Personalized treatment of well-differentiated gastric neuroendocrine tumors based on clinicopathological classification and grading:A multicenter retrospective study

Background:The incidence of well-differentiated gastric neuroendocrine tumors(G-NET)is increasing annually,and while they have a good prognosis and low mortality rate,their high recurrence rate makes treatment options *** study aims to determine the relationship between individualized treatment plans and the recurrence of ***:We performed a multicenter,retrospective study of 94 patients with highly differentiated G-NET and treated at Peking Union Medical College Hospital,Yantai Yuhuangding Hospital,and Beijing Zhong-Neng-Jian Hospital from November 2015 to September *** factors for recurrence of G-NETs were investigated using chi-squared test and multifactorial logistic regression ***:After a median follow-up of 49 months,the overall recurrence rate among the 94 G-NET patients was 14%(13/94).The recurrence rates of endoscopic mucosal resection(EMR),endoscopic submucosal dissection(ESD),somatostatin analog(SSA)therapy,and surgery were 43%(6/14),10%(5/49),5%(1/22),and 11%(1/9),***-treatment recurrence rates were significantly different(P=0.014)among four treatments(EMR,ESD,SSA,and surgery),and further subgroup comparisons revealed lower recurrence rates in the ESD and SSA groups than in the EMR *** the second month onward,SSA therapy considerably reduced the gastrin levels from 1081.0(571.5,2472.8)pg/mL to 461.5(255.3,795.0)pg/mL(Z=-3.521,P<0.001).Both chi-squared test and multifactorial logistic regression analysis suggested that among the clinicopathological parameters studied,only the pre-treatment gastrin level(P=0.018 and 0.005)and the type of treatment(P=0.014 and 0.017)were significantly associated with G-NET ***:Individualized treatment strategies may reduce the risk of relapse after G-NET ***-term SSA therapy may be a secure and efficacious treatment option for type 1 G-NET with more than six lesions,and it substantially decreases the incidence of post-treatment rec

The synthesis of 6-(tert-butyl)-8-fluoro-2,3-dimethylquinoline carbonate derivatives and their antifungal activity against Pyricularia oryzae

A series of novel 6-(/er/-butyl)-8-fluoro-2,3- dimethylquinoline carbonate derivatives were designed and synthesized. Bioassay results showed that some of them exhibited good activity against Pyricularia oryzae (P. oryzae). It was found that the compound 5q (benzyl (6-(tert-butyl)-8-fluoro-2,3-dimethylquinolin-4-yl) carbonate) possessed good activity against P. oryzae whatever protective activity (10 mg·L^-1) or curative activity (25 mg·L^-1), which was better than that of control tebufloquin. In addition, the frontier molecular orbit results revealed that the compound held higher activity against P. oryzae when the total energy was low and the ClogP was high, which may provide useful information for further design novel fungicides.

Pyrazole carboxamide derivatives represent an important class of fungicides in agrochemicals. To find more novel structural pyrazole carboxamides, a novel series of 3-（trifluoromethyl）-lH-pyrazole-4-carboxamide compounds were prepared from ethyl 4,4,4-trifluoroace- toacetate and triethyl orthoformate as starting materials. All the products were characterized by Fourier transform infrared spectroscopy, 1H nuclear magnetic resonance （NMR）, 13C NMR, 19F NMR and mass spectrography. The bioassay results showed these fluorine-containing pyrazole carboxamides have a weak fungicidal activity but some of them exhibit a good nematocidal activity against M. incognita.

目的探索十二指肠神经内分泌肿瘤(D-NEN)的临床特点和预后相关因素。方法回顾性纳入2017年1月~2023年6月于我院住院的D-NEN患者,分析其临床特点、辅助检查结果、病理特征、免疫组化结果、治疗方式及预后情况。采用单因素及多因素cox回归分析评估影响D-NEN不良预后的因素。结果 38例D-NEN患者病理分级均为G1或G2级,临床分期Ⅰ~Ⅳ期分别为13例、9例、9例和7例。中位随访33.0(13.0,55.0)个月,随访期间2例患者死亡,8例出现复发或转移。单因素cox回归分析结果提示肝脏转移和系统免疫炎症指数(SII)≥435.5均与D-NEN患者不良预后相关(P<0.05),多因素cox回归分析结果提示高SII(≥435.5)是D-NEN患者不良预后的独立预测因素(P<0.05)。结论 D-NEN临床病理特点异质性高,SII可能是其潜在的预后预测指标。

Eosinophilic gastroenteritis(EGE)is a gastrointestinal disorder of unclear etiology that is characterized by eosinophilic infiltration of the stomach and small intestine,and consists of mucosal,muscular,and serosal *** infiltration of the gastrointestinal tract is a fundamental histopathological characteristic of EGE and is driven by several T-helper type 2(Th2)-dependent cytokines and induced by food *** to the lack of a diagnostic gold standard,EGE has a high rate of delayed diagnosis or ***,several new diagnostic strategies have been developed,such as novel genetic biomarkers and imaging *** dietary therapy and corticosteroids remain the common choices for EGE treatment,recent decades have seen the emergence of novel treatment alternatives,such as biologics that target particular molecules involved in the pathogenic *** investigations and clinical trials have demonstrated the efficacy of biologics and provided additional insights for the era of refractory or corticosteroid-dependent EGE biologics.

Optimization of process parameters for preparation of powdered activated coke to achieve maximum SO_(2)adsorption using response surface methodology

Powdered activated coke(PAC)is a good adsorbent of SO_(2),but its adsorption capacity is affected by many factors in the preparation *** prepare the PAC with a high SO_(2)adsorption capacity using JJ-coal under flue gas atmosphere,six parameters(oxygen-coal equivalent ratio,reaction temperature,reaction time,O_(2)concentration,CO_(2)concentration,and H_(2)O concentration)were screened and optimized using the response surface methodology(RSM).The results of factor screening experiment show that reaction temperature,O_(2)concentration,and H_(2)O(g)concentration are the significant ***,a quadratic polynomial regression model between the significant factors and SO_(2)adsorption capacity was established using the central composite design(CCD).The model optimization results indicate that when reaction temperature is 904.74℃,O_(2)concentration is 4.67%,H_(2)O concentration is 27.98%,the PAC(PAC-OP)prepared had a higher SO_(2)adsorption capacity of 68.15 mg/g while its SO_(2)adsorption capacity from a validation experiment is 68.82 mg/g,and the error with the optimal value is 0.98%.Compared to two typical commercial activated cokes(ACs),PAC-OP has relatively more developed pore structures,and its SBET and Vtot are 349 m^(2)/g and 0.1475 cm3/g,significantly higher than the 186 m^(2)/g and 0.1041 cm3/g of AC1,and the 132 m^(2)/g and 0.0768 cm3/g of ***,it also has abundant oxygen-containing functional groups,its surface O content being 12.09%,higher than the 10.42%of AC1 and 10.49%of ***,the SO_(2)adsorption capacity of PAC-OP is also significantly higher than that of both AC1 and AC2,which is 68.82 mg/g versus 32.53 mg/g and 24.79 mg/g,respectively.

目的探讨小肠间质瘤的临床特征及其并发消化道出血的危险因素。方法对我院2012年5月~2021年12月手术治疗的317例原发小肠间质瘤患者进行回顾性分析,其中男167例(52.7%)、女150例(7.3%),中位起病年龄55(46,63)岁,中位病程90(30,365)天。采用多因素logistic回归分析评估小肠间质瘤患者消化道出血的影响因素。结果所有患者中,小肠间质瘤位于十二指肠112例(35.3%)、空肠113例(35.6%)和回肠92例(29.0%);肿瘤中位直径5.2(3.0,8.0)cm;10.1%合并其他恶性肿瘤。消化道出血(46.4%)是最常见的临床表现,其次是腹痛(21.1%)、体重下降(15.1%)、腹胀(7.3%)、腹部包块(6.6%);并发症包括肠梗阻(5.4%)、间质瘤内出血(4.4%)、肠穿孔(1.3%),23.7%患者无症状。病程长、TNM分期为T2和T3期是小肠间质瘤患者消化道出血的危险因素;诊断间质瘤同期诊断恶性肿瘤、恶性肿瘤家族史、腹痛是其保护因素(P<0.05)。结论消化道出血是小肠间质瘤患者最常见的临床表现,对于病程长、TNM分期为T2和T3期小肠间质瘤患者需警惕消化道出血的风险。

胃泌素瘤是一种神经内分泌肿瘤,异位分泌胃泌素,导致卓-艾综合征。大部分胃泌素瘤为散发病例,20%~25%患者作为多发性内分泌腺瘤综合征1型(MEN1)临床表现之一存在。因为散发胃泌素瘤及MEN1治疗策略不同,所以临床上需要将其加以区分。胃泌素瘤药物治疗主要分为控制高胃泌素血症相关症状和控制肿瘤生长两类。散发病例的手术治愈率为20%~40%,而对于MEN1人群是否进行手术治疗尚存在争议。本文对近年来胃泌素瘤诊断及治疗相关临床进展进行阐释,以期为临床工作提供参考。
