Ultrasound-guided paravertebral nerve block anesthesia for percutaneous endoscopic laser unroofing treatment of symptomatic simple renal cysts—An innovative ambulatory surgery mode

Objective:This study was designed to evaluate the feasibility,efficacy,and safety of percutaneous ureteroscopy laser unroofing as an ambulatory surgery for symptomatic simple renal cysts under multilevel paravertebral nerve block ***:From December 2015 to September 2017,33 simple renal cyst patients who had surgical indications were *** ultrasound guidance,the T10/T11,T11/T12,and T12/L1 paravertebral spaces were identified,and 7-10 mL 0.5%ropivacaine was injected at each *** a puncture needle was placed inside the cyst cavity under ultrasonic monitoring.A guidewire was introduced followed by sequential dilation up to 28/30 *** extra parenchymal portion of the cyst wall was dissociated and incised using a thulium laser,and a pathological examination was ***:Sensory loss to pinprick from T8 to L1 and sensory loss to ice from T6 to L2 were observed in all *** of the patients complained of pain during *** serious complications occurred *** the surgery,all patients recovered their lower limb muscle strength quickly,got out of bed,resumed oral feeding,and left the hospital within 24 h of *** pathologic diagnosis of all cyst walls was a simple renal *** mean follow-up was 35.8 *** the end of follow-up,the cyst units were reduced in size by more than 50%compared to the preoperative size,and no patient experienced a ***:Multi-level paravertebral nerve block for percutaneous ureteroscopy laser unroofing as an ambulatory surgery mode is feasible,safe,and effective for the treatment of simple renal cysts in selected patients.

In recent years,materials with asymmetric mechanical response properties(mechanical Janus materials)have been found possess numerous potential applications,*** absorption and vibration *** this study,we propose a novel mechanical Janus lattice whose asymmetric mechanical response can be switched in orientation by a *** finite element analysis and experimental verification,this lattice exhibits asymmetric displacement responses to symmetric ***,with such a plug structure inside,individual lattices can switch the orientation of asymmetry and thus achieve reprogrammable design of a mechanical structure with chained *** reprogrammable asymmetry of this material will offer multiple functions in design of mechanical metamaterials.

背景:术后腹腔粘连的形成是一个复杂的过程,预防术后粘连是临床上亟待解决的问题。目的:旨在从细胞及分子水平分析粘连的发生机制,为间充质干细胞外泌体预防和治疗粘连提供理论依据。方法:以“腹腔粘连,盆腔粘连,术后粘连,上皮间充质转化,间充质干细胞,干细胞外泌体,间充质干细胞外泌体”为中文检索词,以“Abdominal adhesion,pelvic adhesion,postoperative adhesion,epithelial mesenchymal transformation,mesenchymal stem cell,stem cell exosomes,mesenchymal stem cell exosomes”为英文检索词,检索PubMed、中国知网和中国生物医学文献数据库,筛选各数据库建库至2023年8月发表的术后腹腔粘连及间充质干细胞外泌体干预研究的相关文章,并进行系统的整理分析,最终纳入54篇文献进行综述。结果与结论:(1)任何腹膜炎症、机械损伤、组织缺血和异物植入等病理因素均可引起腹膜表面的损伤,引起术后腹腔粘连;腹腔粘连的形成过程包括腹膜间皮细胞修复、炎性反应、纤溶系统及凝血途径等过程的相互作用,涉及多种细胞因子及信号通路,包括Wnt/β-catenin通路能诱导纤维化和血管生成,并与转化生长因子β/Smads信号通路相互协同,能刺激成纤维细胞增殖,引起腹膜纤维化;同时,核转录因子kB信号通路上调细胞炎症因子的表达,促进成纤维细胞增生,组织纤维化的过程中发挥关键作用。(2)干细胞的旁分泌功能是目前以再生医学为基础的分子干预腹腔粘连的重要方向,可以参与作用于腹腔粘连中的多种复杂细胞因子及信号通路。(3)相对于传统治疗腹腔粘连的方法,间充质干细胞外泌体具有生物活性、使用安全、无需特殊培养和扩增、更低的免疫原性及较长的稳定性等优势,能通过多种途径引导一种正常的修复和愈合。(4)间充质干细胞外泌体在以往研究中均被证明可以参与调节粘连形成的上述各过程,在临床研究中显示出潜在的应用前景,但需要进一步临床研究以探索适当的间充质干细胞外泌体治疗方案,来解决临床转化的问题。

Carbon dot-based artificial light-harvesting systems with sequential energy transfer and white light emission for photocatalysis

In this work, we designed and synthesized cationic carbon dots(CDs) with a size distribution of 1.6–3.7 nm, which exhibited dark blue fluorescence in the aqueous solution. Based on its excellent luminescence properties, we used it as an energy donor to construct a sequential artificial light-harvesting system(LHS) by employing the energy-matching dyes eosin Y disodium salt(EY) and sulforhodamine101(SR101), which could regulate the white light emission(Commission Internationale de l'Eclairage(CIE) coordinate:(0.30, 0.31)) with the energy transfer efficiency(ΦET) of 53.9% and 20.0%. Moreover, a single-step artificial LHS with white light emission(0.32, 0.28) can be constructed directly using CDs and dye solvent 43(SR) with ΦETand antenna effect(AE) of 48.8% and 6.5, respectively. More importantly,CDs-based artificial LHSs were firstly used in photocatalytic of α-bromoacetophenone, with a yield of90%. This work not only provides a new strategy for constructing CDs-based LHSs, but also opens up a new application for further applying the energy harvested in CDs-based LHSs to the field of the aqueous solution photocatalysis.

Covalent organic frameworks doped with manganese-metal organic framework for peroxymonosulfate activation

A novel N, O modified Mn_(3)O_(4)@porous carbon catalyst(NOC-Mn_(3)O_(4)) was prepared by direct carbonization using the manganese-metal organic framework(Mn-MOF) and covalent organic framework(COF) as precursors to activate peroxymonosulfate(PMS) for the degradation of bisphenol A(BPA) and rhodamine B(Rh B). Benefiting from the N and O co-doping of COF, larger specific surface area, faster electron transfer and Mn cycling, the optimum 1NOC-Mn_(3)O_(4)could significantly improve the degradation performance of BPA and Rh B(92.1% and 96.9% within 30 min) as compared to C-Mn_(3)O_(4)without COF doping. In addition, 1NOC-Mn_(3)O_(4) showed good reusability and strong anti-interference ability. Radical quenching experiments, X-ray photoelectron spectroscopy(XPS), Electron paramagnetic resonance spectrometer(EPR) and electrochemical tests showed that the 1NOC-Mn_(3)O_(4)/PMS system degraded BPA and Rh B by both radical and non-radical pathways. Moreover, the possible degradation pathways of BPA and Rh B were proposed by liquid chromatography-mass spectrometry(LC-MS). Except for that, the toxicity of BPA, Rh B and their intermediates were evaluated. This study opens up a new prospect for the design of COF-doped PMS catalysts.

Boosting the interfacial stability between electrolyte and Li-rich cathode material at high operating voltage is vital important to enhance the cycling stability of Li-rich cathode materials for high-performance Li-ion *** this work,vinyltrimethylsilane as a new type of organic silicon electrolyte additive is studied to address the interfacial instability of Li-rich cathode material at high operating *** cells using vinyltrimethylsilane additive shows the high capacity retention of 73.9%after 300 cycles at 1 C,whereas the cells without this kind of additive only have the capacity retention of 58.9%.The improvement of stability is mainly attributed to the additive helping to form a more stable surface film for Li-rich cathode material,thus avoiding direct contact between the electrolyte and the cathode material,slowing down the dissolution of metal ions and the decomposition of the electrolyte under high operating *** findings in this work shed some light on the design of stable cycling performance of Li-rich cathode toward advanced Li-ion batteries.

Physical and numerical investigations of target stratum selection for ground hydraulic fracturing of multiple hard roofs

Ground hydraulic fracturing plays a crucial role in controlling the far-field hard roof,making it imperative to identify the most suitable target stratum for effective *** experiments are conducted based on engineering properties to simulate the gradual collapse of the roof during longwall top coal caving(LTCC).A numerical model is established using the material point method(MPM)and the strain-softening damage constitutive model according to the structure of the physical *** simulations are conducted to analyze the LTCC process under different hard roofs for ground hydraulic *** results show that ground hydraulic fracturing releases the energy and stress of the target stratum,resulting in a substantial lag in the fracturing of the overburden before collapse occurs in the hydraulic fracturing *** hydraulic fracturing of a low hard roof reduces the lag effect of hydraulic fractures,dissipates the energy consumed by the fracture of the hard roof,and reduces the abutment ***,it is advisable to prioritize the selection of the lower hard roof as the target stratum.

Sensitive and Specific Y-Shaped Ratio Biosensor for Detecting Serum mi R-18a:Potential Early Scanning Tool for Non-Small Cell Lung Cancer

miR-18a has been identified as a significantly expressed microRNA(miRNA)in non-small cell lung cancer(NSCLC)and plays a vital role in cancer cell transformation,metastasis,and ***,a pair of binary probes from numerous probe pairs based on single nucleotide polymorphism analyses of miR-18a and miR-18b was first designed and screened to develop a Y-shaped ratio biosensor for accurate detection of serum miR-18a in *** special structure of the binary probes combined with the hairpin showed strong specificity for miR-18a,which was confirmed by polyacrylamide gel electrophoresis assay and square wave voltammetry ***,it is beneficial to immobilize single-stranded DNA(ssDNA)probes due to the large specific surface area of nanoporous gold,thereby improving the sensitivity of the *** Y-shaped ratio biosensor exhibited a wide detection range and can quantify the concentration of miR-18a in the range of 10 fmol/L–100 pmol/L,with a limit of detection of 0.211 fmol/L(S/N=3).Moreover,it exhibits excellent detection capabilities in serum samples since the biosensor showed a high selectivity toward the coexistence of miR-18a and ***,the prepared Y-shaped ratio biosensor is a highly sensitive and specific miR-18a detection tool,capable of identifying microscopic amounts of miR-18a in serum samples,providing great potential for early screening of NSCLC.

Single-ended Time Domain Fault Location Based on Transient Signal Measurements of Transmission Lines

Precise fault location plays an important role in the reliability of modern power *** the in-creasing penetration of renewable energy sources,the power system experiences a decrease in system inertia and alterations in steady-state characteristics following a fault *** existing single-ended phasor domain methods assume a certain impedance of the remote-end system or consistent current phases at both *** problems present challenges to the applicability of con-ventional phasor-domain location *** paper presents a novel single-ended time domain fault location method for single-phase-to-ground faults,one which fully considers the distributed parameters of the line *** fitting of transient signals in the time domain is real-ized to extract the instantaneous amplitude and ***,to eliminate the error caused by assumptions of lumped series resistance in the Bergeron model,an im-proved numerical derivation is presented for the distrib-uted parameter line *** instantaneous symmet-rical components are extracted for decoupling and inverse transformation of three-phase recording *** on the above,the equation of instantaneous phase constraint is established to effectively identify the fault *** proposed location method reduces the negative effects of fault resistance and the uncertainty of remote end pa-rameters when relying on one-terminal data for ***,the proposed fault analysis methods have the ability to adapt to transient processes in power *** comparisons with existing methods in three different systems,the fault position is correctly identified within an error of 1%.Also,the results are not affected by sampling rates,data windows,fault inception angles,and load conditions.
