Three children with severe atopic dermatitis were noted at routine follow- up to have developed multiple small pigmented macules during long- term therapy with topical tacrolimus 0.1% (Protopic., Fujisawa). Representative lesions in two of the three cases were confirmed histologically as simple lentigines. The focal distribution of lentigines to sites of tacrolimus use, and the temporal association between use of tacrolimus and development of lesions, suggest that topical tacrolimus is of direct aetiological relevance to their development. Careful long-term follow-up will be required to assess the clinical implications of these findings and whether they represent an increase in risk for melanocytic neoplasia.

目的研究CT源性血流参数是否能够预测新辅助化疗在原发性乳腺癌中病理学的完全缓解(pCR)。方法 20例可能为原发性乳腺癌病人经过新辅助化疗(NAC)通过容积CT灌注经反卷积分析获得整个肿瘤区域的血流量(BF)、血容量(BV)和流量提取(FE)。如果病理学没有检测到肿瘤残留,即为病人获得病理学完全缓解。用Mann-WhitneyU检验研究BF、BV、FE、肿瘤的大小和数量与病理学完全缓解之间的基线关系。受试者操作特征曲线(ROC)分析是最能进行评估预测缓解的参数。用Bland-Altman法评估组内差异和组间差异率。结果完成4次新辅助化疗的20例病人中有17例实现了病理学完全缓解。实现病理学完全缓解者的BF和FE的基线高于没有实现病理学完全缓解者(P=0.032);肿瘤的大小和数量无明显的差异(P>0.05)。ROC分析表明,BF和FE能够有效地识别缓解者(AUC=0.87;P=0.03)。组内差异和组间差异率基本一致。结论原发性乳腺癌在治疗前容积CT表现出高灌注水平时,行新辅助化疗更可能获得完全性病理缓解。要点①CT源性血流参数可能对乳腺癌的治疗有帮助。②CT灌注有助于预测新辅助化疗对乳腺癌的疗效。③在病理学完全缓解者中,BF和FE的基线是较高的。

Non-Herlitz junctional epidermolysis bullosa (jEB) is an autosomal recessive genodermatosis characterized by skin fragility and blistering. It is usually caused by mutations in the genes encoding the basement membrane proteins laminin 5 or type XVII collagen. Clinically, impairedwound healing and chronic erosions cause major morbidity in affected patients. Previously it was thought that these individuals, unlike patients with dystrophic EB, did not have an increased risk of developing skin cancer. However, we describe three patients with non-Herlitz jEB (aged 42, 56 and 75 years) who developed cutaneous squamous cell carcinomas (SCCs). The tumours were well-differentiated in two cases, but one patient had multiple primary SCCs that were either well-or moderately differentiated. Most cases of SCC in non-Herlitz jEB described have occurred in those with laminin 5 defects and on the lower limbs. These clinicopathological observations have important implications for the management of patients with this mechanobullous disorder as well as providing further insight into the biology of skin cancer associated with chronic inflammation and scarring.

Aim-The aim of this study was to determine the feasibility of patient’s satisfaction assessment after endoscopy using an interactive voice response(IVR) ***-A specific IVR system was developed for this study and proposed to patients by 161 private gastroenterologists after an endoscopic *** reminder was used for the patients not calling spontaneously the ***-After endoscopy,31% of the patients called the IVR and 1052 answered the entire *** answers obtained by the IVR and a face-to-face interview were concordant for 98.8% of the *** endoscopy was carried out with anesthesia for 94% of the patients and 95% stated they would agree to undergo the procedure again under the same *** was independently associated with the presence of explanations about the procedure before its realization,male gender and,for the patients having had a colonoscopy,difficulties in taking the bowel cleaner and the presence of pain after the ***-This study shows the feasibility of patient’s satisfaction assessment with endoscopy using an IVR system under routine conditions of endoscopy practice.

We are now preparing for the third generation (e,e' K+) Λ hypernuclear spectroscopic experiment at Hall C,jefferson Lab (USA). The goal of the experiment is the precise spectroscopy of hypernuclei in wide mass region. We have constructed a new high resolution electron spectrometer "HES" dedicated to (e,e' K+) hypernuclear study in japan and it was shipped to jLab in February,2008. We will discuss about the physics of the (e,e' K+) hypernuclear study at jLab and report the current preparation status of the third generatrion experiment.
