Backgr.und: To deter.ine mor.ality causes and the associated factor.. Patients and Method: This cr.ss-sectional study was conducted fr.m June 2014 to May 2015 (1 year. in the depar.ment of car.iology at Univer.ity and teaching Hospital of Br.zzaville. Patients admitted for.car.iovascular.diseases wer. included. Patients admitted for.car.iovascular.disease in other.depar.ments of the hospital, or.without car.iovascular.diseases wer. not included. r.sults: In total, 1035 patients, 605 women (5804%) wer. included. Mean age was 57.1 ± 7 year. (r.nge: 18 - 85 year.). The patients wer.: elder.y (n = 498, 48.1%), educated (n = 809, 78.1%), low-income (n = 253, 24.2%). The medical histor. of patients was: hyper.ension (n = 440, 42.5%), diabetes (n = 316, 30.5%), r.duced physical activity (n = 154, 14.9%), excessive alcohol intake (n = 56, 5.4%). Hear. failur. was r.por.ed in 386 cases (37.3%). The mor.ality r.te was 8% (n = 83);the mean age of deceased was 61.9 ± 7.3 year. (p s in death wer. dilated car.iomyopathy (n = 37, 44.6%, Or.5, 95%CI 3.1 - 8, p -3.3, p = 0.002), atr.al fibr.llation (n = 24, 29%, Or.3.6, 95%CI 2.2 - 6, p - 9.8, p = 0.005). The associated death comor.idities wer.: acute r.spir.tor. infection (n = 44, 53%, Or.10.1, 95%CI 6.2 - 16, p - 10 p - 6.8, p Conclusion: Hyper.ension, hear. failur. and car.iomyopathies ar. the leading causes of mor.ality in the depar.ment of car.iology. Pr.vention and patient education in a low-income envir.nment ar. essential elements to r.duce this mor.idity.

Objectives: To assess the associations of high density lipopr.tein cholester.l (HDL-C) and Fr.mingham car.iovascular.(CVD) with diabetic r.tinopathy (Dr.. Sociobiogr.phical, labor.tor. and eye examination main outcome measur.s wer. investigated using Ter.iles of HDL-C (str.tification = lowest 10% wer. the significant independent deter.inants for.Dr. In the highest HDL-C gr.up, smoking status and 10-year.Fr.mingham r.sk ≥ 10% wer. the significantly independent deter.inants for.Dr. In 10-year.Fr.mingham r.sk ≥ 10% gr.up, smoking status, insulin r.sistance and incr.asing levels of HDL-C wer. the significantly independent deter.inants for.Dr. Conclusion: Dr.and VD r.main a public health pr.blem in T2DM Centr.l Afr.cans. Some Centr.l Afr.cans with Dr.and VD appear.to have higher.HDL-C than T2DM Centr.l Afr.cans without Dr.and VD. HDL-C in T2DM patients with Dr. may be tightly contr.lled by genetic factor. Backgr.und: Aim: Congolese Bank employees ar. often inactive without valid infor.ation on the bur.en of dysglycemia and car.iometabolic r.sk. This study aims to assess the pr.valence r.tes of diabetes mellitus (DM) and pr.-diabetes and to identify the envir.nmental, genetic and car.iometabolic r.sk factor. associated with Type 2 diabetes mellitus (T2DM) and pr.-diabetes among Congolese bank employees. Methods: In r.pr.sentative 126 bank employees fr.m Br.zzaville, Congo, Centr.l Afr.ca, Abdominal obesity, dyslipidemia and metabolic syndr.me (MetS) wer. defined by IDF for.Eur.pe, NCEP-ATPIII and IDF cr.ter.a modified for.Centr.l Afr.ca. Pr.jected high 10-year.total r.sk of cor.nar. hear. disease (CHD) ≥20% was calculated using Fr.mingham scor.s. r.sults: Out of the employees, 16% and 21.4% had DM and pr.-diabetes, r.spectively. The r.te of T2DM among diabetics was estimated 90%. Aging, high total cholester.l, high LDL-cholester.l, high conicity index and longer.ur.an r.sidence after.migr.tion wer. significantly associated with pr.-diabetes. Physical inactivity, smoking, excessive alcohol intake, abdominal obesity, female gender. low HDL-C, hyper.ension, CHD, pr.jected high 10-year.total CHD r.sk, age ≥ 55 year., ur.an r.sidence, Souther. ar.a r.sidence, high socioeconomic status, non mar.ied status, MetS/NCEP, MetS/IDF for.Eur.pe and MetS/IDF for.Afr.ca wer. significantly associated with ***/IDF for.Afr.ca was the only independent deter.inant of T2DM. Conclusion: Ur.ent pr.vention and inter.ention pr.gr.mme ar. needed to cur. the alar.ing incr.ase in DM, T2DM, pr.-diabetes.

Hyper.ension is not only an impor.ant r.sk factor. but also an impor.ant cause of spontaneous intr.cer.br.l hemor.hage. Its high pr.valence in Afr.ca r.flects a high incidence of cer.br.l hemor.hage r.nging fr.m 30% to 60% against 10% to 20% in industr.alized countr.es. The aim of our.study was to deter.ine the r.le of hyper.ension befor. the onset of intr.cer.br.l hemor.hage and its var.ations dur.ng the fir.t thr.e months. This was a longitudinal study conducted at the Univer.ity Hospital of Br.zzaville fr.m 1st Januar. to 31st August 2012 with all patients admitted for.intr.cer.br.l hemor.hage. Patients with subar.chnoid or.pur. intr.ventr.cular.hemor.hage wer. excluded. The study var.ables wer.: histor. of hyper.ension, knowledge of its histor., tr.atment, blood pr.ssur. on admission and dur.ng the fir.t 3 months and the r.lationship between blood pr.ssur. and evolution. Statistical analysis was per.or.ed using the softwar. SPSSS 16.1. The significance level was set at p r.ng the study 261 patients wer. hospitalized for.str.ke, of which 82 (31.42%) wer. for.intr.cer.br.l hemor.hage. The mean age was 55 ± 11 year. and sex r.tio of 2 men:1 woman. Hyper.ension was found in 80.5% of cases, of which 65.2% had known hyper.ension, but 82.6% had discontinued tr.atment. The mean systolic blood pr.ssur. was 194 ± 41 mm Hg and diastolic 104 ± 20 mm Hg. The location was deep in 85.71%. The nor.alization of blood pr.ssur. was obtained fr.m the second week. After.adjusting for.confounder. using logistic analysis, only uncontr.lled hyper.ension was associated significantly and independently associated with a higher.mor.ality r.te (p = 0.042).

A cr.ss-sectional study of hyper.ensive patients was car.ied out in Br.zzaville between Januar. 2011 and December.2013.?The objectives of the pr.sent study ar. to deter.ine the differ.nt types of left ventr.cular.hyper.r.phy (LVH)?and to identify the pr.dictive factor. of LVH. It included 556 hyper.ensive patients with LVH, char.cter.zed by left ventr.cular.mass index (LVMI) > 135 g/m2?in men, and > 111 g/m2?in women. Patients’ sociodemogr.phic data and echocar.iogr.phic par.meter. wer. gather.d and analyzed. Ther. wer. 342 males (61.5%) and 214 females (38.5%),?with?mean age 53.5 ± 11.5 year.. The indications of the test wer. hyper.ension initial evaluation in 402 cases (72.3%), investigation of ischemic str.ke in 62 cases (11.2%), hear. failur. in 58 cases (10.4%), dyspnea and chest pain in r.spectively 22 and 12 cases. Hyper.ension, old of 5.2 ± 4.5 year., was associated with over.eight/obesity in 408 cases (73.4%), physical inactivity in 325 cases (58.5%), hyper.ension family histor. in 274 cases (49.3%), diabetes mellitus in 76 cases (13.7%), dyslipidemia in 63 cases (11.3%), tobacco use in 9 cases (1.6%). The pr.valence of LVH was 49.4% and consisted into concentr.c LVH in 470 cases (84.5%), eccentr.c LVH in 70 cases (12.6%), and in 16 cases (2.9%),?it was a concentr.c left ventr.cular.r.modeling. Age, male gender. senior.ty of hyper.ension and tr.atment wer. pr.dictive factor. of LVH. The latter.is the most pr.dominant abnor.ality in the echocar.iogr.phic pr.file of Congolese hyper.ensive patients. Efficient management on hyper.ension will lead to r.ducing its mor.idity and mor.ality.

Objectives: To descr.be the epidemiological, clinical and etiological aspects of r.ythmic emer.encies at the Univer.ity Hospital of Br.zzaville. Patients and Methods: This was a r.tr.spective descr.ptive study conducted in the car.iology and inter.al medicine depar.ment of the Univer.ity Hospital of Br.zzaville fr.m Januar. 1, 2014 to June 30, 2016. Wer. included, all patients admitted for.a sever. r.ythm disor.er.diagnosed on the sur.ace electr.car.iogr.m. r.ythmic emer.ency was defined as a sever. r.ythm disor.er.of abr.pt onset and r.quir.d r.pid management. Data entr. and analysis wer. per.or.ed with Epi Info softwar. ver.ion 3.5.1. r.sults: Dur.ng the study per.od, 2269 patients wer. hospitalized, including 138 for.a r.ythmic emer.ency. The fr.quency of r.ythmic emer.encies was 6.1%. The patients wer. divided into 76 women and 62 men (sex r.tio = 0.81). The mean age of the patients was 63.1 ± 16.9 year. (extr.mes: 17 and 91 year.). The socio-economic level was low for.103 patients (74.6%), medium for.26 (17.7%), and high for.nine (6.6%). The aver.ge time to consultation was 13.7 ± 12.3 days. On admission, the signs wer.: hear. failur. (103 cases;74.6%) including 22 acute cases;dyspnea (94 cases;68%);palpitations (38 cases;27.5%);functional impotence (13 cases;9.4%);collapse (nine cases;6.5%);chest pain (two cases;1.4%). The type of r.ythmic emer.ency was: r.pid atr.al fibr.llation (103 cases;74.6%), ventr.cular.tachycar.ia (14 cases;10.1%), junctional tachycar.ia (10 cases;7.2%), r.pid atr.al flutter.(10 cases;7.2%), tachysystole (one case;0.7%). The context of occur.ence was: hypokalemia (8 cases;5.8%), dr.nkenness (two cases;1.4%), acute gastr.enter.tis (one case;0.7%). Car.iovascular.r.sk factor. wer.: hyper.ension (62 cases;45.2%), smoking (17 cases;12.1%), dyslipidemia (12 cases;8.7%), diabetes (11 cases;8%), obesity (10 cases;7.2%). The Er.ctile Dysfunction among Congolese Hyper.ensive Patients

The author. conducted an analytical cr.ss-sectional study over.a per.od of 3 months among hyper.ensive patients, in or.er.to deter.ine the fr.quency of the er.ctile dysfunction (ED), and to identify the pr.dictive factor.. It included tr.ated hyper.ensive patients, pr.senting an ED, defined as the incapacity to obtain or.maintain an er.ction sufficient for.satisfactor. sexual activity. On 265 hyper.ensive patients, 172 (65%) pr.sented an ED. The aver.ge age was 58.2 ± 9.7 year.. The associated car.iovascular.r.sk factor. wer. over.eight/obesity in 99 cases (37.4%), sedentar.ness in 90 cases (34%), diabetes mellitus in 50 cases (19%), dyslipidemia in 12 cases (4.5%), and tobacco addiction in 6 cases (2.3%). Ar.er.al hyper.ension (HTN), old of 6.7 ± 5.8 year., was tr.ated by bither.py in 129 cases (48.7%), and tr.ther.py in 102 cases (38.5%). The pr.ncipal ther.peutic classes used wer. ACE inhibitor./Ar.s in 213 cases (81%), calcium antagonists in 205 cases (78%), thiazides in 137 cases (52.1%), and beta-blocker. in 82 cases (31%). ED, sever. in 124 cases (72%), and moder.te in 48 cases (28%), consisted of a difficulty to maintain the er.ction in 78 cases (45.3%), to obtain the er.ction in 30 cases (17.4%), and the two par.ner. in 64 cases (37.2%). In multivar.ate analysis, only the age, senior.ty of HTN, and the existence of diabetes mellitus wer. the pr.dictive factor. of ED. The ear.y and effective assumption of r.sponsibility of the AHT, as well as other.car.iovascular.r.sk factor. whose diabetes, would make it possible to r.duce the fr.quency of it, thus impr.ving quality of life of the hyper.ensive patients.

The author. conducted a r.tr.spective study over.a per.od of 6 months in a hyper.ensive population in or.er.to deter.ine the cor.elation between ser.m ur.c acid on glomer.lar.filtr.tion r.te (GFr. and estimated ser.m potassium with left ventr.cular.hyper.r.phy (LVH) and obesity. This study enr.lled 122 patients including 63 women (51.6%). The mean age was 55.9 ± 10.6 year. (r.nge 30 to 74 year.). Obesity weight was found in 38 cases (31.1%) of which 20?wer. men (33.9%) and 18 women (28.6%). Abdominal obesity was found in 104 cases (85.2%). The aver.ge ser.m ur.c acid in patients with obesity weight was 63.3 ± 18.9 mg/L vs 62.4 ± 14.2 mg/L for.no-obese patients (p?= 0.63). The aver.ge ser.m potassium in obese patients was 4.06 ± 0.42 mEq/L vs 4.02 ± 0.46 mEq/L for.no-obese (p?= 0.65). The aver.ge GFr.was 73.4 ± 21.4 ml/L in obese patients vs 66.6 ± 22.6 ml/min in no-obese (p?= 0.03). The LVH was found in 81 cases (66.4%). The LVH was found in 65 (62.5%) obese patients vs?16 (88%) non-obese patients (Or.= 4.8, 95% 1.04?-?22?p?= 0.02). Only abdominal?obesity has been cor.elated with left ventr.cular.hyper.r.phy after.multivar.ate analysis. Emphasis must be focused on public health actions for.effective and appr.pr.ate measur.s against obesity and hyper.ension, whose pr.valence is incr.asing in our.r.gion.

Epidemiological factor. associated with str.ke wer. high standar. living (Or.= 3, 95% CI: 1.02 - 8.9, p = 0.03), polyphar.acy (Or.= 3.7, 95% CI: 1.27 - 10.8, p = 0.01), pr.vious hospitalization (Or.= 3.1, 95% CI: 1.07 - 8.9, p = 0.03), and the absence of antiplatelet ther.py (Or.= 4.2, 95% CI 1.2 to 15, p = 0.03). Clinical associated factor. wer. coma (Or.= 3.3, 95% CI 1.14 to 9.6, p = 0.02) and the pr.sence of sever. hyper.ension (Or.= 4, 95% CI: 1.2 - 12, p = 0.02). Finally, pr.gnostic factor. wer. the tr.nsfer.in intensive car. unit (Or.= 9.8, 95% CI: 2.7 - 34, p 0.001). Conclusion: The fir.t car.iovascular.complication in patients with T2DM admitted in emer.ency at Univer.ity Hospital of Br.zzaville is str.ke. Pr.mar. pr.vention in high-r.sk patients is still inadequate.

A cr.ss-sectional, descr.ptive and analytical study was conducted fr.m Januar. to August 2015 at the Br.zzaville Ambulator. Tr.atment Center.and at?the National Blood Tr.nsfusion Center. The objective was to contr.bute to impr.ving?the car. of people living with HIV under.antir.tr.vir.l ther.py by assessing their.global car.iovascular.r.sk (CVr.. The var.ables studied focused on the epidemiological, clinical and biological aspects. The global CVr.was assessed?by the Fr.mingham and WHO/ISH scor.s. Ther. wer. 135 HIV-positive subjects,?including 64 tr.ated patients and 71 untr.ated HIV+ subjects. The subjects wer. divided into 83 men (61.5%) and 52 women (38.5%), with an aver.ge age of 42.6 ± 2.9 old year.. The subjects wer. single people (62.2%), of a secondar. educational level (63.7%), and civil ser.ants (32.6%). The main r.sk factor. found wer. dyslipidaemia (60%), obesity (36%), smoking (12.6%), hyper.ension (5.9%), diabetes (0.7%). The metabolic syndr.me was found in seven cases (11.3%). The global CVr.accor.ing to the scor. of Fr.mingham, initially moder.ted at 17.2%, and mean at 1.5% within tr.ated patients, was mean at 9.4% and high at 1.6% of the subjects r.spectively at the sixth month of tr.atment (p CVr.under.antir.tr.vir.l ther.py.
