Treatment of functional dyspepsia with sertraline:A double-blind randomized placebo-controlled pilot study

AIM:To evaluate sertraline,a selective serotonin reuptake inhibitor in the treatment of patients with functional ***:Consecutive tertiary hospital patients with a clinical diagnosis of functional dyspepsia(FD) according to the Rome Ⅱ criteria with a Hong kong dyspepsia index(HkDI) of greater than 16 were *** commenced enrolment prior to the inception of the Rome Ⅲ criteria for functional *** patients were ethnic Chinese,had a normal upper endoscopy and were Helicobacter pylori negative prior to *** patients were randomized to receive sertraline 50 mg or placebo daily for 8 *** symptom scores,quality of life,hospital anxiety and depression(HAD) scale and global symptom relief were evaluated before,during and after *** effects were monitored during and after ***:A total of 193 patients were randomized in the intention to treat(ITT),and 150 patients were included in the per protocol(PP) *** both the ITT and PP,there was no difference in the primary outcome of global dyspepsia symptoms between the sertraline and placebo groups at week *** the ITT analysis,98 and 95 patients were randomized to the sertraline and placebo groups respectively.A total of 43 patients withdrew from the study(22.3%) by week 8,with 23 of the 24 drop-outs in the sertraline group occurring prior to week 4(95.8%).In contrast,in the placebo arm,11 of 19 patients dropped out by week 4(57.9%).Utilizing the last response carried forward to account for the drop-outs,there were no differences between the sertraline and placebo groups at baseline in terms of the HkDI,HkDI 26.08 ± 6.19 vs 26.70 ± 5.89,P = 0.433;and at week 8,HkDI 22.41 ± 6.36 vs 23.25 ± 7.30,P = 0.352 *** the PP analysis,74 and 76 patients were randomized to the sertraline and placebo groups *** baseline,there were no statistically significant differences between the sertraline and placebo groups,HkDI 25.83 ± 6.313 vs 27.19 ±

Femoral neck stress fracture and medial tibial stress syndrome following high intensity interval training: A case report and review of literature

BACkGROUND Femoral and tibial stress injuries are commonly found in long distance running *** fractures have rarely been reported in athletes performing high intensity interval training(HIIT)*** objective of this study was to report a case of a patient who presented with medial tibial stress syndrome and femoral neck stress fracture after performing HIIT *** SUMMARY A 26 year old female presented with bilateral medial tibial *** had been performing HIIT exercise for 45 min,five times weekly,for a seven month *** tibial pain was gradual in onset,and was now severe and worse on exercise,despite six weeks of *** resonance imaging(MRI)revealed bilateral medial tibial stress *** she was taking norethisterone for birth control,a dual energy X-ray absorbitometry scan was performed which demonstrated normal bone mineral density of her lumbar spine and femoral *** was managed conservatively with analgesia and physiotherapy,but continued to exercise against medical *** presented again six months later with severe right hip *** of her right hip demonstrated an incomplete stress fracture of her subtrochanteric *** symptoms resolved with strict rest and *** HIIT may cause stress injury of the tibia and femur in young individuals.

Generating diverse motor behaviors critical for survival is a challenge that confronts the central nervous system(CNS)of all *** movement execution,the CNS performs complex calculations to control a large number of neuromusculoskeletal *** theory of modular motor control proposes that spinal interneurons are organized in discrete modules that can be linearly combined to generate a variety of behavioral *** modules have been previously represented as stimulus-evoked force fields(FFs)comprising isometric limb-endpoint forces across workspace ***,we ask whether FFs elicited by different stimulations indeed represent the most elementary units of motor control or are themselves the combination of a limited number of even more fundamental motor *** probe for potentially more elementary modules,we optogenetically stimulated the lumbosacral spinal cord of intact and spinalized Thy1-ChR2 transgenic mice(n=21),eliciting FFs from as many single stimulation loci as possible(20-70 loci per mouse)at minimally necessary *** found that the resulting varieties of FFs defied simple categorization with just a few *** used gradient descent to further decompose the FFs into their underlying basic force fields(BFFs),whose linear combination explained FF *** mice,we identified 4-5 BFFs with partially localizable but overlapping representations along the spinal *** BFFs were structured and topographically distributed in such a way that a rostral-to-caudal traveling wave of activity across the lumbosacral spinal cord may generate a swing-to-stance gait *** BFFs may represent more rudimentary submodules that can be flexibly merged to produce a library of motor modules for building different motor behaviors.

Gastrointestinal bleeding in patients on novel oral anticoagulants:Risk,prevention and management

Novel oral anticoagulants(NOACs), which include direct thrombin inhibitor(dabigatran) and direct factor Xa inhibitors(rivaroxaban, apixaban and edoxaban), are gaining popularity in the prevention of embolic stroke in non-valvular atrial fibrillation as well as in the prevention and treatment of venous thromboembolism. However, similar to traditional anticoagulants, NOACs have the side effects of bleeding, including gastrointestinal bleeding(GIB). Results from both randomized clinical trials and observations studies suggest that high-dose dabigatran(150 mg b.i.d), rivaroxaban and high-dose edoxaban(60 mg daily) are associated with a higher risk of GIB compared with warfarin. Other risk factors of NOAC-related GIB include concomitant use of ulcerogenic agents, older age, renal impairment, Helicobacter pylori infection and a past history of GIB. Prevention of NOAC-related GIB includes proper patient selection, using a lower dose of certain NOACs and in patients with renal impairment, correction of modifiable risk factors, and prescription of gastroprotective agents. Overt GIB can be managed by withholding NOACs followed by delayed endoscopic treatment. In severe bleeding, additional measures include administration of activated charcoal, use of specific reversal agents such as idarucizumab for dabigatran and andexanent alfa for factor Xa inhibitors, and urgent endoscopic management.

Risk of gastric cancer development after eradication of Helicobacter pylori

Helicobacter pylori(H. pylori) infection is the most important risk factor for gastric cancer(gc) development through the correa's gastric carcinogenesis cascade. However, H. pylori eradication alone does not eliminate gc, as pre-neoplastic lesions(atrophic gastritis, intestinal metaplasia and dysplasia) may have already developed in some patients. It is therefore necessary to identify patients at high-risk for gastric cancer after H. pylori eradication to streamline the management plan. If the patients have not undergone endoscopy with histologic assessment, the identification of certain clinical risk factors and non-invasive testing(serum pepsinogen) can predict the risk of atrophic gastritis. For those with suspected atrophic gastritis, further risk stratification by endoscopy with histologic assessment according to validated histologic staging systems would be advisable. Patients with higher stages may require long-term endoscopic surveillance. Apart from secondary prevention to reduce deaths by diagnosing gc at an early stage, identifying medications that could potentially modify the gc risk would be desirable. The potential roles of a number of medications have been suggested by various studies, including proton pump inhibitors(PPIs), aspirin, statins and metformin. However, there are currently no randomized clinical trials to address the impact of these medications on gc risk after H. pylori eradication. In addition, most of these studies failed to adjust for the effect of concurrent medications on gc risk. Recently, large population-based retrospective cohort studies have shown that PPIs were associated with an increased gc risk after H. pylori eradication, while aspirin was associated with a lower risk. The roles of other agents in reducing gc risk after H. pylori eradication remain to be determined.

Life skill-based training is essential for intellectual disabled individuals to regain autonomy and social *** reality(VR)could deliver a nascent application that is more engaging and *** objective of this study was to evaluate the training effects of the VR-based multiple life skill training program on life skill perfor-mance,self-efficacy,memory,cognitive and behavioral functions via a multicenter randomized controlled trial.A total of 145 intellectual disabled participants were recruited for a randomized controlled trial with three in-tervention arms:VR(n=42);traditional(n=53);and control(n=50).The life skill tasks for the interventions included 1)grocery shopping,2)cooking,and 3)kitchen *** measures were performance scores for the three tasks,self-efficacy scale,digit span score,and Frontal Assessment Battery score,graded by blinded ***-after effects of each group were evaluated using separated Wilcoxon Signed-rank *** sig-nificantly improved cooking,cleaning performance,and memory span,while traditional training significantly improved shopping,cooking,and cleaning *** generalized estimating equation evaluated effects between groups adjusted for age,gender and intelligence quotient(IQ).VR had significantly larger improvement effects in cooking and cleaning than the control group and memory span compared to traditional training and controls *** appeared to be a significant confounder on the training *** work may consider developing artificial intelligence to customize programs depending on IQ level.

Malaria transmission can be affected by multiple or even hidden factors,making it difficult to timely and accurately predict the impact of elimination and eradication programs that have been undertaken and the potential resurgence and spread that may continue to *** approach at the moment is to develop and deploy surveillance systems in an attempt to identify them as timely as possible and thus to enable policy makers to modify and implement strategies for further preventing the *** of the surveillance data will be of temporal and spatial *** an interdisciplinary point of view,it would be interesting to ask the following important as well as challenging question:Based on the available surveillance data in temporal and spatial forms,how can we build a more effective surveillance mechanism for monitoring and early detecting the relative prevalence and transmission patterns of malaria?What we can note from the existing clustering-based surveillance software systems is that they do not infer the underlying transmission networks of ***,such networks can be quite informative and insightful as they characterize how malaria transmits from one place to *** can also in turn allow public health policy makers and researchers to uncover the hidden and interacting factors such as environment,genetics and ecology and to discover/predict malaria transmission patterns/*** network perspective further extends the present approaches to modelling malaria transmission based on a set of chosen *** this article,we survey the related work on transmission network inference,discuss how such an approach can be utilized in developing an effective computational means for inferring malaria transmission networks based on partial surveillance data,and what methodological steps and issues may be involved in its formulation and validation.
