Does ethnicity and education influence preoperative disability and expectations in patients undergoing total knee arthroplasty?

AIM To investigate whether minority ethnicity and the duration of education influence preoperative disability and expectations in patients undergoing total knee *** We prospectively included 829 patients undergoing primary unilateral total knee arthroplasty(TKA) from April 2013 to December 2014 at a single centre. Patients filled in pre-operative questionnaires with information regarding place of birth, duration of education, expectations for outcome of surgery and baseline characteristics. Patients were stratified based on ethnicity. Majority ethnicity was defined as born inthe study country and minority ethnicity was defined as born in any other country. Similarly, patients were stratified based on duration of education in groups defined as rs, 9-12 years and > 12 years, *** We found that 92.2% of patients were of majority ethnicity. We found that 24.5%, 44.8% and 30.8% of patients had an education of rs, 9-12 years and > 12 years, respectively. The mean preoperative(preOP) oxford knee score(OKS) in the total population was 23.6. Patients of minority ethnicity had lower mean pre-OP OKS(18.6 vs 23.9, P r pain levels(VAS 73.0 vs 58.7, P r levels of post-OP pain(VAS 14.1 vs 6.1, P = 0.02) and of overall symptoms(VAS 16.6 vs 6.4, P = 0.006). Patients with > 12 years education had lower mean pre-OP OKS(21.5 vs 23.8 and 24.6, P r pre-OP VAS pain(65.4 vs 59.2 and 56.4, P red to groups with shorter education. One year post-operative(post-OP) patients of minority ethnicity had lower mean OKS, higher pain and lower QoL. The mutational dynamics of the SArS-CoV-2 virus in serial passages in vitro

Mineralogy and geochemistry of sands of the lower course of the Sanaga river, Cameroon: implications for weathering,provenance, and tectonic setting

This study focuses on the mineralogy and bulk chemical composition of 19 sediment samples,collected from the Sanaga river bed,between Nanga-Eboko and the Atlantic Ocean in Cameroon,to infer provenance,weathering,and tectonic *** textural analysis revealed that these sediments are mainly coarse-grained sands.X-ray diffraction and heavy mineral analyses show that these sands are made of quartz,microcline,plagioclase,muscovite,ilmenite,anatase,magnetite,opaques,and *** Index of Chemical Variability values(1.07–2.68)indicates that the sands are *** Chemical Index of Alteration(53–66%),Plagioclase Index of Alteration(PIA;57–75%),and Mafic Index of Alteration(54–67%)values revealed a moderate intensity of weathering for these *** to the compositional maturity diagram,the sands are mainly *** studied sands show enrichment in light rare earth elements relative to heavy rare earth elements and a negative anomaly in Eu(Eu/Eu*=0.32–0.83).These sands were derived from felsic metamorphic rocks dated Meso to Neoproterozoic from the Pan-African(700–1000 Ma)domain of south ***/U ratios(mean=5.40;n=19)reveal that these sands originating from felsic source rocks and are low to moderately *** the tectonic discrimination diagrams,all the samples are plotted in the rift and passive margin domains,which is consistent with the tectonic history of Pan African in southern Cameroon.

Strongyloides stercoralis prevalence and diagnostics in Vientiane, Lao People’s Democratic republic

Background:Despite the high prevalence of strongyloidiasis in the Laotian population,Laotian hospitals still lack diagnostic capacity to appropriately diagnose Strongyloides stercoralis *** cross-sectional hospital-based study was conducted to assess the prevalence of Strongyloides stercoralis infection among hospitalized patients treated at Mahosot Hospital,the primary reference hospital of Lao People’s Democratic republic(Lao PDr),and to validate feasible methods for diagnosing *** infection at hospital’s ***:Between September and December 2018,stool samples of 104 inpatients were investigated for *** infection by wet smear,Baermann technique,Koga Agar plate culture(KAPC),and real-time detection polymerase chain reaction(rTD-PCr)at the Infectious Diseases Ward of the Mahosot Hospital in *** sensitivity,the specificity,the negative predictive value(NPV)of each diagnostic test,as well as their combination(s)was calculated using a composite reference standard(CrS).The correlation of the different test methods was assessed by chi-square or Fisher’s exact ***’s kappa coefficient was used to assess the diagnostic agreement of the different test ***:The overall prevalence of *** infections among the study population was 33.4%.The cumulative infection prevalence statistically significantly increased from the lowest age group of 40 years and below(22.4%),to the medium(40.0%)and to the oldest age group of 61 year and above(72.7%)(P=0.003).The cumulative infection prevalence of CrS was considerably higher in male(40.4%)compared to female patients(28.1%),but not statistically different(P=0.184).The diagnostic sensitivity of Baermann technique,KAPC,rTD-PCr,and the combination of Baermann technique and KAPC were 60.0,60.0,74.3,and 77.1%,*** 13 patients(37.1%)of the total 35 *** patients diagnosed with any technique had a simultaneously positive diagnostic test with Baer

Pharmacogenetic Treatments for Alcohol and Stimulants

Pharmacogenetics uses genetic tests to predict the effectiveness of medication treatments for individual patients with drug dependence such as alcohol and opiates. An important shared neurobiological component of the reinforcement and dependence on alcohol and opiates is alcohol’s enhancement of β- endorphin release. The physiological and subjective effects of both alcohol and opiates are associated with β-endorphin and the mu opiate system. A pharmacotherapy that has been effective for both disorders is naltrexone, which increases baseline β-endorphin release blocking further release by alcohol. Naltrexone’s action as an alcohol pharmacotherapy is facilitated by a putative functional single nucleotide polymorphism (SNP) in the opioid mu receptor gene (Al18G) which alters its receptor function. Patients with this SNP have significantly lower relapse rates to alcoholism when treated with naltrexone. This SNP is particularly common among northern Chinese suggesting an important role for naltrexone treatment of alcoholism in China. Another important genetic polymorphism related to stimulant and perhaps other types of dependence frequently occurs (about 40% of the population) in the promoter region of the gene coding for dopamine beta hydroxylase (DβH). This polymorphism leads to 10 to 100 times lower levels of the DβH enzyme, which converts dopamine to norepinephrine in neurons. Stimulant abusers with normal levels of this enzyme show significant reductions in stimulant abuse when treated with DβH inhibitors, while those with the polymorphism for low DβH levels show no treatment efficacy with these medications. Pharmacogenetics has great potential for improving treatment outcome as we identify gene variants that affect pharmacodynamic as well as the more commonly studied pharmacokinetic factors. These mutations can guide pharmacotherapeutic agent choice for optimum treatment of alcohol and stimulant abuse and reduce relapse after successful initial abstinence.

Purification and Characterisation of Xanthine Oxidoreductases from Local Bovids in Malta

Xanthine oxidoreductase (XOr) is a molybdoflavoprotein mainly involved in purine catabolism. It exists in two forms, the oxidase (XO) and dehydrogenase (XDH) which are inter-convertible within mammalian cells. Although various researchers have reported the extraction of mammalian XOr, no extractions have yet been carried out in Malta and subsequently no characterizations are available. In this study, XOr was successfully purified from bovine, caprine and ovine milk through a multistep purification process involving both chemical and chromatographic techniques. The molecular weights of the native enzyme were found to be 295 kDa, 281 kDa and 275 kDa, representing the bovine, caprine and ovine XOr respectively. Western blot showed XOr to be represented on SDS-PAGE by a minimum of three major bands having molecular weights of 151 kDa, 131 kDa and 85 kDa. While all samples showed activity on native PAGE, spectrophotometric assays revealed the bovine XOr to be the most active. Surprisingly, the addition of NAD+ to the assay mixture inhibited enzyme activity of the bovine and caprine XOr whereas the ovine XOr doubled its activity in response to NAD+. The latter also showed a lower binding affinity to heparin. Following incubation with trypsin, XOr was irreversibly converted to its oxidase form in all samples as reflected by the observed increase in XO activity.

目的比较中国高校留学生与中国学生的性知识、性态度和性行为现状及性行为影响因素,为更好推进高校性教育提供理论和数据支撑。方法于2019年9—11月,采取按年级分层抽样方法抽取中国东、中、西部3个地区9所高校中国学生5989人,采用方便抽样方法抽取中国96所高校留学生1004人,采用线上问卷调查方式,应用SPSS 21.0软件统计分析数据。结果留学生性知识得分(5.11±2.20)显著低于中国学生得分(5.31±2.17)(t=2.751,P<0.01)。对一夜情所持态度,留学生(25.10%)比中国学生(14.59%)更开放(χ^(2)=73.04,P<0.01);对同性恋所持态度,中国学生(53.35%%)比留学生(43.23%)更开放(χ^(2)=73.83,P<0.01)。性行为发生率留学生(20.42%)稍高于中国学生(18.88%);多个性伴侣现象留学生(58.05%)比中国学生(35.46%)普遍(χ^(2)=27.26,P<0.01);初次性行为年龄<18岁的比例留学生(30.73%)显著高于中国学生(15.12%),差异有统计学意义(χ^(2)=29.28,P<0.01)。男生、高年级、高消费水平、无宗教信仰以及双性恋的留学生更有可能发生性行为(P<0.01);而男生、高年级、文科类学生、高消费水平、同性恋和双性恋的中国学生更有可能发生性行为(P<0.01)。结论中国学生与留学生在性知识、性态度和性行为的差异提示大学阶段仍需加强性健康教育,并且要考虑大学生不同文化背景和性别等因素,从而促进大学生性健康。
