Surgical outcomes for unilateral superior oblique palsy in Chinese population: a retrospective study

AIM: To evaluate the outcome after surgery for unilateral superior oblique(SO) palsy in ***: The medical records of 39 patients that underwent surgery for unilateral SO palsy between January 2003 and December 2012 at Caritas Medical Centre, Hong Kong, were retrospectively reviewed. All surgeries were performed by a single surgeon. Pre-operative assessments for vertical deviation, cyclo-deviation, and Knapp’s classification were obtained to determine the nature and degree of surgical *** deviation was measured at 1wk; 1, 6mo and on last follow-up day post-operatively. Cyclo-deviation was measured on last follow-up day ***: During the 10 y period, 39 subjects were recruited. The most common etiology was congenital(94.9%). Knapp’s Type III(66.7%) and Type I(12.8%)classifications were the most common subtypes. To treat SO palsy, the most common surgical procedures were:isolated inferior oblique(IO) anteriorization(41.0%),isolated IO myectomy(10.3%), and isolated IO recession(10.3%). At 3.5 ±2.1y post-operatively, the vertical deviation was significantly reduced(15.1 ±6.2 PD versus0.5±1.4 PD, P <0.0001) without significant improvement in cyclo-deviation(P =0.5). Initial vertical deviation was correlated with cyclo-torsion(r =0.4, P =0.007). Those with over-correction had greater initial vertical deviation(19.4±7.2 PD versus 13.2±4.3 PD, P =0.003). After a single operation, 84.6% of subjects achieved a vertical deviation within ±3 PD.· CONCLUSION: The majority of subjects achieved corrected vertical deviation after a single surgery although there was no improvement in *** with over-correction of primary position deviation had greater preoperative vertical deviation and it may be related to simultaneous multiple muscle surgery.

局部特征选择(LFS)方法将样本空间划分为多个局部区域,并为每个区域选择最优特征子集以反映局部异质信息。然而,现有的LFS方法以每个样本为中心划分局部区域并找到最优特征子集,导致优化效率低下且适用场景受限。为了解决这个问题,提出一种进化双层自适应局部特征选择(BiLFS)算法。LFS问题被建模为双层优化问题,特征子集和待优化局部区域是该问题的两个决策变量。在问题的上层,使用非支配排序遗传算法-Ⅱ求解被选择的局部区域的最优特征子集,区域纯度和被选择特征比率是目标函数;在问题的下层,根据上层求解的最优特征子集,首先使用局部区域聚类分析得到区域内的中心样本,然后通过局部区域融合消除非必要区域并更新必要区域的种群。在11个UCI数据集上的测试结果表明,相较于基于进化算法的非自适应LFS方法,BiLFS的平均分类准确率达到前者的98.48%,而平均所需计算用时仅为前者的9.51%,运算效率得到大幅提升,且达到基于线性规划的LFS方法的水准。对迭代过程中BiLFS算法选择的用于优化的局部区域进行可视化分析,结果表明,BiLFS选择必要局部区域具有稳定性和可靠性。

Spinal root avulsion:an excellent model for studying motoneuron degeneration and regeneration after severe axonal injury

Spinal root avulsion is an excellent model for studying the re- sponse of motoneurons to severe injury to their axons （Koliat- sos et al., 1994）. In this model （‘Avulsion Model＇）, spinal roots are torn off from spinal cord without removing the vertebra at different levels of spinal segments, usually at cervical and lum- bar segments. Step-by-step procedures are described in detail elsewhere （Chu and Wu, 2009）. The Avulsion Model resembles very well brachial plexus injuries in human beings. Around 70% of severe brachial plexus injuries in human involved avulsion of one or more roots （Narakas, 1985） and the main causes of traumatic brachial plexus injuries were motor vehicle accidents, sport injuries and difficult deliveries （Terzis et al., 2001）. The Avulsion Model involves injury to both central nervous system （CNS） and peripheral nervous system （PNS） while nerve axoto- my, transection and crush injuries only involve PNIS.

Objective In prenatal diagnostics,both microarray comparative genomic hybridization (array CGH) and single nucleotide polymorphism(SNP) genotyping have proven to be powerful genomic technologies utilized for the evaluation of fetal structural *** only SNP arrays enable the detection of copy number neutral regions of absence of heterozygosity(AOH), they have limited ability to detect small copy number variants(CNVs) due to the uneven distribution of SNPs across the *** provide comprehensive prenatal testing for both CNVs and copy neutral *** We improved our target designed high-resolution oligonucleotide array that has targeted coverage of 1,650 genes with 15,000 SNP probes,referred to as Fetal DNA chip v2.0(8x60K format).Results Of the 240 cases evaluated by this array, clinically significant CNVs were detected in 25 cases including 21 cases with pathogenic CNV *** addition,31 cases(13.0%) showed at least one AOH region >3 Mb,four of them with AOH >10 Mb demonstrated to be of clinical *** Our data demonstrate that even though this array has a lower density of SNP probes than commercially available SNP arrays, it reliably detected AOH events >3 Mb as well as Pathogenic ***,combining targeted array CGH with SNP probes into one platform provides clinically useful prenatal genetic screening in a more efficient manner.

Objectives The predisposition of intracranial atherosclerotic disease(ICAD)to East Asians over Caucasians infers a genetic basis which,however,remains largely *** prevalence of vascular risk factors(VRFs)in Chinese over Caucasian patients who had a stroke,and shared risk factors of ICAD with other stroke subtypes indicate genes related to VRFs and/or other stroke subtypes may also contribute to *** Unrelated symptomatic patients with ICAD were recruited for genome sequencing(GS,60-fold).Rare and potentially deleterious single-nucleotide variants(SNVs)and small insertions/deletions(InDels)were detected in genome-wide and correlated to genes related to VRFs and/or other stroke *** aneuploidies,copy number variants(CNVs)and chromosomal structural rearrangements were also ***,candidate genes were used for pathway and gene ontology enrichment *** Among 92 patients(mean age at stroke onset 61.0±9.3 years),GS identified likely ICAD-associated rare genomic variants in 54.3%(50/92)of ***-eight patients(52.2%,48/92)had 59 rare SNVs/InDels reported or predicted to be deleterious in genes related to VRFs and/or other stroke *** of the 59 rare variants were identified in local subjects without ICAD(n=126).31 SNVs/InDels were related to conventional VRFs,and 28 were discovered in genes related to other stroke *** study also showed that rare CNVs(n=7)and structural rearrangement(a balanced translocation)were potentially related to ICAD in 8.7%(8/92)of ***,candidate genes were significantly enriched in pathways related to lipoprotein metabolism and cellular lipid catabolic *** Our GS study suggests a role of rare genomic variants with various variant types contributing to the development of ICAD in Chinese patients.

Single incision laparoscopic right hemicolectomy： different approaches

Background Single incision laparoscopic colectomy has been performed in recent years, and has been shown to be feasible and safe. This study was to assess the feasibility of single incision laparoscopic right hemicolectomy and to compare the differences in different approaches. Methods This retrospective study included eighteen patients with carcinoma of caecum and ascending colon, undergoing single incision laparoscopic right hemicolectomy. This study also compared single incision laparoscopic right hemicolectomy using different approaches：（1） single incision multiport, （2） single access port and （3） glove port. Results There was no statistical difference in surgical outcomes. Concerning the surgeon＇s satisfaction toward three methods, overcrowding and durability were similar but the single incision multiport was associated with the highest gas-leak and the ＂glove＂ port was associated with poor durability. However, the method of single incision multiport has the lowest average cost of the special trocar or port in each operation. The operative time and blood loss of the operations in this study were comparable to previous publications. Conclusion There was no significant difference between different approaches of single incision laparoscopic right hemicolectomy for colonic cancer in right side colon.

Background Asthma is a significant chronic health problem worldwide. Management aims at disease control by reducing functional impairment and exacerbations and improving quality of life (QoL). We report a multi-center study to survey asthma control and QoL in four cities in the Pearl River Delta. Methods The conjoint survey involved ten Hong Kong pediatric hospitals/units, two Shenzhen hospitals, two Macao hos-pitals, and two Guangzhou hospitals on asthma control (using Asthma Control Test) and QoL (Pediatric Allergic Disease Quality of Life Questionnaire, PADQLQ). Acceptability of a treatment is graded as very good/good/fair/poor. Results Good asthma control was only reported in 80% subjects in Hong Kong, but higher in sister cities (85–94%, P<0.001). Allergic rhinitis, 'incense burning', and 'smoker in family' were prevalent among the four cities. Logistic regres-sion showed better control of asthma was associated with better PADQLQ (B=?0.029, P < 0.001), better acceptability of bronchodilator (B=?1.488, P = 0.025), negatively with 'smoker in family' (B=?0.83, P = 0.015) and various PADQLQ domains. Conversely, worse PADQLQ was associated with allergic rhinitis severity (B=4.77, P <0.001), poor control of asthma (B=7.56, P <0.001), increased frequency of traditional Chinese medicine use (B=1.7, P < 0.05), increased fre-quency of bronchodilator usage (B=1.05, P < 0.05), 'smoker in family' (B=4.05, P < 0.05), and incense burning at home (B=3.9, P < 0.05). Conclusions There are some clinical and cultural differences among the four southern Chinese cities within the Guangdong province. This study identifies potentially modifiable environmental and treatment factors associated with poor asthma control and QoL for health-care interventions. Having a smoker in the family is independently associated with poor asthma control and QoL.
