Univariate Risk Factors for Prolonged mechanical Ventilation in Patients Undergoing Prosthetic Heart Valves Replacement Surgery

Data from 736 patients undergoing prosthetic heart valve replacement surgery and concomitant surgery （combined surgery） from January 1998 to January 2004 at Union Hospital were retrospectively reviewed. Univariate logistic regression analyses were conducted to identity risk factors for prolonged mechanical ventilation. The results showed that prolonged cardiopulmonary bypass duration, prolonged aortic cross clamp time and low ejection fraction less than 50 percent （50 %） were found to be independent predictors for prolonged mechanical ventilation. meanwhile age, weight, and preoperative hospital stay （days） were not found to be associated with prolonged mechanical ventilation. It was concluded that. for age and weight, this might be due to the lower number of old age patients （70 years and above） included in our study and genetic body structure of majority Chinese population that favor them to be in normal weight, respectively.

This study is aimed to evaluate the quality of life （QOL） for individuals living with human immunodeficiency virus （HIV）/acquired immunodeficiency syndrome （AIDS） in Hubei province-central China by using WHOQOL-BREF instrument （Chinese version）. One hundred and thirty six respondents （HIV/AIDS individuals） attending out-patient department of Chinese Center for Disease Control and Prevention （Chinese CDC） were administered a structured questionnaire developed by investigators. QOL was evaluated by using WHOQOL-BREF instrument （Chinese version）. The results showed that the mean score of overall QOL on a scale of 0-100 was 25.8. The mean scores in 4 domains of QOL on a scale of 0-100 were 82.9 （social domain）, 27.5 （psychological domain）, 17.7 （physical domain） and 11.65 （environmental domain）. The significant difference of QOL was noted in the score of physical domain between asymptomatic （14.6） and early symptomatic individuals （12）（P=0.014）, and between patients with early symptoms （12） and those with AIDS （10.43）（P〈0.001）. QOL in psychological domain was significantly lower in early symptomatic （12.1）（P〈0.05） and AIDS patients （12.4）（P〈0.006） than in asymptomatic individuals （14.2）. The difference in QOL scores in the psychological domain was significant with respect to the income of patients （P〈0.048） and educational status （P〈0.037）. Significantly better QOL scores in the physical domain （P〈0.040） and environmental domain （P〈0.017） were noted with respect to the occupation of the patients. Patients with family support had better QOL scores in environmental domain. In our research, QOL for HIV/AIDS individuals was associated with education, occupation, income, family support and clinical categories of the patients. It was concluded that WHOQOL-BREF Chinese version was successfully used in the evaluation of QOL of HIV/AIDS individuals in Chinese population and proved to be a reliable and useful tool.

Sharing sensitive data is a specific challenge for research infrastructures in the field of life sciences. For that reason a toolbox has been developed, providing resources for researchers who wish to share and use sensitive data, to support the workflows for handling these kinds of digital objects. Common and community approved annotations are required to be compliant with FAIR principles(Findability, Accessibility,Interoperability, Reusability). The toolbox makes use of a tagging(categorisation) system, allowing consistent labelling and categorisation of digital objects, in terms relevant to data sharing tasks and activities. A pilot study was performed within the Horizon 2020 project EOSC-Life, in which 2 experts from 6 life sciences research infrastructures were recruited to independently assign tags to the same set of 10 to 25 resources related to sensitive data management and data sharing(in total 110). Summary statistics of agreement and observer variation per research infrastructure are provided. The pilot study has shown that experts were able to attribute tags but in most cases with a considerable observer variation between experts. In the context of CWFR(Canonical Workflow Frameworks for Research), this indicates the necessity for careful definition,evaluation and validation of parameters and processes related to workflow descriptions. The results from this pilot study were used to tackle this issue by revising the categorisation system and providing an updated version.

Patient and Provider Perspectives of Disrespect and Abuse during Childbirth in Tanzania: A Literature Review

Objective: Disrespectful and abusive (D&A) maternity care is a deterrent to facility childbirth and can thus contribute to child and maternal mortality. This study will review existing literature on D&A in Tanzania to better understand and contextualize the issue. Methods: A comprehensive literature search was conducted to find relevant publications on D&A during childbirth in Tanzania. The search was conducted on the National Center for Biotechnology Information (NCBI) and PubMed (U.S. National Library of Medicine at the National Institutes of Health). The inclusion criteria were as follows: qualitative, quantitative, and ethnographic studies conducted in Tanzania on obstetrical violence;published in English;focused on prevalence, incidence, root causes, historical trends, interventions, and policy recommendations for obstetrical violence in Tanzania. Results: Twenty-one studies were selected for this literature review. Up to 73.1% of women reported experiencing at least one form of D&A during labor. Commonly reported D&A events include non-dignified care, non-confidential care, and physical abuse. Other forms of D&A, including detention in facilities and asking for unofficial payments, are also reported. Ninety-six percent of nurses, midwives, and nursing assistants in Tanzania self-reported engaging in at least one form of D&A. Lack of training and provider support, long work hours, fear of blame, and limited infrastructure were reported as factors leading to D&A by providers.
