Intrapartum application of the continuous glucose monitoring system in pregnancies complicated with diabetes: A review and feasibility study

Intrapartum maternal normoglycemia seems to play an important role in the prevention of adverse perinatal, maternal and neonatal outcomes. Several glucose monitoring protocols have been developed, aiming to achieve a tight glucose monitoring and control. Depending on the type of diabetes and the optimal or suboptimal glycemic control, the treatment options include fasting status of the parturient, frequent monitoring of capillary blood glucose, intravenous dextrose infusion and subcutaneous or intravenous use of insulin. Continuous glucose monitoring system(CGMS) is a relatively new technology that measures interstitial glucose at very short time intervals over a specifi c period of time. The resulting profi le provides a more comprehensive measure of glycemic excursions than intermittent home blood glucose monitoring. Results of studies applying the CGMS technology in patients with or without diabetes mellitus(DM) have revealed new insights in glucose metabolism. Moreover, CGMS have a potential role in the improvement of glycemic control during pregnancy and labor, which may lead to a decrease in perinatal morbidity and mortality. In conclusion, the use of CGMS, with its important technical advantages compared to the conventional way of monitoring, may lead into a more etiological intrapartum management of both the mother and her fetus/infant in pregnancies complicated with DM.

Dynamic Mechanical Properties of Tissue after Long-Term Implantation of Collagen and Polypropylene Meshes in Animal Models

Purpose: Pelvic floor reconstructive surgery has grown significantly in recent years. A wide variety of available types of meshes exist but the safety and success has not been adequately proven. We sought to evaluate the effects on dynamic biomechanical properties of tissue after long-term implantation of synthetic and biological grafts. Methods: A total of 96 New Zealand white female rabbits (approximately 3 kg) were used, 72 of which were surgically implanted with acellular, collagen mesh (n = 36) or nonabsorbable monofilament polypropylene mesh (n = 36). There was a no mesh-rupture of fascia group (n = 12) and a second, no-mesh, no-fascia rupture control group (n = 12). In the 59 rabbits, of 72 (13 died) tissue was harvested 3 months (n = 24), 6 months (n = 23) and 9 months (n = 12) later, while in the fascia rupture group, tissue was harvested 6 months later. Tissue samples (2 × 2 cm) underwent dynamic mechanical analysis (DMA) testing during which the dynamic rigidity and tissue damping capacities were measured. The statistical analysis was performed with General Linear Model with Tukeys post hoc testing (sPss v.17.0). Results: With respect to mesh type, the rabbit tissue in which polypropylene mesh was used showed the greatest dynamic rigidity. Those with biological mesh delivered the lowest rigidity results, while the two other groups had almost similar behavior. The meshes exhibited their highest relative dynamic tissue stiffening effect at 9 months. Conclusions: Biological mesh causes lower tissue rigidity, resulting in inferior mechanical response and thus seems to be inferior to polypropylene.

Spectrum of magnetic resonance imaging findings in congenital lumbar spinal stenosis

AIM: To investigate whether congenital lumbar spinal stenosis(CLSS) is associated with a specific degenerative changes of the lumbar spine. METHODS: The lumbar spine magnetic resonance imaging studies of 52 subjects with CLSS and 48 control subjects were retrospectively evaluated. In each examination, the five lumbar levels were assessed for the presence or absence of circumferential or shallow annular bulges, annular tears, anterior or posterior disc herniations, epidural lipomatosis, Schmorl's nodes,spondylolisthesis, pars defects, and stress reactions of the posterior vertebral elements. RESULTS: Compared to control individuals, subjects with CLSS exhibited increased incidence of circumferential and shallow annular bulges, annular tears, discherniations and spondylolisthesis(P < 0.05). CONCLUSION: CLSS is associated with increased incidence of degenerative changes in specific osseous and soft-tissue elements of the lumbar spine.

Cost analysis and outcomes of simple elbow dislocations

AIM: To evaluate the management, clinical outcome and cost implications of three different treatment regimes for simple elbow ***: Following institutional board approval, we performed a retrospective review of all consecutive patients treated for simple elbow dislocations in a Level Ⅰ trauma centre between January 2008 and December 2010. Based on the length of elbow immobilisation(LOI), patients were divided in three groups(Group I, 3 wk). Outcome was considered satisfactory when a patient could achieve a pain-free range of motion ≥ 100°(from 30° to 130°). The associated direct medical costs for the treatment of each patient were then calculated and ***: We identified 80 patients who met the inclusion criteria. Due to loss to follow up, 13 patients were excluded from further analysis, leaving 67 patients for the final analysis. The mean LOI was 14 d(median 15 d; range 3-43 d) with a mean duration of hospital engagement of 67 d(median 57 d; range 10-351 d). Group Ⅲ(prolonged immobilisation) had a statistically significant worse outcome in comparison to Group Ⅰ?and Ⅱ(P = 0.04 and P = 0.01 respectively); however, there was no significant difference in the outcome between groups Ⅰ?and Ⅱ(P = 0.30). No statistically significantdifference in the direct medical costs between the groups was ***: The length of elbow immobilization doesn't influence the medical cost; however immobilisation longer than three weeks is associated with persistent stiffness and a less satisfactory clinical outcome.

Low-grade central osteosarcoma of distal femur, resembling fibrous dysplasia

We report a case of a 32 year-old male, admitted for a lytic lesion of the distal femur. One month after the first X-ray, clinical and imaging deterioration was evident. Open biopsy revealed fibrous dysplasia. Three months later, the lytic lesion had spread to the whole distal third of the femur reaching the articular cartilage. The malignant clinical and imaging features necessitated excision of the lesion and reconstruction with a custom-made total knee arthroplasty. Intraoperatively, no obvious soft tissue infiltration was evident. Nevertheless, an excision of the distal 15.5 cm of the femur including 3.0 cm of the surrounding muscles was finally performed. The histological examination of the excised specimen revealed central low-grade osteosarcoma. Based on the morphological features of the excised tumor, allied to the clinical findings, the diagnosis of low-grade central osteosarcoma was finally made although characters of a fibrous dysplasia were apparent. Central low-grade osteosarcoma is a rare, well-differentiated sub-type of osteosarcoma, with clinical, imaging, and histological features similar to benign tumours. Thus, initial misdiagnosis is usual with the condition commonly mistaken for fibrous dysplasia. Central low-grade osteosarcoma is usually treated with surgery alone, with rare cases of distal metastases. However, regional recurrence is quite frequent after close margin excision.

Origin of de novo daptomycin non susceptible enterococci

Effect of Cigarette Smoke on Diabetic Skin and Protection with Topical Administration of Pinus halepensis Extract

Compared to normal, diabetic skin is characterized by great sensitivity. Oxidative stress is directly involved, contributing to accelerated skin aging, xerodermia and poor wound healing. In the last 10 years, cigarette smoke (CS) exposure has been associated with several skin and dermatological conditions and is directly related to the oxidative stress affecting the skin. However, limited data exist concerning the effect of CS on diabetic skin. Some of the effects of cigarette smoke exposure on the skin of hairless diabetic mice were hereby studied and the potential skin protection by topical applications of Pinus halepensis bark extract was investigated. Female hairless SKH-2 diabetic mice were exposed for 8 days to tobacco smoke and topical applications were performed twice daily. Biophysical parameters such as transepidermal water loss (TEWL), skin elasticity and erythema were measured. In addition, the oxidative stress was evaluated. The results show that diabetes and CS have a synergistic negative action on skin condition, with the development of xerosis and high ROS levels whilst topical applications of Pinus halepensis bark extract protect efficiently the toxic effect of CS on skin, by decreasing skin dryness, oxidative stress and blood glucose levels.
