Background -The comparative importance of physical inactivity and obesity as predictors of coronary heart disease(CHD) risk remains unsettled. Methods and Results -We followed 88 393 women, 34 to 59 years of age, in the Nurses’Health study from 1980 to 2000. These participants did not have cardiovascular disease and cancer at baseline. We documented 2358 incident major CHD events(including nonfatal myocardial infarction and fatal CHD) during 20 years of follow-up, including 889 cases of fatal CHD and 1469 cases of nonfatal myocardial infarction. In a multivariate model adjusting for cardiovascular risk factors, overweight and obesity were significantly associated with increased risk of CHD, whereas increasing levels of physical activity were associated with a graded reduction in CHD risk(Pses of body mass index(BMI) and physical activity in women who had a healthy weight(BMI, 18.5 to 24.9 kg/m2) and were physically active(exercise ≥3.5 h/wk) as the reference group, the relative risks of CHD were 3.44(95%confidence interval[CI], 2.81 to 4.21) for women who were obese(BMI ≥30 kg/m2) and sedentary(exercisese, and 1.48(95%CI, 1.24 to 1.77)for women who had a healthy weight but were sedentary. In combined analyses of waist-hip ratio and physical activity, both waist-hip ratio and physical activity were significant predictors of CHD, and the highest risk was among women in the lowest category of physical activity and the highest tertile of waist-hip ratio(relative risk=3.03; 95%CI, 1.96 to 4.18). Even a modest weight gain(4 to 10 kg) during adulthood was associated with 27%(95%CI, 12%to 45%) increased risk of CHD compared with women with a stable weight after adjusting for physical activity and other cardiovascular risk factors. Assessment of Genetic Diversity in Nepalese Populations of <i>swertia chirayita</i>(Roxb. Ex Fleming) H. Karst Using RAPD-PCR Technique

Owing to the high demand, swertia chirayita populations in the wild are being depleted beyond its regeneration capacity. s. chirayita is one of the most valuable medicinal plants of Nepal in trade. Present Molecular investigation was undertaken to understand the level of genetic diversity in five s. chirayita populations of Nepal using Polymerase Chain Reaction (PCR)-based Random amplified polymorphic DNA (RAPD) technique. Thirty four accessions of s. chirayita along with six outlier accessions were analyzed using 26 arbitrary primers. Of the total 285 amplified bands scored for s. chirayita, 263 bands (92.28%) were polymorphic. Two major clusters were revealed in the phenogram generated from cluster analysis using NTsYs-PC software (version 2.21i) for the geographic populations under study. Principal Coordinate Analysis further substantiated the results of the phenograms. swertia chirayita populations from sankhuwasabha and Terathum were found to be genetically closest (68%, similar) whilst Nagarjun and Terathum were found to be most distant (33%, similar).The high genetic polymorphism reflected in s. chirayita populations indicates the good survival potentiality and adaptability in changing environmental scenario. The results thus produced might be helpful to plant breeders for elite cultivar development. The RAPD-PCR technique is found to be the rapid and effective tool for genetic diversity assessment in s. chirayita populations and generated insights for the formulation of conservation strategy of this vulnerable species together with its phytochemical distinctiveness.

The paper highlights the Phyto-diversity and indigenous uses an ecosystem existing in Lahaul Valley of tribal district of Lahaul and spiti in Himacjhal Pradesh, India. This is one of the most preferred tourist spot especially for foreigners. The cold desert area of India is known for its specific topography, sense climate and unique vegetation. The valley is rich in high value medicinal wealth and local inhabitants (Buddhist) practice the Tebetan Amchi system of Medicine. Keeping in view the continuous pressure on nature, this paper reflects the Phyto-diversity of Valley, with reference to medicinal and aromatic plants and conserva-tion mechanisms to protect the valuable wealth of the valley and also suggests effective ecotourisim planning of the study area.

目的:分析比较3mL和5mL利多卡因眼球筋膜囊下麻醉对白内障超声乳化手术中眼压,镇痛效果和眼外肌失运动能力的作用效果。眼球筋膜囊下麻醉是白内障超声乳化术中常用的局麻方式。作为一种剂量依赖型的阻滞方式,良好的眼压、镇痛效果以及眼外肌失运动是手术成功需要考虑的重要因素。方法:在获得伦理批准和患者的知情同意后,随机纳入70例患者作为研究对象,分别给予3m L利多卡因(组一)和5m L利多卡因(组二)作眼球筋膜囊下麻醉。麻醉前测量眼压,麻醉后10min评估眼压,镇痛效果和眼外肌失运动能力,并记录球结膜水肿和结膜下出血等并发症。结果:在麻醉管理期间,由于两名患者要求镇静,故从本研究中排除。比较组内及组间患者在麻醉前、后的基本特征,如年龄、体质量、身高、眼轴长度以及AsA生理状态,均无明显差异。与组一相比,组二患者眼外肌失运动能力及镇痛效果均明显较好。组二中有13例患者(38.2%)可达到眼球完全不运动,而组一没有患者可达到该程度。组一中有18例患者(56.3%)术中完全无痛觉,14例患者(43.7%)存在轻微痛觉,组二中有28例患者(82.4%)无痛觉,6例患者(17.6%)术中有轻微痛觉。此外,两组患者在麻醉前与麻醉后10min,其眼压的变化均无统计学差异(P<0.05)。两组患者并发症发生情况无明显差异。结论:3m L利多卡因和5m L利多卡因作眼球筋膜囊下麻醉对眼压均无影响,但在白内障超声乳化手术中给予5m L利多卡因作麻醉,在镇痛及眼外肌失运动能方面效果更好。

since the advent of imaging studies such as magnetic resonance imaging (MRI), the role of electroencephalograms (EEGs) has diminished. simultaneously, computerized scanning and miniaturization of the EEG and its components have allowed us to obtain lengthier recordings in an ambulatory setting. We report on 261 ambulatory electroencephalograms performed consecutively in the two year period of 2011 and 2012 in a busy neurology and neuropsychiatry practice with predominantly geriatric patient population. 23% of these patients had abnormal AEEGs demonstrating clear-cut epileptogenic discharges. The role of these findings in clinical practice, especially in geriatric and psychiatric populations is discussed.

横斑腹小鸮(Athene brama)是巴基斯坦旁遮普中部农业生态系统中最常见但研究匮乏的猛禽。我们通过分析吐弃块(n=498)以了解其季节性和地域性食性变化。结果显示:小型哺乳类占所有食物生物量的98.4%,而昆虫类在数量上占据绝对优势(56.4%)。在小型哺乳类食物中,鼠类(38.8%)是稳定的食物成分。而在昆虫食物中,甲虫最多(42.8%)。横斑腹小亦捕食鼩鼱类(2.3%)和鸟类(1.7%)。昆虫类食物还有蟑螂(6.4%)、臭虫(3.3%)、蝗虫(6.7%)和蚂蚁(3.2%)。小家鼠(Mus musculus)和柔毛鼠(Rattus meltada)在鼠类食物中出现频次最多。不同地区中,Hafizabad的食物多样性最高(2.07),而秋季的食物多样性最高(1.99)。

本研究旨在通过比较沙棘籽油(Hippophae rhamnoides)和防腐软膏(5%碘伏)、草本软膏(印度草药研究与供应有限公司)及一种刺激性温和的基础药膏(液体石蜡),来研究沙棘籽油对牛的皮肤伤口愈合效果。这个实验是将12头雄性牛犊分为4组,6头牛都有皮肤伤口,3头胸背部脊椎区的两边都用当地的镇痛药。伤口分别用液体石蜡、5%的碘伏软膏、沙棘籽油(沙棘油)及草本软膏处理28d,并分为Ⅰ、Ⅱ、Ⅲ、Ⅳ四组。不同处理的功效是建立在对经过0d、3d、7d、10d、14d和28d的不同临床血液参数的研究基础之上的。在研究中,所有组的心脏、呼吸率和血液学的各种参数及肛温都维持在正常的生理极限内。沙棘油和草本软膏对伤口处理的临床研究中发现,在伤口愈合的初期,伤口保持了一定的干燥度,并且抑制了炎症初期伤口的恶化。不同的组之间,在持续28d的实验中,用草本软膏和沙棘油做处理的组中伤口的愈合率最高。这些组的伤口试验同时还表现出早治疗,早治愈的特征。但是,在草本软膏和沙棘油处理组中,草本软膏组处理的伤口的整体愈合能力还是略优于沙棘油的疗效。

Poncet's disease: An unusual presentation of tuberculosis in a diabetic lady

Authors describe a 53-year-old woman who presentedto their diabetes clinic with a three week history of multiple painful and swollen joints. she had been diagnosed with type 2 diabetes 5 years back. On examination, both knee joints and left ankle were swollen. A soft tissue swelling appeared over the medial end of the left clavicle few days later. Rheumatoid arthritis, collagen vascular diseases and other common causes of polyarthritis were ruled out by appropriate investigations. Non steroidal anti-inflammatory drugs failed to give satisfactory pain relief and the arthritis persisted. Conventional cultures of synovial fluid samples including cultures for tuberculosis were negative. Computed tomography showed a space occupying lesion involving the left sternoclavicular joint. Fine needle aspiration from the lesion was performed and acidfast bacilli were demonstrated in the smear using ZiehlNeelsen stain. The explanation of her arthritis was therefore tuberculous arthritis in left sternoclavicular joint and reactive arthritis in the rest of the joints. A diagnosis of Poncet's disease was considered in her case. We treated her with standard anti-tuberculosis drugs and the arthritis resolved within a few days. she remained symptom-free at her 2 years' follow-up.

用碱性髓鞘蛋白加福氏完全佐剂免疫ＳＪＬ／Ｊ小鼠，建立实验性变态反应性脑脊髓炎动物模型。取发病动物淋巴结作细胞培养，并用碱性髓鞘蛋白激活培养３周。其间２次经Ｆｉｃｏｌｌ－Ｈｙｐａｑｕｅ分离纯化激活的细胞，并加Ｘ线照射的脾细胞，应用３Ｈ－ＴｄＲ掺入法测定细胞对碱性髓鞘蛋白的反应状况。细胞经ＰＫＨ２－ＧＬ标记及ＰＥ－抗ＣＤ４荧光染色，流式细胞仪分析细胞阳性情况。ＰＫＨ２－ＧＬ标记的细胞静脉注入同品系健康小鼠，约１０天后，动物发生典型的实验性变态反应性脑脊髓炎症状。随之分离发病动物的中枢神经系统和淋巴结淋巴细胞，发现于中枢神经系统中ＰＫＨ２－ＧＬ标记细胞中４５％为ＣＤ４＋细胞，其中９０％以上细胞为激活的母细胞。而在淋巴结中ＣＤ４＋细胞低于４％，母细胞少于１０％，结果说明对碱性髓鞘蛋白反应的ＣＤ４＋细胞，可选择性地进入中枢神经系统并介导实验性变态反应性脑脊髓炎的免疫病理反应。
