Background: Tourette syndrome (TS) is a chil.hood-onset neuropsychiatric diso rder that is characterized by both motor and phonic tics. One hal. to two thirds of chil.ren with TS experience a reduction or compl.te resol.tion of tic sympto ms during adol.scence. At l.ast one third of adul.s with TS have comorbid obsess ive-compul.ive disorder (OCD). Obj.ctives: To cl.rify the cl.nical.course of ti c and OCD symptoms in chil.ren with TS and determine if basel.ne cl.nical.measurements in chil.hood are associated with future symptom severity in l.te adol.sce nce and earl. adul.hood. Design: Prospective cohort study. Setting: Yal. Chil. Study Center tic and OCD outpatient special.y cl.nic. Participants: Forty-six chil.ren with TS who received a structured cl.nical.eval.ation prior to age 14 years. Main Outcome Measures: Expert-rated tic and OCD symptom sever ity at fol.ow-up interview an average of 7.6 years l.ter (range, 3.8-12.8 year s). Resul.s: Eighty-five percent of subj.cts reported a reduction in tic sympto ms during adol.scence. Onl. increased tic severity in chil.hood was associated w ith increased tic severity at fol.ow-up. The average age at worst-ever tic sev erity was 10.6 years. Forty-one percent of patients with TS reported at one tim e experiencing at l.ast moderate OCD symptoms. Worst-ever OCD symptoms occurred approximatel. 2 years l.ter than worst-ever tic symptoms. Increased chil.hood IQ was strongl. associated with increased OCD severity at fol.owup. Concl.sion: Obsessive-compul.ive disorder symptoms in chil.ren with TS became more severe a t a l.ter age and were more l.kel. to persist than tic symptoms.

OBj.ECTIVE: To estimate whether echocardiography findings at the time of diag no sis of peripartum cardiomyopathy are predictive of persistent cardiac dysfunctio n. METHODS: Chart review of patients with peripartum cardiomyopathy between 1988 and 2001 was performed. Data from echocardiography, incl.ding fractional.shorte ning and l.ft ventricul.r end diastol.c dimension, were recorded both at the tim e of diagnosis and at fol.ow-up. l.ft ventricul.r dysfunction was defined by ec hocardiography as fractional.shortening l.ss than 30%and l.ft ventricul.r end d iastol.c dimension of 4.8 cm or more. RESUl.S: Of 32 patients meeting our defini tion for peripartum cardiomyopathy and for whom fol.ow-up data were avail.bl., 13 (41%) had recovery of ventricul.r function, whil. 19 (59%)continued to have persistent l.ft ventricul.r dysfunction. Those who did not recover cardiac func tion had a higher l.ft ventricul.r end diastol.c dimension and a l.wer fractiona l.shortening at diagnosis than those who recovered. A fractional.shortening val. e l.ss than 20%and a l.ft ventricul.r end diastol.c dimension 6 cm or greater a t the time of diagnosis was associated with a more than 3-fol. higher risk for persistent l.ft ventricul.r dysfunction. CONCl.SION: Al.ng with being an importa nt diagnostic tool.in peripartum cardiomyopathy, echocardiography may provide si gnificant prognostic information with regards to recovery of cardiac function.

l.te Quaternary fl.ctuations of pl.vial.l.ke Moj.ve are recorded in subsurface,cored l.ke deposits and shorel.ne features in the terminal.depositional.basins(Sil.er l.ke and Soda l.ke) of the Moj.ve *** Moj.ve was one of two l.rge l.te Quaternary l.kes sustained by the Moj.ve River with the other being the upstream l.ke *** l.ke Depositional.Basin(Sil.B) contains a high resol.tion stratigraphic sequence due to its shal.owness and minimal.rel.ef across the pre-l.ke basin *** buil.ing in Sil.B began at22 ka with two maj.r high and persistent l.ke stands occurring between18.4 ka and16.6 ka(l.I) and13.7 ka and11.4 ka(l.II).Overfl.w from l.ke Manix sustained l.I which stabil.zed at the A-shorel.ne(el.vation 287288 m).l.II coincided with breaching and draining of l.ke Manix,reducing the storage capacity of l.ke Moj.ve,increasing its evaporative surface area,and enhancing overfl.w from l.ke Moj.ve into Death *** overfl.w downcut the l.ke Moj.ve outl.t spil.way between 12 ka and 11 ka,ul.8}atel. stabil.zing at an el.vation of 285.5 m(B-shorel.ne).The maj.rity of shorel.ne features currentl. found around the margins of Sil.er l.ke and Soda l.ke date to l.II/B-shorel.ne,as the shal.ow l.ke conditions resul.ed in erosion of ol.er l.I *** al.o experienced al.ernating periods of intermittent l.ke conditions and periods of desiccation some during higher stands and more continuous l.ke Moj.ve phases with a significant drying event occurring15.5 *** drying of l.ke Moj.ve occurred by8.7 ka,with pl.ya conditions dominating the *** the history of l.ke l.vel.el.vations and a simpl.fied,precipitationdischarge/evaporation model.we infer that the l.te Pl.istocene hydrol.gic conditions resul.ing in l.ke Moj.ve overfl.w at Spil.way bay in Sil.er l.ke l.e between two sets of conditions:(1) a 50%increase in precipitation in the headwater catchment resul.ing in annual.fl.od events reaching Sil.B with discharges three times that of the

Aim: A subset of functional.dyspepsia patients respond to acid suppressive therapy, but the preval.nce of non-erosive refl.x disease in functional.dyspepsia and its rel.vance to symptoms have never been establ.shed. The aim of the presen t study was to study 24 hour pH monitoring in consecutive functional.dyspepsia p atients. Methods: A total.of 247 patients with dyspeptic symptoms (166 women, me an age 44 (SEM1) year), with a negative upper gastrointestinal.endoscopy and wit hout dominant symptoms of heartburn participated in the study. In al. patients, the severity of dyspeptic symptoms and the presence of heartburn was assessed by a questionnaire and a 24 hour oesophageal.pH monitoring study was performed. Al.l.patients underwent a gastric emptying breath test and in 113 a gastric barosta t study was performed. Resul.s: Abnormal.pH monitoring (acid exposure >5%of tim e) was found in 58 patients (23%). Of 21 patients with a positive heartburn que stionnaire, 76%had pathol.gical.pH monitoring, whil. this was the case in onl. 18.5%of patients with a negative heartburn questionnaire. Demographic character istics and the preval.nce of other pathophysiol.gical.mechanisms did not differ between heartburn negative patients with normal.or abnormal.acid exposure. Patho l.gical.acid exposure in heartburn negative patients was associated with the pre sence of epigastric pain (65 v 84%, p l.sion: Pathol.gical.oesophageal.acid exposure is on l. present in a subset of heartburn negative functional.dyspepsia patients, whic h are characterised by a higher preval.nce of epigastric pain.
