Last line therapy with sorafenib in colorectal cancer: A retrospective analysis

AIM: To analyze the efficacy of last line sorafenib treatment in colorectal cancer patients. METHODS: All patients receiving chemotherapy for colorectal cancer in the outpatient clinic of the University of Mainz since 2006 were retrospectively analyzed for last line sorafenib exposure. charts of identified patients were analyzed for clinic-pathological parameters, like data on gender, age, date of initial diagnosis, UIcc stage, number and kind of the pretherapies, therapy start and end of sorafenib, sorafenib mediated treatment cessation, side effects, response rates, time to progression and overall survival. RESULTS: Ten patients with a median of 3.0 prior chemotherapy lines had received a last line sorafenib therapy either alone(10%) or in combination with 5-fluorouracil derivates(90%). All patients suffered from colorectal cancer stage UIcc 4 and were routinely seen in 2-wk intervals in the oncology outpatient clinic. Median duration of treatment was 142.0 d. At 8 wk 80% of patients showed stable disease but we did not observe any remissions. Median time to progression was 140.5 d(4.7 mo), while median overall survival reached 176.5 d. One patient ceased treatment due to side effects. Reason for treatment stop was bleeding complication in one case and non-specified sorafenib intolerance in another case. Due to the retrospective approach we did not further quantify side ***: This retrospective analysis encourages further investigation of sorafenib in colorectal cancer last line therapy.

Introduction: In côte d’Ivoire, there is a scarcity of data on children’s purulent pleurisies. Objective: This study aims to elucidate the epidemiological, diagnostic, therapeutic, and evolutionary facets of non-tuberculous purulent pleurisies in pediatric patients. Methods: A retrospective analysis was conducted using the medical records of children aged one month to fifteen years with purulent pleurisies at Bouaké University Hospital center from January 2017 to December 2021. Results: The study identified 124 cases of purulent pleurisies, constituting 18% of lower respiratory tract infections and 0.8% of all hospitalizations. The majority of these cases (69%) were in children between 1 and 24 months of age. Prominent symptoms included dyspnea (85.5%), O2 saturation below 95% in room air (76.6%), respiratory distress (68.5%), cutaneous-mucosal pallor (63.7%), and fever (43.5%). Radiological findings predominantly showed right-sided pleurisy (62.1%). The pleurisy was often extensive (78.2%), accompanied by pneumothorax (37.1%), alveolo-interstitial opacities (8.1% of cases), and abscess formations (1.6%). Pleural fluid cultures were positive in 46.9% of cases, with Staphylococcus aureus (75%, methi-S) identified among 32 bacteria. Initial antibiotic treatment was empirical, favoring oxacillin (53.2%) or amoxicillin-clavulanic acid (53.2%) in dual (42%) or triple therapy (33%) with gentamicin (64.1%) and/or metronidazole (21.8%). Treatments also included pleural drainage (68.5%) or repeated evacuation punctures (33.1%), and blood transfusion (39%). The mortality rate was 18.8%. conclusion: Non-tuberculous purulent pleurisy remains a significant concern in pediatric hospitalizations at the cHU of Bouaké, marked by high mortality rates.

Instability following total hip arthroplasty(THA) is an unfortunately frequent and serious problem that requires thorough evaluation and preoperative planning before surgical intervention. Prevention through optimal index surgery is of great importance, as the management of an unstable THA is challenging even for an experienced joints surgeon. However, even after well-planned surgery, a significant incidence of recurrent instability still exists. Non-operative management is often successful if the components are well-fixed and correctly positioned in the absence of neurocognitive disorders. If conservative management fails, surgical options include revision of malpositioned components; exchange of modular components such as the femoral head and acetabular liner; bipolar arthroplasty; tripolar arthroplasty; use of a larger femoral head; use of a constrained liner; soft tissue reinforcement and advancement of the greater trochanter.

[目的]建立高效液相色谱-紫外检测器(HPLc-UV)法测定银杏内酯A、B、c的浓度,并对其进行方法学验证。[方法]考察了包括检测波长、8种不同的流动相体系及其比例、进样量、流速和柱温。方法验证涵盖了专属性、精密度、准确度、线性范围、重复性、系统适用性、加样回收率和稳定性。[结果]色谱柱为Agilent Tc-5 c_(18),检测波长为220 nm,流动相为水-甲醇(70∶30),进样量为20μL,柱温30℃,流速为1.0 mL/min且线性关系良好,色谱峰之间无互相干扰。[结论]该方法准确、有效,经济且易得,操作简单,为其他银杏内酯药物含量测定提供了可靠的参考。

This paper adopts the NGI-ADP soil model to carry out finite element analysis,based on which the effects of soft clay anisotropy on the diaphragm wall deflections in the braced excavation were *** than one thousand finite element cases were numerically analyzed,followed by extensive parametric *** models were developed via ensemble learning methods(ELMs),including the e Xtreme Gradient Boosting(XGBoost),and Random Forest Regression(RFR)to predict the maximum lateral wall deformation(δhmax).Then the results of ELMs were compared with conventional soft computing methods such as Decision Tree Regression(DTR),Multilayer Perceptron Regression(MLPR),and Multivariate Adaptive Regression Splines(MARS).This study presents a cutting-edge application of ensemble learning in geotechnical engineering and a reasonable methodology that allows engineers to determine the wall deflection in a fast,alternative way.

Based upon the powerful Hirota method for unearthing soliton solutions to nonlinear partial differential evolution equations,we investigate the scattering properties of a new coupled integrable dispersionless system while surveying the interactions between its self-confined travelling wave *** a result,we ascertain three types of scattering features depending strongly upon a characteristic *** such findings to depict soliton solutions with nonzero angular momenta,we derive an extended form of the dispersionless system,which is valuable for further physical applications.

Ever since the disastrous floods of 1998, the chinese government has used the Natural Forest Protection and Sloping Land conversion Programs to promote afforestation and reforestation as means to reduce runoff, control erosion, and stabilize local livelihoods. These two ambitious programs have been reported as large-scale successes, contributing to an overall increase in china’s forest cover and to the stated goals of environmental stabilization. A small-scale field study at the project level of the implementation of these two programs in Baiwu Township, Yanyuan county, Sichuan, casts doubt upon the accuracy and reliability of these claims of success; ground observations revealed utter failure in some sites and only marginal success in others. Reasons for this discrepancy are posited as involving ecological, economic, and bureaucratic factors. Further research is suggested to determine whether these discrepancies are merely local aberrations or represent larger-scale failures in reforestation programs.

In the present work, hybrid sintering technique which couples the resistive heating and microwave heating is employed to sinter infrared transparent La0.15Y1.85O3 to 99.2% of the theoretical density for the first time to the best of our knowledge. The presence of La3+ in the yttria matrix improves the hardness properties to a greater extent without affecting the transmittance properties, but there is a deterioration in the thermal properties of the sample. So we have limited our studies to La0.15Y1.85O3 which shows better optical, thermal, and hardness properties. The pellets fabricated from the ultra-fine nano powder with average particle size of ~12 nm synthesized by combustion technique and sintered at 1430 ℃ with an average grain size of 0.22 μm show ~80.1% transmittance in the UV–visible region and 81% in mid infrared region. For a comparative study of the optical, mechanical, and thermal properties, two other variants of sintering strategies namely conventional sintering and microwave sintering are also employed. A comprehensive analysis on the hardness reveals that the hardness of the pellets sintered via hybrid heating is 9.73 GPa and is superior to the pellets sintered using the other two techniques. The thermal conductivity of the sample is also analyzed in detail. The results clearly indicate that the La0.15Y1.85O3 ultra-fine nano powder synthesised by the single-step combustion method and sintered via microwave hybrid heating shows better transmittance properties without compromising the mechanical properties, and can be used very effectively for the fabrication of infrared transparent windows and domes.
