vs. 3 percent, P=0.05). coNcLUSIoNS: Intraoperative hypothermia did not improve the neurologic outcome after craniotomy among good- grade patients with aneurysmal subarachnoid hemorrhage.

Metabolic engineering strategies have been successfully implemented to improve the production of isobutanol,a next-generation biofuel,in Saccharomyces ***,we explore how two of these strategies,pathway re-localization and redox cofactor-balancing,affect the performance and physiology of isobutanol producing *** equipped yeast with isobutanol cassettes which had either a mitochondrial or cytosolic localized isobutanol pathway and used either a redox-imbalanced(NADPH-dependent)or redox-balanced(NADH-dependent)ketol-acid reductoisomerase *** then conducted transcriptomic,proteomic and metabolomic analyses to elucidate molecular differences between the engineered *** localization had a large effect on isobutanol production with the strain expressing the mitochondrial-localized enzymes producing 3.8-fold more isobutanol than strains expressing the cytosolic ***-balancing did not improve isobutanol titers and instead the strain with the redox-imbalanced pathway produced 1.5-fold more isobutanol than the balanced version,albeit at low overall pathway *** genomic analyses suggested that the poor performances of the cytosolic pathway strains were in part due to a shortage in cytosolic Fe-S clusters,which are required cofactors for the dihydroxyacid dehydratase *** then demonstrated that this cofactor limitation may be partially recovered by disrupting iron homeostasis with a fra2 mutation,thereby increasing cellular iron *** resulting isobutanol titer of the fra2 null strain harboring a cytosolic-localized isobutanol pathway outperformed the strain with the mitochondrial-localized pathway by 1.3-fold,demonstrating that both localizations can support flux to isobutanol.

obj.ctive:To better evaluate tertiary Gleason pattern reporting and to evaluate the impact of tertiary Gleason pattern 5(TP5)on prostate cancer pathological features and biochemical recurrence at our large single ***:We retrospectively reviewed 1962 patients who underwent radical prostatectomy(RP)for prostate cancer;TP5 was reported in 159 cases(8.1%).Men with Gleason score(GS)7 and GS 8 disease were divided into subgroups with and without TP5,and histopathological features were *** analyses were conducted to assess the impact on TP5 on biochemical-free survival(BFS).Results:Tumors possessing GS 3+4 with TP5 were more likely to exhibit extraprostatic extension(EPE)and had a larger tumor diameter(TD)than GS 3+4 *** 3+4 with TP5 was also associated with positive surgical margins(SM),seminal vesicle involvement(SVI),and higher pre-operative prostate-specific antigen(PSA)values,but without statistical *** 4+3 with TP5 more commonly presented with EPE,positive SM,SVI,and greater TD and pre-operative PSA level than GS 4+3 *** multivariate analysis,Gleason score,EPE,and TP5 were overall independent risk factors for PSA recurrence in this ***,GS 4+3 with TP5 was associated with shorter time to recurrence versus GS 4+3 ***:our results emphasize the importance of TP5 and suggest that criteria for tertiary pattern reporting in prostate cancer should be *** studies are needed to evaluate the role of tertiary patterns in prognostic models.

Purpose The aim of this study is to illustrate a new method and surgical technique in the treatment of j.fferson fractures which preserves the motion of the upper cervical *** and methods A retrospective review was performed of twelve patients with unstable j.fferson fractures treated with transoral osteosynthesis of cI between j.ly 2008 and December 2011.A steel plate and c1 lateral mass screw fixation were placed to repair the unstable j.fferson *** study group included eight males and four females with the average age of 33 years(range,23-62 years).Results The patients were followed up for between 12 and 28 months after surgery(average 16 months).Range of motion of the cervical spine was approximately physiologic:average flexion 35°(range 28°-40°),average extension 42°(range 30°-48°).Lateral bending to the right and left averaged 30° and 28°,respectively(range,12°-36°,14°-32°,respectively).The average postoperative rotation of the atlantoaxial j.int,evaluated by functional cT scan,was 60°(range 35°-72°).Total average lateral displacement of the lateral masses was 7.0 mm before surgery(range,5-12 mm) which improved to 3.5 mm after surgery(range 1-6.5 mm).The total average difference of the atlanto-dens interval in flexion-extension after surgery was 1.0 mm(range 1-3 mm).conclusions Transoral osteosynthesis of the anterior ring using c1 lateral masses screws is a novel technique for treating unstable j.fferson fractures which allows maintenance of rotation at the c1-c2 j.int and restoration of congruency of the atlanto-occipital and atlantoaxial j.ints.
