Critical analysis of feeding jejunostomy following resection of upper gastrointestinal malignancies

AIM To assess nutritional recovery,particularly regarding feeding jejunostomy tube(FJT)utilization,following upper gastrointestinal resection for malignancy. MeTHODS A retrospective review was performed of a prospectively-maintained database of adult patients who underwent esophagectomy or gastrectomy(subtotal or total)for cancer with curative intent,from January 2001 to June 2014. Patient demographics,the approach to esophagectomy,the extent of gastrectomy,FJT placement and utilization at discharge,administration of parenteral nutrition(PN),and complications were evaluated. All patients were followed for at least ninety days or until *** The 287 patients underwent upper GI resection,comprised of 182 esophagectomy(n=107 transhiatal,58.7%; n=56 Ivor-lewis,30.7%)and 105 gastrectomy [n=63 subtotal(SG),60.0%; n=42 total(TG),40.0%]. 181 of 182 esophagectomy patients underwent FJT,compared with 47 of 105 gastrectomy patients(99.5% vs 44.8%,P ergone TG(n=39,92.9% vs n=8 SG,12.9%,P edian length of stay was similar between esophagectomy and gastrectomy groups(14.7 d vs 17.1 d,P=0.076). Upon discharge,87 esophagectomy patients(48.1%)were taking enteral feeds,with 53(29.3%)fully and 34(18.8%)partially dependent. Meanwhile,20 of 39 TG patients(51.3%)were either fully(n=3,7.7%)or partially(n=17,43.6%)dependent on tube feeds,compared with 5 of 8 SG patients(10.6%),all of whom were partially dependent. Gastrectomy patients were significantly less likely to be fully dependent on tube feeds at discharge compared to esophagectomy patients(6.4% vs 29.3%,P=0.0006). PN was administered despite FJT placement more often following gastrectomy than esophagectomy(n=11,23.4% vs n=7,3.9%,P=0.0001). FJT-specific complications requiring reoperation within 30 d of resection occurred more commonly in the gastrectomy group(n=6),all after TG,compared to 1 esophagectomy patient(12.8% vs 0.6%,P=0.0003). We discuss a possible interpretation of the open charm mesons D＊s0 （2317）,D s1 （2460） and the hidden charm mesons X（3872）,Y（3940） and Y（4140） as hadron *** a phenomenological lagrangian approach we review the strong and radiative decays of the D＊ s0 （2317） and D s1 （2460） *** X（3872） is assumed to consist dominantly of molecular hadronic components with an additional small admixture of a charmonium *** the radiative （γJ/ψ and γψ（2s）） and strong （J/ψ2π and J/ψ3π） decay modes we show that the present experimental observation is consistent with the molecular structure assumption of the X（3872）.Finally,we give evidence for molecular interpretations of the Y（3940） and Y（4140） related to the observed strong decay modes J/ψ＋ω or J/ψ＋φ,respectively.

Observations of wild chelonians in Indochina are uncommon with most records and specimens coming from the commercial wildlife trade. During the surveys of the Xe Sap National Protected Area in southern laos between February and May 2012, four impressed tortoises, Manouria impressa(IUCN Red list: Vulnerable) were found in three separate field locations. There are few published observations of this species in the wild. We also present data on threats and local value of chelonians in the wildlife trade.

Plasmonic Ag@ZnO core@she.l nanoparticles are formed by synthesis inside helium droplets with subsequent deposition and controlled *** particle size and shape can be controlled from spherical sub-10 nm particles to larger elongated *** advantage of the method is the complete absence of solvents,precursors,and other chemical *** obtained particle morphology and elemental composition have been analyzed by scanning transmission electron microscopy(STeM)and energy dispersive X-ray spectroscopy(eDS).The results reve.l that the produced particles form a closed and homogeneous ZnO layer around a 2–3 nm Ag core with a uniform thickness of(1.33±0.15)nm and(1.63±0.31)nm for spherical and wire.like particles,*** results are supported by ultraviolet photoelectron spectroscopy(UPS),which indicates a fully oxidized she.l layer for the particles studied by *** plasmonic properties of the produced spherical Ag@ZnO core@she.l particles are investigated by two-photon photoelectron(2PPe)*** excitation of the.localized surface plasmon resonance in Ag at around 3 eV,plasmonic enhancement leads to the.liberation of electrons with high kinetic *** is observed for both Ag and Ag@ZnO particles,showing that even if a Ag cluster is covered by the ZnO layer,a plasmonic enhancement can be observed by photoelectron spectroscopy.

Pancreatic adenocarcinoma is the third leading cause of cancer death in the United States. Unfortunately, at diagnosis, most patients are not candidates for curative resection. Surgical palliation, a procedure performed with the intention of relieving symptoms or improving quality of life, comes to the forefront of management. This article reviews the palliative management of unresectable pancreatic cancer, including obstructive jaundice, duodenal obstruction and pain control with celiac plexus block. Although surgical bypasses for both biliary and duodenal obstructions usually achieve good technical success, they result in considerable perioperative morbidity and mortality, even when performed laparoscopically. The effectiveness of selfexpanding metal stents for biliary drainage is exce.lent with low morbidity. Surgical gastrojejunostomy for duodenal obstruction appears to be best for patients with a life expectancy of greater than 2 mo while endoscopic stenting has been shown to be feasible with good symptom relief in those with a shorter life expectancy. Regardless of the palliative procedure performed, all physicians involved must be adequately trained in end of life management to ensure the best possible care for patients.

Background： Sedentary behavior （SB）, physical activity （PA）, and attributes of physical fitness have been shown to be related to depressive symptoms in adolescents. The purpose of the present study was to investigate whether SB and fitness-producing activity predicted depression in active adolescents over and above gender and fitness attributes. Methods： Participants were 249 adolescents （age： 12.85 ± 0.89 years, mean ± SD） from 3 public middle schools who wore Actical accelerometers to assess their SB and PA. Participants also completed the FITNeSSGRAM he.lth-related fitness assessment and a brief depression questionnaire. A 3-step hierarchical regression analysis was conducted with gender and fitness attributes （i.e., body mass index （BMI）, maximal volume oxygen uptake （VO2max）, curlups, and pushups）, moderate- and vigorous-intensity activity, and SB entered in respective steps. Results： Regression analysis indicated activity variables （i.e., moderate- and vigorous-intensity activity） significantly predicted depression （△R^2= 0.12, p 〈 0.01） beyond gender and fitness attributes. Overall, gender, fitness attributes, activity variables, SB explained 31% of the variance in depression. Structure coefficients reve.led VO2max （rs= -0.77）, moderate-intensity activity （rs= -0.67）, vigorous-intensity activity （rs = -0.81）, and SB （rs = 0.57） were substantially correlated with the criterion variable; thus, they were the strongest predictors of depression. Conclusion： The findings of the current study indicated SB and PA were both significant predictors of depression; however, sufficient fitness-producing activity and adequate cardiorespiratory fitness may nullify the negative influence of SB on depressive symptoms in active adolescents.2018 Published by elsevier B.V. on behalf of Shanghai University of Sport. It has been reported that trivalent cerium salts are effective inhibitors for corrosion of AA2024-T3 in neutral to alkaline co rrosive enviro nments,but poor in acidic *** this study,work was done for extending corrosion resistance provided by Ce(NO_(3))_(3)to acidic pHs as low as 2.5 through the addition of iodide *** this end,potentiodynamic polarization was used to ascertain the optimum Ce(Ⅲ)concentration for the inhibition of AA2024-T3 corrosion in 3.5%NaCl *** optimum concentration is found to be in a range between 0.1 and 0.3 mmol/l Ce(Ⅲ).This optimum concentration also gives a maximum width of the passive region under the pitting potential(epit).Based on this optimization,further work was performed as a function of pH without and in the presence of 0.01 mol/l KI at five pH values(2.5,4,5,7,and 8).Without iodide the best inhibition provided by the Ce(Ⅲ)cation is at pH=7 but active corrosion is observed at pH=2.5 with an estimated inhibitor efficiency(-37.6%).The addition of iodide ions(I^(-))reverses the poor corrosion resistance whereby the inhibitor efficiency becomes 47.7%.Moreover,at pH=4 the efficiency is also increased from 54.0%to 81.0%with iodide addition providing a significant improvement over Ce(Ⅲ)cations alone under acidic *** is a synergistic effect between I-anion and Ce(Ⅲ)cation,which is determined to be 2.4 and 2.1 at pH=2.5and 4.0,*** pH=5 and 7 there is virtually no change in inhibition but at pH=8 the inhibition falls away due to a considerable amount of cerium precipitating out of solution as cerium *** protective film formed on sites in the aluminium alloy surface at different pH values was examined by both scanning electron microscopy with energy dispersive X-ray analysis and X-ray photoelectron spectroscopy to determine the role of these ions as a function of pH in surface *** is proposed that at pH=2.5 protection is provided by a bilayer adsorption of iodide with"cap

Species distribution models are increasing in popularity for mapping suitable habitat for species of management con- cern. Many investigators now recognize that extrapolations of these models with geographic information systems （GIS） might be sensitive to the environmental bounds of the data used in their development, yet there is no recommended best practice for ＂clamping＂ model extrapolations. We relied on two commonly used modeling approaches： classification and regression tree （CART） and maximum entropy （Maxent） models, and we tested a simple alteration of the model extrapolations, bounding ex- trapolations to the maximum and minimum values of primary environmental predictors, to provide a more re.listic map of suit-able habitat of hybridized Africanized honey bees in the southwestern United States. Findings suggest that multiple models of bounding, and the most conservative bounding of species distribution models, like those presented here, should probably re.lace the unbounded or loosely bounded techniques currently used [Current Zoology 57 （5）： 642-647,2011].

Aim:Quadriceps strength and knee extension,the most important factors limiting the ability to rise from a chair,are crucial for walking at an appropriate speed,ascending and descending stairs,and performing activities such as running,dancing,and *** of the anterior compartment of the thigh,including all four quadriceps muscles,for the treatment of a sarcoma is uncommon;however,when necessary,it is very debilitating and adversely affects a patient's quality of life without functional ***,there are a limited number of complex and difficult reconstructions to restore quadriceps function that have been described with variable *** describe a simple technique that employs a single gracilis functional muscle transfer to re.lace essential quadriceps ***:This is a case series describing the use of either a free or pedicled single gracilis muscle to restore quadriceps function following sarcoma ***:Four patients underwent an anterior compartment sarcoma resection that resulted in a large segmental defect and/or denervation of all four quadriceps muscles such that no quadriceps function would remain without *** four patients underwent a functional reconstruction using a single *** of the.living patients achieved British Medical Research Counsel Grade 4 strength,can achieve full knee extension,are able to navigate stairs,and are able to ambulate without a *** fourth patient unfortunately was deceased in under three months following his tumor ***:Despite its small size in comparison to the quadriceps muscles,with physiotherapy and training,the gracilis muscle demonstrates the capacity to hypertrophy and re.lace quadriceps function following limb salvage surgery.
