We aim to evaluate the incidence of incontinence following laser endoscopic enucleation of the prostate(EEP)comparing en-bloc(Group 1)versus 2-lobe/3-lobe techniques(Group 2).We performed a retrospective review of patients undergoing EEP for benign prostaticenlargement in 12 centers between January 2020 and January *** were presented as median and interquartile range(IQR).Univariable and multivariable logistic regression analysis was performed to evaluate factors associated with stress urinary incontinence(SUI)and mixed urinary incontinence(MUI).There were 1711 patients in Group 1 and 3357 patients in Group *** in Group 2were significantly younger(68[62–73]years vs 69[63–74]years,P=0.002).Median(interquartile range)prostate volume(PV)wassimilar between the groups(70[52–92]ml in Group 1 vs 70[54–90]ml in Group 2,P=0.774).There was no difference in preoperativeInternational Prostate Symptom Score,quality of life,or maximum flow ***,morcellation,and total surgical time weresignificantly shorter in Group *** 1 month,overall incontinence rate was 6.3%in Group 1 versus 5.3%in Group 2(P=0.12),and urge incontinence was significantly higher in Group 1(55.1%vs 37.3%in Group 2,P<0.001).After 3 months,the overall rate ofincontinence was 1.7%in Group 1 versus 2.3%in Group 2(P=0.06),and SUI was significantly higher in Group 2(55.6%vs 24.1%in Group 1,P=0.002).At multivariable analysis,PV and IPSS were factors significantly associated with higher odds of transient SUI/***,surgical time,and no early apical release technique were factors associated with higher odds of persistent SUI/MUI.

Dynamic Modeling, Testing, and Stability Analysis of an Ornithoptic Blimp

In order to study omithopter flight and to improve a dynamic model of flapping propulsion, a series 0f tests are conducted on a flapping-wing blimp. The blimp is designed and constructed from mylar plastic and balsa wood as a test platform for aerodynamics and flight dynamics. The blimp, 2.3 meters long and 420 gram mass, is propelled by its flapping wings. Due to buoyancy the wings have no lift requirement so that the distinction between lift and propulsion can be analyzed in a flight platform at low flight speeds. The blimp is tested using a Vicon motion tracking system and various initial conditions are tested including accelerating flight from standstill, decelerating from an initial speed higher than its steady state, and from its steady-state speed but disturbed in pitch angle. Test results are used to estimate parameters in a coupled quasi-steady aerodynamics/Newtonian flight dynamics model. This model is then analyzed using Floquet theory to determine local dynamic modes and stability. It is concluded that the dynamic model adequately describes the vehicle＇s nonlinear behavior near the steady-state velocity and that the vehicle＇s linearized modes are akin to those of a fixed-wing aircraft.

The non-ionizing and penetrative characteristics of terahertz(THz)radiation have recently led to its adoption across a variety of *** effectively utilize THz radiation,modulators with precise control are *** most recent THz modulators manipulate the amplitude,frequency,or phase of incident THz radiation,considerably less progress has been made toward THz polarization *** methods for polarization control suffer from high driving voltages,restricted modulation depth,and narrow band capabilities,which hinder device performance and broader ***,an ideal THz modulator that offers high modulation depth along with ease of processing and operation is *** this paper,we propose and realize a THz metamaterial comprised of microelectromechanical systems(MEMS)actuated by the phase-transition material vanadium dioxide(VO_(2)).Simulation and experimental results of the three-dimensional metamaterials show that by leveraging the unique phase-transition attributes of VO_(2),our THz polarization modulator offers notable advancements over existing designs,including broad operation spectrum,high modulation depth,ease of fabrication,ease of operation condition,and continuous modulation *** enhanced features make the system a viable candidate for a range of THz applications,including telecommunications,imaging,and radar systems.

Thulium：YAG VapoEnucleation of the prostate in large glands： a prospective comparison using 70- and 120-W 2-μm lasers

This study compared the efficacy of 70- and 120-w 2-μm thulium：YAG VapoEnucleation of the prostate （ThuVEP） for patients with benign prostatic obstruction （BPO）. A prospective analysis of 84 patients with symptomatic BPO and prostatic enlargement （t〉 60 ml） who underwent either 70-w （n=44） or 120-w ThuVEP （n=40） non-randomly was carried out. Patient demographics and perioperative and 12-month follow-up data were analysed. The mean prostate volume was 79.90---27.49 ml in patients who had received 70-w ThuVEP, which was less than in those who had received 120-w ThuVEP （88.53＋-25.10; P=0.033）. The mean enucleation （resected weight/laser time, 2.16±1.21 g min-1 vs. 1.23±0.60 g min-1; P=-0.013）, operation efficiency （resected weight/total operation time, 0.76±0.35 g min-1 vs. 0.42＋-0.27 g min-1; P=-0.000） and percentage of resected tissue （66.93%±22.79% vs. 45.41%±23.33%; P=-0.000） were higher with 120-w treatment compared to 70-w ThuVEP. One patient （1.2% of total patients）（in the 120-w group） required a blood transfusion postoperatively. Sixty-one patients （73%） were available for review at the 12-month follow-up time point. The quality of life （QoL）, International Prostate Symptom Score （IPSS）, maximum urinary flow rate （Qmax）, postvoiding residual urine （PVR） and prostate volume improved significantly after treatment （P≤ 0.035） and were not significantly different between those treated with the different devices （70- and 120-w）. The median prostate volume reduction was 81.70% and 82.19% with 70- and 120-w ThuVEP, respectively. The incidence of complications was low and did not differ between groups treated with the different devices. Two patients （2.4%）（one per group） had a bladder neck contracture at the follow-up. ThuVEP is a safe and efficacious procedure for the treatment of symptomatic BPO. The incidence of complications was low with both devices. The 120-w thulium：YAG device enhances the effectiveness of Th,VEP with reeard to the oercentaee of resected tissue

Contemporary role of multi-tract percutaneous nephrolithotomy in the treatment of complex renal calculi

Complex renal calculi remain to be a challenge for the treating urologist due to sheer bulk and the technicalities *** nephrolithotomy(PCNL)remains the treatment modality of choice in dealing with these large and complex *** limiting factor in their treatment continues to be the need for using additional tracts or the use of flexible nephroscopy for complete stone *** systematic review focuses on the need for multi-tract PCNL for complex renal *** literature review was performed using PubMed database using the keywords“multiple tract PCNL”or“multiperc”.We identified original articles published on the usage of multiple tracts for stone clearance in renal calculi between January 2000 to October 2018,and the search was restricted to available literature in English language *** studies with n>20 were included for the final *** analyzed the technical efficacy with respect to the number of tracts and stages that were required for stone clearance,outcomes and complications,especially,procedural bleeding and post-procedure infective complications of multiple-tract PCNL for large burden renal *** is found to be safe,feasible and effective for the management of large burden complex renal calculi with respect to stone clearance and morbidity associated with the *** is cost effective and complete stone clearance as a single procedure is higher in comparison to flexible ureteroscopy and shockwave lithotripsy.

Alcohol binging causes peliosis hepatis during azathioprine therapy in Crohn's disease

Patients with inflammatory bowel disease have normal life expectancy and, due to modern immunosuppressive therapies, also a normal quality of life. Since mostly young people are affected, their social behaviour suits this environment. Alcohol binging is an increasingly disturbing factor among young people. We describe a patient with Crohn's disease, treated with azathioprine, who developed peliosis hepatis after three epsiodes of alcohol binging. Liver toxicity was not observed previously during the course of the treatment. Azathioprine-induced peliosis hepatis is thought to be idiosyncratic in humans. From animal studies, however, it is clear that hepatic depletion of glutathione leads to azathioprine toxicity to the sinusoidal endothelial cells. Damage of these cells causes peliosis hepatis. Since alcohol binging leads to hepatic glutathione depletion, we conclude that in our patient the episodes of binging have reduced liver gluathione content and therefore this has increased azathioprine toxicity causing peliosis hepatis. The problem of alcohol binging has not yet been addressed in IBD patients undertaking immunosuppressive therapy. This should be reviewed in future considerations regarding patients advice.

Serum levels of soluble Fas, nitric oxide and cytokines in acute decompensated cirrhotic patients

AIM: To evaluate plasma levels of nitrite/nitrate (NOx), soluble Fas (sFas) antigen, tumor necrosis factor alpha (TNF-α) and interleukin-6 (IL-6) in patients with com- pensated and acute decompensated cirrhosis and to evaluate mediators causing acute decompensation in liver cirrhosis. METHODS: This prospective study was conducted in the medical intensive care unit of an academic tertiary center. Fifty-five patients with acute decompensation (gastrointestinal hemorrhage, encephalopathy, hydropic decompensation) and twenty-five patients with compen- sated liver cirrhosis were included. Blood samples were taken for analyses of sFas, Nox, IL-6, TNF-α. Liver en- zymes and kidney functions were also tested. RESULTS: In patients with acute decompensation, plas- ma sFas levels were higher than in non-decompensated patients (15 305 ±4646 vs 12 458 ±4322 pg/mL,, P < 0.05). This was also true for the subgroup of patients with alcoholic liver cirrhosis (P < 0.05). The other media- tors were not different and none of the parameters pre- dicted survival, except for ALT (alanine-aminotransfer- ase). In patients with portal-hypertension-induced acute hemorrhage, NOx levels were significantly lower than in patients with other forms of decompensation (70.8 ± 48.3 vs 112.9 ± 74.9 pg/mL, P < 0.05). When NOx lev- els were normalized to creatinine levels, the difference disappeared. IL-6, TNF-α and sFas were not different between bleeders and non-bleeders. In decompensated patients sFas, IL-6 and NOx levels correlated positively with creatinine levels, while IL-6 levels were dependent on Child class. CONCLUSION: In acute decompensated cirrhotic pa- tients sFas is increased, suggesting a role of apoptosis in this process and patients with acute bleeding have lower NOx levels. However, in this acute complex clinical situa-tion, kidney function seems to have a predominant influ- ence on mediator levels.

Recording neural signals from delicate autonomic nerves is a challenging task that requires the development of a lowinvasive neural interface with highly selective,micrometer-sized *** paper reports on the development of a three-dimensional(3D)protruding thin-film microelectrode array(MEA),which is intended to be used for recording low-amplitude neural signals from pelvic nervous structures by penetrating the nerves transversely to reduce the distance to the *** gold pillars(O 20 or 50μm,~60μm height)were fabricated on a micromachined polyimide substrate in an electroplating *** sidewalls were insulated with parylene C,and their tips were optionally modified by wet etching and/or the application of a titanium nitride(TiN)*** microelectrodes modified by these combined techniques exhibited low impedances(~7 kΩ at 1 kHz for ∅ 50μm microelectrode with the exposed surface area of~5000μm^(2))and low intrinsic noise *** functionalities were evaluated in an ex vivo pilot study with mouse retinae,in which spontaneous neuronal spikes were recorded with amplitudes of up to 66μ*** novel process strategy for fabricating flexible,3D neural interfaces with low-impedance microelectrodes has the potential to selectively record neural signals from not only delicate structures such as retinal cells but also autonomic nerves with improved signal quality to study neural circuits and develop stimulation strategies in bioelectronic medicine,e.g.,for the control of vital digestive functions.
