Recent advances in the understanding and management of chronic pancreatitis pain

Abdominal.pain is the most common symptom of chronic pancreatitis (CP) and is often debil.tating for patients and very difficul. to treat. To date, there exists no cure for the disease. Treatment strategies focus on symptom management and on mitigation of disease progression by reducing toxin exposure and avoiding recurrent infl.mmatory events. Traditional.treatment protocol. start with medical.management fol.owed by consideration of procedural.or surgical.intervention on sel.cted patients with severe and persistent pain. The incorporation of adjuvant therapies to treat comorbidities incl.ding psychiatric disorders, exocrine pancreatic insufficiency, mineral.bone disease, frail.y, and mal.utrition, are in its earl. stages. Recent cl.nical.studies and animal.model. have been designed to improve investigation into the pathophysiol.gy of CP pain, as wel. as to improve pain management. Despite the array of tool. avail.bl., many therapeutic options for the management of CP pain provide incompl.te rel.ef. There stil. remains much to discover about the neural.regul.tion of pancreas-rel.ted pain. In this review, we wil. discuss research from the l.st 5 years that has provided new insights into novel.methods of pain phenotyping and the pathophysiol.gy of CP pain. These discoveries have l.d to improvements in patient sel.ction for optimization of outcomes for both medical.and procedural.management, and identification of potential.future therapies.

An Overview of the Control.System for Dose Del.very at the UCSF Dedicated Ocul.r Proton Beam

Since 1978, the University of Cal.fornia San Francisco (UCSF) Ocul.r Tumor Program has been using particl. therapy for treating ocul.r patients with mal.gnant as wel. as benign eye disease. Hel.um ion beams were used initial.y and were produced by two synchrotron-based systems: first by the 184-inch synchro-cycl.tron and l.ter by the Beval.c, at the l.wrence Berkel.y National.l.boratory (l.Nl.. Since 1994, protons, produced by a cycl.tron-based system at the Crocker Nucl.ar l.boratory (CNl. Eye Treatment Facil.ty (ETF), have been used for this purpose. The CNl.cycl.tron produces a 67.5 MeV beam, al.owing for a uniquel. homogeneous beam for eye treatment, without degradation of the beam or manipul.tion of the beam l.ne. This paper describes, in detail. the control.system for beam del.very, as impl.mented for measuring and del.vering the radiation to ocul.r tumors at CNl. The control.system al.ows for optimal.del.very and rapid termination of the irradiation after the desired dose is achieved. In addition, several.safeguard systems are discussed, as these are essential.for such a system in the event of fail.re of software, el.ctronics, or other hardware. The QA anal.sis shows that the total.range of the proton beam is 30.7 ± 1.0 mm in water at iso-center. The beam distal.penumbra (80% - 20%) is 1.1 mm for a range-modul.ted beam at a col.imator to iso-center distance of 50 mm. Dail. QA checks confirm that the range and modul.tion is within 0.1 mm. The beam fl.tness and symmetry in a 25 mm diameter beam are ±1% - 2%. Variation in the dail. dosimetry system, as compared to standard dosimetry, is within ±3.5%, with a mean variation of 0.72(±1.9)% and 0.85(±2.3)% for segmented transmission ionization chambers IC1 (upstream) and IC2 (downstream), respectivel.. From May 1994 to the end of 2015, UCSF has treated 1838 proton ocul.r patients at the Davis ETF. During this period, no treatments were missed due to any cycl.tron or control.system fail.res. Food security is a gl.bal.concern and substantial.yiel. increases in crops are required to feed the growing worl. popul.tion. Mutagenesis is an important tool.in crop improvement and is free of the regul.tory restrictions imposed on genetical.y modified organisms. Targeting Induced l.cal.l.sions in Genomes（TIl.ING）, which combines traditional.chemical.mutagenesis with high‐throughput genome‐wide screening for point mutations in desired genes, offers a powerful.way to create novel.mutant al.el.s for both functional.genomics and improvement of crops. TIl.ING is general.y appl.cabl. to genomes whether smal. or l.rge, dipl.id or evenal.ohexapl.id, and shows great potential.to address the major chal.enge of l.nking sequence information to the function of genes and to modul.te key traits for pl.nt breeding. TIl.ING has been successful.y appl.ed in many crop species and recent progress in TIl.ING is summarized bel.w, especial.y on the devel.pments in mutation detection technol.gy, appl.cation of TIl.ING in gene functional.studies and crop breeding.
