Long-term survival following radiofrequency ablation of colorectal liver metastases: A retrospective study

AIM: To retrospectively evaluate the long-term survivalof patients that received radiofrequency ablation(RFA) therapies of colorectal liver metastases. MeTHODS: In 2005 to 2008, RFA of 105 colorectal liver metastases(CRLM) were performed on 49 patients in our institution. The liver metastases were evaluated, both before and after ablation therapies, with contrast enhanced computerised tomography and contrast enhanced ultrasonography. Histological evidence of malignant liver metastases was obtained in the few instances where contrast enhanced ultrasonography gave equivocal results. Accesses to the CRLM were guided ultrasonically in all patients. The data obtained from records of these ablations were retrospectively analysed and survival data were compared with existing studies in the ***: 1-, 2-, 3-, 4- and 5-year survival rates, when no stringent selection criteria were applied, were 92%, 65%, 51%, 41% and 29% respectively. To explore the impact of the number and size of CRLM on patients' survival, an exclusion of 13 patients(26.5%) with number of CRLM ≥ 5 and tumour size ≥ 40 mm resulted in 1-, 2-, 3-, 4- and 5-year survival rates improving to 94%, 69%, 53%, 42% and 31% respectively. It is of note that 9 of 49 patients developed extra-hepatic metastases, not visible or seen on pretreatment scans, just after RFA treatment. These patients had poorer survival. The development of extra-hepatic metastases in nearly 20% of the patients included in our study can partly account for modestly lower survival rates as compared with earlier studies in the ***: Our study underscores the fact that optimum patients' selection before embarking on RFA treatment is vitally important to achieving a superior outcome.

Dentoalveolar Segmental Osteotomy Combined with Orthodontic Treatment for an Impacted and Ankylosed Upper Canine: A Case Report

Ankylosis is the abnormal adhesion of alveolar bone to dentin or cementum and commonly seen after traumatic dental injuries. Treatment of impacted and ankylosed teeth solely by orthodontics alignment may be challenging. Consequently, several treatment alternatives have been proposed for the management of ankylosed teeth involving extraction, surgical luxation, and osteotomy followed by dentoalveolar distraction or conventional orthodontic treatment. This case report describes a 55-year-old female who was referred by her orthodontist due to an impacted and ankylosed maxillary upper left canine. A dentoalveolar segmental osteotomy with immediate repositioning of an upper canine was performed. One year after final orthodontic treatment a satisfying occlusal and esthetic treatment outcome was obtained.

Acute inflammatory demyelinating polyneuropathy associated with pegylated interferon α 2a therapy for chronic hepatitis C virus infection

The combination of pegylated interferon (Peg-IFN) and ribavirin is the standard of care for chronic hepatitis C virus (HCV) infection treatment. In general, common side effects related to this combination therapy are mild and are very well tolerated. However, peripheral neuropathy including demyelinating polyneuropathy related to Peg-IFN is extremely rare. We present the first case of an acute inflammatory demyelinating polyneuropathy (AIDP) associated with Peg-IFN-α 2a (Pegasys) after 16 wk of a combination therapy with Pegasys and ribavirin in a 65-year-old woman with chronic HCV infection. She developed tingling, numbness, and weakness of her upper and lower extremities and was hospitalized for acute neurological deficits. Her clinical course, neurological findings, an electromyogram (eMG), nerve conductions studies (NCS), muscle biopsy, and a sural nerve biopsy were all consistent with AIDP likely related to Pegasys use. The patient recovered completely with the use of intravenous immunoglobulin (IVIG) including physical therapy and neurological rehabilitation. It is very important that gastroenterologists and/or hepatologists recognize this rare neurological complication related to Peg-IFN treatment very early, since it requires a prompt discontinuation of therapy including an immediate referral to a neurologist for the confirmation of diagnosis, management, and the prevention of long-term neurological deficits.

environmental Profile of NO<sub>x</sub>Reduction by a Photocatalytic Surface Coating and a Vehicle Catalytic Converter

Nitrogen oxides (NOx) in urban air close to ground have significant health implications. Restrictions in traffic, mandatory use of catalytic converters on vehicles, and novel photocatalytic coatings on surfaces contribute to reducing the level of NOx in cities. The aim of this study is to establish environmental profiles of NOx removal by a Three-Way Catalyst (TWC) car converter and by a photocatalytic surface coating (for asphalt and concrete pavements) for fostering technological development in reducing the levels of NOx in urban air. We assessed the environmental performance for the removal of 1 kg NOx by the two technologies with Life Cycle Assessment (LCA;eF.3 impact assessment method). In order to do so, we established Life-Cycle-Inventory (LCI) data representing production, operation and end-of-life of the two technologies based on data from literature and industry. The production of photocatalytic surface coatings, used on concrete and asphalt, has environmental loads two orders of magnitude lower than the environmental benefits of NOx reduction expressed as a reduction in Photochemical Ozone Formation (POF), Acidification (A), and Terrestrial eutrophication (Te). The vehicle catalytic converter shows similar results except that the use of rare earth elements in the production constitutes a significant load to Freshwater ecotoxicity (FeT) and that additional use of fuel during operation induces a modest Climate Change (CC) impact. For both technologies, the environmental benefits of reducing NOx far exceed any adverse environmental aspects of the production of the technologies.

Objective:This article aimed to compare the peripheral blood flow intensity(BFI)in the maternal right and left kidney in uncomplicated third trimester pregnancies and to study possible associations between parenchymal BFI in the kidneys and maternal,labor and new-born ***:We conducted a prospective cohort study in third trimester pregnancies at Trondheim University Hospital,Norway,from January to April *** BFI in the maternal kidneys was examined with color Doppler ultrasound and the peripheral BFI was calculated using the Pixel Flux-method which is a method based on pixel wise calculation of color in an *** was coded by the color bar in the ultrasound *** peripheral BFI was calculated as the product of velocity and area encoded by all colored pixels divided by all pixels within the cortical layer by a software examining the color in each pixel of a standardized region of interest of the renal parenchyma in video-clips through a complete heart ***(cm/s)was thus calculated as follows:Results:In all,51 pregnant women were included in the study,but 17 were excluded,mainly due to movement artifacts from the pulsating aorta,leaving 34 women in the final study *** found significantly lower BFI in the cortex of the left kidney compared with the right kidney,0.37 cm/s versus 0.69 cm/s,respectively(P=0.04).Dividing into regions,the BFI was significantly lower in the proximal 25%of the left renal cortex compared to the corresponding right side(P=0.01),and in the proximal 50%cortex(P=0.02),but the differences were not significantly different in the distal 25%(P=0.06)or in the distal 50%(P=0.20)of the renal ***:The BFI in the left renal cortex was lower compared with the right renal cortex,and BFI in the left renal cortex was negatively correlated with blood pressure,but not significantly correlated with maternal age,body mass index or newborn *** did not observe any significant correlations

Construction,expression and binding specificity of bispecific CD3×VeGFR-2 and CD3×NCAM antibodies in the single chain and diabody format

Bispecific antibodies are recombinant proteins with novel immunological properties and therapeutic potential. Recombinant protein quality and activity of several bispecific antibodies comprising different variable domain combinations with respect to the parental monospecific single chain fragments (scFv) were evaluated after expression in bacteria or mammalian cells. The parental scFv proteins humanized anti-NCAM scFv, murine anti-VeGFR-2 scFv, murine and humanized anti-CD3 scFv, respectively, could successfully be expressed in e. coli, whereas the murine anti-NCAM scFv version could not be reliably detected. Bispecific CD3 × VeGFR-2 and CD3 × NCAM anti-bodies were expressed in the bispecific single chain and the single chain diabody format. However, the diabody derived from the murine anti-NCAM scFv could not efficiently be expressed in e. coli or in mammalian cells. Significant binding of the CD3 × NCAM single chain diabody comprising the humanized version of anti-CD3 and humanized version of anti-NCAM was efficient to both antigens. Nevertheless, binding of the bispecific single chain version to the NCAM antigen was inefficient in comparison to CD3 binding. In conclusion, the data could indicate that the result of scFv expression in bacteria may be predictive for the chances of success for functional expression of more complex bispecific derivatives.
