Recent Advances in Ketamine Research: An Update

Ketamine is a sedative N-methyl-D-aspartate receptor antagonist, considered as a dissociative anesthetic medication. Ketamine inhibits the voltage-gated Na+ & K+ channels and serotonin and dopamine re-uptake and affects specific receptors, such as α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA), kainate and aminobutyric acid A receptors. It is commonly administered by a parenteral route. On administration, ketamine has particular properties that are potentially involved during anesthetic induction including the enhancement of descending inhibition and anti-inflammatory effects. Recent literature reviews report that ketamine possesses various clinically beneficial properties. Sub-dissociative dose/Lower dose of ketamine (LDK) has potential as well as safe effects in clinical practice for the management of acute and chronic pain in postoperative room as well as Emergency Department (ED), along with cognitive function and depression in Psychiatric Department. Moreover, pharmacology of ketamine includes bronchodilators, neuro-protective actions, anti-shock, anti-suicide, and anti-tumor action immune system disorder. The purpose of this review is to illustrate recent advances in mechanism of action, mode of administration and indication of clinical uses of ketamine. In this article various new uses of ketamine have been shown, mainly related to its NMDAR antagonism and the clinical implementation and significance of low dose/sub dissociative dose of ketamine. In future, beyond being used as the adjuvant general anesthesia, it also can be used as a rapid acting antidepressant and anti-suicidal agent for mental disorders, and adjuvant analgesia to avoid potential risk and side effects of opioids in emergency department and in post-operative care.

Study of Type-B QPOs Observed in Black Hole X-Ray Binary Swift J1728.9-3613

Significance of including lid thickness and particle shape factor in numerical modeling for prediction of particle trap efficiency of invert trap

Sediment accumulation on the bed of open sewers and drains reduces hydraulic efficiency and can cause localized *** invert traps installed underneath the bed of open sewers and drains can eliminate sediment build-up by catching sediment *** three-dimensional(3D)computational studies have examined the particle trapping performance of invert traps of different shapes and depths under varied sediment and flow conditions,considering particles as *** two-dimensional and 3D numerical modeling,researchers assumed the lid geometry to be a thin line and a plane,*** this 3D numerical study,the particle trapping efficiency of a slotted irregular hexagonal invert trap fitted at the flume bottom was examined by incorporating the particle shape factor of non-spherical sewage solid particles and the thicknesses of upstream and downstream lids over the trap in the discrete phase model of the ANSYS Fluent 2020 R1 *** volume of fluid(VOF)and the realizable k-turbulence models were used to predict the velocity *** two-dimensional particle image velocimetry(PIV)was used to measure the velocity field inside the invert *** results showed that the thicknesses of upstream and downstream lids affected the velocity field and turbulent kinetic energy at all flow *** joint impact of the particle shape factor and lid thickness on the trap efficiency was *** both the lid thickness and particle shape factor were considered in the numerical modeling,trap efficiencies were underestimated,with relative errors of-8.66%to-0.65%in comparison to the experimental values of Mohsin and Kaushal(2017).They were also lower than the values predicted by Mohsin and Kaushal(2017),which showed an overall overestimation with errors of-2.3%to 17.4%.

This work aims to present and explore thermal management techniques for the wire arc additive manufacturing(WAAM)of IN718 *** heat can be mitigated via air or water *** this study,the material was deposited under four different heat-input conditions with air or water *** air cooling,the layer is deposited in a normal atmospheric air environment,whereas with water cooling,the material is deposited inside a water tank by varying the water *** validate the air and water cooling thermal management techniques,IN718 single-pass and multilayer linear walls were deposited using the bidirectional gas metal arc welding based WAAM setup under four different heat input *** the deposition of single layers,the temperature profiles were recorded,and the geometric and microstructural features were *** multilayer wall structures,the mechanical properties(hardness,tensile strength,and elongation)were determined and assessed using the corresponding microstructural features explored through scanning electron microscopy(SEM),energy dispersive spectroscopy(EDS),and electron backscatter diffraction(EBSD)*** microstructure observed through SEM analysis in the building direction was found to be nonhomogenous compared with that in the deposition ***,water cooling was found to govern bead characteristics,such as wall width and *** grain size and anisotropy of the mechanical properties also decreased in the water-cooled ***,water cooling is an economical and efficient method to mitigate excessive heat accumulation in WAAM-deposited IN718.

Liver transplantation as an alternative for the treatment of non-resectable liver colorectal cancer: Advancing the therapeutic algorithm

Colorectal cancer is a leading cause of cancerrelated mortality,with nearly half of the affected patients developing liver *** three decades,liver resection(LR)has been the primary curative strategy,yet its applicability is limited to about 20%of *** transplantation(LT)for unresectable metastases was attempted unsuccessfully in the 1990s,with high rates of perioperative death and *** is now more interest in this strategy due to improvements in systemic therapies and surgical techniques.A significant study conducted by the Oslo group showed that patients receiving liver transplants had a 60%chance of survival after five *** better results have been achieved by using advanced imaging for risk stratification and further refining selection criteria,especially in the Norvegian SECA *** review carefully charts the development and history of LT as a treatment option for colorectal cancer liver *** revolutionary path from the early days of exploratory surgery to the current situation of cautious optimism is traced,highlighting the critical clinical developments and improved patient selection standards that have made LT a potentially curative treatment for such challenging very well selected cases.

Multi-criteria decision-making method for evaluation of investment in enhanced geothermal systems projects

Deep geothermal energy presents large untapped renewable energy potential could significantly contribute to global energy needs. However, developing geothermal projects involves uncertainties regarding adequate geothermal brine extraction and huge costs related to preparation phases and consequently drilling and stimulation activities. Therefore, evaluating utilization alternatives of such projects is a complex decision-making problem effectively addressed using multi-criteria decision-making (MCDM) methods. This study introduces the MCDM method utilizing analytic hierarchy process (AHP) and weighted decision matrix (WDM) to assess different utilization alternatives (electricity generation, direct heat use and cogeneration). The AHP method determines the weight of each criterion and sub-criterion, while the WDM calculates the final project grade. Five criteria groups - technological, geological, economic, societal and environmental – comprising twenty-eight influencing factors were selected and used for the assessment of investment in Enhanced Geothermal Systems (EGS) projects. The AHP-WDM method was used by 38 experts from six categories: industry, educational institution, research and technology organization (RTO), small- and medium-sized enterprises (SME), local community and other. These diverse expert inputs aimed to capture varying perspectives and knowledge influence investment decisions in geothermal energy. The results were analysed accordingly. The results underscore the importance of incorporating different viewpoints to develop robust, credible, and effective investment strategies for EGS projects. Therefore, this method will contribute to more efficient EGS project development, enabling thus a greater penetration of the EGS into the market. Additionally, the proposed AHP-WDM method was implemented for a case study examining two locations. Locations were assessed and compared on scenario-based evaluation.
