Research on Using IMRT Plan for Preoperative Rectal Cancer Patients

Aims: Research the possibility of using IMRT for rectal cancer patients in preoperative radiotherapy. Methods and Material: The research object is the preoperative radiotherapy plan for rectal cancer patients. The research group made two plans (IMRT, 3DCRT) for each image series of 34 rectal cancer patients who have received preoperative radiotherapy in Hanoi Oncology Hospital;and then compared the dose distribution on PTV, bladder, intestine, femoral bones, the average MU, and QA results of two types of plan. Results: The 95% isodose line and 50% isodose of IMRT plan are closer than those of 3DCRT plan. The average dose of PTV in IMRT plan and 3DCRT plan are 5006 ± 23 cGy and 5036 ± 42 cGy, respectively. The HTCI and HI values of IMRT and 3D plan are 0.97 ± 0.026 and 5.37 ± 1.32;1.00 ± 0.003 and 7.08 ± 0.88. About the dose of organ at risk: The maximum dose, average dose on the right, left femoral head in the IMRT plan are less than those values in the 3DCRT plan (6.2 Gy, 6 Gy, 7.4 Gy, 9 Gy, respectively). The maximum dose and average dose on the bladder of the IMRT plan are smaller than those values of the 3DCRT plan (5.3 Gy, 1.5 times, respectively). The maximum dose and average dose of intestine in the IMRT plan was less than those values in the 3DCRT plan (4.3 Gy, 1.54 times, respectively). The MU number of IMRT plan is 1.5 times bigger than that of 3DCRT plan. Gamma index of IMRT plan is better than that of 3DCRT plan (99% compared with 97%). Conclusions: Using IMRT plan in preoperative radiotherapy for rectal cancer patients can still ensure covered PTV as well as the 3D PLAN. Furthermore, the dose of PTV in the IMRT plan is more uniform than those in the 3D plan, and the dose effect on the OAR surrounding PTV is much lower than when using the 3D plan. When IMRT plan were used to treat the preoperative rectal cancer patients, the LINAC took more time than when using 3DCRT plan.

Analyzing Planning Techniques for Whole Brain Radiotherapy

Aims: This study compares data between the Field in Field planning and Wedge planning techniques to figure out which technique has better dose coverage and distribution for PTV, and, if using FiF technique for whole brain treatment, how many beams will have better plan. Methods: 56 patients, who need to radiate whole brain with 30 Gy/10 fractions, have been selected for this study. Four plans have been made for each patient (FiF1—one subfield per field plan, FiF2—two subfields per field plan, FiF3—three subfields per field plan, and a Wedge plan). Results: The results of Field in Field plans including Compare dose distribution on the transverse CT slice, plan evaluation using DVH, number MU of plan, Dmax, HI, HTCI, DmaxPTV, DmeanPTV. Volume of PTV with the dose over 105% prescribed dose, dose of organ at risk, and Quality Assurance (QA) plan, are better than those of Wedge plan. Conclusions: Plans using Field in Field technique has better coverage, is more homogeneous in dose distribution than plan using Wedge technique. When using Field in Field technique for whole brain radiotherapy, using three subfields per field has better result than two subfields per field and one subfield per field.

In this paper,the authors investigate the differences between the dose results calculated with AAA(Anisotropic Analytical Algorithm)algorithm in Eclipse 13.0 treatment planning system,which has just commissioned,with the measured *** AAA algorithm in the Eclipse software is used to calculate the absolute dose,relative dose in profile line,and percent depth dose of some specific field size in Blue *** calculated dose results were compared with measured dose based on Technical Reports Series No.430 of International Atomic Energy Agency(IAEA No.430).The authors have used CC13 ion chamber,Famer Chamber,water phantom,and Omnipro software to measure relative,absolute doses of these fields *** AAA algorithm was also used to calculate the doses in IMRT(Intensity Modulated Radiotherapy)plan of fifty head and neck cancer *** IMRT plans were controlled quality in some points with plastic phantom,and famer chamber *** differences between calculated dose,and measured doses are in the limit of IAEA 430 *** results of the QA IMRT plan processing are fitting with ESTRO(European Society Therapeutic Radiation Oncology)Booklet No.9.

Phototropin-and photosynthesis-dependent mitochondrial positioning in Arabidopsis thaliana mesophyll cells^FA

Mitochondria are frequently observed in the vicinity of chloroplasts in photosynthesizing cells, and this association is considered necessary for their metabolic in-teractions. We previously reported that, in leaf palisade cel s of Arabidopsis thaliana, mitochondria exhibit blue-light-dependent redistribution together with chloroplasts, which conduct accumulation and avoidance responses under the control of blue-light receptor phototropins. In this study, precise motility analyses by fluorescent micro-scopy revealed that the individual mitochondria in palisade cel s, labeled with green fluorescent protein, exhibit typical stop-and-go movement. When exposed to blue light, the velocity of moving mitochondria increased in 30 min, whereas after 4 h, the frequency of stoppage of mi-tochondrial movement markedly increased. Using different mutant plants, we concluded that the presence of both phototropin1 and phototropin2 is necessary for the early acceleration of mitochondrial movement. On the contrary, the late enhancement of stoppage of mitochondrial movement occurs only in the presence of phototropin2 and only when intact photosynthesis takes place. A plasma-membrane ghost assay suggested that the stopped mi-tochondria are firmly adhered to chloroplasts. These re-sults indicate that the physical interaction between mi-tochondria and chloroplasts is cooperatively mediated by phototropin2- and photosynthesis-dependent signals. The present study might add novel regulatory mechanism for light-dependent plant organel e interactions.

Utilization and Satisfaction with HIV Services at 7 Provinces in Vietnam: A Cross-Sectional Study

The study was carried out to determine utilization and satisfaction with essential HIV services from HIV individuals and high-risk populations in 7 socioeconomic provinces nationwide. A cross-sectional approach was applied in the study. Data were collected through face-to-face interviews using self-report questionnaires. Findings from 2380 respondents indicated a high percentage of utilization as well as a high level of satisfaction, in particular among men who have sex with men (MSM) and female sex worker (FSW) sub-group. Needle, syringe, and condom programs had lower usage and satisfaction mean scores in comparison with other services. The proportion of people who used drugs (PWID) and participated in the needle and syringe program was 53.6%. HIV counseling and testing uptake proportion of overall respondents in the last 12 months was 82.2% while the rate of antiretroviral treatment among infected PWID was 74.9%. The level of satisfaction with treatment services was found to be higher than preventive interventions (p < 0.05). Analysis using univariable logistic regression suggested an association between some socioeconomic factors such as income, employment, educational attainment, and marital status with subjects’ satisfaction with several HIV services. Preventive services, especially needle, syringe and condom programs should be prioritized to scale up the coverage and effectiveness. Innovative approaches targeting at PWID should be promoted to increase their access to prevention programs as well as HIV care when needed.

Genetic variants of interferon regulatory factor 5 associated with chronic hepatitis B infection

AIM To investigate possible effects of IRF5 polymorphisms in the 3' UTR region of the IFR5 locus on susceptibilityto hepatitis B virus(HBV) infection and progression of liver diseases among clinically classified Vietnamese *** Four IFR5 SNPs(rs13242262 A/T, rs77416878 C/T, rs10488630 A/G, and rs2280714 T/C) were genotyped in clinically classified HBV patients [chronic hepatitis B(CHB). n = 99; liver cirrhosis(LC), n = 131; hepatocellular carcinoma(HCC), n = 149] and in 242 healthy controls by direct sequencing and Taq Man realtime PCR assays. RESULTS Comparing patients and controls, no significant association was observed for the four IFR5 variants. However, the alleles rs13242262 T and rs10488630 G contributed to an increased risk of liver cirrhosis(LC vs CHB: OR = 1.5, 95%CI: 1.1-2.3, adjusted P = 0.04; LC vs CHB: OR = 1.7, 95%CI: 1.1-2.6, adjusted P = 0.019). Haplotype IRF5*TCGT constructed from 4 SNPs was observed frequently in LC compared to CHB patients(OR = 2.1, 95%CI: 1.2-3.3, adjusted P = 0.008). Haplotype IRF5*TCAT occurred rather among CHB patients than in the other HBV patient groups(LC vs CHB: OR = 0.4, 95%CI: 0.2-0.8, adjusted P = 0.03; HCC vs CHB: OR = 0.3, 95%CI: 0.15-0.7, adjusted P = 0.003). The IRF5*TCAT haplotype was also associated with increased levels of ALT, AST and bilirubin. CONCLUSION Our study shows that IFR5 variants may contribute as a host factor in determining the pathogenesis in chronic HBV infections.

Hip Tuberculosis at Stage IV: Outcomes and Some Conditions for Total Hip Replacement

Background: Treatment of hip disorders during active hip tuberculosis has long been a controversial issue. Some authors have reported performing hip replacement with different strategies with very good outcomes. This study aimed to describe the surgical outcomes and necessary conditions for total hip replacement in active hip tuberculosis. Methods: We conducted a quasi-experimental study. The study enrolled 40 patients with 42 active tuberculosis hips at stage IV treated by total hip replacement from October 2016 to December 2019 at the National Lung Hospital. We followed up with the patients for at least 12 months, evaluated surgical outcomes, and investigated the factors associated with these outcomes by logistic regression analysis. Results: Surgical outcomes showed that 37 patients (88.1%) had excellent hip function, no abscesses, and no sinus tract formation. Four cases (9.5%) had sinus tract formations. One case (2.4%) had good hip function. Binary logistic regression models revealed that sinus tract formation was associated with preoperative tuberculosis infection syndrome. The average time to obtain antituberculosis drug treatment preoperatively was 4.6 weeks. Conclusion: Total hip replacement for active hip tuberculosis is a practical and promising treatment method. Surgeons should consider improving patients’ conditions before performing total hip replacement, administering antitubercular drugs, and arthrotomy to eliminate all abscesses, and decrease the risk of tuberculosis infection syndrome and the inflammatory response.

Acetabular Defects of Hip Tuberculosis: Recommended Classification and Reconstruction in Hip Arthroplasty

Background: This study aimed to propose a classification of acetabular defects in advanced-stage hip tuberculosis based on preoperative radiographs and intraoperative evaluation, thereby providing options for reconstructing each type of defect in hip arthroplasty. Materials and Methods: We conducted a prospective study with 45 patients with active hip tuberculosis at stage IV who were treated with a total hip replacement strategy. We collected the patients’ acetabular defect data from preoperative radiographs and intraoperative observations. The defects were classified into types based on defect severity and reconstructive complexity. The hip replacement outcomes were evaluated at least 12 months after the operation. Results: The classification system includes three types: type I: localized or extensive defects inside the acetabulum, not affecting the acetabular rim, medial wall, or columns. Type II: extensive defects inside the acetabulum, affecting the acetabular rim or (and) medial wall. Type III: extensive defects inside the acetabulum, causing the medial wall and the acetabular column to lose function. Each type has a corresponding reconstruction. The outcome includes that 41/45 patients had excellent results;3/45 patients had good results. The patients had good results due to nerve and muscle damage caused by tuberculosis not related to the acetabular defects. Conclusion: This is a simple, practical classification system of acetabular defects in hip tuberculosis. The outcome of hip reconstruction using this classification was excellent without any cases of dislocation or protrusion.
