Diagnosis and treatment recommendations for pediatric respiratory infection caused by the 2019 novel coronavirus

Since December 2019,an epidemic caused by novel coronavirus (2019-nCoV) infection has occurred unexpectedly in *** of 8 pm,31 January 2020,more than 20 pediatric cases have been reported in *** these cases,ten patients were identified in Zhejiang Province,with an age of onset ranging from 112 days to 17 *** the latest National recommendations for diagnosis and treatment of pneumonia caused by 2019-nCo V (the 4th edition) and current status of clinical practice in Zhejiang Province,recommendations for the diagnosis and treatment of respiratory infection caused by 2019-nCoV for children were drafted by the National Clinical Research Center for Child Health,the National Children's Regional Medical Center,Children's Hospital,Zhejiang University School of Medicine to further standardize the protocol for diagnosis and treatment of respiratory infection in children caused by 2019-nCoV.

To recognize the clinical characteristics and outcomes of congenital tuberculosis,the authors retrospectively review 3 cases in their department and compared their data with 62 cases reported in the available Chinese literature between January 1984 and December *** of the cases were located the eastern of China,and clinical manifestation of 53.85%of the cases occurred at 1-3 weeks of *** manifestations with fever,cough,abdominal distension and hepatosplenomegaly are common in congenital *** occurred in86.15%of the patients,61.53%with hepatosplenomegaly,40%with cough,21.53%with abnormal liver function,and 16.92%with *** persisted and hepatosplenomegaly showed improvement.5 infants developed tuberculosis meningitis.34 cases died.27(41.5%) infant's mothers did not have any symptoms of tuberculosis before delivery.

Hemophagocytic syndrome(HPS) represents a severe hyperinflammatory condition with hypercytokinemia owing to a highly stimulated but ineffective immune response,leading to uncontrolled phagocytosis of platelets,erythocytes,lymphocytes,and then hematopoietic precursors throughout the reticuloendothelial *** may be either primary or *** hemophagocytic syndrome is best known to be associated with viral infection but other associated diseases have also been implicated including tuberculosis(TB).Reports of HPS associated with TB in neonates are *** describe a case of tuberculosis-associated HPS in a *** manifestations with fever,abdominal distension and hepatosplenomegaly are common in congenital *** our case,the infant presented with similar clinical features and pancytopenia,hemophagocytosis in bone *** to the Histiocyte Society Protocol entitled HPS 2004,the diagnosis of HLH can be established if 5 out of 8 diagnostic criteria are ful?lled(i) Fever(2=7 days),(ii) Splenomegaly,(iii)Cytopenia(≥2 lineage)-anaemia(hemoglobin2400 u/*** our case 6 out of 8 criteria were ful?lled,so the diagnosis of tuberculosis associated with hemophagocytic syndrome was given.

Objective:Hypoglycemia is one of the common metabolic problems in contemporary neonatal *** features of infants who su?ered from brain damage as a result of neonatal hypoglycemia were shown to have similar patterns,and to a?ect the parietal and occipital lobes most *** purpose was to assess the patterns of neuroradiologic changes injury observed in patients who suffered brain damage consequent to neonatal *** used magnetic resonance imaging(MRI) and cranial ultrasonography(US) scans.A secondary aim was to address the question of whether brain magnetic resonance imaging(MRI) is more informative by comparing cranial ultrasonography(US) to magnetic resonance imaging(MRI) ***:The imaging studies and clinical records of thirty three patients of neonatal hypoglycemia were reviewed *** of injury were compared with those described in the literature and those seen in neonatal hypoxic-ischemic ***:We hypothesized that MRI features of neonatal hypoglycemia could be *** or US showed evidence of abnormality in 54.5%the hypoglycemic *** most common observed abnormal findings were injury primarily to the parietal and occipital lobes of the brain,and it showed as hyperintense *** summary,the abnormalities are located predominantly on the posterior aspects of the cerebral cortex associated with periventricular white matter ***,abnormal signals also exist in the basal ganglia and posterior limb of internal ***:MRI detected more abnormalities in the brains of neonatal hypoglycemia than *** pattern of bilateral occipital cortical injury is quite specific for neonatal *** with occipital diffusion restriction were more likely to have experienced neonatal seizures,but these findings were not statistically significant.

Objectives:Neonatal infants with fever were more likely to undergo invasive procedures like lumbar puncture(LP).To provide the evidence of LP related with febrile neonates in the clinic,we discussed the risk factors of abnormal cerebrospinal fluid(CSF) test for febrile ***:A retrospective cohort study was conducted on 566 febrile neonates with *** characteristics were compared and non-conditional logistic regression analysis was performed to determine independent predictors for LP with abnormal CSF test in febrile ***:There were 146(25.80%) cases of abnormal CSF test in febrile *** logistic regression analysis,the following factors independently predicted abnormal CSF test in febrile neonates:enterovirus infection[odds ratio(OR):4.652;95% confidence interval(CI):2.254-12.248],highest temperature>38.35℃(OR:2.948;95% CI:1.733-5.014),age of onset≤9.5 days(OR:2.341;95%CI:1.435-3.831),positive blood culture(OR:2.163;95%CI:1.327-3.678),premature rupture of membrane(OR:2.088;95%CI:1.245-3.496),prior antibiotic use≤1.5 days(OR:1.898;95%CI:1.278-2.819) and duration of fever>3.5 days(OR:1.072;95%CI:1.021-1.125).Conclusion:The factors which may predict the risk of abnormal CSF test in febrile neonates were enterovirus infection,highest temperature>38.35 ℃,age of onset≤9.5 days,positive blood culture,premature rupture of membrane,prior antibiotic use≤1.5 days and duration of fever>3.5 *** data indicated that LP should be routinely considered in the high risk febrile neonates.
