Metastatic pancreatic and lung cancer patient in complete remission following immunotherapy: A case report and review of literature

BACKGROUnD Metastatic pancreatic ductal adenocarcinoma(PDAC)is a lethal malignancy with dispiriting survival *** is a promising approach to many cancer types,but achieves poor outcomes in advanced PDAC due to its immunosuppressive tumor *** describe a case of metastatic PDAC effectively treated with *** SUMMARY We report the case of a 67-year-old woman with unresectable locally advanced PDAC,treated with gemcitabine plus nab-paclitaxel followed by radiotherapy plus *** nine months,pancreatic tumor progression was observed at the level of the hepatic hilum with the appearance of a new pulmonary nodule suggestive of a second primary,confirmed by left lung *** immunotherapy was then initiated with pembrolizumab,an immune checkpoint inhibitor targeting programmed cell death protein-1 that covers the two tumor *** patient showed a complete metabolic response that was maintained throughout the *** patient continues to be disease-free at 5.6 years since the start of *** These results suggest that the administration of pembrolizumab after chemoradiotherapy has a beneficial effect in patients with metastatic *** our knowledge,this is the first reported case of a patient with metastatic PDAC and metastatic lung cancer showing such a long-lasting complete response after pembrolizumab treatment without curative *** studies are required to determine biomarkers that identify PDAC patients most likely to benefit from this immunotherapy.

A methodological framework for improving air quality monitoring network *** to environment management

This work aims to provide a methodology framework which allows to improve the performance and efficiency of an air quality monitoring network(AQMn).It requires to be constituted by a minimum and reliable number of measurement ***,the AQMn efficiency should be assessed over time,as a consequence of the possible emergence of new emission sources of air pollutants,which could lead to variations on their spatial distribution within the target ***_(10)particles data monitored by the Community of Madrid's(Spain)AQMn between 2008 and 2017 were used to develop a methodology to optimize the AQMn *** annual spatial distribution of average PM_(10)levels over the studied period monitored by all current stations vs those more representative was provided by a geographic information system(GIS),and the percentage of similarity between both postulates was quantified using simple linear regression(>95%).As one innovative tool of this study,the practical application of the proposed methodology was validated using PM_(10)particles data measured by AQMn during 2007 and 2018,reaching a similitude degree higher than 95%.The influence of temporal variation on the proposed methodological framework was around 20%.The proposed methodology sets criteria for identifying non-redundant stations within AQMn,it is also able to appropriately assess the representativeness of fixed monitoring sites within an AQMn and it complements the guidelines set by European legislation on air pollutants monitoring at fixed stations,which could help to tackle efforts to improve the air quality management.

An optimized approach for estimating benzene in ambient air within an air quality monitoring network

Benzene is a carcinogenic air pollutant for which European legislation has set an annual limit and criteria for the number of fixed monitoring sites within air quality networks(AQMn).However,due to the limited number of fixed sites for benzene measurement,exposure data are *** the relationship between benzene levels and other variables monitored within an AQMn,such as nO_(2),O_(3),temperature,solar radiation,and accumulated precipitation,this study proposes an approach for estimating benzene air concentrations from the related *** the data of the aforementioned variables from23 fixed stations during 2016-2017,the proposed approach was able to forecast benzene concentration for 2018 with high confidence,providing enriched data on benzene exposure and its ***,the spatial distribution of the estimated versus the most representative benzene levels was quite ***,an artificial neural network identified the most representative fixed benzene monitoring sites within the AQMn.

In order to address the study of complex systems,the detection of patterns in their dynamics could play a key role in understanding their *** particular,global patterns are required to detect emergent concepts and trends,some of them of a qualitative *** concept analysis(FCA) is a theory whose goal is to discover and extract knowledge from qualitative data(organized in concept lattices).In complex environments,such as sport competitions,the large amount of information currently available turns concept lattices into complex *** authors analyze how to apply FCA reasoning in order to increase confidence in sports predictions by means of detecting regularities from data through the management of intuitive and natural attributes extracted from publicly available *** complexity of concept lattices-considered as networks with complex topological structure- is *** is applied to building a knowledge based system for confidence-based reasoning,which simulates how humans tend to avoid the complexity of concept networks by means of bounded reasoning skills.

Taeniasis vs cysticercosis infection routes

Although cysticercosis caused by Taenia solium(T. solium) is considered a neglected disease, its life cycle has been well known for more than two centuries. T. solium not only causes cysticercosis but also taeniasis in humans. These two diseases have totally different infection routes. To acquire taeniasis(the presence of the adult stage of T. solium in the intestine), humans have to ingest the larval stage(cysticercus) that infects a variety of organs and viscera in pigs, its intermediate hosts. Therefore, taeniasis is acquired when eating raw or undercooked infected pork. The adult stage in the human intestine release eggs that contain a hexacanth embryo, the oncosphere. If humans accidentally ingest the eggs of T. solium, the released oncospheres penetrate the intestine and become cysticerci. Therefore, cysticercosis is acquired by the ingestion of eggs that contaminate water, vegetables, hands etc. These facts should not be forgotten to avoid misinformation in scientific publications.

Importance of BRCA mutation for the current treatment of pancreatic cancer beyond maintenance

In this editorial,we comment on pancreatic cancer(PC),one of the most aggressive and lethal *** minimal improvements in survival rates have been achieved over recent *** chemotherapeutic regimens have little impact,and surgical resection remains the only reliable curative *** address current treatment options for these patients,focusing on the usefulness of breast cancer(BRCA)gene mutation as a prognostic biomarker and predictor of response to *** survival outcomes have been reported in patients with PC and mutant BRCA gene treated with first-line platinum-based ***,it appears appropriate to include BRCA gene status among clinical criteria used to select the chemotherapy *** addition,maintenance treatment with poly(ADP-ribose)polymerase inhibitors has been found to improve progression-free survival in patients with PC and mutated BRCA whose disease does not progress after first-line platinum-based *** combination has therefore been proposed as the optimal treatment regimen for these patients.

Dear Editor, The synthesis of tetrapyrroles, including chlorophylls, is central for chloroplast function. The metabolic pathway of tetrapyrrole biosynthesis in Arabidopsis is initiated with the formation of amino levulinic acid （ALA）, which is con- verted by a series of common reactions to protoporphy- tin IX （Proto IX）（Tanaka et al., 2011）. Then the pathway diverges into two branches： the synthesis of heme/bilin and chlorophylls. The insertion of Mg2＋ into Proto IX, cata- lyzed by Mg-chelatase, is the first committed reaction of the chlorophyll branch and is considered a key step for the regulation of the whole pathway. Mg-chelatase is a het- erotrimeric enzyme composed of subunits CHLI, CHLD, and CHLH, the reaction mechanism of which has been estab- lished. It is a two-step process consisting in the Mg-ATP- dependent activation of the enzyme, which implies the formation of a ternary complex of subunits CHLI and CHLD with ATP-Mg2＋, and Mg2＋ chelation, which is catalyzed by CHLH driven by ATP hydrolysis, CHLI providing ATPase activity to the complex （Tanaka et al., 2011）. In Arabidopsis, CHLH and CHLD are encoded by single genes, whereas two genes, CHLI-I and CHLI-2, encode the two isoforms of CHLI.

Autoimmune pancreatitis(AIP),a chronic inflammation caused by the immune system attacking the pancreas,usually presents imaging and clinical features that overlap with those of pancreatic ductal adenocarcinoma(PDAC).Serum biomarkers,substances that quantitatively change in sera during disease development,are a promising non-invasive tool with high utility for differentiating between these *** this way,the presence of AIP is currently suspected when serum concentrations of immunoglobulin G4(IgG4)antibody are ***,this approach has some ***,IgG4 antibody concentrations are also elevated in sera from some patients with *** review focuses on the most recent and relevant serum biomarkers proposed to differentiate between AIP and PDAC,evaluating the usefulness of immunoglobulins,autoantibodies,chemokines,and *** proposed serum biomarkers have proven useful,although most studies had a small sample size,did not examine their presence in patients with PDAC,or did not test them in *** addition,current evidence suggests that a single serum biomarker is unlikely to accurately differentiate these diseases and that a set of biomarkers will be needed to achieve adequate specificity and sensitivity,either alone or in combination with clinical data and/or radiological images.
