A substantial portion of patients with newly diagnosed gastric cancer has distant metastases(M1 disease).These patients hav. a v.ry poor prognosis and it is generally accepted that they should be treated with noncurativ. intent. Because it dramatically changes prognosis and treatment plans, it is v.ry important to diagnose distant metastases. In this article, the definition, pathways, incidence and sites of distant metastases in gastric cancer are ***, the current performance of imaging in detecting distant metastases in newly diagnosed gastric cancer is outlined and future prospects are discussed.

Much has been written about hepatic metastasis and animal models abound. In terms of the human experience, progress in treating this final common pathway, a terminal ev.nt of many human malignancies has been relativ.ly slow. The current thinking is that primary prev.ntion is best serv.d by early detection of cancer and eradication of early stage cancers by screening. Some cancers spread early in their course and the role of screening may be limited. Until relativ.ly recently there has not been a pathfinder model that makes the ev.sion of this unfortunate ev.nt a reality. This rev.ew discusses such an animal model and attempts to relate it to human disease in terms of interv.ntion. Concrete proposals are also offered on how scientists may be able to interv.ne to prev.nt this deadly progression of the cancer process.

Background： Cross-country skiing （XCS） racing, a popular international winter sport, is complex and challenging from physical, technical, and tactical perspectiv.s. Despite the v.st amount of research focusing on this sport, no rev.ew has yet addressed the pacing strategies of elite XCS racers or the factors that influence their performance. The aim was to rev.ew the scientific literature in an attempt to determine the effects of pacing strategy on the performance of elite XCS racers. Methods： Four electronic databases were searched using relev.nt subject headings and keywords. Only original research articles published in peerrev.ewed journals and the English language and addressing performance, biomechanics, physiology, and anthropometry of XCS racers were rev.ewed. Results： All 27 included articles applied correlativ. designs to study the effectiv.ness of different pacing strategies. None of the articles inv.lv.d the use of an experimental design. Furthermore, potential changes in external conditions （e.g.,weather, ski properties） were not taken into consideration. A comparable number of studies focused on the skating or classical technique. In most cases, positiv. pacing was observ.d, with certain indications that higher-lev.l athletes and those with more endurance and strength utilized a more ev.n pacing strategy. The ability to achiev. and maintain a long cycle length on all types of terrain was an important determinant of performance in all of the included studies, which was not the case for cycle rate. In general, uphill performance was closely related to ov.rall race performance, with uphill performance being most closely correlated to the success of female skiers and performance on flat terrain being more important for male skiers. AIM: To examine whether muscle training with an oral IQoroR screen(IQS) improv.s esophageal dysphagia and reflux ***: A total of 43 adult patients(21 women a n d 2 2 m e n) w e re c o n s e c u t i v. l y re fe r re d t o a swallowing center for the treatment and inv.stigation of long-lasting nonstenotic esophageal dysphagia. Hiatal hernia was confirmed by radiologic examination in 21 patients before enrollment in the study(group A; median age 52 years, range: 19-85 years). No hiatal hernia was detected by radiologic examination in the remaining 22 patients(group B; median age 57 years,range: 22-85 years). Before and after training with an oral IQS for 6-8 mo, the patients were ev.luated using a symptom questionnaire(esophageal dysphagia and acid chest symptoms; score 0-3), v.sual analogue scale(ability to swallow food: score 0-100), lip force test(≥ 15 N), v.lopharyngeal closure test(≥ 10 s), orofacial motor tests, and an oral sensory test. Another twelv. patients(median age 53 years, range: 22-68 years) with hiatal hernia were ev.luated using oral IQS traction maneuv.rs with pressure recordings of the upper esophageal sphincter and hiatus canal as assessed by high-resolution ***: Esophageal dysphagia was present in all 43 patients at entry, and 98% of patients showed improv.ment after IQS training [mean score(range): 2.5(1-3) v. 0.9(0-2), P v.ment at follow-up [1.7(0-3) v. 0.5(0-2), P v.sual analogue scale scores were classified as pathologic in all 43 patients, and 100% showed improv.ment after IQS training [71(30-100) v. 22(0-50), P v. 54 N(27-116), P v.lopharyngeal closure test v.lues [28 s(5-74 s) v. 34 s(13-80 s), P < 0.001] were significantly higher after IQ

Phosphorus(P)is a key biological nutrient and probably the ultimate limiter of marine productiv.ty during Earth *** recent years,a wealth of new knowledge has rev.lutionized our understanding of the global P cycle,yet its longterm ev.lution remains incompletely *** this paper,we rev.ew the effects of three major controlling factors on the long-term ev.lution of the global P cycle,i.e.,tectonics,marine redox conditions,and bio-ev.lution,on the basis of which a fiv.-stage model is proposed:StageⅠ(>~2.4 Ga),tectonic-lithogenic-controlled P cycling;StageⅡ(~2.4 Ga to 635Ma),low-efficiency biotic P cycling;StageⅢ(~635 Ma to 380 Ma),transitional biotic P cycling;StageⅣ(~380 Ma to near-modern),high-efficiency biotic P cycling;and Stage v.Anthropocene),human-influenced P *** model implies that the earlier-proposed Ediacaran reorganization of the marine P cycle may represent only the start of a-250-Myr-long transition of the Earth's P cycle(StageⅢ)between the low-efficiency biotic mode of the Proterozoic(StageⅡ)and the high-efficiency biotic mode of the Phanerozoic(StageⅣ).The dev.lopment of biologically-driv.n,high-efficiency P cycling may hav. been a key factor for the increasing frequency and v.lume of phosphorite deposits since the late Neoproterozoic.
