There has been a long-standing need for guidelines on the diagnosis and treatment of keloids and hypertrophic scars that are based on an understanding of the pathomechanisms that underlie these skin fibrotic *** is particularly true for clinicians who deal with Asian and African patients because these ethnicities are highly prone to these *** contrast,Caucasians are less likely to develop keloids and hypertrophic scars,and if they do,the scars tend not to be *** ethnic disparity also means that countries vary in terms of their differential diagnostic *** lack of clear treatment guidelines also means that primary care physicians are currently applying a hotchpotch of treatments,with uneven *** overcome these issues,the Japan Scar Workshop(JSW)has created a tool that allows clinicians to objectively diagnose and distinguish between keloids,hypertrophic scars,and mature *** tool is called the JSW Scar Scale(JSS)and it involves scoring the risk factors of the individual patients and the affected *** tool is simple and easy to *** a result,even physicians who are not accustomed to keloids and hypertrophic scars can easily diagnose them and judge their *** JSW has also established a committee that,in cooperation with outside experts in various fields,has prepared a Consensus Document on keloid and hypertrophic scar treatment *** guidelines are simple and will allow even inexperienced clinicians to choose the most appropriate treatment *** Consensus Document is provided in this *** describes(1)the diagnostic algorithm for pathological scars and how to differentiate them from clinically similar benign and malignant tumors,(2)the general treatment algorithms for keloids and hypertrophic scars at different medical facilities,(3)the rationale behind each treatment for keloids and hypertrophic scars,and(4)the body site-specific treatment protocols for these *** believe that t

Endoscopic transgastric drainage of a gastric wall abscess after endoscopic submucosal dissection

A 63-year-old woman was referred to our hospital for further examination because of an incidental finding of early gastric *** submucosal dissection(ESD)was successfully performed for complete resection of the *** the first post-ESD day,the patient suddenly complained of abdominal pain after an episode of *** computed tomography(CT)showed delayed perforation after *** patient was conservatively treated with an intravenous proton pump inhibitor and *** the fifth post-ESD day,CT revealed a gastric wall abscess in the gastric *** revealed a gastric fistula at the edge of the post-ESD ulcer,and pus was found flowing into the *** intradrainage stent and an extradrainage nasocystic catheter were successfully inserted into the abscess for endoscopic transgastric *** the procedure,the clinical symptoms and laboratory test results improved *** months later,a follow-up CT scan showed no collection of ***,the intradrainage stent was *** the gastric wall abscess recurred 2 wk after stent removal,it recovered soon after endoscopic transgastric ***,after stent removal and oral antibiotic treatment for 1 mo,no recurrence of the gastric wall abscess was found.

Real-time in situ three-dimensional integral videography and surgical navigation using augmented reality: a pilot study

To evaluate the feasibility and accuracy of a three-dimensional augmented reality system incorporating integral videography for imaging oral and maxillofacial regions, based on preoperative computed tomography data. Three-dimensional surface models of the jawbones, based on the computed tomography data, were used to create the integral videography images of a subject＇s maxillofacial area. The three-dimensional augmented reality system （integral videography display, computed tomography, a position tracker and a computer） was used to generate a three-dimensional overlay that was projected on the surgical site via a half-silvered mirror. Thereafter, a feasibility study was performed on a volunteer. The accuracy of this system was verified on a solid model while simulating bone resection. Positional registration was attained by identifying and tracking the patient/surgical instrument＇s position. Thus, integral videography images of jawbones, teeth and the surgical tool were superimposed in the correct position. Stereoscopic images viewed from various angles were accurately displayed. Change in the viewing angle did not negatively affect the surgeon＇s ability to simultaneously observe the three-dimensional images and the patient, without special glasses. The difference in three-dimensional position of each measuring point on the solid model and augmented reality navigation was almost negligible （〈1 mm）; this indicates that the system was highly accurate. This augmented reality system was highly accurate and effective for surgical navigation and for overlaying a three-dimensional computed tomography image on a patient＇s surgical area, enabling the surgeon to understand the positional relationship between the preoperative image and the actual surgical site, with the naked eye.

A Neuro-Based Software Fault Prediction with Box-Cox Power Transformation

Software fault prediction is one of the most fundamental but significant management techniques in software dependability assessment. In this paper we concern the software fault prediction using a multilayer-perceptron neural network, where the underlying software fault count data are transformed to the Gaussian data, by means of the well-known Box-Cox power transformation. More specially, we investigate the long-term behavior of software fault counts by the neural network, and perform the multi-stage look ahead prediction of the cumulative number of software faults detected in the future software testing. In numerical examples with two actual software fault data sets, we compare our neural network approach with the existing software reliability growth models based on nonhomogeneous Poisson process, in terms of predictive performance with average relative error, and show that the data transformation employed in this paper leads to an improvement in prediction accuracy.

Laparoscopic and endoscopic co-operative surgery for nonampullary duodenal tumors

AIM To assess the safety and feasibility of laparoscopic and endoscopic co-operative surgery(LECS) for early nonampullary duodenal *** Twelve patients with a non-ampullary duodenal tumor underwent LECS at our hospital. One patient had two mucosal lesions in the duodenum. The indication for this procedure was the presence of duodenal tumors with a low risk for lymph node metastasis. In particular, the tumors included small(less than 10 mm) submucosal tumors(SMT) and epithelial mucosal tumors, such as mucosal cancers or large mucosal adenomas with malignant suspicion. The LECS procedures, such as full-thickness dissection for SMT and laparoscopic reinforcement after endoscopic submucosal dissection(ESD) for epithelial tumors, were performed for the 13 early duodenal lesions in 12 patients. Here we present the short-term outcomes and evaluate the safety and feasibility of this new *** Two SMT-like lesions and eleven superficial epithelial tumor-like lesions were observed. Seven and Six lesions were located in the second and third parts of the duodenum, respectively. All lesions were successfully resected en bloc. The defect in the duodenal wall was manually sutured after resection of the duodenal SMT. For epithelial duodenal tumors, the ulcer bed was laparoscopically reinforced via manual suturing after ESD. Intraoperative perforation occurred in two out of eleven epithelial tumor-like lesions during ESD; however, they were successfully laparoscopically repaired. The median operative time and intraoperative estimated blood loss were 322 min and 0 mL, respectively. Histological examination of the tumors revealed one adenoma with moderate atypia, ten adenocarcinomas, and two neuroendocrine tumors. No severe postoperative complications(Clavien-Dindo classification grade Ⅲ or higher) were reported in this series, but minor leakage secondary to pancreatic fistula occurred in one *** LECS can be a safe and minimally invasive treatment

Availability Importance Measures for Virtualized System with Live Migration

This paper presents component importance analysis for virtualized system with live migration. The component importance analysis is significant to determine the system design of virtualized system from availability and cost points of view. This paper discusses the importance of components with respect to system availability. Specifically, we introduce two different component importance analyses for hybrid model (fault trees and continuous-time Markov chains) and continuous-time Markov chains, and show the analysis for existing probabilistic models for virtualized system. In numerical examples, we illustrate the quantitative component importance analysis for virtualized system with live migration.

Aperiodic Checkpoint Placement Algorithms—Survey and Comparison

In this article we summarize some aperiodic checkpoint placement algorithms for a software system over infinite and finite operation time horizons, and compare them in terms of computational accuracy. The underlying problem is formulated as the maximization of steady-state system availability and is to determine the optimal aperiodic checkpoint sequence. We present two exact computation algorithms in both forward and backward manners and two approximate ones;constant hazard approximation and fluid approximation, toward this end. In numerical examples with Weibull system failure time distribution, it is shown that the combined algorithm with the fluid approximation can calculate effectively the exact solutions on the optimal aperiodic checkpoint sequence.

This paper proposes a quantitative security evaluation for software system from the vulnerability data consisting of discovery date, solution date and exploit publish date based on a stochastic model. More precisely, our model considers a vulnerability life-cycle model and represents the vulnerability discovery process as a non-homogeneous Poisson process. In a numerical example, we show the quantitative measures for contents management system of an open source project.

AIM To clarify the roles of TWEAK and its receptor Fn14 in 5-fluorouracil(5-FU)-induced *** Diarrhea was induced in wild-type(WT), Fn14 knockout(KO), and IL-13 receptor(IL-13R)α1 KO BALB/c mice using a single injection of 5-FU. Histological analysis, cytokine analysis, and flow cytometry was performed on ileal tissues and cells. Murine colon carcinomabearing mice were co-treated with an anti-TWEAK antibody and 5-FU. Embryonic fibroblast response to cytokines was also ***5-FU induced high Fn14 expression in epithelial cells. The severity of 5-FU-induced diarrhea was lower in Fn14 KO mice compared with WT mice. Administration of anti-TWEAK antibody reduced 5-FU-induced diarrhea without affecting the antitumor effects of 5-FU in vivo. 5-FU-induced expression of IL-13, IL-17 A, TNF-α, and IFN-γ in the ileum was Fn14 dependent. The severity of 5-FU-induced diarrhea was lower in IL-13Rα1 KO mice, indicating major role for IL-13 signaling via IL-13Rα1 in pathogenesis. We found that IL-13Rα2, an IL-13 neutralizing/cell protective receptor, was strongly induced by IL-33 in vitro and in vivo. IL-13Rα2 was upregulated in the ileum of 5-FU-treated Fn14 KO mice. Thus, the deletion of Fn14 upregulated IL-13Rα2 expression, which reduced IL-13 expression and activity. CONCLUSION Disruption of the TWEAK/Fn14 pathway affects several interconnected pathways, including those associated with IL-13, IL-33, and IL-13Rα2, to attenuate 5-FUinduced intestinal side effects.

Development of the Japanese Medication Adherence Scale and Verification of Its Reliability and Validity in Hypertensive Patients

A 32-Item Japanese Medication Adherence Scale had been developed as a tool for evaluating the medication-taking behavior of hypertensive patients and predicting therapeutic efficacy, and an Internet survey of 990 hypertensive patients throughout Japan was performed. As a result, factor 1 “Expectation of pharmacological efficacy” (9 items), factor 2 “Motivation to be self-controlled in taking medication” (6 items), and factor 3 “Negative feelings about taking medication” (4 items) were identified, comprising a total of 19 items. The scale was highly reliable. Because it proved capable of discriminating between the 2 groups with different medication-taking status and blood pressure, its validity had also been confirmed.
