BACKGROUND Acute carpal tunnel syndrome(ACTS)is commonly caused by repetitive strain,trauma,or inflammatory ***,ACTS due to tophaceous gout is a clinical event that remains poorly understood and *** rare manifestation necessitates prompt diagnosis and intervention to prevent irreversible *** SUMMARY A 51-year-old man who had poorly controlled hyperuricemia presented with ACTS secondary to tophaceous *** of rapid symptom progression symptoms and severe median nerve compression within 3 mo,the patient underwent emergency decompression surgery for both wrists at different time ***,he exhibited complete recovery of sensory and motor functions,with no recurrence at long-term *** outcomes were achieved through immediate decompression surgery,anti-inflammatory medications,postoperative active and passive range-of-motion exercises,and intermittent wrist *** diagnosis and surgical intervention,when necessary,are crucial for preventing long-term complications and obtaining favorable outcomes in patients with *** optimal gout management strategy involving pharmacologic therapy and lifestyle modifications may help minimize ACTS recurrence and improve clinical *** Prompt surgical intervention and optimal gout management are crucial for preventing irreversible nerve damage and ACTS recurrence.

Jurassic Black Shales Facies from Qiangtang Basin (Northern Tibet): Rare Earth and Trace Elements for Paleoceanographic Implications

The Biluo Co and Amdo 114 station, northern Tibet, cropping out the Early Toarcian and Middle-Late Tithonian （Jurassic） organic-rich black shales, have been a focus to petroleum geologists in discussing their oil-producing potential. This paper first reports the trace elements and rare earth elements to discuss the paleoenvironments, redox conditions and sedimentary mechanisms of those black shales. Both sections exhibit variation in trace element abundances with concentrations 〈0.1 ppm to 760 ppm, mostly enriched in V, Cr, Ni, Cu, Zn, Mo, Ba and U. Element ratios of Ni/Co, V/Cr, U/Th and V/（V＋Ni） plus U were used to identify redox conditions. The shale-normalized rare earth element （REE） patterns are characterized by the flat-shale type with instable Ce anomalies and very weekly positive Eu anomalies. Positive Ceanom values are significant with values varying between - 0.064 and 0.029 in Biluo Co, which may be interpreted as release of REE and input of riverine terrestrial matter with rich Ce （resulting in pH change） during the anoxic conditions. In the middle parts of Amdo 114 station, distinct negative Ceanom values are observed （-0.238 to -0.111） and associated surface water warming were interpreted as being related to a major sea level rise. In contrast, the formation of the black shales in the lower and upper part of the studied succession took place during a cooler （Ceanom values 〉-0.10）, lower surface water productivity, and lower sea-level stage. Thus, we emphasize the role of different factors that control the formation of local and regional black shales. The most important factors are sea-level fluctuations and increasing productivity.

Paleoceanographic Indicators for Early Cambrian Black Shales from the Yangtze Platform, South China: Evidence from Biomarkers and Carbon Isotopes

The lower Cambrian Niutitang Formation, a widespread black shale deposition, is of geological interest because of its polymetallic beds, Cambrian explosion, depositional ages, dramatic environmental changes and so on. Previous study focused mainly on inorganic geochemistry and few studies have investigated the organic fractions of upper Neoproterozoic-lower Cambrian strata in South China. Here we report a study of biomarkers plus organic carbon isotopes for black shales from Ganziping, Hunan Province （China）. All the saturated hydrocarbon fractions have a unimodal distribution of n-alkanes, a high content of short-chain alkanes and maximize at C 19 or C 20 （C 23 for sample Gzh00-1）. The C 27 /C 29 sterane ratio ranges from 0.77 to 1.20 and 4-methylsteranes are in low abundance. These parameters indicate that algae and bacteria are the important primary producers. Furthermore, biomarker maturity proxies show the samples to be higher maturity. The low Pr/Ph values （0.7） suggest that the samples were deposited under anoxic conditions and, likely, under stratified water columns. In addition, 25-norhopanes and gammacerane are present as diagnostic indicators of normal marine salinity and dysoxic to anoxic conditions. During the Early Tommotian, known to coincide with a transgression event, small shelly fossils increased in abundance and diversity. Moreover, positive δ 13 C org excursions close to 1.4‰ occur at the base of the Tommotian stage. In summary, the Early Cambrian black shales were deposited under dramatic paleoenvironmental changes, including oceanic anoxia, higher primary productivity and sea-level rise.

The association of non-alcoholic fatty liver disease and metabolic syndrome in a Chinese population

BACKGROUND： Non-alcoholic fatty liver disease（NAFLD） is associated with features of metabolic syndrome. The aim of this study was to investigate the association between NAFLD and metabolic syndrome in a Chinese population. METHODS： Data from subjects were retrospectively collected from 2006 to 2009. The exclusion criteria included significant consumption of alcohol and chronic hepatitis B and C. The patients were assigned to two groups according to ultrasound findings： normal group and fatty liver group. The liver function of patients was determined by assessing serum alanine aminotransferase（ALT）. Metabolic syndrome was diagnosed based on the 2005 International Diabetes Federation criteria. RESULTS： A total of 7568 subjects were enrolled and 5736（75.8%） and 1832（24.2%） patients were assigned to the normal and fatty liver groups, respectively. The fatty liver group had significant male predominance（69.7% vs 56.0%）, higher body mass index（mean, 26.67 vs 23.55 kg/m^2） compared with the normal group. There were 441（7.7%） and 377（20.6%） cases with metabolic syndrome in the normal and fatty liver groups, respectively, with significant difference（P=0.001）, and the subgroup of 385 cases with fatty liver and elevated ALT had higher prevalence（28.8%） of metabolic syndrome. The strongest association of an individual component of metabolic syndrome with NAFLD was hyperlipidemia（adjusted OR=2.55, 95% CI： 2.22-2.94）.CONCLUSION： The individuals with NAFLD had a higher ratio of metabolic syndrome. Hyperlipidemia had the strongest positive association with NAFLD.

Risk factors and metabolic abnormality of patients with non-alcoholic fatty liver disease: Either non-obese or obese Chinese population

Background: Non-alcoholic fatty liver disease(NAFLD) occurs not only in obese individuals but also in non-obese ones. The aim of this study was to focus on the association between NAFLD and metabolic events in a non-obese or obese Chinese ***: Data collected from subjects registered at Taichung Veterans General Hospital from January to December 2009 were analyzed. The exclusion criteria were alcoholics, chronic hepatitis B or C. Patients included in analyses were assigned to four groups according to sonography of their liver(normal or NAFLD), and body mass index(BMI) levels(non-obese if BMI tients enrolled in four groups labeled non-obese normal liver(group A), non-obese NAFLD(group B), obese normal liver(group C) and obese NAFLD(group D),respectively. The highest ratio of metabolic syndrome existed in the group B(26.9%), followed by group A(11.7%), group D(10.9%) and finally the group C(5.2%). The positive association with NAFLD in non-obese individuals was significant in triglyceride(OR = 1.01; 95% CI: 1.01–1.02) and glucose(OR = 1.02; 95% CI:1.01–1.03), while the positive association with NAFLD in obese subjects was only significant in triglyceride(OR = 1.01; 95% CI: 1.01–1.02). The positive association was most significant in all cases(adjusted OR = 2.41; 95% CI: 1.78–3.24), especially in non-obese individuals(OR = 2.81; 95% CI: 1.92–4.12).Conclusions: Non-obese NAFLD subjects displayed a higher proportion of metabolic abnormality. Hyperlipidemia and hyperglycemia had the most positive strength association with NAFLD.

BACKGROUND Inverted papilloma is an uncommon neoplasm in the nasal *** is a histologically benign tumor,but has a high recurrence and local invasion *** addition,nasal or skull base lymphoma is another rare *** coexistence of these two tumors in one case makes the diagnosis and related treatment *** SUMMARY We report a case of an immunocompetent patient,who had a history of inverted papilloma 20 years *** patient presented with an infiltrated mass lesion in the nasal cavity with extension to the frontal *** repeated biopsies revealed inverted papilloma without any malignant *** the patient underwent a frontobasal craniotomy with total tumor excision,the final pathological examination revealed nasal inverted papilloma coexisting with diffuse large B-cell lymphoma of the skull *** Based on this case report,while managing a case of an aggressive recurrent inverted papilloma,not only squamous cell carcinoma transformation,but also other invasive malignancy,such as lymphoma,should be considered.

Design of Digital Linear Power Supply with Low Current Ripple

A linear power supply is a circuit architecture directly driven by AC. Multiple high-voltage LEDs (light-emitting diodes) are connected in series to withstand the input voltage. The LEDs are divided into multiple segments through a segment switching mechanism to adapt to the pulsating DC changes after bridge rectification. Due to imperfect switching between control signals, there may be signal overlap or misalignment, which reduces circuit performance and component lifespan. Firstly, signal overlap occurs when the next segment opens before the previous segment is about to close. This state causes overlapping intervals where two segments of current combine, resulting in a high combined current that can lead to significant power losses and generate heat, potentially reducing the lifespan of the components. Secondly, signal misalignment occurs when the next segment prepares to open after the previous segment has completely closed. In this state, there is a time gap with no current flow, resulting in reduced output power and exacerbating LED flickering. Both of these undesirable phenomena introduce ripple into the LED output current. In this paper, a digitalized design is proposed to precisely compensate for the timing between alternating signals, significantly reducing output current ripple. This approach helps enhance power supply performance, extend component lifespan, and reduce LED index flicker.

Generally,in Hot Strip Mill or Cold Mill,the roll ID is etched on head/tail end side face of the roll for visual *** in Z-Mill the work roll is too small(6500mm)to etch roll ID and because of the thrust bearing for work roll,etching roll ID on side face is not *** only way to identify the rolls is to etch the ID on the head side neck which is a small ***,the Z-Mill has to change work rolls at least one time for each coil rolled,which means larger amount of work rolls compare with conventional *** rolls have to be stored in the pallet which can store up to6 rolls and the pallet can be piled up to 5 layers,it is difficult for visual identification in the storage *** roll ID identification,CSC R&D department designed the ring shape RFID tags(2mm ring width and thickness,20/30mm diameter)which fits and glued inside the center hole of the roll head/tail end *** Electrical&Control department configured and integrated the interfaces between the fixed RFID readers and the Roll Shop Management System(RSMS)for automatic roll ID identification;and also design the Android tablet computer application which use Blue Tooth to connect to the portable RFID reader for roll ID、pallet ID and location ID readings,and use wireless network(Wi-Fi)to update the RSMS yard database to facilitate the roll storage yard management.
