Background:Aspirin-exacerbated respiratory disease(AERD)is an aggressive inflammatory disorder of the upper and lower respiratory ***,leukotriene modifiers,endoscopic sinus surgery(ESS),aspirin(ASA)desensitization,and biological immu-nomodulators are currently used to treat the ***:The objective of this study was to determine the psychosocial impact of ESS and ASA desensitization on AERD ***:All AERD patients who underwent complete ESS were divided into two cohorts based on ASA desensitization *** psychosocial metrics of the SNOT-22 were collected and analyzed at the following time points:pre-operative,1-month,3-month,6-month,and 12-month after ***:One hundred and eighty-four AERD patients underwent ESS from November 2009 to November *** this group,130 patients underwent ASA desensitization(AD cohort)and 54 patients remained non-desensitized(ND cohort).AD patients showed a significantly greater reduction in total SNOT-22 scores over the study period compared to ND patients(p=0.0446).Analysis of SNOT-22 psychosocial metrics showed a significantly greater improvement in patient productivity in the AD cohort when compared to the ND cohort(p=0.0214).Further,a sub-group analysis accounting for subject attrition showed a significantly greater improvement in both productivity and concentration in AD patients when compared to the ND cohort(productivity:p=0.0068;concentration:p=0.0428).Conclusions:ESS followed by ASA desensitization decreases the overall psychosocial burden in AERD patients with a significant improvement in perceived productivity and *** has significant implications given the psychosocial impact of chronic diseases.

Objectives:The use of topical corticosteroids to manage postoperative sinonasal symptoms after endoscopic skull base surgery (ESBS) has not been well *** quantified long‐term impact of postoperative steroid irrigations (SIs) on quality of life of patients after ***:Retrospective review of patients at the University of Pennsylvania undergoing ESBS from 2010 to *** on patient demographics and postoperative treatment with nasal saline irrigation twice daily with and without dissolved steroids (mometasone or budesonide) was ***,and 1‐,3‐,6‐,12‐,18‐,and 24‐month postoperative Sino‐Nasal Outcome Test (SNOT‐22) scores were ***:A total of 727 patients were assessed (53.4% males),with 479 patients in the no SI group and 248 patients in the SI *** SNOT‐22 scores did not differ significantly (P = 0.19).1‐,3‐,6‐,12‐,18‐,and 24‐month post‐op SNOT‐22 scores did not significantly differ between ***,mometasone irrigations resulted in significantly lower postoperative 2‐year SNOT‐22 scores compared to budesonide (P line (P = 0.03).Conclusions:Though corticosteroid irrigations are routine in managing inflammatory sinus disease,their role in postoperative management after ESBS for tumors is *** findings suggest that mometasone irrigation may be effective at improving postoperative quality of life in patients after ESBS.

Germanium （Ge） has gained much interest due to the potential of becoming a direct band gap material and an efficient light source for the future complementary metal-oxide-semiconductor （CMOS） compatible photonic integrated circuits. In this paper, highly biaxial tensile strained Ge quantum wells （QWs） and quantum dots （QDs） grown by molecular beam epitaxy are presented. Through relaxed step-graded InGaAs buffer layers with a larger lattice constant, up to 2.3% tensile-strained Ge QWs as well as up to 2.46% tensile-strained Ge QDs are obtained. Characterizations show the good material quality as well as low threading dislocation density. A strong increase of photoluminescence （PL） with highly tensile strained Ge layers at low temperature suggests the existence of a direct band gap semiconductor.

AIM To characterize the clinical course and outcomes of nasal intermittent mandatory ventilation(NIMV) use in acute pediatric respiratory *** We identified all patients treated with NIMV in the pediatric intensive care unit(PICU) or inpatient general pediatrics between January 2013 and December 2015 at two academic *** who utilized NIMV with other modes of noninvasive ventilation during the same admission were *** included demographics,vital signs on admission and prior to initiation of NIMV,pediatric risk of mortality Ⅲ(PRIsM-Ⅲ) scores,complications,respiratory support characteristics,PICU and hospital length of stays,duration of respiratory support,and *** who did not require escalation to mechanical ventilation were defined as NIMV responders;those who required escalation to mechanical ventilation(MV) were defined as NIMV *** responders were compared to NIMV *** Forty-two patients met study ***(14%) failed treatment and required *** majority of the patients(74%) had a primary diagnosis of *** median age of these 42 patients was 4 mo(range 0.5-28.1 mo,IQR 7,P = 0.69).No significant difference was measured in other baseline demographics and vitals on initiation of NIMV;these included age,temperature,respiratory rate,O2 saturation,heart rate,systolic blood pressure,diastolic blood pressure,and PRIsM-Ⅲ *** duration of NIMV was shorter in the NIMV nonresponder vs NIMV responder group(6.5 h vs 65 h,P < 0.0005).Otherwise,NIMV failure was not associated with significant differences in PICU length of stay(LOs),hospital LOs,or total duration of respiratory *** patients had aspiration pneumonia,pneumothorax,or skin *** Most of our patients responded to *** failure is not associated with differences in hospital LOs,PICU LOs,or duration of respiratory support.

Adverse birth outcomes are a leading cause of mortality in children in China, but the environmental influences of these conditions remain largely unexplained in this population. We aimed to evaluate the levels of polycyclic aromatic hydrocarbons (PAHs) in Chinese pregnant women and their newborns, and to examine the association between levels of PAHs and infant birth weight. We conducted a cross-sectional study including 81 pairs of mothers and newborns from four hospitals in four different cities in China. High Performance Liquid Chromatography was used to measure the concentration of nine PAHs in maternal and cord blood and multiple linear regression analyses were used to evaluate the associations of these PAHs with infant birth weight. Anthracene (ANT) had the highest average concentration and detection rate (geometric mean = 69.54 ng/g and 76.5%, respectively) in maternal serum samples, while fluoranthene (FLT) had the highest concentration and detection rate (geometric mean = 68.4 ng/g and 50.6%, respectively) in the cord blood. Most of the measured PAHs in maternal serum and three PAHs in cord blood were inversely but non-significantly associated with birth weight. The strongest associations were observed for higher concentrations of benzo (a) pyrene (BaP) in maternal serum (230.7 g decrease for levels > median vs. < LOD;p = 0.151) and for ANT in cord blood (153.1 g decrease for levels < median vs. < LOD;p = 0.208). Ant and FLT were the predominant PAHs in the maternal and cord blood serum. Serum concentrations of several measured PAHs were associated with a decreased birth weight, although not significantly, suggesting that further studies with larger sample sizes are needed to validate our findings.

基因治疗的提出为人类遗传疾病和重大疾病的治愈带来了重要机遇,经过几十年曲折的发展,基因治疗逐渐得到人们的认可,且相关药品开始进入市场.但与人们的预期相比,基因治疗的发展比较缓慢,一个关键的因素就是缺乏安全高效的基因载体,尤其是非病毒基因载体.在过去十多年,各种各样的非病毒基因载体得到了极大的发展.我们课题组以大环多胺[12]aneN_(3)为正电单元,利用非病毒基因载体的结构可调性,引入单光子、双光子及聚集诱导和近红外荧光显像单元、酯酶和谷胱甘肽刺激响应单元以及胆酸、生物素靶向单元,设计合成了一系列多功能非病毒基因载体,通过凝胶电泳实验、细胞毒性、细胞摄取、细胞器逃逸、定性定量转染实验以及各种荧光成像技术,研究了它们对质粒DNA和siRNA的递送转染性能及其转染过程,发展了一些生物相容性好、基因转染效率高的非病毒基因载体,进一步研究了它们在诊疗一体化、联合治疗等方面的潜力.本文从结构设计与性质研究两个方面介绍了我们在多功能型非病毒基因载体方面的工作进展,概括总结了各种结构的设计与合成,系统总结了基因转染效率提升与机理探究.最后,对非病毒基因载体未来的发展方向进行了展望.
