Acute bleeding after argon plasma coagulation for weight regain after gastric bypass: A case report

BAcKGROUND Roux-en-Y gastric bypass(RYGB) is the most commonly performed surgical procedure used to treat obesity worldwide. Despite satisfactory results in terms of weight loss, over time many patients experience weight regain. There are many factors that contribute to weight regain after RYGB, including the diameter of the gastric-jejunal anastomosis(GJA). One of the most commonly performed endoscopic procedures for weight regain after RYGB is argon plasma coagulation(APc). We report a case of hematemesis after outlet revision with APc. We highlight several treatment modalities that can be used to treat this *** SUMMARY A 45-year-old female with a history of weight regain after RYGB was referred for possible endoscopic treatment for weight regain. On endoscopic evaluation, the diameter of the GJA was 22 mm. Due to the dilated GJA, treatment with APc was performed. Several months later she reported a return of poor satiety and an increased appetite. A repeat endoscopy was then performed. The GJA was approximately 15 mm and was incompetent. APc was performed. One day post procedure she had four episodes of hematemesis. An endoscopy was performed and a large ulcer with a visible arterial vessel was visualized at the *** was attempted using a coagrasper and after initial contact with the vessel, the vessel started oozing. Due to fibrosis and the depth of ulceration in the area, clips and repeat APc could not be used. Therefore, an attempt to inject epinephrine injection was made. However, persistent oozing was noted. As a result, hemostatic powder was applied to the region of the bleeding ***, no more bleeding was observed. On follow-up, the patient remained hemodynamically stable and a second look endoscopy was not performed. Background： Hamburger is the most consumed beef product in North America, but lacks in nutritional appeal due to its high fat content and high proportion of saturated fatty acids （SFA）. Objectives of the present study were to improve the FA profiles of hamburgers made with perirenal fat （PRF） and subcutaneous fat （ScF） when feeding steers different diets along with examining differences in sensory attributes and oxidative stability. Diets included a control diet containing 70：30 red clover silage： barley based concentrate, a diet containing sunflower-seed （SS） substituted for barley, and diets containing SS with 15% wheat dried distillers＇ grain with solubles （DDGS-15） or 30% DDGS （DDGS-30）. Hamburgers were made from triceps brachii and either PRF or ScF （80：20 w/w）. Results： Perirenal fat versus ScF hamburgers FA had 14.3% more （P 〈0.05） 18：0, 11.8% less cis （c）9-18：1 （P 〈0.05）, and 1.82% more total trdns （t）-18：1 mainly in the form of tl 1-18：1. During sensory evaluation, PRF versus ScF hamburgers had greater （P 〈0.05） mouth coating, but the difference was less than one panel unit. Examining effects of steer diet within PRF hamburgers, feeding the SS compared to the control diet increased （P 〈0.05） t-18：1 by 2.89% mainly in the form of tl 1-18：1, feeding DGGS-15 diet led to no further changes （P 〉0.05）, but feeding DDGS-30 diet reduced the proportions of （P 〈0.05） of t-18：1 chiefly tl 1-18：1. Feeding SS and DDGS diets had small but significant （P 〈0.05） effects on hamburger sensory attributes and oxidative stability. conclusions： Feeding high-forage diets including SS and 15% DDGS, and taking advantage of the FA heterogeneity between fat depots offers an opportunity to differentially enhance beef hamburgers with 18：2n-6 biohydrogenation products （Le., t11-18：1） with potential human health benefits without compromising their sensory attributes and oxidative stability during retail display.

Binocular treatment of amblyopia:from the laboratory to clinical trials

Over the last 10 years,numerous psychophysical studies have indicated that binocular mechanisms are structurally intact but functionally suppressed in amblyopia(1-8).Many of these studies have used a contrast balancing approach,whereby the contribution of each eye to binocular vision is equated or“balanced”by presenting higher contrast stimulus elements to the amblyopic eye than the fellow eye(6).Using this approach,the strength of suppression can be quantified by measuring the magnitude of interocular contrast difference required for normal binocular *** suppression strength has been correlated with other clinical measures,stronger suppression has been associated with worse stereoacuity and worse amblyopic eye visual acuity indicting a link between the monocular and binocular deficits in amblyopia(5,7).

comprehensive review of hepatocellular carcinoma with portal vein tumor thrombus:State of art and future perspectives

Background:Despite advances in the diagnosis of patients with hepatocellular carcinoma(Hcc),70%-80%of patients are diagnosed with advanced stage *** vein tumor thrombus(PVTT)is among the most ominous signs of advanced stage disease and has been associated with poor survival if *** sources:A systematic search of MEDLINE(PubMed),Embase,cochrane Library and Database for Systematic Reviews(cDSR),Google Scholar,and National Institute for Health and clinical Excellence(NIcE)databases until December 2022 was conducted using free text and MeSH terms:hepatocellular carcinoma,portal vein tumor thrombus,portal vein thrombosis,vascular invasion,liver and/or hepatic resection,liver transplantation,and systematic ***:centers of surgical excellence have reported promising results related to the individualized surgical management of portal thrombus versus arterial chemoembolization or systemic *** elements to the individualized surgical management of Hcc and portal thrombus include precise classification of the portal vein tumor thrombus,accurate identification of the subgroups of patients who may benefit from resection,as well as meticulous surgical *** review addressed five specific areas:(a)formation of PVTT;(b)classifications of PVTT;(c)controversies related to clinical guidelines;(d)surgical treatments versus non-surgical approaches;and(e)characterization of surgical techniques correlated with classifications of ***:current evidence from chinese and Japanese high-volume centers demonstrated that patients with Hcc and associated PVTT can be managed with surgical resection with acceptable results.

The epidemiology of hyperferritinaemia

AIM： To discover the causes of markedly raised ferritin levels in patients seen at a teaching hospital in Newcastle Upon Tyne, United Kingdom. METHODS： Demographic and medical data were collected for all patients over 18 years who had a serum ferritin levels recorded as ≥ 1500 μg/L during the period January to September 2002. The cause or causes for their hyperferritinaemia were identified from their medical notes. Patients from a defined local population were identified. RESULTS： A total of 19 583 measurements were provided of which 406 from 199 patients were ≥ 1500 μg/L. An annual incidence for the local population was determined to be 0.44/1000. 150/199 medical notes were scrutinised and 81 patients were identified as having a single cause for their raised ferritin level. The most common single cause was alcoholic liver disease in the local population and renal failure was the most common single cause in the overall population. confirmed hereditary haemochromatosis was the 10^th most common cause. Liver disease contributed to hyperferritinaemia in 44% of the patients. Weight loss may have contributed to hyperferritinaemia in up to 11%. cONcLUSION： Alcohol related liver disease, haematological disease, renal failure and neoplasia are much more common causes of marked hyperferritinaemia than haemochromatosis. The role of weight loss in hyperferritinaemia may warrant further investigation.

A Study of Fluid Flow in Sediments and the Effect of Tidal Pumping

Offshore drilling and production operations can result in spills or leaks of hydrocarbons into seabed sediments, which can potentially contaminate these sediments with oil. If this oil later mi- grates to the water surface it has the potential for negative environmental impacts. For proper contin- gency planning and to avoid larger consequences in the environment, it is essential to understand me- chanisms and rates for hydrocarbon migration from oil containing sediments to the water surface as well as how much will remain trapped in the sediments. It is believed that the amount of oil transported out of the sediment can be affected by tidal pumping, a common form of subterranean groundwater dis- charge （SGD）. However, we could find no study experimentally investigating the phenomenon of fluid flow in subsea sediments containing oil and the effects of tidal pumping. This study presents an experi- mental investigation of tidal pumping to determine if it is a possible mechanism that may contribute to the appearance of an oil sheen on the ocean surface above a sediment bed containing oil. An experimen- tal apparatus was constructed of clear PVc pipe allowing for oil migration to be monitored as it flowed out of a sand pack containing oil, while tidal pressure oscillations were applied in three different man- ners. The effect of tidal pumping was simulated via compression of air above the water （which simulated the increasing static head from tidal exchange）. Experimental results show that sustained oil release oc- curred from all tests, and tests with oscillating pressure produced for longer periods of time. Further- more, the experimental results showed that the oil migration rate was affected by grain size, oil satura- tion, and oscillation wave type. In all oscillating experiments the rate and ultimate recovery was less than the comparable static experiments. Does foraging mode affect metabolic responses to feeding？ A study of pygopodid lizards

Foraging mode （ambush vs. active） profoundly affects many aspects of organismal biology, including metabolic rates and their relationship with food intake. Previous studies on snakes suggest that ambushers tend to have lower standard metabolic rates （SMR） and higher energetic costs of digestion and assimilation of prey （specific dynamic action, or SDA） than do active foragers. However, phylogenetic considerations may be at least partly responsible for such patterns, as foraging mode is strongly conserved evolutionarily and most SDA studies have focused on species from only two lineages of ambush foragers （pythonid and viperid snakes） and one lineage of active foragers （colubrid snakes）. We sought to deconfound the effects of phylogeny and foraging mode, investigating SMR and SDA in two closely related pygopodid lizards, the common scaly-foot Pygopus lepidopo- dus （active forager） and Burton＇s legless lizard Lialis burtonis （ambush forager）. consistent with the pattern seen in snakes, L. burtonis exhibits a significantly lower SMR and a higher SDA than does P lepidopodus. The magnitude of SDA in L. burtonis is comparable to that of some pythons and vipers, providing yet more evidence for the remarkable convergence between this species and ambush-foraging snakes [current Zoology 59 （5）： 618-625, 2013].

Modulating Wnt signaling to improve cell replacement therapy for Parkinson＇s disease

clinical trials have demonstrated the capacity for dopamine neurons, transplanted ectopicaUy into the striatum, to structurally inte- grate, restore dopamine transmission, and induce long-term functional benefits for Parkinson＇s disease （PD） patients. Despite this proof of principle, a number of limitations have hindered the development of cell replacement therapy over the past 20 years, particu- larly tissue availability, graft survival, and adequate reinnervation of the host brain. With a greater understanding of failure in prior clinical trials, increased knowledge of midbrain dopamine development （now including Wnts）, and the development of pluripotent stem cell technologies, we are better equipped than ever to re-address a number of these challenges. This review summarizes the trials, tribulations, and progress in cell replacement therapy for PD. We discuss the prospects of modulating canonical and non-canon- ical Wnt signalingto improve cell therapy based upon their roles in dopamine neural development and the adult brain. This will include the potential of Wnts to （i） expand fetaUy derived tissue in vitro and foUowing transplantation, （ii） promote the differentiation of pluripotent stem cells, （iii） increase graft integration and restoration of neural circuitry, and finally （iv） enhance graft survival.

Implementing a multispecies size-spectrum model in a datapoor ecosystem

Multispecies ecological models have been used for predicting the effects of fishing activity and evaluating the performance of management strategies. Size-spectrum models are one type of physiologically-structured ecological model that provide a feasible approach to describing fish communities in terms of individual dietary variation and ontogenetic niche shift. Despite the potential of ecological models in improving our understanding of ecosystems, their application is usually limited for data-poor fisheries. As a first step in implementing ecosystem-based fisheries management（EBFM）, this study built a size-spectrum model for the fish community in the Haizhou Bay, china. We describe data collection procedures and model parameterization to facilitate the implementation of such size-spectrum models for future studies of data-poor ecosystems. The effects of fishing on the ecosystem were exemplified with a range of fishing effort and were monitored with a set of ecological indicators. Total community biomass, biodiversity index, W-statistic, LFI（Large fish index）, Mean W（mean body weight） and Slope（slope of community size spectra） showed a strong non-linear pattern in response to fishing pressure, and largest fishing effort did not generate the most drastic responses in certain scenarios. We emphasize the value and feasibility of developing size-spectrum models to capture ecological dynamics and suggest limitations as well as potential for model improvement. This study aims to promote a wide use of this type of model in support of EBFM.
