目的探讨应变和应变率在发现早期糖尿病性心肌病中的应用价值。方法应用GE Vivid5超声诊断仪及应变和应变率分析软件，检测137例受试者的心肌左心室6个室壁节段应变和应变率，其中单纯糖尿病组36例，糖尿病合并左心室肥厚组23例，单纯左心室肥厚组42例，正常对照组36例。结果患者组的心脏收缩及舒张功能均低于正常对照组（P〈0．001），最大收缩期应变和应变率绝对值亦均低于正常对照组（P〈0．05）；最大收缩期应变和应变率在糖尿病合并左心室肥厚组低于单纯糖尿病组（P〈0．05），亦低于单纯左心室肥厚组（P〈0．01）。结论应变和应变率可以早期发现无明显心脏病表现的糖尿病性心肌病。

局部心肌功能的评价在临床心脏病学中占有非常重要的地位.传统的心脏功能评价指标包括:射血分数、缩短分数、射血量等,它们都代表的是心脏的整体功能,这些参数在临床应用中的不足之处是:它们都有很明显的前负荷和后负荷依赖性,重复检查结果的变化较大.局部心肌功能评价,虽可根据心内膜的运动距离和心肌增厚程度,通过视觉给与一个主观判断,但评价结果的可靠性很大程度上依赖于超声图像的质量和超声图像读图人员的经验,而且,由于其受心脏旋转运动、心肌运动传导以及相邻心肌运动牵拉的影响,而使临床医生在应用这些检查结果时,产生踟躇心理.最近,出现两个新的评价局部心肌变形能力的超声技术指标,它们就是心肌应变(strain)和应变率(strain rate)超声成像技术.心肌应变和应变率图像可取自心肌组织速度图,或者直接取自二维超声图.

运动性高血压反应[hypertensive response to exercise，hRE；定义为诊室血压正常但运动时收缩压≥210（男性），≥190mmhg（女性），或舒张压≥105mmhg]可以独立地预测死亡率。该研究评估hRE患者中隐蔽性高血压的患病率及其与心血管危险因素（包括左心室质量指数）的相关性。方法：入选（54±9）岁未治疗的hRE患者72例（60％男性，经运动负荷超声心动图检查无冠状动脉性疾病）进行全面的临床和超声心动图（包括中心动脉压和脉搏波传导速度）检查及24h动态血压监测。

INTRODUCTION Recent technological readings of stone artefacts from Guanyindong Cave site,south-west China, have identified the Levallois strategy in sedimentary deposits dated to approximately 170 000–80 000 years ago. Some specimens in the assemblage are susceptible to misinterpretation. here, we provide further detailed descriptions of these specimens to clarify that they possess attributes consistent with the Levallois concept, as it has been widely employed elsewhere.

Objective: Screening for coronary artery disease is constrained by its low prevalence in unselected patients. We compared the ability of clinical scores to identify a high-risk group with diabetes mellitus and investigated a Bayesian strategy by combination with exercise echocardiography(ExE). Methods: The Framingham risk score(FRS), a score based on the American Diabetes Association(ADA)screening guidelines, the United Kingdom Prospective Diabetes Study(UKPDS)risk engine, and a disease-specific diabetic cardiac risk score(DCRS)were calculated in 199 asymptomatic patients with type 2 diabetes mellitus undergoing ExE. The frequency of abnormal ExE and the proportion of these with coronary stenoses were sought in groups designated as high risk on the basis of optimal cutoffs for each sco re. All patients were followed up for 1 year. Results: high risk was identified in fewer patients with the DCRS(27%)than FRS(38%, P=.02), ADA(41%, P=.004), a nd UKPDS(43%, P=.001). Exercise echocardiography was positive in 27(14%); 11 o f 23 proceeding to angiography showed significant stenoses. Areas under the rece iver operator characteristic curves for prediction of a positive ExE were simila r for DCRS, UKPDS, and FRS but less for ADA(P=.04). Positive ExE was uncommon in low-risk patients(8%-11%)and most were false positives(58%-80%). Cardiov ascular events(n=9)were more likely in the high-risk compared with the low-ris k UKPDS(9%vs 2%, P=.03)and DCRS(12%vs 2%, P=.01). Conclusion: Combination of the UKPDS or DCRS with ExE may optimize detection of coronary artery disease an d cardiac events in asymptomatic patients, while minimizing the numbers of ExE a nd false-positive rate.

This study was designed to predict the response and prognosis after cardiac re synchronization therapy(CRT) in patients with end-stage heart failure(hF). Card iac resynchronization therapy improves hF symptoms, exercise capacity, and left ventricular(LV) function. Because not all patients respond, preimplantation iden tification of responders is needed. In the present study, response to CRT was pr edicted by the presence of LV dyssynchrony assessed by tissue Doppler imaging. M oreover, the prognostic value of LV dyssynchrony in patients undergoing CRT was assessed. Eighty-five patients with end-stage hF, QRS duration >120 ms, and le ft bundle-branch block were evaluated by tissue Doppler imaging before CRT. At baseline and six months follow-up, New York heart Association functional class, quality of life and 6-min walking distance, LV volumes, and LV ejection fracti on were determined. Events (death, hospitalization for decompensated hF) were ob tained during one-year follow-up. Responders(74%) and nonresponders(26%) had comparable baseline characteristics, except for a larger dyssynchrony in respon ders (87±49 ms vs. 35±20 ms, pharacteristic curve analysis demonstrated that an optimal cutoff value of 65 ms for LV dyssynchrony yielded a sensitivity and specificity of 80%to predict clinical improvement an d of 92%to predict LV reverse remodeling. Patients with dyssynchrony ≥65 ms ha d an excellent prognosis(6%event rate) after CRT as compared with a 50%event r ate in patients with dyssynchronyh LV dyssynchron yhave an excellent prognosis after CRT.
