Pancreas preserving distal duodenectomy: A versatile operation for a range of infra-papillary pathologies

To investigate the range of pathologies treated by pancreas preserving distal duodenectomy (PPDD) and present the outcome of *** lesions of the duodenum are treated conventionally by pancreaticoduodenectomy. Lesions distal to the major papilla may be suitable for a pancreas-preserving distal duodenectomy, potentially reducing morbidity and mortality. We present our experience with this procedure. Selective intraoperative duodenoscopy assessed the relationship of the papilla to the lesion. After duodenal mobilisation and confirmation of the site of the lesion, the duodenum was transected distal to the papilla and beyond the duodenojejunal flexure and a side-to-side duodeno-jejunal anastomosis was formed. Patients were identified from a prospectively maintained database and outcomes determined from digital health records with a dataset including demographics, co-morbidities, mode of presentation, preoperative imaging and assessment, nutritional support needs, technical operative details, blood transfusion requirements, length of stay, pathology including lymph node yield and lymph node involvement, length of follow-up, complications and outcomes. Related published literature was also ***-four patients had surgery with the intent of performing PPDD from 2003 to 2016. Nineteen underwent PPDD successfully. Two patients planned for PPDD proceeded to formal pancreaticoduodenectomy (PD) while three had unresectable disease. Median post-operative follow-up was 32 mo. Pathologies resected included duodenal adenocarcinoma (n = 6), adenomas (n = 5), gastrointestinal stromal tumours (n = 4) and lipoma, bleeding duodenal diverticulum, locally advanced colonic adenocarcinoma and extrinsic compression (n = 1 each). Median postoperative length of stay (LOS) was 8 d and morbidity was low [pain and nausea/vomiting (n = 2), anastomotic stricture (n = 1), pneumonia (n = 1), and overwhelming post-splenectomy sepsis (n = 1, asplenic patient)]. Photometric investigation of eight ultra-short period eclipsing binaries from OGLE

We performed a detailed photometric analysis of eight ultra-short period eclipsing binaries(USPEBs) using the Wilson-Devinney method. We present the modeled light curves and derived photometric solutions. The USPEBs with period(P)≤0.21 d considered in our study belong to W-subtype having shallow contact factor(f)~0.7 and later spectral types. The absolute parameters for these short-period binaries were derived applying empirical relations. We discuss the evolutionary stage of these USPEBs using the mass-radius, color-density and period-color diagrams. The objects showed poor metallicities, and some objects were even found to be existing around fully convective limits. The period distribution of USPEBs exhibited a sharp cut-off at 0.22 d;however, we observed significant deficits for our objects in the literature. We examined the statistics of USPEBs studied to date(in terms of the distribution of period, mass ratio and component temperatures of USPEBs) and observed that a dominant distribution of component temperatures for these USPEBs was towards lower temperatures.

Early warning system for shallow landslides using rainfall threshold and slope stability analysis

A combined cluster and regression analysis were performed for the first time to identify rainfall threshold that triggers landslide events in Amboori, Kerala, India. Amboori is a tropical area that is highly vulnerable to landslides. The 2, 3, and 5-day antecedent rainfall data versus daily rainfall was clustered to identify a cluster of critical events that could potentially trigger landslides. Further, the cluster of critical events was utilized for regression analysis to develop the threshold equations. The 5-day antecedent(xvariable) vs. daily rainfall(y-variable) provided the best fit to the data with a threshold equation of y = 80.7-0.1981 x. The intercept of the equation indicates that if the 5-day antecedent rainfall is zero, the minimum daily rainfall needed to trigger the landslide in the Amboori region would be 80.7 mm. The negative coefficient of the antecedent rainfall indicates that when the cumulative antecedent rainfall increases, the amount of daily rainfall required to trigger monsoon landslide decreases. The coefficient value indicates that the contribution of the 5-day antecedent rainfall is～20% to the landslide trigger threshold. The slope stability analysis carried out for the area, using Probabilistic Infinite Slope Analysis Model(PISA-m), was utilized to identify the areas vulnerable to landslide in the region. The locations in the area where past landslides have occurred demonstrate lower Factors of Safety(FS) in the slope stability analysis. Thus, rainfall threshold analysis together with the FS values from slope stability can be suitable for developing a simple, cost-effective, and comprehensive early-warning system for shallow landslides in Amboori and similar regions.

AIM：To investigate the association of variations in the cyclooxygenase-2 （COX2） and uridine diphosphate glucuronosyltransferase 1A6 （UGTIA6） genes and non-steroidal anti-inflammatory drugs （NSAIDs） use with risk of colon ***： NSAIDs, which are known to reduce the risk of colon cancer, act directly on COX2 and reduce its activity. Epidemiological studies have associated variations in the COX2 gene with colon cancer risk, but others were unable to replicate this finding. Similarly,enzymes in the UGT1A6 gene have been demonstrated to modify the therapeutic effect of NSAIDs on colon adenomas. Polymorphisms in the UGTIA6 gene have been statistically shown to interact with NSAID intake to influence risk of developing colon adenomas, but not colon cancer. Here we examined the association of tagging single nucleotide polymorphisms （SNPs） in the COX2 and UGTIA6 genes, and their interaction with NSAID consumption, on risk of colon cancer in a population of 422 colon cancer cases and 481 population ***： No SNP in either gene was individually statistically significantly associated with colon cancer, nor did they statistically significantly change the protective effect of NSAID consumption in our sample. Like others, we were unable to replicate the association of variants in the COX2 gene with colon cancer risk （P 〉 0.05）,and we did not observe that these variants modify the protective effect of NSAIDs （P 〉 0.05）. We were able to confirm the lack of association of variants in UGT1A6 with colon cancer risk, although further studies will have to be conducted to confirm the association of these variants with colon ***： Our study does not support a role of COX2 and UGTIA6 genetic variations in the development of colon cancer.

Through a bioreplication approach, we have fabricated artificial visual decoys for the invasive species Agrilus planipen- nis--commonly known as the Emerald Ash Borer （EAB）. The mating behavior of this species involves an overflying EAB male pouncing on an EAB female at rest on an ash leaflet before copulating. The male spots the female on the leaflet by visually detecting the iridescent green color of the female＇s elytra. As rearing EAB and then deploying dead females as decoys for trapping is both arduous and inconvenient, we decided to fabricate artificial decoys. We used a dead female to make a negative die of nickel and a positive die of epoxy. Decoys were then made by first depositing a quarter-wave-stack Bragg reflector on a polymer sheet and then stamping it with a pair of matched negative and positive dies to take the shape of the upper surface of an EAB female. As nearly 100 artificial decoys were fabricated from just one EAB female, this bioreplication process is industrially scalable. Preliminary results from a field trapping test are indicative of success.

The impact of iodinated X-ray contrast agents on formation and toxicity of disinfection by-products in drinking water

The presence of iodinated X-ray contrast media（ICM） in source waters is of high concern to public health because of their potential to generate highly toxic disinfection by-products（DBPs）. The objective of this study was to determine the impact of ICM in source waters and the type of disinfectant on the overall toxicity of DBP mixtures and to determine which ICM and reaction conditions give rise to toxic by-products. Source waters collected from Akron,OH were treated with five different ICMs, including iopamidol, iopromide, iohexol,diatrizoate and iomeprol, with or without chlorine or chloramine disinfection. The reaction product mixtures were concentrated with XAD resins and the mammalian cell cytotoxicity and genotoxicity of the reaction mixture concentrates was measured. Water containing iopamidol generated an enhanced level of mammalian cell cytotoxicity and genotoxicity after disinfection. While chlorine disinfection with iopamidol resulted in the highest cytotoxicity overall, the relative iopamidol-mediated increase in toxicity was greater when chloramine was used as the disinfectant compared with chlorine. Four other ICMs（iopromide, iohexol, diatrizoate, and iomeprol） expressed some cytotoxicity over the control without any disinfection, and induced higher cytotoxicity when chlorinated. Only iohexol enhanced genotoxicity compared to the chlorinated source water.

Twenty-first century climate change and submerged aquatic vegetation in a temperate estuary:the case of Chesapeake Bay

Introduction:The Chesapeake Bay was once renowned for expansive meadows of submerged aquatic vegetation(SAV).However,only 10%of the original meadows *** restoration effortswill be complicated by accelerating climate change,including physiological stressors such as a predicted mean temperature increase of 2-6℃and a 50-160%increase in CO_(2)***:As the Chesapeake Bay begins to exhibit characteristics of a subtropical estuary,summer heat waves will become more frequent and *** alone would eventually eliminate eelgrass(Zostera marina)from the *** will favor native heat-tolerant species such as widgeon grass(Ruppia maritima)while facilitating colonization by non-native seagrasses(e.g.,Halodule spp.).Intensifying human activity will also fuel coastal zone acidification and the resulting high CO_(2)/low pH conditions may benefit SAV via a“CO_(2)fertilization effect.”Discussion:Acidification is known to offset the effects of thermal stress and may have similar effects in estuaries,assuming water clarity is sufficient to support CO_(2)-stimulated photosynthesis and plants are not overgrown by ***,coastal zone acidification is variable,driven mostly by local biological processes that may or may not always counterbalance the effects of regional *** precarious equipoise between two forces-thermal stress and acidification-will be critically important because it may ultimately determine the fate of cool-water plants such as Zostera marina in the Chesapeake ***:The combined impacts of warming,coastal zone acidification,water clarity,and overgrowth of competing algae will determine the fate of SAV communities in rapidly changing temperate estuaries.
