Characterization of the Ear Canal Bacterial Flora Present in Hearing Aids (HA) Wearing Subjects

The use of hearing aids (HA) is considered a predisposing factor for ear microbial infections. We undertook this study to compare the presence and nature of the microbial flora inhabiting of ears of HA and non-HA (nHA) users. Swab samples of the ears of HA and nHA users were collected from the Institute of Otolaryngology, “Cattolica del Sacro Cuore” University “Agostino Gemelli”, Rome, Italy. Swab samples were taken from the ear canal of 57 HA and 33 nHA users. The components of the microbial flora present on each swab sample were identified and characterized at the level of species. A total of 41 different bacterial species were identified. A statistically significant prevalence of polymicrobial communities was found in ears presenting signs of inflammation (2.5 ± 1.7 vs 2.1 ± 1.3;P = 0.02) and in HA users (2.3 ± 1.2 vs 1.7 ± 1.0;P = 0.002). Few putative pathogens were detected. Candida albicans spp. was not isolated in our study. A small number of swab samples presented no microbial growth. Bacterial species isolated from HA users with and without inflammation were assayed for the ability to form biofilm. Among gram-positive and gram-negative bacteria, S. aureus, CoNS, P. aeruginosa and K. pneumoniae were found to be strong biofilm producers. S. aureus and P. aeruginosa, isolated only from the ears of HA and nHA users presenting signs of inflammation, were further analyzed for their antibiotic-resistance profile and characterized by the Multilocus Sequence Typing (MLST) assay. The highest rates of antibacterial resistance were in S. aureus to penicillin (75.5%) and in P. aeruginosa, to amoxicillin-clavulanic acid, cefotaxime, ertapenem, tigecycline and trime-thoprim-sulfamethoxazole (100%). Moreover, three S. aureus strains (37.5%) were methicillin-resistant (MRSA). Of the eight S. aureus isolates, we identified six sequence types (ST) indicating that 75% are likely independent clones. Alpha-fetoprotein(AFP) behavior in patients with hepatocellular carcinoma(HCC) waiting for liver transplant(LT) represents a perfect biological example of a fractal model in which its progressive modification and possible future prediction of its values are very hard to capture. As a consequence, AFP represents a useful but poorly manageable tool to increase the ability to better select HCC patients waiting for LT. Trying to find a "filrouge" in the recent literature, no definitive answers can be done to several open questions:(1) the best AFP value to adopt;(2) the best cut-off measurement; and(3) the best way to comfortably capture the effective, time-related, fluctuations of this biological marker. More, structured and prospective, studies using serial determination of AFP values within and without the context of locoregional therapies are needed in order to find the "ideal"(static and dynamic) cut-off values allowing to respond to all the still open questions in this field of transplant oncology.

Berberine behind the thriller of marked symptomatic bradycardia

Berberine is used in traditional Chinese medicine for the treatment of congestive heart failure, hypertension, diabetes, and dyslipidaemia and has a good safety profile. We report a case of a 53-year-old sportsman referred to our hospital for the onset of fatigue and dyspnoea upon exertion after he started berberine to treat hypercholesterolaemia. An electrocardiogram showed sinus bradycardia (45 bpm), first-degree atrioventricular block, and competitive junctional rhythm. An ergometric stress test showed slightly reduced chronotropic competence and the presence of runs of competitive junctional rhythm, atrial tachycardia, and sinus pauses in the recovery. After 10 d of wash-out from berberine, the patient experienced a complete resolution of symptoms, and an ergometric stress test showed good chronotropic competence. An electrocardiogram Holtershowed a latent hypervagotonic state. This is the first case report that shows that berberine could present certain side effects in hypervagotonic people, even in the absence of a situation that could cause drug accumulation. Therefore, berberine's use should be carefully weighed in hypervagotonic people due to the drug's bradycardic and antiarrhythmic properties, which could became proarrhythmic, exposing patients to potential health risks.

背景达90％的截肢者在肢体截肢术后会产生幻肢综合征（phantom limb syndroms，PLS）。尽管现在有很多不同的治疗方法，但没有一种是显著有效的。因此，我们评估了高浓度的局麻药在外周神经的持续输注对预防PLS的效果。方法在术前或术中给71例行下肢截肢术的患者放置一周围神经导管，通过弹力输注泵（非电子）在术中开始以5ml／h的速率持续输注0．5％罗哌卡因，并且持续到术后的第4—83天。在术后的第1天，第1、2、3、4周和第3、6、9、12月评估PLS。为了评估患者在接受罗哌卡因输注时PLS是否存在及其严重程度，在每次评估前中断6—12小时的给药（即直至肢体感觉的恢复）。幻肢痛和残肢痛的程度由5分级的VRS评分系统来评分，其中0分代表无疼痛，4分代表不能忍受的疼痛，而幻肢的感觉被记录为存在或者不存在。如果VRS评分大干1分或者有显著的幻肢感觉出现，罗哌卡因立即以5ml／h的速率重新开始输注。如果患者的VRS评分持续为0或1分并且无幻肢感觉达48小时，则输注完全停止并移除导管。结果局麻药的持续输注时间的中位数是30天（95％的可信区间，25-30天）。在术后第1天，73％的患者主诉有严重的或者不能忍受的疼痛（VRS评分大于2分）。但是，在12个月的评估期的最后患者“严重到不能忍受”的幻肢痛的发生率仅为3％。在第12个月末，患者VRS疼痛评分的百分比：84％的患者为0分，10％的患者为1分，3％的患者为2分，3％的患者为3分，没有患者为4分。但是，在12个月的评估期的最后仍有39％的患者有幻肢感觉。所有患者都能在家中应用这种弹力输注系统。结论术后予以0．5％的罗哌卡因于神经周的持续输注可能是下肢截肢术后治疗幻肢痛和幻肢感觉的有效方法。

Reliability in endoscopic diagnosis of portal hypertensive gastropathy

AIM: To analyze reliability among endoscopists in diagnosing portal hypertensive gastropathy (PHG) and to determine which criteria from the most utilized classifications are the most suitable. METHODS: From January to July 2009, in an academic quaternary referral center at Santa Casa of S o Paulo Endoscopy Service, Brazil, we performed this singlecenter prospective study. In this period, we included 100 patients, including 50 sequential patients who had portal hypertension of various etiologies; who were previously diagnosed based on clinical, laboratory and imaging exams; and who presented with esophageal varices. In addition, our study included 50 sequentialpatients who had dyspeptic symptoms and were referred for upper digestive endoscopy without portal hypertension. All subjects underwent upper digestive endoscopy, and the images of the exam were digitally recorded. Five endoscopists with more than 15 years of experience answered an electronic questionnaire, which included endoscopic criteria from the 3 most commonly used Portal Hypertensive Gastropathy classifications (McCormack, NIEC and Baveno) and the presence of elevated or flat antral erosive gastritis. All five endosco- pists were blinded to the patients' clinical information, and all images of varices were deliberately excluded for the analysis. RESULTS: The three most common etiologies of portal hypertension were schistosomiasis (36%), alcoholic cirrhosis (20%) and viral cirrhosis (14%). Of the 50 patients with portal hypertension, 84% were Child A, 12% were Child B, 4% were Child C, 64% exhibited previous variceal bleeding and 66% were previously endoscopic treated. The endoscopic parameters, presence or absence of mosaic-like pattern, red point lesions and cherry-red spots were associated with high inter-observer reliability and high specificity for diagnosing Portal Hypertensive Gastropathy. 在多种焊接节点中产生的疲劳裂缝,也产生在大量细长正交异性钢桥面板中。一部分裂缝在桥梁投入使用短短几年之后便会出现,所以对于桥面焊接节点的疲劳寿命评估较复杂。这同时也是局部压应力会随着很多因素而随机变化的原因,尤其是那些在车胎、公路和钢结构的动力交互作用面上。提出并讨论了可观测裂缝的主要成因,以及具有梯形横截面和纵肋的正交异性桥面上典型焊接和几何细部的极限疲劳寿命的评估结果。以一现存长跨钢桥的桥面为例进行分析。该桥面曾经通过增加钢筋混凝土覆层,采用剪力螺栓连接到钢板面而加固。这些措施是为了防止后续更大的维修工作,并且提高钢桥面的疲劳寿命预期值。采用数值模型、试验性应变测量对疲劳寿命进行评估,还考虑了在该桥32年的使用年限中所有结构上的可变因素、交通流量、车载荷载。
