Health Resource Utilization and Comorbidities in Patients with Mental Disorders: Analysis Based on Health Insurance Claim Data

The number of patients with mental disorders is increasing all over the world and they have a high prevalence of physical complications. To analyze and compare the cost of mental disorders with the total medical costs such as mental disorders and physical disorders, we analyzed the cost that patients with mental disorders incurred. We investigated the distribution of medical costs and the characteristics of diseases by using the health insurance claims of people in City A, Japan from March 2013 to February 2014. The subjects had one or more of 4 mental disorders: schizophrenia, alcohol related disorders, bipolar disorder, and depressive episode. As a result, the total number of patients who met inclusion criteria per year was 7403 (6522 outpatients and 881 inpatients). It was revealed that the hospitalization rate of patients with mental disorders increased with age, and many inpatients stayed in hospital for a long time. Also, it was revealed that many patients with mental disorders were in complicated condition with more than one mental or physical disorders and incurred medical costs for these comorbidities. In conclusion, this analysis indicated that many patients with mental disorders switch from attending the outpatient department to hospitalization as they become older. Further, they incurred a lot of medical expenses for complication management. To improve their QOL, comprehensive assessment of their mental/ physical health, self-management education, coordination of services, and support for decision making regarding treatment are necessary.

The 45-Degree Arthroscope Improves Visualization of the Femoral Attachment of the Anterior Cruciate Ligament

Background: Exposure of the insertion site of the anterior cruciate ligament (ACL) is important for appropriate tunnel placement in ACL reconstruction surgery. However, observing the femoral ACL insertion site via the standard anterolateral portal is sometimes difficult. In this study, we compared views of the femoral ACL insertion site between 30-degree and the 45-degree arthroscopes. Methods: We first inserted the 30-degree and the 45-degree arthroscope into the anterolateral portal of a knee simulator in which we had drawn a lattice pattern on the lateral intercondylar notch based on the quadrant method. Next, we compared the arthroscopic views provided by the 30-degree and 45-degree arthroscopes during ACL reconstruction surgery by measuring the area of the lateral intercondylar notch visible through each of the arthroscopes. Results: In the knee simulator, the 45-degree arthroscope showed the entire area of the lateral intercondylar notch, whereas the 30-degree arthroscope had to be introduced more deeply to show the most superior and posterior quadrant, where the attachment of the anteromedial bundle of ACL is located. During the ACL reconstruction, the area of the lateral intercondylar notch in the field of view was larger through the 45-degree arthroscope than through the 30-degree arthroscope. Conclusion: The 45-degree arthroscope provides a better view of the femoral ACL insertion site via the anterolateral portal, which may be helpful during ACL reconstruction.

Possibility of a Straightening Flow-Meter by Using Woven Screen

In this paper, the possibility of the flow rate measurement for a circular pipe flow by using a wo-ven screen with the property of straightening un-uniform flows is discussed. The resistance coefficient and the flow rate coefficient are estimated from the pressure difference caused by the woven screen under the experiment ranges of the wire Reynolds number, Red = 2.2 × 102-1.8 × 103, and of the open area ratio, β = 0.28-0.65. As a result, the resistance coefficient decreases and the flow rate coefficient increases as the wire Reynolds number Red or the open area ratio β increases. In addition, both coefficients are not affected by the difference between uniform and turbulent pipe flows approaching the woven screen. Therefore, the possibility of a flow-meter having the property to straighten the un-uniform flow is expected.

＜正＞The carbon nanotubes （ CNT）/unsaturated polyester resin （ UPR ） nanocomposites were fabricated, and their mechanical property, electrical conduction and percolation behavior were investigated experimentally and theoretically. The carbon nanotubes used in this study were those by the chemical vapor deposition （ CVD ） method. Two carbon nanotubes, VGCF （ diameter 150 nm）and VGNF（ diameter 80 nm） , were used. The mechanical damping and storage modulus of the composites were measured using a dynamic mechanical analyzer（ DMA）. Damping factor decreases with an increment of the fiber volume fraction. The electrical conductivity of the nanocomposites was measured as a function of carbon nanotube volume fraction in order to understand percolation behavior. The nanocomposites showed the electrical conductivity with low percolation threshold between 2 vol% and 3 vol%. Moreover, the percolation threshold was examined based on simulation calculations. The critical carbon nanotube volumes fraction is in a good agreement with both the theoretical prediction and the experiment results.

Radiofrequent ablation(RFA)for treating hepatocellular carcinoma(HCC)has prevailed in Japan since it was assured legally to use in *** to algorithm of treating HCC,the application of RFA was 3 nodules whose diameter are less then 3cm or single nodule within 5cm in *** instruments are devided into expansive needle type and cool-tip *** tip needle system has been more accepted in our country because of small puncture *** RFA ablation,safety margin of more than 5mm is required to bring about local control. Incidence of local recurrence after RFA for HCC≦3cm and＞3cm were 6 to 21% and 8 to 40% respectively. Incidence of intrahepatic distant recurrence was 33 to 85% three years *** are also some reports decribing the complication and problems of *** presentation is focused on the complication of RFA.

A simplified two-piece mandibular advancement appliance for obstructive sleep apnea

Nowadays mandibular advancement appliances (MA- As) are available and distributed widely in the field of dentistry for the treatment of snoring and ob- structtive sleep apnea (OSA). However, a few stud- ies discussed temporomandibular joint (TMJ) discom- fort and masticatory muscle stiffness during wearing MAAs are found. A new appliance that we introduced in this pilot study has shown significant jaw move- ment and could diminish TMJ and masticatory side effects.

A 1-GHz, 7-mW, 8-Bit Subranging ADC without Resistor Ladder Using Built-In Threshold Calibration

A subranging analog-to-digital converter (ADC) features high-speed and relatively low-power. The limiting factors of power reduction in subranging ADCs are the resistor ladder and the comparator. We propose an ADC architecture combining a capacitive digital-to-analog convertor and built-in threshold calibration to eliminate the resistor ladder, resulting in a low-power subranging ADC. We also propose a calibration technique comprising of metal-oxide-metal capacitor, MOS switch, and scaling capacitor to reduce the power consumption of the comparator and an offset drift compensation technique to enable precise foreground calibration. We designed an 8-bit, 1-GHz subranging ADC by applying these techniques, and post-layout simulation results demonstrated a power consumption of 7 mW and figure of merit of 51 fJ/conv.-step.

Non-steroidal anti-inflammatory drug-induced small bowel injuries identified by double-balloon endoscopy

AIM： To clarify clinical features of the NSAID-induced sma bowel lesions using a new method of endoscopy. METHODS： This is a retrospective study and we analyzed seven patients with small bowel lesions while taking NSAIDs among 61 patients who had undergone doubleballoon endoscopy because of gastro-intestinal bleeding or anemia between September 2000 and March 2004, at .lichi Medical School Hospital in Japan. Neither conventional EGD nor colonoscopy revealed any lesions of potential bleeding sources including ulcerations. Double-balloon endoscopy was carried out from oral approach in three patients, from anal approach in three patients, and from both approaches in one patient. RESULTS： Ulcers or erosions were observed in the ileum in six patients and in the jejunum in one patient, respectively. The ulcers were multiple in all the patients with different features from tiny punched out ulcers to deep ulcerations with oozing hemorrhage or scar. All the patients recovered uneventfully and had full resolution of symptoms after suspension of the drug. CONCLUSION： NSAIDs can induce injuries in the small bowel even in patients without any lesions in both the stomach and colon.

Cloning and analysis of [NiFe]-hydrogenase genes from Arthrospira platensis FACHB341

The complete [ NiFe]-hydrogenase genes were cloned from cyanobacterium, Arthrospira platensis FACHB341, using PCR and in-vitro cloning. The total length is 2078bp, including hoxY 549bp, hoxH 1431bp and the spacer in between 98bp. There is a stem-loop structure, downstream of the hoxY gene, serving as a transcription terminator. The deduced amino acid sequences of HoxY aud HoxH consist of acidic amino acid of 15.8% and 12.0%; alkalitropic amino acid of 11.9% and 15.3%; hydrophobic amino acid of 46.3% and 41.0%, respectively. The similarities of hoxY and hoxH genes in Arthrospira platensis FACHB341 to their homologues in other cyanobacteria were compared respectively. The secondary structures and 3D models of small and large subunits of [ NiFe]-hydrogenase were predicted by using 3D-PSSM.
