Introduction SARS-CoV-2 vaccination is an effective means to prevent severe illness and death due to SARS-CoV-2 infection[1].Vaccine safety has been carefully monitored to be sure that the benefit outweighs the risk of adverse events(AEs).Common AEs of the BNT162b2 mRNA vaccine,for example,include fatigue(45%),headache(40%),and chills(28%)after the first administration in accordance with the adult phase Ⅲ clinical trial[1].

Vaccination is the mainstream measure against coronavirus disease 2019(COVID-19).As of May 2022,more than 80%of the Japanese population has received vaccination doses(1),and booster vaccination programs are in progress.Acute disseminated encephalomyelitis is the most common type of demyelination following vaccinations,and although an increase in neuromyelitis optica spectrum disease(NMOSD)has recently been reported worldwide(2-4),no collective data,especially after newly developed mRNA vaccines against COVID-19,have been reported yet.

背景:结肠镜能检出肿瘤、息肉和炎症性肠病等结直肠疾病。应用深度卷积神经网络(CNN)的计算机辅助诊断(CAD)系统在结肠镜检查中能够识别解剖部位,为内镜医生提供支持。本研究旨在建立一个应用CNN的CAD系统,以区分盲肠、升结肠、横结肠、降结肠、乙状结肠及直肠等解剖部位的内镜影像。方法:我们用包括9,995个结肠镜影像的训练组来建立CNN,并用5,121个独立的结肠镜影像来进行验证CNN的检测效能。验证组影像根据解剖部位分为七类:回肠末端、盲肠、升结肠至横结肠、降结肠至乙状结肠、直肠、肛门以及无法识别的部位。我们检测了2017年1-11月间单中心的结肠镜影像照片,评估内镜医生与CNN的诊断一致性。本研究主要结局指标是CNN判断内镜下解剖部位的灵敏度和特异度。结果:我们建立的CNN识别内镜下解剖部位的曲线下面积为:回肠末端0.979,盲肠0.940,升结肠至横结肠0.875,降结肠至乙状结肠,0.846,直肠0.835,肛门0.992。在验证过程中,CNN系统能准确识别66.6%的内镜下解剖部位。结论:我们建立了一个临床相关的新的CNN系统,该系统可识别结肠镜下的不同解剖部位,这是构建CAD系统的第一步,后者能给结肠镜检查提供质量保障。

An appendiceal neurofibroma(ANF) is a rare neoplasm associated with neurofibromatosis type 1(NF-1),an inheritable neurocutaneous disorder that involves multiple systems including the intraabdominal organs.Appendiceal diverticulitis occasionally ruptures in the absence of intense abdominal pain,which can lead to serious consequences.Recent reports highlight the association between appendiceal diverticulum and appendiceal neoplasms; however,there is still little information on the association between appendiceal diverticulitis and ANF in NF-1.A 51-year-old Japanese male with NF-1 was referred to the division of surgery for mild right lower quadrant pain.It was suspected he had perforated acute appendicitis with periappendiceal abscess based on clinical manifestations and findings of computed tomography.An emergency appendectomy was conducted.The pathological examination revealed diffusely proliferated tumor cells of a neurofibroma,coexistent with multiple appendiceal diverticulums,leading to the diagnosis ofperforated appendiceal diverticulitis associated with ANF.Although he developed a remnant abscess,he recovered with the conservative treatments of antibiotics and drainage.This case suggests that appendiceal diverticulitis might be a complication of appendiceal involvement of NF-1,and that it occasionally ruptures in the absence of intense abdominal pain.Clinicians should recognize that NF-1 can cause various abdominal manifestations.

新型冠状病毒肺炎(COVID-19)于2019年12月开始出现病例报道以来,逐渐在全球范围内传播。日本国内的COVID-19病例从2020年1月中旬开始出现,并随时间推移持续增加,至2020年2月底前被认为是日本国内传播的初期阶段。本文描述截至2020年2月底与日本有关的935例COVID-19病例的传播状况,包括从武汉包机接回的15例日本侨民感染者、大型邮轮“钻石公主”号中的696例感染者以及日本国内的224例感染者状况。同时总结了日本控制疫情扩散的措施,如限制针对新型冠状病毒(SARS-CoV-2)进行RT-PCR检测、降低不必要的前往医疗机构就诊的轻症患者人数、制订SARS-CoV-2感染咨询的准则、取消大型聚会、暂时关闭学校等。进一步提出了日本在预防与控制SARS-CoV-2传播中遇到的问题,如RT-PCR的检测迟缓、医务人员面临被感染风险、日本国内医疗保健服务体系存在地区差异、信息公开与管理存在混乱等。通过以上介绍,加深对COVID-19在日本及全球流行特点的理解,为全球控制COVID-19疫情提供借鉴。

The development of vehicle integrated photovoltaics-powered electric vehicles (VIPV-EV) significantly reduces CO2 emissions from the transport sector to realize a decarbonized society. Although long-distance driving of VIPV-EV without electricity charging is expected in sunny regions, driving distance of VIPV-EV is affected by climate conditions such as solar irradiation and temperature rise of PV modules. In this paper, detailed analytical results for effects of climate conditions such as solar irradiation and temperature rise of PV modules upon driving distance of the VIPV-EV were presented by using test data for Toyota Prius and Nissan Van demonstration cars installed with high-efficiency InGaP/GaAs/InGaAs 3-junction solar cell modules with a module efficiency of more than 30%. The temperature rise of some PV modules studied in this study was shown to be expressed by some coefficients related to solar irradiation, wind speed and radiative cooling. The potential of VIPV-EV to be deployed in 10 major cities was also analyzed. Although sunshine cities such as Phoenix show the high reduction ratio of driving range with 17% due to temperature rise of VIPV modules, populous cities such as Tokyo show low reduction ratio of 9%. It was also shown in this paper that the difference between the driving distance of VIPV-EV driving in the morning and the afternoon is due to PV modules’ radiative cooling. In addition, the importance of heat dissipation of PV modules and the development of high-efficiency PV modules with better temperature coefficients was suggested in order to expand driving range of VIPV-EV. The effects of air-conditioner usage and partial shading in addition to the effects of temperature rise of VIPV modules were suggested as the other power losses of VIPV-EV.

With an increasing global population that is rapidly ageing,our society faces challenges that impact health,environment,and energy demand.With this ageing comes an accumulation of cellular changes that lead to the development of diseases and susceptibility to infections.This impacts not only the health system,but also the global economy.As the population increases,so does the demand for energy and the emission of pollutants,leading to a progressive degradation of our environment.This in turn impacts health through reduced access to arable land,clean water,and breathable air.New monitoring approaches to assist in environmental control and minimize the impact on health are urgently needed,leading to the development of new sensor technologies that are highly sensitive,rapid,and low-cost.Nanopore sensing is a new technology that helps to meet this purpose,with the potential to provide rapid point-of-care medical diagnosis,real-time on-site pollutant monitoring systems to manage environmental health,as well as integrated sensors to increase the efficiency and storage capacity of renewable energy sources.In this review we discuss how the powerful approach of nanopore based single-molecule,or particle,electrical promises to overcome existing and emerging societal challenges,providing new opportunities and tools for personalized medicine,localized environmental monitoring,and improved energy production and storage systems.

The emergence of a novel strain during a pandemic,like the current COVID-19,is a major concern to the healthcare system.The most effective strategy to control this type of pandemic is vaccination.Many previous studies suggest that the existing vaccine may not be fully effective against the new strain.Additionally,the new strain's late arrival has a significant impact on the disease dynamics and vaccine coverage.Focusing on these issues,this study presents a two-strain epidemic model in which the new strain appears with a time delay.We considered two vaccination provisions,namely preinfection and post-infection vaccinations,which are governed by human behavioral dynamics.In such a framework,individuals have the option to commit vaccination before being infected with the first strain.Additionally,people who forgo vaccination and become infected with the first train have the chance to be vaccinated(after recovery)in an attempt to avoid infection from the second strain.However,a second strain can infect vaccinated and unvaccinated individuals.People may have additional opportunities to be vaccinated and to protect themselves from the second strain due to the time delay.Considering the cost of the vaccine,the severity of the new strain,and the vaccine's effectiveness,our results indicated that delaying the second strain decreases the peak size of the infected individuals.Finally,by estimating the social efficiency deficit,we discovered that the social dilemma for receiving immunization decreases with the delay in the arrival of the second strain.

Rare histological subtype of invasive micropapillary carcinoma in the ampulla of Vater: A case report

BACKGROUND Carcinoma of the ampulla of Vater is an uncommon ampullo-pancreatobiliary neoplasm,and the most common histological type is adenocarcinoma with a tubular growth pattern.Invasive micropapillary carcinoma(IMPC)is an aggressive variant of adenocarcinoma in several organs that is associated with lymph node metastasis and poor prognosis.IMPC was first described as a histological subtype of breast cancer;however,IMPC of the ampulla of Vater is extremely rare,with only three articles reported in the English literature.CASE SUMMARY We have reported a case of IMPC of the ampulla of Vater in an 80-year-old man.Microscopically,the surface area of the carcinoma was composed of tubulopapillary structures mimicking intra-ampullary papillary-tubular neoplasm,and the deep invasive front area exhibited a pattern of IMPC.The carcinoma showed lymphatic invasion and extensive lymph node metastasis.The immunohistochemical study revealed mixed intestinal and gastric/pancreatobiliary phenotypes.CONCLUSION This rare subtype tumor in the ampulla of Vater showed a histologically mixed phenotype and exhibited aggressive behavior.

A Game-Theoretic Modeling Approach to Comprehend the Advantage of Dynamic Health Interventions in Limiting the Transmission of Multi-Strain Epidemics

In the situation of inadequate vaccines and rapid mutation of virulent strains, alternative health interventions play a crucial role in the containment of emerging epidemics. This study elucidates the critical aspects of health interventions to control epidemic resurgence. Besides, human behavioral response to epidemics plays an instrumental role in bringing the success of control efforts. The appearance of multi-strain epidemics has become a global health concern that requires special attention. Here, we introduce a novel mean-field epidemic game approach to predict the evolutionary dynamics of flu-like epidemics having multiple disease strains. Our model illustrates the importance of multiple provisions alongside their timely execution for better disease attenuation. In addition to vaccination, we introduce self-protection as a potential alternative that yields safeguard against either strain. Both these imperfect provisions render better efficacy against primary (resident) strain than secondary (mutant) to contain epidemic transmission. The simulation-backed model analysis further sheds some light on the crucial impacts of control interventions to limit the invasion of virulent strains from qualitative and quantitative viewpoints. It explicates how vaccination and self-protection complement each other as per situation demands. Our full-fledged theoretical approach further illustrates the dynamic trade-off between the cost and efficacy of a certain intervention. We confirm that the disease dies out when the basic reproduction number of individual strains is less than one and becomes endemic if it is greater than one. Finally, the model addresses the evolutionary consequences when mutation takes place from primary to secondary strain. Some impressive facts while employing dual provisions have been reinforced using a game-theoretic framework.
