北洺河铁矿采用大结构参数无底柱分段崩落法开采,多年来大块率高、爆破隔墙事故与眉线破坏等爆破问题,一直没有得到很好解决。通过对扇形炮孔爆破机理的分析得出,形成推墙现象的主要原因,是扇形面内炮孔过于密集。以减小扇面炮孔的密度,将炮孔爆破的主导方向调整到抵抗线方向为出发点,进行了改进装药结构的现场试验,通过优化装药结构,改进了爆破效果。

弓长岭井下矿中央区开采急倾斜中厚矿体,随着采深的增大,采准工程出现地压显现的部位增多、规模逐渐增大,进入-280m中段开采后,采准工程的塌冒已经严重影响生产的正常进行。经过调查巷道发生破坏的部位与破坏区岩性变化,查明上盘楔形体围岩的附加力作用是引发回采巷道大规模破坏的主要原因,为此提出卸压开采方案,即在高应力区先回采上分段沿脉进路,为下分段卸掉垂直压力后,再掘进下分段沿脉进路。此外,根据巷道破坏过程与地压显现形式,分析了现用工字钢金属架支护方法的不适应性,提出根据岩性与地应力大小选择树脂锚杆、喷锚网联合支护的改进方案。初步试验表明,改进方案可有效解决上盘沿脉巷道的塌冒难题,并可较好地适应弓长岭井下矿深部矿岩条件,不失为弓矿实用技术。

Surgical Treatment of Upper Urinary Tract Lithiasis in Conakry

Introduction: The management of urinary lithiasis in our settings has long been conventional surgery. The recent introduction of endoscopy of the upper urinary tract represents a major advance in our department. The aim was to contribute to the study of the treatment of lithiasis of the upper urinary tract in Conakry and to report on our initial experience of ureteroscopy in Conakry. Methodology: This is a prospective descriptive study lasting one year, from January 1 to December 31, 2022, carried out at the Urology Department of the Ignace Deen National Hospital and at the PERCHIN Urological Clinic. The study included all patients treated surgically for lithiasis of the upper urinary tract who met the selection criteria. Results: The study included 57 patients. The mean age was 30 years, with extremes of 20 and 73 years;the most represented age group was 31 to 40 years, followed by 20 to 30 years with 29.9% and 26.3% respectively. Males predominated in 36 cases (63.15%), with a M/F sex ratio of 1.71. Left-sided renal colic was the most common, at 75.43% (n=43), and right-sided renal colic at 40.35%, associated with digestive signs at 43.85%. 77.19% had normal creatinine levels before surgery, versus 22.81% with elevated creatinine levels, i.e. 15.78% improvement in renal function after surgery. Urinary tract infection was found in 47.36%, and Escherichia coli in 31.57%, followed by staphylococcus aureus in 8.77%. Overall, 82.45% of patients had organic damage to the kidneys or ureters. Right ureterohydronephrosis was noted in 56.14%, followed by left hydronephrosis in 26.32%. Open surgery was predominant in 52.63% of cases, with endoscopic surgery (URS) accounting for a significant 47.36% during the study period. The endoscopic treatment used was laser ureteroscopy. In our study, 73.68% underwent drainage of the upper excretory tract, including 64.91% with a JJ catheter. Epidemiological, Clinical and Biological Characteristics of Patients with Chronic Hepatitis B Infection Followed-Up at the University Hospital of Conakry, Guinea

Introduction: Very little information is available in Guinea on chronic hepatitis B infections. The objective of this study was to describe the epidemiological, clinical and biological features of patients who are chronic carriers of the hepatitis B virus. Patients and Methods: This is a retrospective study carried out from January 2017 to May 2020, based on the medical records of patients seen via consultation or hospitalized with a record of positive HBs antigen for more than 6 months. Clinical and paraclinical data were collected and analyzed. Results: Seven hundred and sixteen patients with a mean age of 35.6 ± 12.2 (sex ratio 2.05), were included. The HBs antigen was discovered incidentally in 36% of cases (n = 258). A history of dental care and surgical procedures was found in 46.3% (n = 290) and 21.1% (n = 138) of cases, respectively. The median value of ALAT enzymes was 34 (21 - 47) IU/L. HBeAg was positive in 20.8% (n = 55/265) of cases. The median B viral load was 458.5 (87 - 3827) IU/ml and 29% (n = 94) of patients had a viral load over 2000 IU/ml. Anti-HCV antibody was present in 10.4 % of cases (n = 39/374). HIV serology was positive in 2.7% (n = 8/298). A total of 19.4% (n = 139) of the patients had cirrhosis and 4.5% (n = 32) had hepatocellular carcinoma. Conclusion:

Introduction: Scleroderma is a generalized condition of the connective tissue, arterioles and micro-vessels characterized by tissue fibrosis, vascular obliteration and abnormal humoral and cellular immune responses. The aim of this study was to describe the epidemiological, clinical, therapeutic and evolutionary aspects of scleroderma at the Department of Dermatology-Venerology at Donka National Hospital. Methods: This was a descriptive cross-sectional study with a retrospective collection of data for a period of 11 years, from January 1, 2000, to December 31, 2010, covering all inpatient records of Scleroderma in the service. Results: We collected 17 cases of scleroderma out of 3289, a frequency of 0.04%. The average age of patients was 33.33 years with extremes of 10 and 60 years. A female predominance was noted with a sex ratio M/F of 0.13. Cutaneous induration was the main symptom associated with the phenomenon of Raynaud, and disorders of pigmentation. The extra-skin manifestations were mainly dominated by gastrointestinal involvement (32%), pulmonary involvement (24%), cardiac (20%), articular (20%) and renal (12%) involvement. The systemic form was the most common (99.45%). Corticotherapy was the most prescribed treatment (24.59%). Clinical improvement was noted in (93.2%) of the cases. Conclusion: Scleroderma, although rare, deserves special attention at all levels for early diagnosis.

Experimentation of a Forced Convection Solar Dryer for Drying Sweet Potatoes at the Higher Institute of Technology of Mamou-Guinea

This study was carried out at the Mamou Higher Institute of Technology during the period from March 10 to April 15, 2022, with the aim of designing and testing a solar dryer with forced convection by drying potatoes. The dryer was designed using local materials. Its main geometric parameters are: 1) height of the drying chamber (90 cm), 2) length of the drying chamber (50 cm), 3) width of the drying chamber (43 cm), 4) surface of the racks (0.1806 m2), 5) surface of the heat accumulator (0.2537 m2). The experiment focused on the vacuum test of the dryer for two days and that of the drying of the sweet potato for three days from 8:30 a.m. to 5:30 p.m. The average vacuum test temperature values of the three environments are respectively accumulator (43°C), dryer chamber (41°C) and ambient environment (34°C). Four kilograms (4 kg) of boiled sweet potato were dried. The average temperatures in the accumulator and in the drying chamber during the three days of drying are respectively 33°C and 39°C. The final mass of the dried product is 1.2 kg, with a quantity of water extracted of 2 liters or 63% of the initial mass of the product. The average drying rate is 0.074 kg/h. The drying kinetics showed a decreasing rate in the absence of the heating period and the constant rate period.

Introduction: The therapeutic pathway affects the patient who changes care facilities. This change is linked to factors such as financial means, geographical accessibility of care facilities, quality of care and the socio-cultural perception of the patient. In countries with limited resources such as Guinea, the therapeutic itinerary remains multiple and hampered by obstacles. The objective of this study was to describe the therapeutic itinerary of patients living with hepatitis B and C at the Donka National Hospital. Material and methods: This was a retrospective, descriptive study lasting 43 months (24 January 2017 - 27 August 2020);it focused on patients living with hepatitis B or C or viral cirrhosis. We collected sociodemographic, clinical and treatment history variables. The data were analysed with SPSS software version 21.0. Results: Out of 5400 patients, the proportion of viral hepatitis B or C represented 393 patients (7.3%). The mean age of our patients was 35 ± 10 years, with extremes of 16 - 77 years. The sex ratio was 2.5. Three hundred and forty-two patients (87%) had recourse to modern medicine, mostly in private facilities (50.6%). One hundred and twenty-five patients (31.8%) had received previous treatment, half of whom (57.6%) had received inappropriate treatment. The delay in treatment was long in 45% of cases. The main reasons for delaying treatment were feeling unwell in 33.3%;not knowing to consult a specialist in 29.9% and feeling that they were being treated with useful drugs in 24.3%. Conclusion: The lack of training of general practitioners on the management of viral hepatitis and the scarcity of awareness campaigns on viral hepatitis contributed to the inadequacy of the management.

This paper focuses on people living in Medina Gounass where has been experiencing annually flood events during the rainy season. Besides the climatic conditions, the lack of formal amenity plan has led to the emergence of highly vulnerable urban communities, particularly through informal settlements and inadequate land management. Flood has disrupted their livelihood and caused huge loss of properties. Thus, inhabitants have a long experience of flood events. The study was carried out to assess critical aspects that led Medina Gounass inhabitants vulnerable to flood. Consequently, people in the study area have been practicing various methods at household to mitigate the probable losses. The study found that flood adaptation measures can be made more effective through community initiatives and their participation than at household level which sometimes causes harm in the neighborhood. Structural and non-structural flood management approach seems to be more efficient to alleviate Medina Gounass inhabitants from the flood problems.

In the town of Mamou,all homes with sanitation systems are equipped with autonomous installations not connected to a sewer *** emptying service is provided by manual emptiers and by the living environment and sanitation service of the urban municipality which has a single 6 m^(3)*** objective of this work is to determine the quantity of fecal sludge produced within the city for sustainable *** methodology adopted is based on a field survey and the use of three methods for evaluating the quantities of sludge(specific production,quantity of sludge collected by the sludge truck and the total production of sludge produced in the various sludge works).The results obtained show that,out of the 2,940 sanitation works identified,2,936 works have been emptied at least once since their *** 2,307 structures are emptied manually or 78.57%,against;619 structures emptied mechanically or 21.08%.The structures are emptied on average every 3 years for septic tanks and every 5 years for dry *** specific production of sludge is 685,241,532 m^(3)/year;the production of sludge by the mechanical emptying technique varies from 588,641.57 m^(3)/year to 724,800.46 m^(3)/year and the production of sludge by manual emptying is 1,573,709.33 m^(3)/year,for a total quantity of sludge produced including between 2,162,350 m^(3)/year to 685,241,531.9 m^(3)/year.
